1.Functional characterization of CYP81C16 involved in the tanshinone biosynthetic pathway in Salvia miltiorrhiza.

		                        			Danshen, the dried roots and rhizomes of Salvia miltiorrhiza Bunge (S. miltiorrhiza), is widely used in the treatment of cardiovascular and cerebrovascular diseases. Tanshinones, the bioactive compounds from Danshen, exhibit a wide spectrum of pharmacological properties, suggesting their potential for future therapeutic applications. Tanshinone biosynthesis is a complex process involving at least six P450 enzymes that have been identified and characterized, most of which belong to the CYP76 and CYP71 families. In this study, CYP81C16, a member of the CYP71 clan, was identified in S. miltiorrhiza. An in vitro assay revealed that it could catalyze the hydroxylation of four para-quinone-type tanshinones, namely neocryptotanshinone, deoxyneocryptotanshinone, and danshenxinkuns A and B. SmCYP81C16 emerged as a potential broad-spectrum oxidase targeting the C-18 position of para-quinone-type tanshinones with an impressive relative conversion rate exceeding 90%. Kinetic evaluations andin vivo assays underscored its highest affinity towards neocryptotanshinone among the tested substrates. The overexpression of SmCYP81C16 promoted the accumulation of (iso)tanshinone in hairy root lines. The characterization of SmCYP81C16 in this study accentuates its potential as a pivotal tool in the biotechnological production of tanshinones, either through microbial or plant metabolic engineering.

2.Construction of a prediction model for difficult tracheal intubation in patients with obstructive sleep apnea-hypopnea syndrome

		                        			Objective:To construct a prediction model for difficult tracheal intubation in the patients with obstructive sleep apnea-hypopnea syndrome (OSAHS).Methods:A total of 324 patients with OSAHS undergoing surgery with general anesthesia, admitted to our hospital from June 2019 to June 2021, were included in model group, and 175 patients with OSAHS undergoing surgery with general anesthesia, admitted from July 2021 to July 2022, were selected and served as validation group. The patients in model group were divided into occurrence group and non-occurrence group according to whether difficult tracheal intubation occurred. Logistic regression was used to construct the prediction model, and R4.2.1 software was used to draw the risk nomogram and calibration curve. The predictive accuracy of the model was evaluated by the area under the receiver operating characteristic curve.Results:Body mass index (BMI), sagittal minimum cross-sectional area, horizontal minimum cross-sectional area and mandibular distance were risk factors for difficult tracheal intubation in OSAHS patients ( P<0.05). A prediction model was developed using the above factors: Logit P=33.726+ 1.411×BMI score-0.014×sagittal airway minimum cross-sectional area-0.013×airway horizontal minimum cross-sectional area-0.312× mandibular distance. The area under the receiver operating characteristic curve was 0.846, Youden index 0.585, sensitivity 0.831, specificity 0.755, and the accuracy 0.889 (Hosmer-Lemeshow test χ2=9.24, P=0.322) in model group. The area under the external validation curve was 0.802, Youden index 0.545, sensitivity 0.636, specificity 0.908, and the accuracy 0.893 (Hosmer-Lemeshow test χ2=10.24, P=0.287) in validation group. Conclusions:The prediction model based on BMI, sagittal minimum cross-sectional area of airway, horizontal minimum cross-sectional area of airway and mandibular distance has a high value in predicting the risk of difficult tracheal intubation in patients with OSAHS.
		                        		
