1.Characteristics of myocardial injury in patients with acute myocardial infarction complicated with pleural effusion and its influence on long-term prognosis

Objective To explore the characteristics of myocardial injury in patients with acute myocardial infarction(AMI)complicated by pleural effusion and its effect on long-term prognosis.Methods It was a prospective single-center study.Patients with AMI who were admitted to hospital within 15 days from symptom onset and performed echocardiography and cardiac magnetic resonance imaging(CMR)during hospitalization were consecutively enrolled and assigned to the with-pleural effusion group and the without-pleural effusion group according to the echocardiography result.Baseline data,cardiac magnetic resonance myocardial injury index and echocardiography characteristics were compared between the two groups.The occurrence of major adverse cardiovascular and cerebrovascular events(MACCE)was recorded through outpatient follow-up and telephone follow-up,including all-cause death,re-infarction,revascularization,rehospitalization for congestive heart failure and stroke.Cox regression analysis was performed to analyze influencing factors of all-cause death.Results Among 211 patients,31(14.7%)patients had pleural effusion and 180(85.3%)had no pleural effusion.Compared with the group without pleural effusion,the left ventricular end-diastolic diameter was larger,and left ventricular ejection fraction assessed by echocardiography was lower in the group with pleural effusion(P<0.05).There were no significant differences in infarct size,left ventricular end-diastolic volume,left ventricular end-systolic volume,left ventricular ejection fraction and the presence of microvascular obstruction and intramyocardial hemorrhage between the two groups in CMR(all P>0.05).At a median follow-up of 31 months,MACCE occurred in 43(20.4%)patients,and there was no significant difference between the two groups(χ2=3.160,P=0.075).Six cases(2.8%)had all-cause death.The incidence of all-cause death was higher in the group with pleural effusion than that in the group without pleural effusion(9.7%vs.1.7%,P<0.05).There was no significant difference in the incidence of other adverse events between the two groups(P>0.05).Multivariate Cox regression analysis showed that advanced age and presence of pleural effusion were independent risk factors of all-cause death during follow-up.Conclusion Patients with AMI combined with pleural effusion have more severe myocardial injury and higher all-cause mortality.
2.Analysis of the curative effect of combined treatment of renal calculi or upper ureteral calculi with double endoscopy

Objective To investigate the efficacy of negative pressure combined ureteroscopy combined with flexible ureteroscopy holmium laser lithotripsy in the treatment of renal calculi or upper ureteral calculi with a maximum diameter of 2-4cm.Methods The data of patients with renal calculi or upper ureteral calculi with a maximum diameter of 2-4cm who were hospitalized inAffiliated Hospital of Youjiang Medical University for Nationalities from March 2023 to March 2024 were collected and divided into observation group(n=40)and control group(n=40).The observation group was treated with negative pressure combined ureteroscopy combined with flexible ureteroscopy for holmium laser lithotripsy,and control group was treated with nephroscope for percutaneous nephrolithotomy(PCNL).The operation time,postoperative hospital stay,changes of blood routine white blood cell(WBC)before and after operation,changes of urine routine WBC before and after operation,postoperative visual analogue score(VAS),one-stage stone clearance rate and postoperative complications were compared between two groups.Results The postoperative hospitalization time and VAS of observation group were significantly lower than those of control group(P<0.05).There was no significant difference in operation time,stone clearance rate,postoperative blood routine WBC and postoperative urine routine WBC between two groups(P>0.05).However,the blood routine WBC of two groups after operation was higher than that before operation(P<0.05).The overall stone clearance rate of two surgical methods was 86.25%,of which the stone clearance rate of observation group was 82.5%,and control group was 90%.There was no significant difference between two groups(P>0.05).Conclusion It is safe and effective to treat renal calculi;or upper ureteral calculi with a maximum diameter of 2-4cm by negative pressure combined ureteroscopy combined with flexible ureteroscopy holmium laser lithotripsy.It is expected to partially replace PCNL and become another safe and reliable operation for the treatment of renal calculi or upper ureteral calculi with a maximum diameter of 2-4cm.
3.Correlation between the integrity of interdigitation zone defect and visual function before and after idiopathic macular epiretinal membrane surgery

