Objective:To introduce the progress in research of rash and fever syndrome (RFS) surveillance and early warning both at home and abroad, and provide reference for surveillance and prevention of RFS in China. Methods:The keywords "fever" "rash" and "surveillance" and others were used for a literature retrieval by using China National Knowledge Infrastructure, Wanfang Data Knowledge Service Platform, PubMed and Web of Science. The languages of literatures were limited in Chinese and English. The key information of the literatures were collected and analyzed with Excel. Results:A total of 36 study papers (21 in Chinese and 15 in English) were included. The studies mainly focused on the pathogen surveillance of RFS ( n=19). The pathogens included measles virus, varicella-zoster virus, rubella virus, enterovirus, human B19 virus, dengue virus, streptococcus group A, Salmonella typhi and Salmonella paratyphoid, human herpesvirus, mumps virus and adenovirus. Eight studies were about the surveillance in major events, such as sport game, World Expo and religious gathering, or sudden natural disasters, such as earthquake and tropical storm, during 2010-2015. Eight studies focused on case or epidemic surveillance, most of which were studies from other counties. The surveillance sites were medical institutions. RFS was diagnosed according to the International Classification of Diseases, 9 th (ICD-9) and symptoms descripted in chief-complaint. Only one study in Mongolia conducted RFS epidemic prediction. The analysis methods of 36 papers included simple descriptive analysis, time-based early warning models (such as regression analysis, fixed threshold method, Hugh Hart control chart method and cumulative sum control chart method) and time series analysis method. Conclusions:In the future, RFS surveillance system should cover both known pathogens and emerging pathogens. Automatic surveillance using information capture and intelligent modelling can be applied to improve the sensitivity and specificity of RFS surveillance and early warning.

Chinese Journal of Epidemiology 2024;45(6):892-898
Medical and preventive integration effectively bridges the gap between "treating diseases" and "preventing diseases". Over the years, medical and preventive integration research has focused on chronic and chronic infectious diseases, with insufficient attention to acute ones. Confronting newly emerging infectious diseases establishing continuous monitoring, early warning, emergency response, and appropriate treatment will be a key focus for developing and reforming the healthcare system. Interoperability and sharing of medical and health data are essential prerequisites for bridging the gap between medical treatment and disease prevention and are also important for promoting intelligent surveillance and early warning of infectious diseases. Informatization is necessary to achieve efficient collaboration between medical treatment and disease prevention. Reviewing the development of medical and health informatization in the United States and Europe, this paper compares and discusses the problems and challenges in developing medical and health informatization in China. The aim is to provide references for the development of medical and health informatization and the innovation of medical and preventive integration mechanisms in the country.