		                        		
		                        		
		                        	
3.Intrathecal administration of Maresin 1 attenuates bone cancer pain by inhibiting the glial cells acti-vation and neuroinflammation in the spinal cord

		                        			Objective To investigate the effects of intrathecal administration of Maresin 1 on pain behavior and glial cells activation in spinal dorsal horn of mice with bone cancer pain(BCP).Methods Experiment Ⅰ,twenty-eight clean male C57BL/6 mice,aged 4-6 weeks,weighing 18-22 g,were ran-domly divided into two groups:sham 1 group(group S1)and BCP 1 group(group B1),14 mice in each group.BCP model was established in group B1 by injecting PBS 20 μl containing 2 × 105 lewis lung carcino-ma cells into the intramedullary space.Group S,received PBS 20 μl without tumor cells.Six mice in each group were randomly selected to determine the mechanical withdrawal threshold(MWT)and thermal with-drawal latency(TWL)1 day before implantation,3,7,14,21 days after implantation.Four mice were randomly sacrificed on days 7,14 and 21 in group S,and group B1.The expressions of spinal glial fibrillary acidic protein(GFAP)and ionized calcium-binding adapter molecule 1(Iba1)were detected by Western blot.Experiment Ⅱ,thirty-six clean male C57BL/6 mice were randomly divided into three groups:sham 2 group(group S2),BCP 2 group(group B2),and Maresin 1 group(group M),12 mice in each group.BCP models were established in groups B2 and M while group S2 received PBS 20 μl without tumor cells.Maresin 1 50 ng/5 μl were intrathecally injected from days 14 to 16 after tumor cells implantation.Six mice in each group were randomly selected to determine the MWT and TWL on days 14 to 18 after implantation.Mice were sacrificed on day 18 after pain behavior tests.The expressions of spinal GFAP and Iba1 were de-tected by Western blot and immunofluorescence staining.The levels of spinal IL-1 β,IL-6,and TNF-α were detected by ELISA.Results Experiment Ⅰ:compared with group S1,the MWT and TWL were significantly lower on days 7,10,14,and 21 after implantation in group B,(P<0.05),the expressions of spinal GFAP and Iba1 were upregulated on days 7,14,and 21 after implantation in group B1(P<0.05).Experiment Ⅱ:compared with group S2,the levels of spinal GFAP,Iba1,IL-1β,IL-6,and TNF-α were increased on day 18 after implantation in group B2(P<0.05).Compared with group B2,the MWT and TWL were significantly higher on days 15-18 after implantation while levels of spinal GFAP,Iba1,IL-1β,IL-6,and TNF-α on day 18 were decreased in group M(P<0.05).Conclusion Intrathecal administra-tion of Maresin 1 attenuates BCP maybe by inhibiting the glial cells activation and neuroinflammation in the spinal cord.
		                        		
		                        		
		                        		
		                        	
4.Detection rate and influencing factors of suspected scoliosis among primary and middle school students in Dali Bai Autonomous Prefecture

		                        			Objective:To investigate the epidemiological characteristics and influencing factors of scoliosis among primary and middle school students in Dali Bai autonomous prefecture.Methods:A total of 176,729 students aged 6-18 years from 380 primary and secondary schools in eight counties of Dali Bai Autonomous Prefecture were screened for spinal health by visual method, Adams forward bend test and scoliometer measuring the angle of trunk rotation from September to November 2021. The angle of trunk rotation ≥5° or visual method retest positive was used as a positive standard to confirm suspected scoliosis population. Demographic data of the screening population including name, gender, age and ethnicity were collected. The average altitude of the community or village committee in the screening area was recorded. Binary logistic regression analysis was used to screen the influencing factors of scoliosis.Results:There were 176,729 people planned to complete the screening. A total of 140,026 people were actually completed screening with completion rate 79.23%. A total of 3,190 (2.28%) suspected scoliosis positive people were detected. The detection rate of female was 2.52%, which was higher than that of male 2.03% (χ 2=37.18, P<0.001). The detection rate of 10-18 years old group was 2.90%, which was higher than that of 6-9 years old group 0.70% (χ 2=620.79, P<0.001). There was a significant difference in the detection rate among different age groups from 6 to 18 years old (χ 2=1,451.00, P<0.001). The detection rate of Han population was 2.67%, which was higher than that of non-Han population 2.15% (χ 2=31.06, P<0.001). In non-Han population, the detection rate of Bai population was 2.07%, which was lower than that of non-Bai population 2.26% (χ 2=4.02, P=0.045). Trend chi-square test showed significant difference in the detection rate of scoliosis among the general population, 10-18 years old population and 6-9 years old population in the altitude <1,500 m, 1,500-1,599 m, 1,600-1,699 m, 1,700-1,799 m, 1,800-1,899 m, 1,900-1,999 m, 2,000-2,099 m, 2,100-2,199 m, 2,200-2,299 m and ≥2,300 m groups (χ 2=249.02, 195.64, 24.46, P<0.05). Binary logistic regression analysis showed that the risk of scoliosis was lower in males than that in females ( OR=0.82, P<0.001). The risk of scoliosis was increased in 10-18 years old compared with that in 6-9 years old ( OR=0.12, P<0.001). The risk of scoliosis was low in Bai population compared with that in Han populations ( OR=0.75, P<0.001). The altitude ≥2,000 m was a risk factor ( OR=1.52, P<0.001). Conclusion:The detection rate of suspected scoliosis in primary and middle school students in Dali Bai Autonomous Prefecture was 2.28%. 5.Effect of "Internet +" blood glucose management model in patients with gestational diabetes mellitus

