Shanghai Journal of Preventive Medicine 2025;37(7):620-624
ObjectiveTo investigate the mortality level and leading causes of death among the registered residents in Taishun County, Zhejiang Province from 2016 to 2023, as as to provide evidence for the formulation of relevant disease prevention and control strategies. MethodsDeath data of the registered residents in Taishun County were collected from the population death information registration and management system of Zhejiang Province Chronic Disease Surveillance Information Management System and Wenzhou City Disease Prevention and Control Information System. Excel 2021 software was used to calculate the mortality rates and cause-specific rankings of different genders, age groups, and surveillance years. ResultsDuring 2016‒2023, the crude mortality rate (CMR) of the registered residents in Taishun County was 663.97/105, with a standardized mortality rate of 448.33/105. The CMR for males (833.15/105) was 1.73 times higher than that for females (480.77/105). The mortality rate tended to decrease and then increase with age increased, with 78.25% of deaths occurring among individuals aged ≥65 years. The top five causes of death were malignant tumors, cerebral vascular diseases , cardiovascular diseases, chronic respiratory diseases, and injuries/poisoning, collectively accounting for 73.69% of the total deaths. The ranking orders for males and females were not exactly the same. Chronic diseases dominated the causes of death, accounting for 83.85% of the total deaths, from which, malignant tumors and cerebral vascular diseases accounted for 24.45% and 17.79%, respectively. ConclusionStatistically significant differences of mortality rates are observed among individuals from different ages and genders in Taishun County. Chronic diseases, particularly malignant tumors and cerebral vascular diseases, are the main causes of death among the registered residents in Taishun County. Therefore, it is needed to place weight on the prevention and management of chronic diseases, to alleviate the disease burden caused by premature deaths, and to promote the health of the whole population.

Objective:To explore the effect of timely induction intervention on postpartum urination in primipara during vaginal delivery, so as to provide the evidence for preventing the occurrence of postpartum urinary retention and relieving the pain of primipara. Methods:This study adopted a randomized controlled trial design, and selected 400 cases of primipara who were hospitalized for vaginal delivery in the Obstetric Department of Dalian Women and Children's Medical Group Sports New Town Hospital from June 2021 to September 2022 as the study objects by convenience sampling method. They were divided into the intervention group and the control group with 200 cases each by random number table method, and the control group received routine postpartum care. Instruct active urination within 6 hours after delivery. The intervention received timely induction urination intervention. The general condition and bladder urine volume of the women in the intervention group were evaluated at 2, 4, 6 h after delivery, respectively, and personalized guidance was implemented, including the frequency of massage of the bottom of the uterus, the control of water intake, the selection of methods and timing of inducing urination, etc., and routine postpartum care was given when the women completed their first urination and had no complaints of discomfort. The first urination time, first urination volume, first bladder irritation during the first urination and the incidence of postpartum urinary retention in different periods were compared between the two groups. Results:The patients in the control group were (29.60 ± 3.20) years old, while the patients in the intervention group were (28.81 ± 3.42) years old. The first urination time in the intervention group was (6.89 ± 2.18) h, which was shorter than that in the control group (9.11 ± 3.86) h, and the difference was statistically significant ( t=-2.49, P<0.01). The first urination volume in the intervention group was (322.36 ± 120.15) ml, which was higher than that in the control group (262.93 ± 105.68) ml, and the difference was statistically significant ( t=3.39, P<0.05). The incidence of the first bladder irritation in the intervention group was 22.0%(44/200), which was lower than that in the control group 33.5%(67/200), and the difference was statistically significant ( χ2=6.60, P<0.05). The incidence of postpartum urinary retention within 24 h in the intervention group was 5.5%(11/200), which was lower than that in the control group 11.5%(23/200), and the difference was statistically significant ( χ2=4.63, P<0.05). The incidence of postpartum urinary retention within 1 week in the intervention group was 9.5%(19/200), which was lower than that in the control group 16.5%(33/200), and the difference was statistically significant ( χ2=4.33, P<0.05). There was no significant difference in the incidence of postpartum urinary retention within 24 to 72 h between the two groups ( P>0.05). Conclusions:Timely induction intervention can reduce the incidence of postpartum urinary retention, shorten the time of first urination, increase the volume of first urination and improve the comfort of first urination, which is worthy of clinical application.

