2.Advances in population pharmacokinetics of meropenem in critically ill adult patients

Meropenem (MEM) is one of the important drugs for the treatment of severe infections, but the standard dose is often difficult to achieve an effective therapeutic concentration target. This article reviews the related studies on the population pharmacokinetics of MEM in patients with severe infection. It is found that the apparent volume of distribution (Vd) and clearance rate are the most important factors affecting the dose adjustment, and the factors affecting Vd include serum albumin, age, overall weight, shock status, and chest/abdomen/cerebrospinal fluid drainage. The main factors affecting the clearance rate were renal function, renal replacement therapy treatment mode and combination therapy. For adult patients with severe infections in China, MEM is recommended to be administered in an individualized manner based on glomerular filtration rate, with a dosage range of 500 to 1 500 mg given every 4 to 6 hours, and prolonged infusion is preferred. When the minimum inhibitory concentration (MIC) of the pathogenic bacteria reaches 64 mg/L, therapeutic drug monitoring is required. For therapeutic efficacy, it is essential to ensure that the trough concentration remains above the MIC; to prevent drug resistance, it should be maintained above 4×MIC. Regarding safety, it is recommended that the upper limit of the trough concentration be 32 mg/L, and blood sampling for monitoring can be conducted as early as after 1 to 2 doses of administration.

The pathogenesis of chronic sinusitis is complex, involving a variety of inflammatory cells and inflammatory mediators, and neutrophils play a key role in the pathological process of chronic sinusitis. Understanding the molecular basis of the pathogenesis of CRS is helpful for the precise diagnosis and treatment of chronic sinusitis. This article will systematically review the mechanism of neutrophils in the inflammatory response of the body, their role in the pathogenesis and treatment progress in CRS, and look forward to future research directions.

OBJECTIVES:
To evaluate the protective effect of Schistosoma japonicum cystatin (rSj-Cystatin) in a mouse mode of "two-hit" sepsis.
METHODS:
Sixty male C57BL/6 mice randomized equally into sham-operated group, protein group, "two-hit" modeling group, and protein intervention group. In the former two groups, the mice received an intraperitoneal injection of 100 μL PBS followed by exposure of the cecum and then by intraperitoneal injection of 100 μL PBS or 25 μg rSj-Cystatin 30 min later; In the latter two groups, 100 μL PBS containing LPS (5 mg/kg) was injected intraperitoneally 24 h before cecal ligation and puncture (CLP), and 100 μL PBS or 25 μg rSj-Cystatin were injected 30 min after CLP. At 12 h after rSj-Cystatin treatment, 6 mice from each group were sacrificed for detection of TNF-α, IL-6, IL-10, TGF-β, iNOS and Arg-1 in the serum, spleen, liver, lung and kidney tissues using ELISA, for examinations of liver, lung and kidney pathologies with HE staining, and for analysis of CD3+CD4+CD25+Foxp3+ T cell percentage in the spleen using flow cytometry. The remaining mice were observed for general condition and 72-h survival.
RESULTS:
The 72-h survival rates in the 4 groups were 100%, 100%, 0% and 20%, respectively, showing significant differences between the latter two groups. The mouse models of "two-hit" sepsis exhibited obvious tissue pathologies and significant elevations of TNF-α and IL-6 in both the serum and tissue homogenate, which were significantly ameliorated by rSj-Cystatin treatment. Treatment with rSj-Cystatin also increased IL-10 and TGF-β levels and spleen CD3+CD4+CD25+Foxp3+ T cell percentage. The septic mouse models also showed increased iNOS levels in all the detected tissues and a decreased Arg-1 level in the kidney, and these changes were obviously improved by rSj-Cystatin treatment.
CONCLUSIONS
rSj-Cystatin has a protective effect against "two-hit" sepsis in mice by regulating the inflammatory microenvironment.

6.13-Docosenamide Enhances Oligodendrocyte Precursor Cell Differentiation via USP33-Mediated Deubiquitination of CNR1 in Chronic Cerebral Hypoperfusion.

Chronic cerebral hypoperfusion leads to white matter injury (WMI), which plays a significant role in contributing to vascular cognitive impairment. While 13-docosenamide is a type of fatty acid amide, it remains unclear whether it has therapeutic effects on chronic cerebral hypoperfusion. In this study, we conducted bilateral common carotid artery stenosis (BCAS) surgery to simulate chronic cerebral hypoperfusion-induced WMI and cognitive impairment. Our findings showed that 13-docosenamide alleviates WMI and cognitive impairment in BCAS mice. Mechanistically, 13-docosenamide specifically binds to cannabinoid receptor 1 (CNR1) in oligodendrocyte precursor cells (OPCs). This interaction results in an upregulation of ubiquitin-specific peptidase 33 (USP33)-mediated CNR1 deubiquitination, subsequently increasing CNR1 protein expression, activating the phosphorylation of the AKT/mTOR pathway, and promoting the differentiation of OPCs. In conclusion, our study suggests that 13-docosenamide can ameliorate chronic cerebral hypoperfusion-induced WMI and cognitive impairment by enhancing OPC differentiation and could serve as a potential therapeutic drug.

