1.Exploration on the molecular mechanism of Sanhuang Yishen Capsules for the treatment of diabetes based on network pharmacology and experimental verification

Objective:To explore the material basis and potential mechanism of Sanhuang Yishen Capsules in the treatment of diabetes through network pharmacology, molecular docking and experimental verification.Methods:The active components and targets of Sanhuang Yishhen Capsules were screened using China Natural product chemical composition database and SymMap database, and the related targets of T2DM were screened by GeneCards database. The "Chinese materia medica-active component-target" network was constructed, and the intersection genes were enriched by GO and KEGG using R language. Key active components were selected for molecular docking verification with potential core targets. 60 rats were divided into normal group, model group, and Sanhuang Yishen Capsules group according to random number table method, with 15 rats in each group. In addition to the normal group, the diabetic rat model was prepared in the other groups, and the corresponding drugs were intragastric in each group for 8 weeks. The levels of fasting blood glucose (FBG), fasting insulin (FINS) and insulin resistance index (HOMA-IR) were measured by radioimmunoassay. Western blotting was used to detect protein expressions of epidermal growth factor receptor (EGFR), epidermal growth factor (EGF), Akt serine/threonine kinase 1 (AKT1), recombinant tumor protein p53 (TP53), and recombinant caspase 3 (CASP3).Results:A total of 160 active components and 298 targets of Sanhuang Yishen Capsules, 2194 targets related to T2DM, and 166 intersection targets were obtained. GO and KEGG analyzed a series of biological reaction processes mainly involved in signal transduction, oxidative stress, apoptosis, etc., and mainly involved in the regulation of P13K/Akt, P53, CASP3 and other targets. The results of molecular docking showed that the main active components obtained by screening had strong binding with the target. Compared with model group, FBG, FINS, HOMA-IR, TP53 and CASP3 in Sanhuang Yishen Capsules group decreased ( P<0.05), EGFR, EGF and Akt1 proteins increased ( P<0.05). Conclusion:The mechanism of Sanhuang Yishen Capsules for the treatment of may be related to the regulation of EGF/EGFR/P13K/Akt signaling pathway, TP53 signaling pathway, CASP3 signaling pathway, PPARG signaling pathway, ESR1 signaling pathway, PTGS2 signaling pathway, CAT signaling pathway and CTNNB1 signaling pathway.
2.Screening and preliminaryfunctional analysis of differentially expressed lncRNAs and mRNAs in vascular endothelial cells treated by tritiated water

Objective:To investigate the expression changes of lncRNAs and mRNAs in human umbilical vein endothelial cells(HUVEC) treated by tritiated water.Methods:HUVEC cells were divided into two groups, the control group cultured in DMEM medium, and the tritiated water exposure group cultured in a medium containing tritiated water with a final concentraion of 3.7×10 3 Bq/ml. After culture for 48 h, cells were collected for RNA extract.The differentially expressed lncRNAs and mRNAs were screened by high-through put chip technology and then analyzed. Results:Compared with the control group, 1 717 lncRNAs were significantly up-regulated and 3 994 lncRNAs significantly down-regulated, and 4 562 mRNAs were significantly up-regulated and 1 433 mRNAs down-regulated. Through co-expression analysis of differential mRNAs and lncRNAs, some key genes including SQSTM1, CXCL8, ITPR1, GADD45A, NF-kB1 and VDAC1 were obtained.Conclusions:Tritiated water exposure can induce multiple changes of mRNAs and lncRNAs in vascular endothelial cells, which may lead to toxic effects through signaling pathways including some key genes such as SQSTM1, CXCL8, and ITPR1.
3.Minimally invasive osteotomy and manual reduction for hallux valgus.

OBJECTIVE:
To explore clinical effects of minimally invasive osteotomy and manual reduction in treating hallux valgus.
METHODS:
From January 2018 to May 2019, 31 patients (42 feet) with hallux valgus were treated with minimally invasive osteotomy and manual reduction, including 3 males and 28 females aged from 18 to 76 years old with an average of (50.1± 4.9) years old. Preoperative and postoperative hallux valgus (HVA), intermetatarsal angles(IMA), length difference between 1 and 2 metatarsals were recorded and compared, and American Orthopedic Foot and Ankle Society (AOFAS)score were observed and measured.
RESULTS:
Thirty-one patients (42 feet) were followed up from 14 to 18 months with an average of (15.1± 1.2) months. HVA, IM before operation were (38.5±5.4)°, (13.0± 1.1)°, and improved to (14.3±4.7)°and (9.1±1.5)°after operation respectively(
CONCLUSION
Minimally invasive osteotomy and manual reduction in treating hallux valgus have advantages of shorter operation time, less length of incision, and could correct hallux valgus deformity, improve front feet and receive good clinical effect in further.