In recent years, immune checkpoint inhibitors (ICIs) have made great progress in the treatment of tumor patients, prolonging their survival. However, the expansion of immunity against tumors with ICIs may also cause an imbalance in immune tolerance, leading to immune-related adverse events (irAEs). Immune-mediated liver injury caused by ICIs (ILICI) is one of the more common types of irAEs. In this review paper, the definition, epidemiology, risk factors, pathogenesis, pathology, clinical manifestations, treatment, recurrence, and re-treatment of ILICI were summarized to provide a basis for clinical diagnosis and treatment.

Objective To investigate the clinical effect of direct-acting antiviral agent (DAA) in the treatment of chronic hepatitis C (CHC) patients with thrombocytopenia and its effect on platelet count (PLT). Methods A retrospective analysis was performed for 83 CHC patients with thrombocytopenia (PLT < 150×10 9 /L) who received the DAA treatment regimen without interferon for 12-24 weeks in Tianjin Third Central Hospital from April 2018 to March 2019, and the changes in virologic response, liver function parameters, PLT, and liver stiffness measurement (LSM) were evaluated at the end of treatment (EOT) and at week 12 after EOT. Quantitative data accord with normal distribution were compared by repeated measures ANOVA. Normal transformation was performed before the comparison between skewed data, then repeated measures ANOVA was carried out. A logistic regression analysis was used to investigate the predictive factors for PLT elevation, and the receiver operating characteristic (ROC) curve was plotted to analyze the value of LSM in predicting PLT elevation after treatment. Results Among the 83 CHC patients with thrombocytopenia, 61.4% had liver cirrhosis, and the rate of sustained virologic response at week 12 after the end of treatment (SVR12) was 98.8%. From baseline to EOT and SVR12, the patients had significant reductions in the serum levels of aspartate aminotransferase, alanine aminotransferase, gamma-glutamyl transpeptidase, total bilirubin, and globin, a significant increase in the serum level of albumin, and a significant reduction in LSM (all P < 0.05). For all patients, PLT at EOT and SVR12 was significantly higher than that at baseline [EOT vs baseline: (110.4±44.6)×10 9 /L vs (97.8±33.2)×10 9 /L, P < 0.01; SVR12 vs baseline: (109.0±47.7)×10 9 /L vs (97.8±33.2)×10 9 /L, P < 0.01]. At SVR12, there were significant differences in the proportion of patients with liver cirrhosis, baseline LSM, and baseline white blood cell count between the PLT elevation group and the non-PLT elevation group (all P < 0.05). The multivariate logistic regression analysis showed that LSM was an independent predictive factor for significant PLT elevation after DAA treatment (odds ratio=0.929, 95% confidence interval: 0.864-0.999, P < 0.05). Baseline LSM had an area under the ROC curve of 0.644 in predicting PLT elevation, with a sensitivity of 81.0% and a specificity of 48.6% at a cut-off value of 20.15 kPa. The patients with PLT > 100×10 9 /L at baseline had a greater increase in PLT( P < 0.05). Conclusion CHC patients with thrombocytopenia have significant improvements in liver function and LSM after receiving DAA treatment and obtaining SVR12, and baseline LSM is an independent predictive factor for PLT elevation. There is a significant increase in PLT from baseline to EOT and SVR12.

Objective:To analyze the risk factors of death within two years of the patients with liver cirrhosis after transjugular intrahepatic portosystemic shunt(TIPS), and to explore the predictive value of 6 common clinical evaluation systems on the risk of death after TIPS.Methods:TIPS clinical data from 132 patients with liver cirrhosis were analyzed retrospectively. According to the 2-year clinical outcome after TIPS, the patients were divided into the death group and the survival group. Logistic regression was used to analyze the risk factors of death within 2 years after TIPS. According to the scores of CTP, MELD, MELD Na, BioCliM, FIB-4, and ALBI evaluation systems, the prediction efficiency of death risk of the six evaluation systems was evaluated by using the receiver operating characteristic (ROC) curves and the area under the curve (AUC).Results:During the 2-year follow-up period after TIPS, the age, urea nitrogen level, platelet count, and proportion of hepatic encephalopathy in the death group were higher than those in the survival group one month after TIPS, and the serum sodium level was lower than those in the survival group (all P<0.05). Multivariate analysis showed that the elderly and hepatic encephalopathy one month after operation were independent risk factors for death (all P<0.05). At 1 week after the surgery, there were significant differences in CTP, MELD, and MELD-Na scores between the survival group and the death group (all P<0.05). One week after operation, the AUC of ROC of CTP, MELD, MELD-Na, and ALBI scores were 0.685, 0.721, 0.805, and 0.658 respectively, and the optimal critical values were 8.5, 12.99, 14.51 and -1.52 respectively. Conclusions:The elderly and the occurrence of hepatic encephalopathy one month after TIPS are independent risk factors for the death of liver cirrhosis patients after TIPS. The evaluation of CTP, MELD, MELD-Na, and ALBI one week after TIPS can predict the death risk of decompensated liver cirrhosis patients within 2 years after TIPS, and MELD-Na has the best predictive effect.

