1.Countermeasure analysis on the education of large medical imaging equipment metrology for the major of Biomedical Engineering

Based on the analysis of current situation of the metrological education of large medical imaging equipment in colleges and universities at home, the advantages of major of Biomedical Engineering (BME) and the internal relation with the requirement of personnel training of metrology were systematically investigated. The necessity of carrying out education of large medical imaging equipment metrology for BME students in colleges and universities was elucidated. The specific thoughts and countermeasures were also proposed from curriculum establishment, teaching design, textbook compilation, conditional construction and teaching staff, respectively. The study is hopeful to provide guidance and help for the education of the large medical imaging equipment metrology in colleges and universities.

Pulmonary blast injury has become the main type of trauma in modern warfare, characterized by externally mild injuries but internally severe injuries, rapid disease progression, and a high rate of early death. The injury is complicated in clinical practice, often with multiple and compound injuries. Currently, there is a lack of effective protective materials, accurate injury detection instrument and portable monitoring and transportation equipment, standardized clinical treatment guidelines in various medical centers, and evidence-based guidelines at home and abroad, resulting in a high mortality in clinlcal practice. Therefore, the Trauma Branch of Chinese Medical Association and the Editorial Committee of Chinese Journal of Trauma organized military and civilian experts in related fields such as thoracic surgery and traumatic surgery to jointly develop the Clinical treatment guideline for pulmonary blast injury ( version 2023) by combining evidence for effectiveness and clinical first-line treatment experience. This guideline provided 16 recommended opinions surrounding definition, characteristics, pre-hospital diagnosis and treatment, and in-hospital treatment of pulmonary blast injury, hoping to provide a basis for the clinical treatment in hospitals at different levels.
3.Clinical application of neutrophil-to-lymphocyte ratio in predicting the prognosis of H7N9 avian influenza

Objective To explore the predictive value of neutrophil-to-lymphocyte ratio on the prognosis of H7N9 avian influenza.Methods A retrospective analysis was conducted on 28 H7N9 avian influenza patients (treatment group) at the First Affiliated Hospital of Soochow University from April 2013 to January 2016.Thirty healthy physical examiners in the same period were enrolled as the healthy control group.The 28 patients were followed up for half a year and divided into the improvement group (18 cases) and the death group (10 cases) according to the clinical prognosis.Inflammatory indicators including white blood cells (WBC),neutrophil (N),lymphocyte (L),monocytes (M),platelet (PLT),creatine kinase (CK),lactate dehydrogenase (LDH),high sensitive C reactive protein were collected at day 1,day 3 and week 1 of admission.Calculation of neutrophil-to-lymphocyte ratio (NLR),platelet-to-lymphocyte ratio (PLR),lymphocyte-to-monocyte ratio (LMR),△NLR3 (day 3 of admission NLR-on day 1 of admission NLR),△NLR7 (week 1 of admission NLR-day 3 of admission NLR) and so on calculating △PLR3,△PLR7,△LMR3,△LMR7.Differences of the above indicators between the improvement group and death group were compared.The measurement data with normal distribution were tested by t-test of two independent samples,and the count data with non-normal distribution were tested by Mann-Whitney U-test.Univariate and multivariate logistic regression analysis to explore the prognostic factors and the working characteristic curve of subjects was used to evaluate the predictive value of inflammatory response indexes for H7N9 avian influenza death.Results In the treatment group,the baseline WBC,L,N,PLT,the proportion of lymphocytes,neutrophils,monocytes,and NLR,PLR,and LMR were all statistically different compared with the healthy control group (all P <0.01).After treatment,day 3 NLR,△NLR3 in improvement group were both significantly decreased to 10.93 (15.71)and0.87 (-15.63),respectively when compared with death group (17.62[23.63] and 7.42[22.68],respectively) (Z =-2.16 and-2.014,respectively,both P<0.05).Day 7 NLR,△NLR7 in improved group were 6.51 (13.23) and-0.37 (-12.38),respectively,which were both lower than those of death group (27.90 [25.64] and 11.54 [-26.22]) with statistically significant differences (Z =-2.444 and -2.111,respectively,both P < 0.05).Multivariate logistic regression analysis indicates that △NLR3 is the main factor that affects the prognosis of the H7N9 infection (odds ratio [OR] =1.153,95% confidence interval [CI]:1.052-1.263,P =0.002).Reciver operating characteristic curve analysis showed that the area under the curve was 0.733 (95 % CI:0.532-0.935,P =0.044).Based on the principle of Youden index,the cutoff value of △NLR3 to predict the death risk of H7N9 avian influenza was 5.453 with sensitivity of 0.700 and the specificity of 0.722.The mortality was higher when △NLR3 was higher than 5.453.Conclusions Dynamic monitoring NLR,especially △NLR3 may reflect the condition and prognosis of H7N9 infection,which is an independent predictor of death.
4.Development and experimental test of ribbed, oxygen-enriched and inflatable tent