		                        			Objective:To explore the effect of "Internet +" blood glucose management model in patients with gestational diabetes mellitus (GDM) .Methods:From January 2020 to March 2021, convenience sampling was used to select 95 GDM patients who were treated in the Outpatient Department of Lu'an People's Hospital of Anhui Province as the research object. According to the random number table method, the patients were divided into the control group ( n=47) and the intervention group ( n=48) . The control group received routine nursing, and the intervention group was given the "Internet +" blood glucose management model on the basis of routine nursing. The scores of the Diabetes Management Self-Efficacy Scale (DMSES) , GDM Self-management Behavior Scale and blood glucose control, including fasting blood glucose, 2-hour postprandial blood glucose (2hPG) , and glycosylated hemoglobin (HbA1c) , were compared before and after management, and the pregnancy outcomes were calculated. Results:After management, the DMSES and GDM self-management behavior scores of the intervention group were higher than those of the control group, and the 2 hPG, fasting blood glucose and HbA1c were lower than those of the control group, and the differences were statistically significant ( P<0.05) . The incidences of cesarean section, abnormal amniotic fluid volume and neonatal hypoglycemia in the intervention group were lower than those in the control group, and the differences were statistically significant ( P<0.05) . The implementation of high-quality perinatal palliative care (PPC) can help pregnant women and families cope with the pain caused by trauma and maximize the quality of life and comfort of the fetus. Perinatal palliative care knowledge, skills, and attitudes of health care workers are closely related to the quality of palliative care services and maternal and infant outcomes. 8.Clinical Observation of Bosentan in the Treatment of Hypoxic Pulmonary Hypertension in Neonates

		                        			OBJECTIVE:To observe the ef fectiveness and safety of bosentan in the treatment of hypoxic pulmonary hypertension(HPH)in neonates. METHODS :From Jan. 2014 to Mar. 2019,a total of 82 HPH neonates hospitalized in the department of neonatology of our hospital were selected as research subjects. According to whether or not receiving bosentan therapy,50 cases were included into bosentan group and 32 cases into non-bosentan group. Meanwhile ,another 25 non-HPH neonates with serum sample retention time and general information such as gestational age at birth and day age matching the HPH group were selected as the control group. All neonates with HPH were given continuous intravenous infusion of Dopamine hydrochloride injection 5 mg/(kg·min)until PASP was normal. On this basis, neonates in the bosentan group were additionally given Bosentan tablets 1 mg/kg(fed after dissolving with appropriate amount of water for injection )for q 12 h,72 h. The relationship between serum ET- 1 levels of neonates with HPH and PASP was analyzed ,as well as PASP before and after treatment and therapeutic efficacy between bosentan and non-bosentan groups ,the changes of arterial blood gas indexes and ADR in 3 groups were compared. RESULTS :Before treatment ,the serum ET- 1 levels of bosentan group was (164.3±115.3)pg/mL,which was significantly higher than (41.9±3.7)pg/mL of control group and positively correlated with PASP level (r=0.864,P<0.001). Total response rate of bosentan group was 90.00%,which was significantly higher than 71.88% of non-bosentan group (P<0.05). After 72 h of treatment ,PASP of 2 groups was decreased significantly ,compared with before treatment (P<0.001),and the bosentan group was significantly lower than the non-bosentan group (P<0.05). The PaO 2,SaO2,PaCO2 and OI in 3 groups was significantly improved compared with that before treatment (P<0.001),and the PaO 2,SaO2 and OI in the bosentan group was significantly higher than that in the non-bosentan group (P<0.05). During the treatment period of bosentan and within one week after drug withdrawal,there was no significant change in serum LDH ,AST,ALT and Scr levels in neonates. There was no statistically significant difference in the incidence of feeding intolerance ,anemia,reduced WBC and reduced PLT in 3 groups(P>0.05). 9.Application of element-tagged immunoassay combined with inductively coupled plasma mass spectrometry in HCG detection