AIM: To explore the correlation between the integrity of the macular microstructures and the visual outcomes before and after idiopathic macular epiretinal membrane(IMEM)surgery.METHODS: A total of 43 patients(43 eyes)with IMEM who were treated in our hospital between January 2017 and December 2019 were selected. The best corrected visual acuity(BCVA)of each patient was examined preoperatively and at 3, 6, 9mo after surgery. Moreover, metamorphopsia(M-chart)examination was given, and spectral domain-optical coherence tomography(SD-OCT)was used to measure central foveal thickness(CFT), subfoveal choroidal thickness(SFCT), the thickness of ganglion cells-inner plexiform layer(GC-IPL)and the length of interdigitation zone(IZ)defect.RESULTS: The BCVA and the metamorphopsia were gradually improved at postoperative 3, 6 and 9 mo(all P<0.05). Moreover, the BCVA was positively correlated with CFT and the length of the IZ defect(P<0.05), while it was not correlated with SFCT and the thickness of GC-IPL(P>0.05). The metamorphopsia assessment was positively correlated with the CFT at postoperative 3, 6, and 9mo(P<0.05), while it was not correlated with SFCT, the thickness of GC-IPL and the length of IZ defect(P>0.05).CONCLUSIONS: The CFT and the length of IZ defect were significantly correlated with the BCVA after IMEM surgery, which can be used as indicators to predict the recovery of visual function after IMEM surgery.
4.Combination pharmacotherapy for macular edema associated with retinal vein occlusion

Retinal vein occlusion(RVO)is the most prevalent retinal vascular disease except for diabetic retinopathy. Macular edema(ME)is a common complication of RVO which is the major causation of vision loss. The pathogenesis of ME is associated with vascular endothelial growth factor(VEGF)which induced by ischemia and hypoxia after venous obstruction and destruction of blood-retinal barrier caused by inflammation. Therefore, intravitreal injection of anti-VEGF medications or corticosteroids is the most common in the clinical treatment. Due to the two medicines have different react mechanism, duration of efficacy, side effects, and prices etc. There are several researches reported that the combination of two medications in order to treat ME secondary to RVO. This essay is a brief overview of the combination therapies' efficacy, the time of injections and relevant precautions during the therapy process.
5.Analysis of Monitoring Focus Drugs in an Obstetrics and Gynecology Hospital Based on ABC-VEN Matrix Analysis

OBJECTIVE:To investigate drug use in an obstetrics and gynecology hospital and confirm the types of drugs that need to be monitored so as to provide reference for rational drug use in clinic. METHODS: Activity based classification (ABC) analysis, Vital-Essential-Nonessential Medicine (VEN) analysis and ABC-VEN matrix analysis were used to statistically analyze the types of drugs in the inpatients and outpatients of this hosptial during Jan. 2016-Dec. 2017, and consumption sum in the hospital so as to determine the types of monitoring focus drugs. RESULTS: The drugs were divided into class A, B, and C by using ABC analysis, and the constitute ratio of them were 6.08%, 7.71% and 86.21%; the constitute ratio of consumption sum were 70.97%, 19.07% and 9.96%, respectively. The drugs were divided into class V, E and N, and the constitute ratio of them were 36.51%, 43.61% and 19.88%; constituent ratios of their consumption sum were 31.89%, 33.89% and 34.22%, respectively. The drugs were divided into group Ⅰ (class AV, AE, AN, BV, CV), group Ⅱ (class BE, CE, BN) and group Ⅲ (class CN) by using ABC-VEN matrix analysis; the constitute ratios of accumulative number of drug type were 40.56%, 44.43% and 15.01%,while those of accumulative consumption sum were 77.29%, 20.52% and 2.19%, respectively. Among class N, the constituent ratio of consumption sum of class AN as Chinese patent medicine, blood substitutes and perfusion solutions were higher, being 12.48% and 7.92%; that of class BN as Chinese patent medicine was higher, being 3.21%; those of class CN as Chinese patent medicine, sex hormones and modulators of the genital system were higher, being 1.14%, 0.50%. CONCLUSIONS: In the Obstetrics and Gynecology Hospital, consumption sum of class A is the main part of the total consumption sum of drugs, and they should be seleted according to therapeutic efficacy. Active regulatory policies should be adopted for class V and E so that more drug types that possess cost- effectiveness advantages; for class N, management control and reasonable utilization should be monitored closely to reduce irrational drug use. Some Chinese patent medicines, blood substitutes and perfusion solutions among class AN should be monitored and controlled emphatically.