4. Clinical analysis of Shenzhuo Decoction combined with massage manipulation, acupotomy and lumbar traction in treatment of lumbar disc herniation

Objective To analyze the clinical effects of Shenzhuo Decoction combined with massage manipulation, acupotomy and lumbar traction in treatment of lumbar disc herniation. Methods A total of 495 cases of lumbar disc herniation from January 2015 to December 2018 were divided into the observation group and control group, with 259 and 236 cases respectively in observation and control groups. The patients in control group were only given regular massage manipulation, acupotomy and lumbar traction, and the patients in observation group were given Shenzhuo Decoction based on the treatment of control group. The clinical effects of the two groups was analyzed after 2 weeks of treatment. Results The total effective rate and JOA Score of observation group were significantly better than that in the control group (P < 0.05). Conclusion Shenzhuo Decoction in the treatment of lumbar disc herniation has good curative effect. It has the advantages of quick curative effect and short courses of treatment with the regular massage manipulation, acupotomy and lumbar traction, which can be promoted in clinical practice as a conservative treatment for lumbar disc herniation.
5.Imaging features of lung ultrasound in patients undergoing lung transplantation

Objective To evaluate the postoperative imaging features of lung ultrasound on patients undergoing lung transplantation ,and to provide the evidence for diagnosis and therapy . Methods Between October 2016 and March 2017 ,51 patients undergoing lung transplantation ( unilateral:37 ,bilateral:14 ) admitted to the ICU in Wuxi People′s Hospital were examined by bedside lung ultrasound ,and imaging features were analyzed . Results The main features on ultrasound of 51 patients undergoing lung transplantation were:①Pneumothorax :The A-line arising at the pleural line was shown in all of 51 patients , mainly on anterolateral parts of the chest wall initially ,then fade away towards anterosuperior parts over time . ② Hydrothorax :An anechoic fluid collection was detected ( up to 50 mm in width ) ,and became narrow over time in most patients . A mass of floccules or progressive growth of pleural effusion indicated the need for emergency surgery ,and were confirmed bleeding after surgery . ③Subcutaneous emphysema:The E-line was detected mainly in anterior and lateral parts around the surgical incision of postoperative patients ,and gradually fade away over time . ④Pulmonary edema:On the first postoperative day ,multiple B-lines were shown in 49 cases ,lung consolidation in 36 cases ( mainly in the inferior and inferoposterior parts) ,lung consolidation sonographic air bronchogram in 12 cases . Then the area of consolidation and B-lines reduced ,the air bronchogram sign became more prevalent ,and the shred sign appeared on the border of consolidation over time . Conclusions The imaging features of lung ultrasound provides clinic diagnostic and therapeutic value for postoperative patients undergoing lung transplantation .
6.The use of the increased IL-27 expression as a diagnostic marker in the serum of hepatocellular carcinoma patients

Objective To investigate the clinical significance of IL-27 in hepatocellular carcinoma ( HCC) in the early stage , and to compare it with alpha-fetoprotein ( AFP) in diagnostic performance .Methods Enzyme-linked immunosorbent assay was used to detect the serum level of IL-27 in the HCC group and control group .Receiver operator characteristic ( ROC) curve was used to ana-lyze two indicators and the logistic regression model predicted value ( PRE) was obtained .Results The study indicated the concentra-tion of IL-27 in HCC group [(364.19 ±177.55)pg/ml] was significantly higher than the control group [(255.49 ±94.33)pg/ml], the area under the curve (AUC) of IL-27 and AFP were (0.804) and (0.818), respectively.Logistic regression obtained the regres-sion model PRE that contains AFP and IL-27, with a predicted area under the ROC curve for HCC (0.901), while the diagnostic sen-sitivity (84.8%) and specificity (96.7%) were significantly higher than the capability of individual diagnosis of each indicator .Con-clusions In this study we obtained the model which can be used for clinical diagnosis of hepatocellular carcinoma in future .
7.Radioprotective effects of FUZHENGJIEDU on rats exposed to radon

Objective To investigate the protective effects of FUZHENGJIEDU,a kind of Chinese herb on rats exposed to radon and it's daughter.Methods Twelve male SD rats were exposed with concentration of 40 000 Bq/m3 radon and the exposed dose was 120 work level month (120 WLM),and then all the rats were randomly divided into 2 groups.One group was 120 WLM group (positive control,n=6 ),the other group was FUZHENGJIEDU treatment group (120 WLM exposed rats were given FUZHENGJIEDU 5.0 g every day,n =6),meanwhile 6 rats which lived in the normal environment ( the concentration of radon and it's daughter was lower than 50 Bq/m3 ) as control group.The levels of tumor markers induding carcinoembryonic antigen ( CEA),neuron-specific enolase (NSE),CYFRA21-1 and p53 antibody were tested by ELISA.The rate of p16 gene methylation was measured by methylation-specific PCR (MSP).Results The levels of CEA and NSE in 120 WLM group were (396.62 ± 148.74) and (9.09 ±0.90) μg/L,respectively,significantly higher than those of control group (t =2.583,2.463,P < 0.05).The levels of CEA and NSE in treatment group were (70.89 ±44.71) and (4.31 ±1.37) μg/L,respectively,remarkably lower than those of 120 WLM group (t =2.921,2.526,P <0.05 ).The concentrations of CYFRA21-1 and p53 antibody were not significantly different among the three groups (t =1.713,1.963,P > 0.05 ).The rate of p16 gene methylation in BALF cells in 120 WLM group was 16.67%,but in control group and treatment group did not arise p16 gene methylation in BALF cells.Conclusions FUZHENGJIDU would decrease the levels of CEA and NSE and the rate of p16 gene methylation,and could exert it's protective effect on rats exposed to high concentration of radon.
8.Application of transperineal biopsy of the prostate guided by contract-enhanced transrectal ultrasound

