1.Analysis of Clonal Rearrangement Characteristics and Clinical Application Value of IGH in B-cell Non-Hodgkin's Lymphoma by Next-generation Sequencing

Objective To investigate the clonal rearrangement characteristics and clinical application value of IGH gene in B-cell non-Hodgkin's lymphoma(B-NHL).Methods Demographic and clinical data as well as IGH sequencing results of 55 patients with B-NHL who underwent next-generation sequencing(NGS)testing were collected,and IGH gene clonal rearrangement was detected.The characteristics of IGH gene clonal rearrangement,IGHV gene usage,and the clinical application value of NGS for IGH clonal rearrangement were analyzed.Results Among 55 patients with B-NHL and IGH clonal rearrangement,single dominant clones were mainly detected(85.45%,47/55);a few patients had two(12.73%,7/55)and three dominant clones(1.82%,1/55).In terms of preference for IGHV gene usage,IGHV3 gene had the highest frequency of access in B-NHL,followed by IGHV4.Among the IGHV subtypes,IGHV3-23 had the highest frequency in chronic lymphocytic leukemia/small lymphocytic lymphoma,and IGHV4-34 had the highest frequency in primary central nervous system diffuse large B-cell lymphoma and not otherwise specified diffuse large B-cell lymphoma.Conclusion A preference for IGHV gene usage in clonal rearr-angement of IGH genes is noted in B-NHL patients with different pathological types.Using NGS to detect IGHclonal rearrangement can identify subclones and clonal correlations,and assist in disease diagnosis.
2.Imaging analysis and clinical follow-up of Ewing sarcoma of bone

Objective:To analyze and summarize the imaging characteristics and clinical follow-up results of Ewing sarcoma of bone.Methods:The imaging data of 23 patients with Ewing sarcoma confirmed by pathology who treatment in Drum Tower Hospital Affiliated to Nanjing University School of Medicine from May 2010 to October 2021 were retrospectively analyzed, and clinical follow-up was performed.Results:Of the 23 patients with Ewing sarcoma of the bone in this group, a total of 18 patients had follow-up results and 5 cases were lost to follow-up. Of the 18 cases, 6 cases died and 12 cases survived. The main cause of death was lung metastasis. There were 27 lesions in total, femoral diaphysis was the most common site of the disease; bone structure destruction and soft tissue mass shadows could be seen in the images of each lesion. Periosteal reaction could be seen in most of the lesions (92.59%, 25/27). There were certain differences in signs of bone destruction and periosteal reaction between different bone types.Conclusions:The imaging of Ewing sarcoma of bone mainly manifests various types of bone destruction, soft tissue masses and periosteal reactions. Ewing sarcoma of bone is mainly bone marrow metastasis and lung metastasis, and lung metastasis is the main cause of death.
3.Inhibition of histone methyltransferase PRMT5 attenuates cisplatin-induced hearing loss through the PI3K/Akt-mediated mitochondrial apoptotic pathway

This study aimed to evaluate the therapeutic potential of inhibiting protein arginine methyltransferase 5(PRMT5)in cisplatin-induced hearing loss.The effects of PRMT5 inhibition on cisplatin-induced auditory injury were determined using immunohistochemistry,apoptosis assays,and auditory brainstem response.The mechanism of PRMT5 inhibition on hair cell survival was assessed using RNA-seq and Cleavage Under Targets and Tagment-quantitative polymerase chain reaction(CUT&Tag-qPCR)analyses in the HEI-OC1 cell line.Pharmacological inhibition of PRMT5 significantly alleviated cisplatin-induced damage to hair cells and spiral ganglion neurons in the cochlea and decreased apoptosis by protecting mitochondrial function and preventing the accumulation of reactive oxygen species.CUT&Tag-qPCR analysis demonstrated that inhibition of PRMT5 in HEI-OC1 cells reduced the accumulation of H4R3me2s/H3R8me2s marks at the promoter region of the Pik3ca gene,thus activating the expression of Pik3ca.These findings suggest that PRMT5 inhibitors have strong potential as agents against cisplatin-induced ototoxicity and can lay the foundation for further research on treatment strategies of hearing loss.
4.Study on a Core Outcome Set(COS)of Myasthenia Gravis in Clinical Trials of Chinese Medicine