[Conclusion]Taoist thought is one of the important thoughts inheritance for thousands of years in China, the quietness of the laissez-faire Taoism, the thought of returning to nature, the prevention treatment of disease thought in tonifying physical and spiritual, the thought of venerating yin and softness, al have guiding significance for TCM in health maintenance.
7.Analysis of allergens in 167 pateints with allergic contact dermatitis

Objective To assess the environmental contact allergens in patients with allergic contact dermatitis (ACD).Methods Totally,167 patients with ACD were included in this study.All the patients underwent patch test.Results Of these patients,92 (55.1%) were diagnosed as facial ACD,and 148 showed positive patch test results (88.6%).The six most common allergens in a decreasing order were nickel sulfate,fragrance mix,paraphenylenediamine,thimerosal,octanoates and amerchol L 101.Conclusion Patch test may be an efficient way to confirm the cause of ACD.
8.Long-term intermittent treatment of localized chronic eczema with triamcinolone acetonide and econazole nitrate cream

Objective To investigate the efficacy and safety of long-term intermittent treatment with topical corticosteroids in localized chronic eczema.Methods A total of 129 patients with localized chronic eczema were treated with triamcinolone acetonide and econazole nitrate cream (Pevisone (R)).Those who achieved a clinical cure within 4-week treatment were recruited into the following long-term study and classifled into 2 groups to be intermittently treated with triamcinolone acetonide and econazole nitrate cream or a moisturizing cream for 8 weeks.The cream was applied topically twice a day for 2 days every week.Patients were followed up on week 4 and 8 after the beginning of treatment and on week 12 after the discontinuation of treatment.The severity of eczema in patients was rated according to SCORAD (SCORing atopic dermatitis) score.Results The SCORAD score was significantly lower in patients treated with triamcinolone acetonide and econazole nitrate cream than in those with moisturizing cream on week 8 in the treatment and week 12 after the discontinuation (t=3.076,2.367,both P<0.05).A statistical decrease was also observed in the recurrence rate of eczema in patients treated with triamcinolone acetonide and econazole nitrate cream compared with moisturizing cream.treated patients at the 3 follow-up time points (x2=4.426,7.683,8.199,all P<0.05).The incidence of adverse events was 3.1% during the treatment with triamcinolone acetonide and econazole nitrate cream.No severe adverse reactions were observed.Conclusion Long-term intermittent treatment with topical corticosteroids is effective for the prevention of exacerbation and postponement of recurrence,of eczema.
9.A comparison between endoscopic retrograde and magnetic resonance cholangiopancreatography for the diagnosis of biliary obstruction diseases

Objective To evaluate the value of magnetic resonance cholangiopancreatography(MRCP) in the diagnosis of biliary obstruction diseases by comparison with endoscopic retrograde cholangiopancreatography (ER-CP). Methods A total of 36 patients with biliary obstruction diseases underwent both MRCP and ERCP. To review the images obtained from ERCP and MRCP ,the diagnosis were compared according to pathology. MRCP and ERCP in the diagnosis and treatment of biliary obstruction disease were analysed. Results 36 patients including 16 biliary tumors, 15 cholelithiases,and five other diseases were analysed. The diagnosis accuracy of MRCP and ERCP for bil-iary obstruction diseases were 86.1% and 91.7 %, there was no significant difference( P > 0.05). Conclusion MR-CP can accurately define the obstructively sites, identify the obstructive causes and is a noninvasive and reliable exam-ination for biliary obstruction diseases.