Objective To investigate the clinical significance of contrast-enhanced transrectal ultrasound(CE-TRUS) in the perineal prostate biopsy. Methods A total of 116 patients was undergone prostate biopsy through the perineum under the direction of tansrectal ultrasound. Prostate biopsy standard was based on 2007 CUA revised guidelines for diagnosis and treatment of urological diseases.Color Doppler ultrasonography was used to check the prostate and to learn the prostate focal lesion,size, number and echo color Doppler flow characteristics. Of the 116 cases, 43 patients was undergone contrast-enhanced transrectal ultrasound. Results The biopsy results confirmed the diagnosis of prostate cancer was 64 cases, Benign prostatic hyperplasia was 52 cases. Of 43 cases who undergone contrast-enhanced transrectal ultrasound, Prostate cancer and Benign prostatic hyperplasia were 25 and 18 cases, respectively. CE-TRUS group and TRUS group showed no statistical difference between two groups. Analyzed the cases with PSA≤30 ng/ml, CE-TRUS group had a higher positive rate of biopsy (P=0.046). Conclusion TRUS guided transperineal biopsy of prostate might be an method for the diagnosis of prostate cancer with a higher accuracy rate. CE-TRUS can improve the biopsy positive rate of prostate cancer.
9.Study on the correlation between syndrome differ classification of knee osteoarthritis and X-ray image.

OBJECTIVETo study the correlation between syndrome differ classification of knee osteoarthritis and X-ray image, so as to provide evidence for clinical diagnosis and treatment.
METHODSFrom Jun. 2007 to Dec. 2007, 78 patients (108 knees) with knee osteoarthritis were reviewed, including 65 females (89 knees) and 13 males (19 knees), ranging in age from 41 to 77 years. According to the standards for the differentiation of syndrome in the treatment of knee osteoarthritis defined in Principle of Clinical Research for New Traditional Herbs, the patients were divided into three types: Type I, insufficiency of the liver and kidney, with stagnation of tendons and muscles, 43 knees; Type II, insufficiency of the spleen and kidney, with dampness infusion into bone and joints, 26 knees; Type I, deficiency of the liver and kidney, with inter-obstruction of phlegm and stasis 39 knees. Normotopia and lateral plain film of knee joint of weigh loading and in erect position, and patellofemoral Skyline plain flim was taken. Joint space narrow, osteophyte generation, subchondral osteosclerosis and subchondral cystic degeneration were evaluated. All data were analyzed by K independent samples nonparametric test in order to find out the correlation between syndrome differ classification of knee osteoarthritis and X-ray image.
RESULTSIt was shown that after K independent samples nonparametric test about syndrome differ classification of knee osteoarthritis and X-ray image: there were significant differences among three types about lateral patella osteophyte, condyles of tibia osteophyte and Type II was the most serious, Type I was secondary, Type II was the lightest. Other index had no obvious difference among the three groups.
CONCLUSIONThere is certain correlation between syndrome differ classification of knee osteoarthritis and X-ray image. There are significant differences among three types about lateral patella osteophyte, condyles of tibia osteophyte, the Type II is the most serious,Type I is secondary, Type II is the lightest. Consequently, it can be deduced that worse osteophyte is one of accordances of Type II-insufficiency of the spleen and kidney, with dampness infusion into bone and joints. And, the Type II is more serious stage in radiologic manifestation.

10.Treatment of Barton fractures with external fixation with paper splints.

OBJECTIVETo study the control of instability of paper splints for the treatment of Barton fractures.
METHODSFrom 1998 to 2007, 30 patients with Barton fractures were treated with manual reduction and external fixation with paper splints. Among the patients, 20 patients were male and 10 patients were female, ranging in age from 20 to 68 years, averaged 45.3 years. The course of the disease ranged from 1 to 15 hours, with a mean of 4.2 h. All the patients had a history of trauma. The X-rays showed palm-side distal radius fractures, fractured fragments displacing toward proximal palm and the radius bone sudislocating toward the palm. Anderson assessment method was adopted.
RESULTSAll the patients were followed up ranging from 0.5 to 2 years (12.3 months on average). When clinical healing, the X-ray showed good position in 26 cases, and the fractured fragments on the palm side displaced 1 to 2.5 mm toward palm in 4 cases. Final results : the motion range and function of wrist and forearm recovered completely in 24 patients; the motion range and function of wrist and forearm were limited slightly without hindering daily life in 6 patients. According to Anderson assessment method, 24 patients got an excellent result and 6 good.
CONCLUSIONThis fixation method possesed such advantages as improving hematoma absorption, reinforcing fixation on the basis of original fixation at any time without pressure sore and necrosis, safe and effective.
