1.Effects of loganin on inflammatory response and intestinal barrier damage in septic rats

OBJECTIVE To investigate the effects of loganin on inflammatory response and intestinal barrier damage in septic rats by regulating the Ras homolog gene family member A (RhoA)/Rho-associated coiled-coil forming protein kinase 1 (ROCK1) signaling pathway. METHODS A sepsis rat model was established by cecal ligation and puncture, and randomly divided into sepsis group, loganin low-dose group (50 mg/kg loganin, gavage), loganin high-dose group (200 mg/kg loganin, gavage), positive control group (0.2 mg/kg atorvastatin, intraperitoneal injection), and loganin high-dose + lysophosphatidic acid (LPA) group (200 mg/kg loganin gavage and intraperitoneal injection of 10 mg/kg RohA activator LPA). An additional sham surgery group was established. Each group consisted of 10 rats, and medications were administered once every 6 hours for 4 times. After 24 hours of the last intervention, the levels of serum inflammatory factors interleukin-6 (IL-6), tumor necrosis factor-α (TNF-α), and IL-1β were detected. The pathological changes of ileal tissue were observed and Chiu’s intestinal mucosal injury score was also performed. The levels of intestinal function-lactate dehydrogenase (D-lactate), D-amino acid oxidase (DAO) and endotoxin, the percentages of zonula occludens-1 protein (ZO-1) and Occludin positive staining area, as well as protein expressions of RhoA, and ROCK1 were all detected. com RESULTS Compared with the sepsis group, the percentages of ZO-1 and Occludin positive areas increased significantly in loganin low-dose and high-dose groups; while the levels of IL-6, TNF-α, IL-1β, DAO, D-lactate and endotoxin, Chiu’s intestinal mucosal injury score as well as protein expressions of RhoA and ROCK1 decreased significantly (P<0.05); the destruction of rat ileal tissue was alleviated, and tissue edema and inflammatory infiltration were significantly reduced; moreover, the improvement effect in loganin high-dose group was superior to that in loganin low-dose group (P<0.05). Compared with loganin high-dose group, RhoA activator LPA reversed the trend of changes in the above indicators (P<0.05). CONCLUSIONS Loganin can alleviate inflammatory response and intestinal barrier damage in septic rats, the mechanism of which may be associated with inhibiting RhoA/ROCK1 signaling pathway.
2.Results and analysis of quality monitoring of radiation health technical services in Jiangsu Province, China

Objective To assess and understand the service capabilities and existing problems of radiation health technical service institutions in Jiangsu Province, China, and provide a basis for improving in-process and post-process supervision as well as enhancing radiation health technical service capabilities. Methods Thirty radiation health technical service institutions in Jiangsu Province were selected as quality monitoring objects from the National Occupational Health Technical Service Institution Management Information System. Evaluations were conducted using a standardized national assessment checklist, and a comprehensive risk assessment was performed by combining the results of laboratory test capability comparisons. Results The 30 institutions all passed the quality monitoring, with an average score of (76.62 ± 5.07). Comprehensive risk assessment identified 8 (26.67%) high-risk institutions, 22 (73.33%) medium-risk institutions, and 0 (0%) low-risk institutions. Conclusion The overall service quality of radiation health technical service institutions in Jiangsu Province is acceptable. However, further training and supervision are needed to improve technical service capacity and reduce service risks.
3.Morphological study of GABAergic neurons in the ventral zona incerta of mice involved in chronic itch