Objective To standardize the selection of clinical research outcome indicators,which can objectively evaluate the clinical efficacy or effect of traditional Chinese medicine in the treatment of myasthenia gravis.This study aims to standardize the construction of the core outcome set of clinical research of traditional Chinese medicine in the treatment of myasthenia gravis.Methods We followed the core outcome set development specification(COS-STAD)to carry out research,established a research working group,which set up a Delphi-method advisory group.Two graduate students of working group conducted a document research and meetings of patients to establishe an outcome set item pool of myasthenia gravis in clinical trials of Chinese medicine under the instruction of other members.With the questionnaire based on the content of item pool,we then carried out Delphi-method expert consultations and a consensus meeting.Results The core outcome set of clinical research on myasthenia gravis treated with traditional Chinese medicine included five outcome domains:endpoint outcome,myasthenia gravis symptom evaluation,medication evaluation,quality of life evaluation and safety outcome;Nine outcome measures:recurrence rate,incidence of hormone complications,incidence of crisis,QMGS scale(MGFA quantitative myasthenia gravis score),daily activity scale of MG patients(ADL),analysis of immunosuppressant dosage,analysis of glucocorticoid dosage,analysis of cholinesterase inhibitor dosage,and incidence of adverse events.Conclusion The five outcome domains and nine outcome measures included in the core outcome set can be used as outcome options for the efficacy evaluation of myasthenia gravis clinical research.
5.Effects of subspecialty group management mode in Operating Room nursing for patients with laryngeal cancer

Objective:To explore the effects of the subspecialty group management mode in Operating Room nursing for patients with laryngeal cancer.Methods:Totally 70 patients with laryngeal cancer admitted to Henan Provincial People's Hospital from January to December 2020 who underwent laryngectomy concurrently with routine surgical nursing were selected as the control group, and 70 laryngeal cancer patients admitted from January to December 2021 who underwent laryngectomy and the subspecialty group management concurrently as the research group by convenience sampling. 84 nurses from the Operating Room of the hospital, including 14 specialist team leaders, were selected for the study. The preoperative waiting time, operation time, operation compliance rate, and nursing satisfaction were compared between the two groups. The nursing competence scores of Operating Room nurses were evaluated and compared before and after the introduction of the subspecialty group nursing management mode.Results:Patients in the research group reported a shorter preoperative waiting time and operation time, higher compliance rates in preoperative device preparation, drug preparation, body position placement, venipuncture, catheter indwelling, device cleaning, and higher nursing satisfaction than patients in the control group, with statistically significant differences ( P<0.05) . One year after the implementation of the subspecialty group nursing management mode, the nursing core competence scores of the specialist team leader and the post competency scores of all nurses in the Operating Room were higher than those before the implementation, and the differences were statistically significant ( P<0.05) . Conclusions:The subspecialty group management mode can significantly improve the nursing competence of Operating Room nurses, shorten the preoperative waiting time and operation time, and improve the compliance rate of surgical work and nursing satisfaction.
6.Effect of simple knee trainer on knee active flexion for hemiplegics after stroke

Objective To observe the clinical efficacy of simple knee trainer on active knee flexion in patients with hemiplegia after stroke.Methods From March, 2020 to May, 2021, 46 hemiplegics after stroke in Beijing Bo'ai Hospital were randomly divided into control group (n = 23) and experimental group (n = 23). Both groups accepted routine rehabilitation training, and the experimental group accepted knee flexion exercise training in sitting position through the simple knee trainer, for eight weeks. They were measured active knee flexion angle in sitting position and maximum flexion angle in walking before and after training, as well as Holden Functional Ambulation Category (FAC).Results Both active knee flexion angle in sitting position and maximum flexion angle in walking significantly increased in both groups after training (|t| > 6.991, P < 0.001), and increased more in the experimental group than in the control group (t > 2.185, P < 0.05). The grade of FAC also improved in both groups (|Z| > 2.828, P < 0.01), and no significant difference was found between groups (Z = -0.821, P = 0.412).Conclusion The application of simple knee trainer can effectively improve the active range of motion of knee, which may be used in clinic- and community-based rehabilitation.