Objective To investigate the role of histone H4 in the polarization of alveolar macrophages (AM) in lipopolysaccharide (LPS)-induced acute respiratory distress syndrome (ARDS) in mice. Methods i) The specific pathogen free male C57BL/6 mice were randomly divided into control group and 2, 4, 6 and 8 mg/kg LPS groups, with six mice in each group. The mice in the LPS groups were intratracheally administered LPS according to their respective doses, while the mice in the control group received an equivalent volume of 0.9% saline. After 12 hours, the arterial blood gas was analyzed, and the pulmonary edema and histopathological changes in lung tissues of mice in each group were observed. The level of histone H4 in bronchoalveolar lavage fluid (BALF) of mice was detected using enzyme-linked immunosorbent assay , and mice AMs of the five group were isolated using adherent method. ii) AMs from normal mice were isolated using adherent method and randomly divided into control group, histone H4 injury group, BALF injury group and anti-histone H4 antibody (anti-H4) intervention group. In the histone H4 injury group, AMs were treated with histone H4 at a final concentration of 20 mg/L. In the BALF injury group and anti-H4 intervention group, AMs were treated with 200 μL BALF supernatant from mice intratracheally administered 6 mg/kg body weight LPS, with the latter group treated with 25 mg/L anti-H4 antibody. The control group AMs were treated with phosphate-buffered saline. iii) After 12 hours of stimulation, the cells were collected, and the relative expression of tumor necrosis factor-α (Tnfa), interleukin-1β (Il1b), differentiation antigen 206 (Cd206) and arginase 1 (Arg1) in AMs was detected using real-time quantitative polymerase chain reaction. Results i) Compared with the control group, mice in all four LPS groups exhibited rapid breathing, inflammatory reaction and lung edema in lung tissues, which were aggravated in a dose-dependent manner. The ratio of partial pressure of arterial oxygen to fraction of inspired oxygen in mice decreased with the increase of LPS dose (P<0.05). The wet/dry weight ratio of lung, the level of histone H4 in BALF and the relative expression of Tnfa and Il1b mRNA in AMs increased with the increase of LPS dose (all P<0.05). The mice in the 6 and 8 mg/kg LPS groups developed ARDS. The level of histone H4 in BALF and the relative expression of Tnfa and Il1b mRNA in AMs of mice in 6 and 8 mg/kg LPS groups were higher than those in the other three groups (all P<0.05). ii) The relative expression of Tnfa and Il1b mRNA increased (both P<0.05), and the relative expression of Cd206 and Arg1 mRNA decreased (both P<0.05) in AMs of histone H4 injury group and BALF injury group compared with the control group. Compared with BALF injury group, the relative mRNA expression of Tnfa and Il1b in AMs of anti-H4 intervention group decreased (both P<0.05), while the relative expression of Arg1 mRNA increased (P<0.05). Conclusion LPS can induce a dose-dependent increase in histone H4 levels in BALF in mice. Histone H4 drives the development of ARDS by activating AMs to M1 polarization. Antagonizing histone H4 to interfere with AM polarization to M1 could be a target for the treatment of ARDS.

ObjectiveTo investigate the regulatory effect of Mankuining Formula (MKNF) on the gut microbiota and the NOD-like receptor (NLR)P3/Caspase-1/gasdermin D (GSDMD) pyroptosis pathway-mediated inflammation in dextran sulfate sodium (DSS)-induced ulcerative colitis (UC) mice. MethodSixty SPF C57BL/6 mice were randomly divided into a blank group, a model group, a MKNF group (20 g·kg-1), and a mesalazine group (0.266 g·kg-1), with 15 mice in each group. The UC model was induced in mice by freely drinking a 3% DSS solution for 7 days. After 12 hours of modeling, the treatment groups received daily oral administration, while the other groups received an equal volume of normal saline by gavage. Daily body weight and disease activity index (DAI) were recorded. On the 8th day, mice were euthanized after anesthesia, and the colon and feces were collected. The colon length was measured, and histopathological changes were observed after hematoxylin-eosin (HE) staining. Tumor necrosis factor-α (TNF-α), interleukin-1β (IL-1β), and interleukin-18 (IL-18) levels in the colon were detected by enzyme-linked immunosorbent assay (ELISA). The differences in gut microbiota among the groups were analyzed using 16S rRNA sequencing technology. The protein content of NLRP3/Caspase-1/GSDMD in colon tissues was detected by Western blot. ResultCompared with the blank group, mice in the model group showed increased DAI (P<0.01), shortened colon length (P<0.01), severe colon mucosal damage, elevated levels of TNF-α, IL-1β, and IL-18 (P<0.01), increased protein content of NLRP3/Caspase-1/GSDMD in colon tissues (P<0.01), altered gut microbiota structure with decreased abundance of Actinobacteria, Bacteroidetes, and Proteobacteria, and increased abundance of Firmicutes at the phylum level. At the genus level, there was a decrease in Lactobacillus, Alloprevotella, and Yersinia, and an increase in Bacteroides, Bacillus, and Lachnospiraceae_NK4A136. Compared with the model group, the MKNF group and the mesalazine group showed a significant reduction in DAI after the 3rd day (P<0.01), a significant increase in colon length (P<0.01), alleviated colon inflammation and mucosal structural damage, and decreased TNF-α, IL-1β, and IL-18 levels in the colon (P<0.01), reduced protein content of NLRP3/caspase-1/GSDMD in colon tissue (P<0.05, P<0.01),an increase in the abundance of Proteobacteria and Bacteroidetes, and a decrease in Firmicutes at the phylum level. ConclusionMKNF can alleviate UC-induced colonic inflammation, reduce colon damage, and improve dysbiosis of the gut microbiota by inhibiting the classical pyroptosis pathway.