Objective:To detect itching,anxiety,depression behaviors in chronic itch models of mice and observe the activation of γ-aminobutiric acid(GABA)neurons in the ventral sector of the zona incerta(ZIv),and provide mor-phological evidence for their involvement in the modulation of itch information.Methods:Diphenylcyclopropenone(DCP)was used in glutamic acid decarboxylase 67-green fluorescent protein(GAD67-GFP)knock-in mice to establish chronic itch model.Itch behaviors were detected by video tracking system to verify whether the models were successfully established.The anxiety,depression behaviors of chronic itch model mice were detected by using elevated plus maze test(EPM)and tail suspention test(TST).By using GAD67-GFP mice,the distribution of GABAergic neurons in va-rious sectors of the zona incerta(ZI)was observed.And combined with immunofluorescence staining method,double labeling of GABAergic neurons with FOS in ZIv were observed respectively in control and DCP group mice.Results:In brain slices of GAD67-GFP mice,GABAergic neurons can be observed within all sectors of ZI and are more concentrat-ed in ZIv.Compared with control group mice,DCP group mice showed a significant increase in the bouts of scratching(P<0.001).The time of immobility in TST was significantly higher in DCP group mice than in control group mice,which displayed depression-like behavior.The EPM test showed that the numbers of entries and proportion of time in the cross region in DCP group mice were less than in control group mice.EPM test revealed that DCP group mice exhibited anxiety-like behavior.The results of immunofluorescence staining showed that the number of FOS-positive cells in ZIv was significantly higher in DCP group mice than in control group mice,and abundant co-labeled neurons of FOS and GABAergic neurons were observed in ZIv.Conclusion:GABAergic neurons were predominantly distributed in ZI,and were more concentrated in ZIv.The activation of GABAergic neurons in ZIv of DCP group mice provides morphological evidence on the involvement of GABAergic neurons in chronic itch and associated negative emotions.
4.Exploring the Effect of Medication-Containing Serum of Pingwei Capsules (平胃胶囊) on MNNG-Induced Inflammatory Cancer Transformation of Human Gastric Mucosal Epithelial Cells Based on Raf/MEK/ERK Pathway

ObjectiveTo explore the possible mechanism of Pingwei Capsules (平胃胶囊) for chronic atrophic gastritis from rapidly accelerated fibrosarcoma / mitogen-activated protein kinase /extracellular-signal-regulated kinase (Raf/MEK/ERK) pathway that influences the activation of fibrosarcoma protein/mitogen. MethodsFifteen SD rats were randomly divided into 5 rats in the blank group and 10 rats in Pingwei Capsules group. The rats in the blank group were given 1 ml/100 g of saline by gavage, and the rats in Pingwei Capsules group were given 0.63 g/(kg·d) of Pingwei Capsule suspension by gavage, and serum was collected for 3 consecutive days. N-methyl-N'-nitro-N-nitrosoguanidine (MNNG) was used to induce human gastric mucosal epithelial cells GES-1 to establish a precancerous lesion cell model. The successful cells were divided into control group (10% fetal bovine serum), blank serum group (10% fetal bovine serum plus 10% blank serum), and medication-containing serum group (serum with medication of Pingwei Capsule), and the volume fraction and time of intervention of Pingwei Capsule-containing serum were screened by CCK-8 assay. Human gastric mucosal epithelial cells GES-1 were divided into normal group, model group, blank serum group, medication-containing serum group, U0126 group, and combined group, with 6 replicate wells in each group. After successful modelling of the cells in all groups except the blank group, an equal volume of fetal bovine serum was added to the normal and model groups, an equal volume of blank serum was added to the blank serum group, a screening volume fraction of Pingwei Capsule-containing serum was added to Pingwei Capsule group, a 10 μmol/L mitogen-activated extracellular signal regulated kinase 1 (MEK1) inhibitor U0126 was administered in the U0126 group, an equal dose of Pingwei Capsule-containing serum plus 10 μmol/L of U0126 was administered to the combined group. After the selected incubation time, the level of interleukin 6 (IL-6) was detected in the cells by ELISA, the expression of IL-6 and MEK1 was detected by immunofluorescence, and the expression of IL-6, Raf, MEK1, and ERK mRNA was detected by RT-qPCR, and the expression of IL-6, Raf, MEK1, and ERK mRNA in the cells was detected by Western blot. ResultsThe 5.35% volume fraction, 48 h intervention of Pingwei Capsule-containing serum was selected for subsequent experiments. Compared with the normal group, the IL-6 content in cell supernatants and the expression of IL-6, Raf, MEK1, ERK mRNA and ERK1/2 proteins in cells increased in the model group and blank serum group (P<0.01). Compared with the model group, all of the above indexes were improved in medication-containing serum group, U0126 group, and combined group (P<0.05 or P<0.01). Compared with medication-containing serum group, the expression of IL-6, MEK1 expression, the expression of IL-6, Raf, MEK1 and ERK mRNA, and the expression of IL-6, Raf, MEK1 and ERK1/2 proteins reduced in the cells of combined group (P<0.05 or P<0.01). Compared with the U0126 group, IL-6 expression reduced and IL-6, MEK1 and ERK1/2 protein expression reduced in cells of combined group (P<0.05 or P<0.01). ConclusionThe Pingwei Capsule-containing serum may play a role in the treatment of chronic atrophic gastritis by improving the inflammation-cancer transformation of GES-1 cells through inhibiting the Raf/MEK/ERK pathway.
5.Construction and application of a management scheme for nursing assistants in unaccompanied hospitals