8.Efficacy and safety of tetrandrine in treatment of rheumatoid arthritis:a meta-analysis

Objective:To explore the efficacy and safety of tetrandrine in the treatment of rheumatoid arthritis.Methods:Randomized controlled studies of tetrandrine in the treatment of rheumatoid arthritis were searched in CNKI,VIP,Wanfang database,SinoMed,PubMed,Springer,Web of Science and Cochrane Central Register of Controlled Trails databases.A meta-analysis was conducted using R 3.5.3 software to evaluate the clinical outcomes,including the total effective rate,erythrocyte sedimentation rate(ESR),C-reactive protein(CRP),rheumatoid factor(RF),visual analogue scale(VAS),disease activity score(DAS),tender joint count(TJC),swollen joint count(SJC),and morning stiffness duration,as well as adverse events of rheumatoid arthritis patients.Results:A total of 10 articles were included in the study.The meta-analysis indicated that tetrandrine significantly improved the total effective rate(OR=3.27,95%CI:2.01-5.37,P<0.01),ESR(SMD=1.12,95%CI:0.06-2.19,P<0.05),CRP(SMD=0.75,95%CI:0.28-1.22,P<0.01),VAS(SMD=0.64,95%CI:0.29-1.00,P<0.01),TJC(SMD=1.16,95%CI:0.58-1.74,P<0.01),SJC(SMD=0.85,95%CI:0.40-1.31,P<0.01),and morning stiffness(SMD=1.09,95%CI:0.68-1.50,P<0.01).However,no statistical significance was found in RF(SMD=1.70,95%CI:-1.10-4.51,P>0.05)and DAS(SMD=0.26,95%CI:-0.59-1.11,P>0.05).The overall incidence of adverse events associated with tetrandrine treatment for rheumatoid arthritis was 20%(95%CI:12%-27%,I2=60%,P<0.05),with mild severity and favorable outcomes.Conclusion:Tetrandrine is effective in the treatment of RA patients with a mild degree of adverse events.
9.Visual analysis of the research hotspots and trends of emergency nursing training at home and abroad based on CiteSpace

Objective:To analyze the status quo, hotspots and fronts of emergency nursing training research at home and abroad in the past ten years, and to provide reference and ideas for the efficient development of emergency nursing training in China.Methods:CiteSpace 6.2.R2 software was used to visually analyze the Chinese and English literatures on emergency nursing training included in CNKI and Web of Science core databases from January 1, 2013 to June 1, 2023.Results:A total of 1 177 Chinese literatures and 1 163 English literatures were included. The number of foreign articles in this field increased year by year, while the number of domestic articles showed a downward trend since 2018. There were many stable core author groups and core institution groups in foreign countries, while there was less cooperation among domestic authors and institutions. The common research hotspots and frontiers at home and abroad focused on broadening the training audience of emergency nursing, innovating the training methods of emergency nursing, strengthening the evaluation of the effect of emergency nursing training, and paying attention to the training experience and needs of nurses. Foreign researches also focused on specialized nurses, interprofessional education and nurses′mental health, etc, and the research direction was diversified.Conclusions:The development stages of emergency nursing training researches at home and abroad are different, and the research hotspots are different. In the future, we should learn from foreign research, strengthen interdisciplinary cooperation, improve the depth and breadth of research, and strengthen the cooperation between authors, institutions and countries to promote the high-quality development of emergency nursing training research in China.
10.Effect of Warming-Needle Moxibustion on Intestinal Flora and Anal Function in Patients with Colorectal Cancer After Enterostomy

Objective To observe the effect of warming-needle moxibustion on intestinal flora and anal function in patients after enterostomy.Methods A total of 94 patients with colorectal cancer enterostomy were randomly divided into observation group and control group,47 cases in each group.The control group was given routine treatment after operation,and the observation group was given warming-needle moxibustion treatment on the basis of the control group.Both groups were treated for two weeks.After two weeks of treatment,the changes of the relative abundance of Enterococcus,Escherichia coli,Lactobacillus,Bifidobacterium before and after treatment in the two groups,as well as the first exhaust time,the first feeding time and the hospitalization time were observed.The changes of constipation score scale(Wexner),low anterior resection syndrome(LARS)score,tumor necrosis factor α(TNF-α),interleukin 8(IL-8)and C-reactive protein(CRP)were compared before and after treatment between the two groups,and the incidence of complications in the two groups was evaluated.Results(1)The first exhaust time,first feeding time and hospitalization time in the observation group were significantly less than those in the control group,and the differences were statistically significant(P<0.05).(2)After treatment,the relative abundance of Enterococcus,Escherichia coli,Lactobacillus,Bifidobacterium in the two groups was significantly improved(P<0.05),and the observation group was significantly superior to the control group in improving the relative abundance of Enterococcus,Escherichia coli,Lactobacillus,Bifidobacterium(P<0.05).(3)After treatment,the Wexner score and LARS score of anal function in the two groups were significantly improved(P<0.05),and the improvement of Wexner score and LARS score of anal function in the observation group was significantly superior to that in the control group,the differences were statistically significant(P<0.05).(4)After treatment,the levels of serum TNF-α,IL-8 and CRP in the two groups were significantly improved(P<0.05),and the improvement of serum TNF-α,IL-8 and CRP levels in the observation group was significantly superior to that in the control group,the differences were statistically significant(P<0.05).(5)The total incidence of complications in the observation group was significantly lower than that in the control group,and the difference was statistically significant(P<0.05).Conclusion The treatment of warming-needle moxibustion for patients after enterostomy can effectively shorten the postoperative recovery time,regulate intestinal flora,improve anal function,and reduce the incidence of complications such as peristomal infection,with significant clinical effect.