		                        			Objective:To establish an inductively coupled plasma mass spectrometry (ICP-MS) based immunoassay method for the quantitative detection of human chorionic gonadotropin (HCG), and evaluate the clinical applicability of this method.Methods:Sm was selected as element tags, and the HCG quantitative detection system was established by double antibody sandwich method. The dosage of biotinylated antibody and reaction time were optimized. According to EP documents of Clinical and Laboratory Standards Institute (CLSI) and related standards, the analytical performance was evaluated after the establishment of the assay, including the limit of blank (LOB), linearity, precision, recovery, cross reactivity and interference test. And 88 clinical samples were measured using the new method compared to the electrochemical immunoassay (ECLIA) method.Results:Total process completed within 30 min after optimization, and the optimal biotinylated antibody dosage was 0.5 μl. The LOB was 0.29 mIU/ml. The linearity was good within the range of 1.16-8 365.62 mIU/ml with the linear correlation coefficient greater than 0.995 ( R2=0.998 0), the recovery was 97.53%-102.01%. Both intra-and inter-assay coefficients of variation of the high-value sample and the low-value sample were less than 10%. And there was no significant cross-reaction with Luteinizing hormone (LH), follicular stimulating hormone (FSH), and thyroid-stimulating hormone (TSH). The interference bias caused by different concentrations of interference substances was less than 10%. When compared with the ECLIA method for clinical sample detection, the proposed method showed a significant correlation( R2=0.960 0). Conclusion:The proposed ICP-MS base immunoassay for HCG detection has good accuracy, high sensitivity and specificity, and the results of analytical performance verification meet the clinical requirements, which provides experimental basis for the clinical application of this method.
		                        		
		                        		
		                        		
		                        	
10.Research on the construction of humanistic care training module for new nurses based on the concept of "craftsman spirits"

		                        			Objective? To construct new nurses' humanistic care training modules based on the concept of "craftsman spirits", so as to provide the basis of training for nursing managers to improve nurses' humanistic care behavior. Methods Underpinned by the "craftsman need-to-know 30 items" in "craftsman spirits", we reviewed the literature about nurses' humanistic care training status and patients' humanistic needs, and selected 20 typical clinical nursing managers and educators from 3 cities in Jiangsu Province and conducted 2 rounds of consultation by Delphi expert consultation method; we used analytic hierarchy process (AHP) for indicators at various levels to determine the contents and weights of humanistic care training modules for new nurses. Results? After two rounds of consultation, the recovery rate was 100%, the expert authority coefficient (Cr) was 0.92 and 0.87 respectively, and the coordination coefficient(W) was 0.32 and 0.23. The training module of humanistic care for new nurses was finally formulated, including 6 primary indicators, 15 secondary indicators and 30 tertiary indicators. Conclusions The construction method of the new humanistic care nurse training module based on the concept of "craftsman spirits" is scientific and vigorous, the contents are specified and easy to apply, with an emphasis on nurses' internal literacy and understanding of the development of the discipline.