Objective To construct a management scheme of nursing assistants in unaccompanied hospitals and explore its application effect.Methods The management team of nursing assistants in unaccompanied hospitals was set up.On the basis of investigating the current situation of nursing assistants'team management,the management scheme of nursing assistants was constructed by ORTCC management model for references,including 5 parts:determining management objectives,perfecting rules system,building hierarchical training system,checking and assessing,and shaping cultural system.Convenience sampling method was used to select 92 nursing assistants in an unaccompanied hospital in Xiamen,Fujian Province from June 2021 to June 2022 as application subjects.The post competence of nursing assistants and the satisfaction of patients and nurses to nursing assistants were compared before the application of the management scheme(June-December 2021)and after the application(January 2022-June 2022).Results After the application of ORTCC management model,the total score of post competency of medical nursing assistants increased from[65(64,67)]to[91(90,92)];the excellent rate of post competency level increased from 1.09%to 67.39%;the satisfaction of patients with nursing assistants increased from 61.99%to 88.62%;the scores of nurses'satisfaction with nursing assistants in all dimensions were significantly higher than before(all P<0.05),and the overall evaluation was improved from 92.73%to 99.24%.Conclusion The use of the ORTCC management model for nursing assistants can effectively improve their post competence,improve the satisfaction of patients and nurses,and help to further stabilize and develop the medical nursing assistants staff.
6.Serological characteristics of HBsAg positive/HBV DNA non-reactive blood donors

【Objective】 To explore the distribution of serological markers related to samples whose serological test results were inconsistent with HBV DNA test results among voluntary blood donors in Xi′an. 【Methods】 A total of 71 HBsAg ELISA positive and NAT non-reactive (ELISA+ /NAT-)blood samples were collected from Shaanxi Blood Center from November 1, 2022 to April 30, 2023. The serological markers of hepatitis B were detected by electrochemiluminescence method, and the HBV S region and C region gene fragments were amplified by nested-PCR. 【Results】 The positive rate of nested-PCR in double ELISA+ /NAT- group(n=30) was statistically higher than that of ELISA+ /NAT- group(n=41)(60% vs 24.4%, P<0.05). Donors in double ELISA+ /NAT- group were all first-time blood donors, with the positive rate of anti-HBc in serum of 100%, and the serological pattern was mainly positive for items 1, 4 and 5 items(80%). Among the ELISA+ /NAT- group, 31.7% were repeat blood donors, with the positive rate of anti-HBc in serum of only 19.51%, and the serological patterns were mainly single anti-HBs positive (43.90%) and all negative (36.58%). 【Conclusion】 There are false positives in the test results of ELISA+ /NAT- group, which leads to unnecessary blood discarding. Meanwhile, the samples with negative NAT may have low levels of HBV DNA, which may lead to missed detection. It is suggested that multiple systems and methods should be applied to trace the blood donors who are HBsAg positive and NAT non-reactive, so as to improve the accuracy of HBV screening of blood donors and reduce blood waste.
7.Clinical characteristics and prognosis of 28 cases of infant acute lymphoblastic leukemia

