1.Exploration and practice of the course of Geriatric Nursing: taking Peking Union Medical College as an example

The Peking Union Medical College School of Nursing has been dedicated to exploring the education of undergraduate students specializing in geriatric nursing since 2016. Through seven years of exploration and practice, aiming at the pain points and difficulties in the process of cultivating geriatric nursing students, the teaching team has developed a progressive and modular curriculum system, with the goal of training geriatric nursing professionals with specialized knowledge and practical innovation abilities, on the platform of a seamless teaching practice base connecting settings from laboratory to community/elderly care institutions/hospital to society, with an emphasis on the integration of ideological and political education with geriatric nursing education. Moreover, a specialized faculty team has been established. This course serves as a model for nurturing excellent nursing professionals possessing the right values and outlook on life as well as ability to cope with the national challenges posed by aging populations.
2.Study on geriatric nursing competency of undergraduate nursing interns of four nursing schools in Beijing

Objective:To investigate the geriatric nursing competency of undergraduate nursing interns in four schools of nursing in Beijing as well as the influencing factors, and to provide a reference for perfecting the geriatric nursing education system in colleges and universities.Methods:From October to November 2022, a cross-sectional survey was performed on 223 undergraduate nursing interns selected by convenience sampling from four schools of nursing in Beijing, with the use of a general information questionnaire and the Gerontological Nursing Competence instrument. A general linear regression model was established to explore the factors influencing the geriatric nursing competency of nursing interns.Results:The mean total score for gerontological nursing competency was (3.19±0.40). In all dimensions, the mean score was highest for "Providing a safe living environment for older persons" (3.40±0.47) and lowest for "Understanding the sexual needs of older persons" (2.78±0.73). The thought about whether gerontological nursing contents were enough and the level of motivation to study gerontological nursing courses were the factors affecting nursing interns' gerontological nursing competency ( P<0.05). Conclusions:The overall gerontological nursing competency of the nursing interns was at a medium level, with uneven competencies in different dimensions. Nursing schools should strengthen gerontological nursing course building and stimulate students' learning motivation to improve their gerontological nursing competency.
3.Influencing factors for nicotine dependence among smoking patients with chronic obstructive pulmonary disease

Objective:
To investigate the nicotine dependence of patients with chronic obstructive pulmonary disease (COPD) who smoke and analyze its influencing factors, so as to provide insights into targeted smoking cessation interventions for smoking COPD patients.
Methods:
The smoking COPD patients were selected from three hospitals in Lanzhou City using the convenience sampling method from October 2022 to April 2023. Demographic data, smoking status, and willingness to quit smoking were collected using questionnaire surveys. Nicotine dependence was assessed according to Fagerstrom Test for Nicotine Dependence. The influencing factors of nicotine dependence in smoking COPD patients were analyzed using a multiple linear regression model.
Results:
A total of 200 questionnaires were distributed, and 171 valid questionnaires were collected, with a response rate of 85.50%. The median age was 48.00 (interquartile range, 10.00) years. There were 158 males, accounting for 92.40%; 53 cases with a college degree or above, accounting for 30.99%. The median smoking duration was 15.00 (interquartile range, 11.00) years. There were 73 patients (42.69%) smoking 21 to 30 cigarettes per day, and 159 patients (92.98%) with the willingness to quit smoking. The median nicotine dependence score was 3.00 (interquartile range, 1.00) points. There were statistically significant differences in nicotine dependence scores among smoking COPD patients with different ages, educational levels, occupations, interpersonal relationships, smoking duration, daily cigarette consumption, awareness of smoking hazards and willingness to quit smoking (all P<0.05). Multiple linear regression analysis revealed that nicotine dependence was related with educational level (β=-0.251), interpersonal relationship (β=-0.246), smoking duration (β=0.038), daily cigarette consumption (β=0.518) and willingness to quit smoking (β=-0.173) among smoking COPD patients (all P<0.05).
Conclusion
The nicotine dependence among smoking COPD patients was influenced by educational level, interpersonal relationships, smoking duration, daily cigarette consumption and willingness to quit smoking.