Objective To develop a ribbed,oxygen-enriched and inflatable tent to fulfill the requirements for mobile enriched oxygen supply under plateau conditions.Methods The tent was developed with ribbed skeleton,PVC material,nested structure,buffer door,channel for fresh air dispersion,exhaust recovery pipeline and etc,and the self-developed plateau oxygen enrichers were involved in to establish a mobile oxygen-enriched room.The enrichers were divided into four grades of 20,50,70 and 100 m3/h,which were used to execute 8-h oxygen-enriched fresh air dispersion in the tent.The change curves of the fractions for oxygen and carbon dioxide were compared and analyzed at 5 disperse points in the tent.Results Optimal effect was acquired with 20 m3/h oxygen enricher,the oxygen concentration reached 27% in the tent after 8-h dispersion,while the carbon dioxide fraction was 1 800 ppm (1 ppm=l×l0-6) and it took 3-h to raise oxygen concentration to 25%.Conclusion The tent combined with 20 m3/h oxygen enricher decreases the equivalent altitude from 5 000 m to less than 3 000 m,and is hopeful to be spreaded in the plateau troops after improvement.
5.Design of test phantom for MRI clinical quality control

Objective To design a test phantom for clinical routine quality control of magnetic resonance imaging (MRI).Methods The commonly used phantom such as Magphan SMR170 and Victoreen 76-903 were used for experiments with the same parameters,and the test data of all the modules were compared.The advantages and disadvantages of all the modules were analyzed,and an optimization scheme was proposed accordingly.Results The phantom could be applied to the detection of high-image-quality MRI machine,and solved the problems in positioning,size matching and etc.Conclusion The new phantom meets the demand of magnetic resonance imaging clinical routine testing,and provides a certain reference for phantom optimization design.
6.Development of new loading device for establishment of stress fracture animal models

Objective To develop a novel stress fracture animal model system based on dynamic cyclic mechanical loading.Methods Ulnae and tibiae of rats were respectively fixed using differently shaped fixtures (including a fixed holder and a movable holder).The axial mechanical loading was applied via a linear actuator based on LabVIEW control program.During the loading process,a load cell and a laser displacement transducer were used to detect the force and displacement changes in the limbs,respectively.The two signals were sampled at real time by PC-based LabVIEW data acquisition program.A compressive loading test (peak force of 50 N) was conducted to examine the consistency and reliability of cyclic loads after completing the loading system.The loading system and an ElectroForce 3220 mechanical testing machine were respectively used to measure and compare the Young's modulus of a standard ultra-high molecular weight polyethylene (UHMWPE)cylinder to calibrate the loading system.Results The compressive loading test (peak force of 50 N) demonstrated that the mechanical loading system was able to apply consistent loads with controllable intensity and time.The calibration experiment indicated the accuracy of the loading system.Conclusion The novel mechanical loading device has characteristics of precision,scientificity and reliability,and it is approaching the real development situation of stress fracture,which may provide a reliable experimental base for exploring the precautionary measures of stress fracture.
7.Variety and clinical significance of T lymphocyte subsets and cytokines in the peripheral blood of patients with avian influenza H7N9 virus infection