Objective:To analyze the clinical characteristics and prognosis of patients with infant acute lymphoblastic leukemia (IALL).Methods:A retrospective cohort study.Clinical data, treatment and prognosis of 28 cases of IALL who have been treated at Beijing Children′s Hospital, Capital Medical University and Baoding Children′s Hospital from October 2013 to May 2023 were analyzed retrospectively. Based on the results of fluorescence in situ hybridization (FISH), all patients were divided into KMT2A gene rearrangement (KMT2A-R) positive group and KMT2A-R negative group. The prognosis of two groups were compared. Kaplan-Meier method and Log-Rank test were used to analyze the survival of the patients.Results:Among 28 cases of IALL, there were 10 males and 18 females, with the onset age of 10.9 (9.4,11.8) months. In terms of immune classification, 25 cases were B-ALL (89%), while the remaining 3 cases were T-ALL (11%). Most infant B-ALL showed pro-B lymphocyte phenotype (16/25,64%). A total of 22 cases (79%) obtained chromosome karyotype results, of which 7 were normal karyotypes, no complex karyotypes and 15 were abnormal karyotypes were found. Among abnormal karyotypes, there were 4 cases of t (9; 11), 2 cases of t (4; 11), 2 cases of t (11; 19), 1 case of t (1; 11) and 6 cases of other abnormal karyotypes. A total of 19 cases (68%) were positive for KMT2A-R detected by FISH. The KMT2A fusion gene was detected by real-time PCR in 16 cases (57%). A total of 24 patients completed standardized induction chemotherapy and were able to undergo efficacy evaluation, 23 cases (96%) achieved complete remission through induction chemotherapy, 4 cases (17%) died of relapse. The 5-year event free survival rate (EFS) was (46±13)%, and the 5-year overall survival rate (OS) was (73±10)%.The survival time was 31.3 (3.3, 62.5) months. There was no significant statistical difference in 5-year EFS ((46±14)% vs. (61±18)%) and 5-year OS ((64±13)% vs. (86±13)%) between the KMT2A-R positive group (15 cases) and the KMT2A-R negative group (9 cases) ( χ2=1.88, 1.47, P=0.170, 0.224). Conclusions:Most IALL patients were accompanied by KMT2A-R. They had poor tolerance to traditional chemotherapy, the relapse rate during treatment was high and the prognosis was poor.

Fragile X syndrome(FXS)is caused by abnormal duplication and amplification of the FMR1 gene CGG.This article reports a pair of brothers diagnosed with FXS by genetic testing.Two patients,aged 15 and 14 years old respectively,both had clinical manifestations such as language disorders,intellectual disabilities,attention deficit disorder,autism spectrum disorder,and FXS's characteristic facial features.The proband had a rare late-onset epileptic seizure,which was well treated with levetiracetam,while his younger brother had no electroencephalogram abnormalities after repeated follow-up.This pair of cases suggests that the clinical phenotype of FXS has diversity and heterogeneity.
9.Regulatory Effect of Xiaoyaosan Combined with Pirfenidone on Idiopathic Pulmonary Fibrosis and Depression in Rats Based on p38 MAPK/NF-κB Inflammatory Pathway