177Lu-prostate specific membrane antigen(PSMA)radio-ligand therapy has been approved abroad for advanced prostate cancer and has been in several clinical trials in China.Based on domestic clinical practice and experimental data and referred to international experience and viewpoints,the expert group forms a consensus on the clinical application of 177Lu-PSMA radio-ligand therapy in prostate cancer to guide clinical practice.
5.Analysis of the Impact of Different Low-density Lipoprotein Cholesterol Levels on the Progression of Intermediate Coronary Stenosis Based on Quantitative Flow Ratio Quantification

Objectives:This study aims to investigate the impact of different Low-Density Lipoprotein cholesterol(LDL-C)levels on progression of intermediate coronary stenosis,and the associated risk factors leading to the progression of such lesions. Methods:Data were collected on 219 consecutive patients admitted at the Fuwai Central China Vascular Hospital from January 2020 to February 2021,underwent angiographic examinations and diagnosed with intermediate coronary stenosis,with at least one follow-up angiography after 11 months.Offline quantitative flow ratio(QFR)analysis was performed on these cases.Patients were divided into two groups:LDL-C controlled group(LDL-C<1.8 mmol/L,148 patients with 191 vessels)and LDL-C uncontrolled group(LDL-C≥1.8 mmol/L,71 patients with 98 vessels).Coronary artery QFR and anatomical indicators such as minimal lumen diameter,minimal lumen area,percentage diameter stenosis,percentage area stenosis were compared within and between the groups.Further analysis was performed to identify influencing factors leading to changes in coronary physiological parameters derived from QFR. Results:Within the LDL-C controlled group,there was no significant difference in the QFR values of the vessels compared to baseline(P>0.05),whereas in the LDL-C uncontrolled group(P<0.05),a notable decline in QFR was observed.Patients in the LDL-C controlled group had lower rates of maximum diameter and area stenosis and higher minimum lumen diameter and area(all P<0.05).Through multifactorial Logistic regression analysis,it was found that a body mass index>28 kg/m2,LDL-C≥1.8 mmol/L,and a history of myocardial infarction were independent risk factors leading to the decline in QFR(all P<0.05). Conclusions:It was found that patients in the LDL-C controlled group had higher coronary artery QFR,minimum lumen diameter and area,lower rates of maximum diameter and area stenosis.
6.Research on the training needs of advanced practice nurses in 1 287 medical institutions

Objective To investigate the training needs and suggestions of advanced practice nurses in medical institutions in China,and to provide references for the formulation of training programs for advanced practice nurses.Methods By a convenient sampling method,from May to July in 2023,the heads of nursing nurses in medical institutions in 29 provinces,autonomous regions and municipalities directly under the Central Government were investigated,and the self-designed questionnaires were distributed in the form of electronic questionnaire.Results 1 287 medical institutions participated in the survey,with tertiary hospitals accounting for 65.50%.93.16%of medical institutions had the use needs of advanced practice nurses,and the top 3 professional fields hoping to prioritize in the APN training were in the order of critical illness,emergency treatment and intravenous treatment.Among the APN core competence,the most important one was direct clinical practice(92.77%),followed by professional development(89.43%)and scientific research and evidence-based practice(86.40%).71.72%of medical institutions suggested that the theoretical training time is 4 weeks,combining online and offline training(50.35%),and the clinical practice time should not be less than 12 weeks(97.66%).Theoretical learning course assignment(92.93%),case report(90.83%)and clinical practice reflection diary(88.19%)were the main process evaluation methods,while 1 case study with the completion of the report and defense(87.65%)and 1 evidence-based nursing practice research plan with the completion of a report and defense(77.93%)were the main final evaluation methods.Conclusion Advanced practice nurses are important roles in the process of nursing professional development,and medical institutions in our country have a high demand for the training of advanced practice nurses.It is imperative to carry out the training of advanced practice nurses.The suggestions of medical institutions on the training content and methods focus more on the training of core competence.On this basis,the admission conditions should be clarified,and the training program of advanced practice nurses for China's national conditions should be explored.
7.Research Progress of Spinal Biomechanics in 2023

Spine is one of the most important skeletal structures in human body.It has the function of protecting the spinal cord,supporting body weight,slowing impact and allowing flexible movement of the trunk.The study of spinal biomechanics is very important for a comprehensive understanding of the structure and function of the spine and the pathogenesis of spinal diseases.In 2023,scholars at home and abroad have done a lot of researches on spine related biomechanics,including the cognition on basic biomechanics of the spine,the changes in mechanical properties of the spine under pathological conditions,and the design of various treatment methods of spinal diseases based on biomechanical researches.This review focuses on the research progress of spinal biomechanics in 2023,and several typical spinal diseases or pathological states are introduced in detail as examples.
8.Meta-analysis of the effect of singing therapy on patients with chronic obstructive pulmonary disease