Objective:To analyze the risk factors of bronchopulmonary dysplasia(BPD)in very preterm infants(VPI), and to provide scientific basis for the prevention and treatment of BPD in VPI.Methods:A prospective multicenter study was designed to collect the clinical data of VPI in department of neonatology of 28 hospitals in 7 regions from September 2019 to December 2020.According to the continuous oxygen dependence at 28 days after birth, VPI were divided into non BPD group and BPD group, and the risk factors of BPD in VPI were analyzed.Results:A total of 2 514 cases of VPI including 1 364 cases without BPD and 1 150 cases with BPD were enrolled.The incidence of BPD was 45.7%.The smaller the gestational age and weight, the higher the incidence of BPD( P<0.001). Compared with non BPD group, the average birth age, weight and cesarean section rate in BPD group were lower, and the incidence of male infants, small for gestational age and 5-minute apgar score≤7 were higher( P<0.01). In BPD group, the incidences of neonatal respiratory distress syndrome(NRDS), hemodynamically significant patent ductus arteriosus, retinopathy of prematurity, feeding intolerance, extrauterine growth restriction, grade Ⅲ~Ⅳ intracranial hemorrhage, anemia, early-onset and late-onset sepsis, nosocomial infection, parenteral nutrition-associated cholestasis were higher( P<0.05), the use of pulmonary surfactant(PS), postnatal hormone exposure, anemia and blood transfusion were also higher, and the time of invasive and non-invasive mechanical ventilation, oxygen use and total hospital stay were longer( P<0.001). The time of starting enteral nutrition, cumulative fasting days, days of reaching total enteral nutrition, days of continuous parenteral nutrition, days of reaching 110 kcal/(kg·d) total calorie, days of reaching 110 kcal/(kg·d) oral calorie were longer and the breastfeeding rate was lower in BPD group than those in non BPD group( P<0.001). The cumulative doses of amino acid and fat emulsion during the first week of hospitalization were higher in BPD group( P<0.001). Multivariate Logistic regression analysis showed that NRDS, invasive mechanical ventilation, age of reaching total enteral nutrition, anemia and blood transfusion were the independent risk factors for BPD in VPI, and older gestational age was the protective factor for BPD. Conclusion:Strengthening perinatal management, avoiding premature delivery and severe NRDS, shortening the time of invasive mechanical ventilation, paying attention to enteral nutrition management, reaching whole intestinal feeding as soon as possible, and strictly mastering the indications of blood transfusion are very important to reduce the incidence of BPD in VPI.

As the incidence of urinary calculi continues to increase, accurate assessment of patient-reported outcomes has become increasingly prominent in the formulation of disease nursing measures, the evaluation of the effectiveness of diagnosis, treatment and nursing, and the improvement of nursing quality. This article reviewed the importance of the patient-reported outcome in patients with urinary tract stones and the five specific measurement scales, and introduced the characteristics, psychometric attributes and application status of each scale, so as to provide a basis and reference for the scientific selection of tools.
9. Progress of inhaled nitric oxide on treatment of persistent pulmonary hypertension of new-born

Persistent pulmonary hypertension is a serious disease among new-borns.Despite the variety of causes, similar physiopathologic changes characterize this syndrome: a persistently raised pulmonary vascular resistance after birth, which leads to severe hypoxemia due to extrapulmonary shunting.The fundamental treatment is to reduce pulmonary vascular pressure and improve hemodynamics.Modern treatments such as inhaled nitric oxide, high-frequency oscillation ventilation, extracorporeal membrane oxygenation, and/or other pulmonary vasodilators could reduce mortality in neonatal persisitent pulmonary hypertension(PPHN). Inhaled nitric oxide is the main method for the treatment of PPHN, which can effectively improve oxygenation and reduce the need for extracorporeal membrane oxygenation(ECMO). The effect of inhaled nitric oxide is different with the gestational age changed.This article aims to summarize the clinical research progress of inhaled nitric oxide in the treatment of PPHN.
10. Efficacy of pulmonary surfactant combined with caffeine citrate on neonatal respiratory distress syndrome treated with synchronized nasal intermittent positive pressure ventilation

Objective:
To investigate the clinical effects of pulmonary surfactant(PS) combined with caffeine citrate on neonatal respiratory distress syndrome(NRDS) treated with synchronized nasal intermittent positive pressure ventilation(SNIPPV).
Methods:
We collected and analyzed 99 neonates who were diagnosed with NRDS and required SNIPPV treatment from January 2016 to June 2019 in NICU of Shengjing Hospital of China Medical University.According to the different treatment, they were divided into PS+ citrate caffeine combination group and control group.The control group(53 neonates)was treated by PS alone.The combination group(46 neonates)was treated with PS combined with caffeine citrate.The indexes of ventilator time, the time of using oxygen, length of hospital stay and complications were compared between the two groups.
Results:
The indexes of ventilator time, the time of using oxygen and length of hospital stay of combination group were significantly shorter than those of the control group.The differences were statistically significant(