ObjectiveTo observe the mechanism of chronic psychological stress aggravating idiopathic pulmonary fibrosis (IPF) in rats and the regulatory effect of Xiaoyaosan. MethodSixty SD rats were acclimatized for one week and then randomly divided into five groups: sham operation group, IPF group, IPF and depression model group, pirfenidone group, and pirfenidone + Xiaoyaosan group, with 12 rats in each group. The IPF group was induced by intratracheal instillation of bleomycin (5 mg·kg-1) and administered 0.9% sodium chloride solution intragastrically. The model group was induced with bleomycin (5 mg·kg-1) combined with chronic unpredictable mild stress to establish a rat model of IPF and depression and administered 0.9% sodium chloride solution intragastrically. Concurrently, the pirfenidone group was administered pirfenidone aqueous solution (50 mg·kg-1) intragastrically, and the pirfenidone + Xiaoyaosan group was administered pirfenidone aqueous solution (50 mg·kg-1) and Xiaoyaosan decoction (19.27 g·kg-1) intragastrically. The experiment lasted for four weeks. Various parameters, including body weight, food intake, sucrose consumption rate, open field behavior, lung function, lung coefficient, pathological changes in lung tissue, and hydroxyproline (HYP) content were compared among the groups. Enzyme-linked immunosorbent assay (ELISA) was used to measure serum cortisol (CORT), corticotropin-releasing hormone (CRH), adrenocorticotropic hormone (ACTH) levels, and serotonin (5-HT) levels in serum and hippocampus. The levels of interleukin-1β (IL-1β), interleukin-6 (IL-6), and tumor necrosis factor-α (TNF-α) in serum and lung tissue were also measured. Real-time fluorescence quantitative polymerase chain reaction (Real-time PCR) was employed to determine the mRNA expression of IL-1β, TNF-α, IL-6, p38 mitogen-activated protein kinase (p38 MAPK), and nuclear transcription factor-κB p65 (NF-κB p65) in lung tissue. Western blot was used to detect the expression of proteins related to the p38 MAPK/NF-κB signaling pathway. ResultCompared with the normal group, IPF and depression model group showed slow weight gain, reduced food intake, decreased sucrose consumption rate, reduced total distance and average speed of movement in the open field test, weakened lung function, increased lung coefficient (P<0.01), significant inflammatory cell infiltration in lung tissue by hematoxylin-eosin (HE) staining, collagen fiber deposition by Masson staining, and increased HYP content (P<0.01). There were elevated levels of serum CORT, CRH, and ACTH (P<0.05, P<0.01), increased protein and mRNA levels of IL-1β, IL-6, and TNF-α in serum and lung tissue, decreased 5-HT levels in serum and hippocampus, and increased relative expression of p-p38 MAPK and p-NF-κB p65 proteins in lung tissue (P<0.01). Compared with the IPF and depression model group, the depression and IPF of rats in the pirfenidone + Xiaoyaosan group were effectively ameliorated, as evidenced by faster weight gain, increased food intake and sucrose consumption rate, increased total distance and average speed of movement in the open field test, enhanced lung function, reduced lung coefficient (P<0.01), decreased inflammatory cell infiltration by HE staining, reduced collagen fiber deposition by Masson staining in lung tissue, and decreased HYP content (P<0.01). Decreased serum CORT, CRH, and ACTH levels (P<0.05, P<0.01), decreased protein and mRNA levels of IL-1β, IL-6, and TNF-α in serum and lung tissue (P<0.05, P<0.01), increased 5-HT levels in serum and hippocampus (P<0.05, P<0.01), and decreased relative expression of p-p38 MAPK and p-NF-κB p65 proteins in lung tissue were also noted (P<0.05, P<0.01), with the effects of Xiaoyaosan + pirfenidone being more significant. ConclusionChronic unpredictable stress exacerbates the progression of IPF in rats. The combination of Xiaoyaosan and pirfenidone not only improves depressive-like behavior but also alleviates pulmonary fibrosis, potentially through the regulation of the p38 MAPK/NF-κB signaling pathway and inhibition of excessive expression of inflammatory factors.
10.Investigation of occupational health of nuclear medicine radiation workers in Jiangsu Province, China, 2023

Objective To investigate the basic situation and occupational health conditions of nuclear medicine radiation workers in Jiangsu Province based on the research protocol developed by the Institute of Radiation Medicine, Chinese Academy of Medical Sciences for the nationwide study on the health effects of nuclear medicine radiation in China, understand the impact of occupational radiation on the physical health of nuclear medicine radiation workers, and provide a basis for improving the occupational protection of nuclear medicine radiation workers and reducing the risk of occupational radiation-related health issues. Methods A census approach was used to collect general data and occupational health information of nuclear medicine radiation workers in Jiangsu Province. The analysis focused on the abnormalities in physical examination indicators among radiation workers of different genders, ages, and working years to evaluate the health effects of occupational radiation exposure. Results The occupational health examination data of 472 nuclear medicine radiation workers were collected from 76 medical institutions in Jiangsu Province. The results showed that the detection rate of abnormal hypothyroidism in female workers (8.90%) was higher than that in male workers (2.54%) (P=0.028). With increasing working years, the detection rates of cataract and continuous decrease in white blood cell count increased (P<0.001). The multivariate logistic regression identified working years as a risk factor for cataract and continuous decrease in white blood cell count (OR=1.59, 95%CI=1.40-3.35, P=
