Objective:To summarize the clinical characteristics of patients with end-stage heart failure who receive heart transplant under extracorporeal membrane oxygenation (ECMO) support. Methods:The clinical data of 12 pediatric patients who received heart transplant with ECMO support in the Seventh Medical Center of Chinese People′s Liberation Army General Hospital and Guangdong Provincial People′s Hospital, from January 2019 to December 2023 was collected. The data included sex, age, weight, diagnosis, pre-ECMO lactate level, left ventricular ejection fraction (LVEF), vasoactive-inotropic score (VIS), and preoperative ECMO running time. Surgical data included cold ischemia time of the donor heart, cardiopulmonary bypass time, intraoperative use of immunosuppressant, postoperative use of ECMO, duration of postoperative ECMO, rate of successful weaning from ECMO, and survival discharge rate. The paired t-test was performed to compare cardiac function indices before and after left ventricular decompression. Results:The 12 patients ranged in age from 1.1 to 15.8 years, and weighted from 8 to 63 kg. Ten children were diagnosed with dilated cardiomyopathy, one with myocardial underdensification, and one with a novel heterozygous mutation of the SCN5A gene causing overlap syndrome complicated by fatal arrhythmia. Before ECMO, the lactate ranged from 0.6 to>15.0 mmol/L, the LVEF from 6.5% to 43%, and VIS from 3 to 108. Four patients underwent left ventricular decompression supported by preoperative ECMO, and their pulse pressure was significantly increased after decompression ((17.8±2.1) vs. (9.8±1.5) mmHg, 1 mmHg=0.133 kPa, t=11.31, P=0.001), while there was no apparent change in LVEF ((26.8±4.4)% vs. (24.9±4.9)%, t=1.75, P=0.178). A total of 7 children received a second run of ECMO after surgery and 3 of them successfully weaned off ECMO and survived to discharge. In the entire cohort, 10 were successfully weaned from ECMO and 8 survived to discharge. Conclusions:For children with end-stage heart failure supported by ECMO, left ventricular decompression can significantly improve pulse pressure. These patients will eventually require heart transplantation.

Chinese Journal of Endemiology 2020;39(8):619-624
Iodine is an important raw material for thyroid hormone synthesis. Sodium iodide symporter (NIS), chloride iodine transporter (Pendrin), iodotyrosine deiodinase (IYD), thyroglobulin (TG), thyroid peroxidase (TPO) and thyroid stimulating hormone receptor (TSHR) are important genes related to thyroid hormone synthesis, all of them are involved in iodine metabolism. The dysfunction of genes related to thyroid hormone synthesis is not only related to thyroid diseases, but also related to other diseases such as breast cancer. In this paper, the function of genes related to thyroid hormone synthesis, the relationship between genes related to thyroid hormone synthesis and diseases, and how to be regulated by iodine are reviewed, in order to provide new ideas for the study of the relationship between iodine and diseases.

Iodine is one of the essential trace elements for human body. Its main physiological function is to synthesize thyroid hormones, which are involved in the development and metabolism of the body. Although China has achieved the goal of eliminating iodine deficiency disorders (IDD), people ignore the general iodine deficiency in the environment, and they are worried about excessive iodine intake and reduce the consumption of iodized salt. This may lead to a comeback of IDD, especially in high-risk groups such as pregnant women, breastfeeding women, adolescents and children. Russia is a geographically iodine-deficient country either. Russia was assessed by Iodine Global Network (IGN) as the third most severely iodine-deficiency country in 2019, and the actual average iodine consumption of Russian residents was less than 1/3 of the minimum standard set by the World Health Organization (WHO). Compared with Russia, China has basically eliminated IDD due to implementation of iodized salt and application of real-time and effective monitoring systems. Therefore, this paper compares the monitoring and prevention strategies of IDD between China and Russia, in order to learn from each other and improve the citizens' awareness of prevention and control of IDD in China and Russia, and to achieve correct, scientific and precise iodine supplementation.