Chinese Medical Sciences Journal 2022;37(3):265-274
Wnt5a is a secreted Wnt ligand that plays a critical role in cellular pathways and inflammatory diseases. The WNT5A gene encodes two protein isoforms, Wnt5a-long and Wnt5a-short, which differ based on different promoter methylation and have distinct functions. However, the mechanisms of the promoter methylation are unclear. Depending on the extent of promoter methylation, Wnt5a exerts both anti-inflammatory and pro-inflammatory effects in inflammatory diseases, which may be involved in different Wnt5a isoforms. Therefore, the Wnt5a isoforms may be potential diagnostic markers for inflammatory diseases and the mechanisms of the WNT5A gene promoter methylation need to be further investigated.

Objective :
To investigate the correlation between nocturnal sleep duration combined with snoring in the first trimester of pregnancyand small for gestational age(SGA) ,large for gestational age(LGA) .
Methods :
Multi- variate Logistic regression model was used to analyze the association between nocturnal sleep duration ,snoring, their combined effects and SGA,LGA.
Results :
Compared to nocturnal sleep duration 7 to 9 h in the first trimester of pregnancy,sleep duration<7 h was positively correlated with SGA in male newborn( OR = 4. 22,95% CI : 1. 69 - 10. 52) ; After stratified by snoring,the sleep duration of snoring women<7 h was positively correlated with SGA ( OR = 5. 68,95% CI : 1. 02-31. 51) ,and the sleep duration of non-snoring women<7 h was positively correlated with LGA ( OR = 2. 10,95% CI : 1. 16 -3. 81) .
Conclusion
Sleep duration<7 h in the first trimester of preg- nancy is a risk factor for SGA and LGA,and snoring may enhance the association between sleep duration<7 h in the first trimester of pregnancy and SGA.Pregnant women should keep adequate nocturnal sleep duration to reduce the risk of abnormal neonatal weight.

Objective:To study the molecular biological characteristics of isolates from the infection site and isolates colonizing in anterior nares of children with invasive Staphylococcus aureus (SA) infection, and to analyze the concordance between the two types of strains from different sources. Methods:A total of 45 strains were collected from children with invasive SA infection treated in the Pediatric Ward of Sichuan Provincial Maternal and Child Health Care Hospital from January 2019 to August 2019, and 28 colonization isolates were obtained from the anterior nares of these patients. The susceptibility test was carried out by broth dilution method. The drug resistance genes mecA and blaZ and the virulence gene panton-valentine leucocidin( pvl) were detected by PCR.The homology of infective and colonizing isolates was detected by pulsed-field gel electrophoresis(PFGE)typing technique. Results:Colonization of SA was found in the nasal vestibule of 62.2% (28/45 cases) of patients with invasive SA infection.A total of 40.0% (18/45 strains) of the infective isolates and 32.1% (9/28 strains) of the colonizing isolates were Methicillin-resistant SA (MRSA), and the difference was not statistically significant( P>0.05). The resistance of infective isolates to Clindamycin, Azithromycin and Erythromycin was stronger than that of colonizing isolates, the difference of drug resistance rate was statistically significant( χ2=7.114, 7.820, 5.359, all P<0.05). There were no differences in the carrying rates of the drug resistance gene blaZ and the virulence gene pvl between the infective and colonizing bacteria( P>0.05). Phenotypically, Methicillin-susceptible SA (MSSA)was more susceptible to concordant colonization than MRSA[16.7%(3/18 cases) vs.48.1%(13/27 cases), χ2=4.671, P<0.05]. PFGE indicated that patients with invasive MSSA infection were significantly more likely to have a concordant MSSA colonization isolate in their anterior nares, compared with patients with invasive MRSA infection[59.3%(16/27 strains) vs.27.8%(5/18 strains), χ2=4.301, P<0.05]. Conclusions:The infective and colonizing strains of invasive SA show no difference in their resistance to some anti-biotics, but they carry almost the same number of drug resistance and virulence genes.Compared with those with MRSA infection, patients with MSSA infection are more likely to have concordant colonizing isolates. It is of potential clinical significance to screen the colonizing SA strains in patients with invasive SA infection.