Objective To investigate the changes and clinical significance of peripheral blood T lymphocyte subsets and cytokines in influenza A (H7N9) virus infection patients.Methods Twenty-three patients with H7N9 virus infection who received treatment at the First Affiliated Hospital of Soochow University from April 2013 to April 2015 were enrolled as case group.Twenty healthy subjects in the outpatient clinic during the same period were selected as control group.The percentages of T lymphocyte subsets in the peripheral blood including CD3+CD4+ T cells, CD3+CD8+ T cells, regulatory T lymphocytes (Treg), Th17, CD3+CD8-interferon (IFN)-γ+ (Th1) cells and CD3+CD8-IL-4+ (Th2) cells were detected by flow cytometry.The changes of plasma cytokines including interleukin (IL)-8, IL-18, interferon-inducible protein (IP)-10, macrophage inflammatory protein (MIP)-1b were quantified by Bio-Plex Pro human cytokine Group Ⅰ 27-plex panel and Group Ⅱ 21-plex panel.The normality test was performed by Kolmogorov-Smirnov test.Two independent samples t test were used to compare the two groups.Results The percentages of CD3+CD4+ and CD3+CD8+ T cells before treatment in case group were significantly lower than control group ([27.90±10.19]% vs [38.75±6.78]%, t=-2.726, P=0.012;[14.82±7.72]% vs [22.79±6.12]%, t=-2.556, P=0.018), while the percentages of Th17 and Th2 before treatment in case group were significantly higher than control group ([2.64±1.40]% vs [0.29±0.21]%, t=4.668, P<0.001;[2.24±2.00]% vs [0.35±0.25]%, t=2.626, P=0.014).After treatment, 15 cases achieved improvement, among which the percentages of CD3+CD4+ T cells, CD3+CD8+ T cells and Treg after treatment were significantly higher than those before treatment ([36.54±9.20]% vs [25.86±7.22]%, t=-3.339, P=0.007;[22.70±5.68]% vs [15.08±8.80]%, t=-2.811, P=0.017;[6.08±1.70]% vs [3.26±1.64]%, t=-6.670, P<0.001), while the percentages of Th17 and Th1 cells after treatment were significantly lower than those before treatment ([2.12±1.28]% vs [2.76±1.33]%, t=2.220, P=0.048;[4.00±2.13]% vs [6.97±1.81]%, t=4.407, P=0.001).A total of 8 patients died with no significance differences of all the above mentioned immunological parameters after treatment compared with before treatment (all P>0.05).Before treatment, levels of IL-8, IL-18, IP-10 and MIP-1b of case group were significantly higher than those in control group (IL-8: [23.19±14.35] vs [12.78±6.76] ng/L, t=2.277, P=0.035;IL-18:[230.55±230.18] vs [72.80±27.91] ng/L, t=2.348, P=0.036;IP-10:[28 870.55±41 815.22] vs [1 356.13±1 093.10] ng/L, t=2.371, P=0.035;MIP-1b: [197.74±119.87] vs [118.51±41.86] ng/L, t=2.198, P=0.043).Conclusions Patients with H7N9 virus infection exhibit an imbalance of T lymphocyte subsets.It is very important to monitor the changes of T lymphocyte subsets in those patients for clinical prognosis.A storm of cytokines could exist during H7N9 virus infection, which may be the main reason for multiple organ failure.

English for science and technology (EST) is the language medium for academic communication among scientists and technologists. The expression of EST is characterized by concise-ness, correctness, objectiveness and understandability. The EST course is regarded as a key process for the undergraduate students to reinforce the English foundation and improve the capacity of English application after their College English study. In this paper, the authors introduced an interest-motivated, output-dominated, snowballing and interactive teaching strategy, according to the accumulated experi-ence of EST teaching in the past few years coupled with the unique medicine-engineering combined characteristics for the major of biomedical engineering.
10.Effect and clinical significance of glucocorticoid on CD4+CD25+ regulatory T cells in patients with hepatitis B virus-related pre-liver failure.

OBJECTIVETo investigate the effect and clinical significance of glucocorticoid on CD4+CD25+ regulatory T cells (Tregs) in patients with hepatitis B virus (HBV)-related pre-liver failure.
METHODSThe subjects of this study included 78 patients with pre-liver failure induced by HBV (cases) and 24 healthy individuals (controls). Among the 78 cases, 42 received glucocorticoid treatment and 36 did not. Between-group differences in Tregs (in peripheral blood) were evaluated by flow cytometry and statistical analysis.
RESULTSTwo weeks of glucocorticoid treatment led to an increase in Treg level compared to baseline (before therapy: 2.76 ± 0.73 vs. 3.88 ± 1.60). In addition, after the two weeks of glucocorticoid treatment, the Treg level of improved patients was significantly higher than that measured at baseline (before therapy: 2.70 ± 0.77 vs 3.97 ± 1.59, P < 0.05).
CONCLUSIONGlucocorticoids up-regulate the expression of Treg cells, which may contribute to the immunological mechanism that protects pre-liver failure patients from deterioration of their condition. Careful inspection and monitoring of Treg levels may help improve prognosis of these patients.