Objective:To investigate the clinical and pathological characteristics of acquired immunodeficiency syndrome (AIDS) patients with intestinal Talaromycosis marneffei (TM) infection. Methods:A total of 64 AIDS patients who underwent colonoscopy in Guangzhou Eighth People′s Hospital from January, 2010 to December, 2018 were retrospectively collected. Among them, 32 patients were co-infected with TM (AIDS with intestinal TM infection group) and 32 patients were not (AIDS without intestinal TM infection group) according to the colonic mucosa pathology. The clinical manifestations and pathological differences were compared between the two groups. Nonparametric rank sum test and Fisher exact probability method were used for statistical analysis. Results:The proportions of patients presented with fever, cough, retroperitoneal lymph nodes tume faction, nausea and vomiting, abdominal muscle tension, abdominal tenderness and rebound pain in AIDS with intestinal TM infection group were 28 (87.5%), 16 (50.0%), 13 (40.6%), 9 (28.1%), 8 (25.0%), 20 (62.5%) and 12 (37.5%), respectively, which were all significantly higher than those in AIDS without intestinal TM infection group 11 (34.4%), 6 (18.8%), 3 (9.4%), 2 (6.2%), 1 (3.1%), 8 (25.0%) and 1 (3.1%), respectively, the differences were statistically significant (Fisher exact test, all P<0.05). The median counts of peripheral blood CD4 + T lymphocyte, lymphocytes, monocytes, hemoglobin, platelet and albumin in AIDS with intestinal TM infection group were 13.5/μL, 0.30×10 9/L, 0.16×10 9/L, 88 g/L, 122×10 9/L and 23.5 g/L, respectively, which were all significantly lower than those in AIDS without intestinal TM infection group 207.0/ μL, 1.35×10 9/L, 0.35×10 9/L, 128 g/L, 201×10 9/L and 37.5 g/L, respectively, the differences were all statistically significant ( Z=-6.111, -6.191, -4.273, -5.353, -2.974 and-6.666, respectively, all P<0.05). Multivariate logistic regression analysis showed that CD4 + T lymphocytes <50/μL, hemoglobin <90 g/L and fecal occult blood positive were independent risk factors for AIDS with intestinal TM infection. The main manifestations of colonoscopy in AIDS with intestinal TM infection group were discontinuous ulcers (31.2%(10/32)), erosion (31.2%(10/32)) or co-exitance of ulcer and erosion (21.9%(7/32)), while suspected tumor-like eminence lesions were less common (15.6%(5/32)). The pathological features of colon mucosa were ulcer and/or erosion (53.1%(17/32)), chronic inflammation (46.9%(15/32)) and inflammatory granuloma (43.8%(14/32)). Oval or round spore with apparent septum could be seen by special staining. In AIDS with intestinal TM infection group, 27 patients were cured or improved, five patients died or deteriorated, while all patients in the AIDS without intestinal TM infection group improved after treatment without death. Conclusions:There are no specific gastrointestinal symptoms in AIDS patients with intestinal TM infection, while the patients present with decreased immunological cells and multiple colony pathological features. Specific fungal spores can be seen.

Chinese Journal of Nervous and Mental Diseases 2015;(8):487-491
Objective This preliminary study aimed to investigate relevant factors of the metacognition of peri?menopausal women. Methods Total 66 perimenopausal women voluntarily participated in this study from October 2012 to July 2013. The Metacognitions Questionnaire 30-item version (MCQ-30) was used to assess metacognition from 5 di?mensions including cognitive confidence (F1), positive beliefs (F2), cognitive self-consciousness (F3), uncontrollability and danger (F4), and need to control thoughts (F5). Eysenck Personality Questionnaire was utilized to measure the person?ality characteristics such as the extraversion/introversion (E), neuroticism/stability (N), psychoticism/socialization (P), and lie (L). Depression and its 4 symptom components including core, cognitive, anxiety, and somatic symptoms were deter?mined by Zung Self-rating Depression Scale (SDS). The linear multiple stepwise regression were performed to analyze the relevant factors of each MCQ dimension. Results The education level (β’=-0.229, P=0.035), N score (β’=0.255, P=0.042), and L score (β’=-0.292, P=0.021) were related to F1. The education level (β’=-0.260, P=0.031) and N score (β’=0.248, P=0.039) were predictors of the dependent variable F2. The core depression symptom (β’=-0.251, P=0.037) and anxiety symptom (β’=-0.248, P=0.039) of SDS were negatively related to F3. Predictors of F4 were the body mass in?dex (β’=0.211, P=0.048) and L score (β’=0.511, P<0.0001). Only P score (β’=0.299, P=0.015) was related to F5. Con?clusion The metacognition level of perimenopausal women is affected by a variety of factors such as personality character?istics and education level, and low self-consciousness and lack of confidence to the cognitive process may be involved in the increased susceptibility to depression.