Objective:To systematically evaluate the effect of singing therapy on patients with chronic obstructive pulmonary disease.Methods:Randomized controlled trials on the effect of singing therapy in patients with chronic obstructive pulmonary disease were searched by computer from PubMed, Web of Science, Cochrane Library, CINAHL, Embase, China National Knowledge Infrastructure, Wanfang Data, China Biology Medicine disc, and VIP. The search period was from the establishment of databases to June 30, 2023. Two researchers independently screened literature, performed a literature quality assessment, and extracted data. Meta-analysis was performed using RevMan 5.4 software.Results:A total of 11 articles were included. Meta-analysis showed that singing therapy for 10 weeks or more could improved anxiety ( MD=-5.01, 95% CI: -5.76~-4.25, P<0.001), and depression ( MD=-4.73, 95% CI: -8.82~-0.64, P=0.020). Singing therapy with a duration of less than 60 min per session and/or five interventions per week or more could improve FEV 1% predicted values ( MD=9.46, 95% CI: 7.66~11.26, P<0.001) in patients with chronic obstructive pulmonary disease. Conclusions:Singing therapy is best achieved at least five times a week, with each session lasting less than 60 minutes and lasting for at least ten weeks. However, more high-quality, multicenter and large sample studies are still needed for further validation.
9.Value of cardiac MRI in evaluation of left atrial function in the postoperative tetralogy of Fallot and pulmonary stenosis patients

Objective:To explore the value of cardiac MRI (CMRI) in evaluating left atrial function in patients with postoperative tetralogy of Fallot (rTOF) and postoperative pulmonary stenosis (rPS).Methods:Totally 67 pediatric patients (49 with rTOF, 18 with rPS) with preserved left ventricular ejection fraction (EF) were recruited between January 2019 and October 2021 in Shanghai Children′s Medical Center, School of Medicine, Shanghai Jiao Tong University. Thirty-three healthy volunteers, matched in gender and age, were included as controls from July 2017 to August 2018. Left atrial EF, strain and strain rate of three phases (reservoir, conduit and pump), left atrial volume (maximum volume index, minimum volume index and pre-atrial contraction volume index) were measured with corresponding cardiac function analysis software. Then, the differences in these parameters were analyzed between the three groups by ANOVA or Kruskal-Wallis test with post hoc comparison and Bonferroni correction.Results:Compared with controls, patients with rTOF had lower reservoir function parameters (EF, strain and strain rate), conduit EF, conduit strain, and left atrial maximum volume index ( P<0.05), but higher pump EF ( P<0.05). In patients with rPS, only the reservoir strain rate decreased compared with controls ( P<0.05), and the remaining data showed no significant difference ( P>0.05). The reservoir and conduit EF and strain in patients with rPS were higher than those in patients with rTOF ( P<0.05). Conclusions:In patients with rTOF and rPS, left atrial function has changed despite the preservation of left ventricular EF, which may be an early marker of left ventricular diastolic dysfunction. In children with rTOF, left atrial reservoir and conduit functions decreased while the pump function increased. The reservoir and pump functions in rPS were better than those in rTOF. In addition, CMRI can detect left atrial dysfunction early before it enlarged.
10.Prolonging negative pressure drainage time to prevent salivary fistula after parotidectomy

Objective:To investigate the effect of prolonged negative pressure drainage time after parotidectomy and analyze its relationship with the incidence of postoperative salivary fistula.Methods:The clinical data of 94 patients with benign parotid gland tumors who received treatment in the Department of Otolaryngology-Head and Neck Surgery of The First Affiliated Hospital of Xiamen University from July 2021 to June 2022 were retrospectively analyzed. These patients were divided into an observation group and a control group ( n = 47 per group). In the observation group, the negative pressure drainage tube was removed after 1 week of simple negative pressure drainage, while in the control group, conventional local bandaging of the parotid gland was performed for 2 weeks, and negative pressure drainage was given for 2-3 days. Postoperative drainage volume, pain degree, and the incidence of salivary fistula were recorded for each group. Results:The total drainage volume in the observation group was (77.93 ± 23.83) mL, which was significantly greater than (47.06 ± 24.71) mL in the control group ( t = 6.17, P < 0.001). The Visual Analogue Scale score in the observation group was (3.021 ± 1.07) points, which was significantly lower than (7.53 ± 1.27) points in the control group ( t = 18.63, P < 0.001). The incidence of postoperative salivary fistula in the observation group was 2.1% (1/47), which was significantly lower than 17.0% (8/47) in the control group ( χ2 = 4.42, P = 0.035). Conclusion:Simple prolongation of negative pressure drainage time can achieve full drainage, improve the quality of life of patients after parotidectomy and reduce the occurrence of postoperative salivary fistula, which is worthy of clinical promotion.