Objective:
To investigate eating behavior and associated family factors of preschool children during the prevalence of Corona Virus Disease(COVID-19), and to provide scientific basis for healthy eating behavior for preschool children.
Methods:
A total of 1 829 preschool children were investigated by electronic questionnaire from 5 kindergartens in Shenzhen from March 18-20, 2020. SPSS 19.0 was used to perform logistic regression analysis on the factors for eating behavior.
Results:
The rate of unhealthy eating behavior during the prevalence of COVID-19 was 47.2%. The rate of unhealthy eating behaviors in boys and girls were 46.1% and 48.4%, respectively. And there was no significant difference between different genders(χ 2=1.02,P=0.31). The rate of unhealthy eating behaviors of preschool children aged 3-, 4-, 5- and 6- were 56.6%, 56.8%, 42.3% and 29.9%, respectively, and the differences between different ages were significant(χ 2=72.17,P<0.01). Logistic regression analysis showed that fathers high education level, mothers education level(OR=0.78,0.77,0.21), and parents satisfaction with children s eating were associated with low nutritional problems of child, while parental permission to play while eating, criticize while eating, force child to eat more and force child to eat foods they do not like, and parental concerns about children s appetite, were associated with more unhealthy eating behavior(OR=6.29,1.80,2.20,1.70,2.16)(P<0.05).
Conclusion
The prevalence of unhealthy eating behaviors of preschool children during the prevalence of COVID-19 is lower than usual, but the health education of preschool children s families still needs to be strengthened.

Objective:To investigate the value of α-synuclein (α-syn) concentration in cerebrospinal fluid (CSF) in predicting postoperative delirium (POD).Methods:One thousand patients underwent elective surgery with combined epidural-spinal anesthesia in our hospital from January 2018 to September 2020 were selected. The epidural puncture was performed at L 3, 4 interspace, and 2 ml of CSF was collected after the needle reaching the subarachnoid space. The concentrations of α-syn, β-amyloid (Aβ)40, Aβ42, total tau protein (T-tau), and phosphorylated tau protein (P-tau) in CSF were determined by enzyme-linked immunosorbent assay. The concentrations of α-syn in CSF and occurrence of POD in patients of different ages were recorded.Patients were divided into POD group and non-POD group according to whether POD occurred, and frequency matching (1∶1) was performed based on five matching variables of age, ASA physical status, education level, duration of operation, and intraoperative blood loss. Results:Eight hundred and forty-one patients were finally included in the study, and the incidence of POD was 15.0%. There were 126 cases in POD group and 126 cases in non-POD group after matching. The concentrations of α-syn in CSF and incidence of POD were gradually increased with age ( P<0.05). Compared with non-POD group, the concentrations of α-syn, T-tau and P-tau in CSF were significantly increased, the concentrations of Aβ40 and Aβ42 were decreased, Aβ40/P-tau, Aβ42/P-tau, Aβ42/Aβ40 and P-tau/T-tau were decreased in POD group ( P<0.05). After confounding factors were corrected by logistic regression analysis, increased concentrations of α-syn, p-tau, and T-tau in CSF were risk factors for POD ( P<0.05). Increased concentrations of Aβ40 and Aβ42 in CSF and increased Aβ40/P-tau and Aβ42/P-tau were protective factors for POD ( P<0.05). Multiple linear regression analysis showed that the concentration of α-syn in CSF was negatively correlated with Aβ40 and Aβ42 concentrations and positively correlated with P-tau and T-tau concentrations ( P<0.05). The area under the receiver operating characteristic curve of concentrations of α-syn in CSF predicting POD was 0.895, Youden index was 0.664, sensitivity was 80.00%, and specificity was 86.36% ( P<0.001). Conclusion:The concentration of α-syn in CSF is related to the occurrence of POD, and it provides higher accuracy in predicting POD.
