1.Clinical application of turbo gradient and spin echo-BLADE diffusion weighted imaging at the sellar region

Objective:To compare the image quality of turbo gradient and spin echo-BLADE diffusion weighted imaging (TGSE-BLADE-DWI) with that of readout segmentation of long variable echo-trains (RESOLVE) at the sellar region.Methods:From September 15 th, 2019 to February 15 th, 2020, 38 patients with suspected sellar abnormalities were enrolled prospectively to perform RESOLVE and TGSE-BLADE-DWI at a 3.0 T MR scanner in Department of Radiology, Union Hospital, Tongji Medical College of Huazhong University of Science and Technology. Totally, 32 patients were identified with sellar lesions. The DWI images were evaluated subjectively and objectively. Two experienced radiologists scored images of the two DWI methods for anatomical structures (including the internal carotid arteries, optic chiasm, pituitary stalk and pituitary gland), lesion conspicuity, susceptibility artifacts, geometric distortions and overall image quality using a five-point scale respectively. Objective parameters on the images of the two DWI methods were analyzed, including lesion size, signal to noise ratio (SNR) and apparent diffusion coefficient (ADC). The consistency of subjective scores of two radiologists was tested by Kappa test. Paired t-test, Wilcoxon signed rank test, Kappa statistics were used for statistical evaluation. Results:TGSE-BLADE-DWI performed significantly better than RESOLVE in depicting the sellar anatomical structures, lesion conspicuity, geometric distortion and overall image quality (all P<0.05). There were no significant differences in SNR, maximum longitudinal diameter of lesions and ADC of lesions between the two DWI methods(all P>0.05). The maximum transverse diameter measured by TGSE-BLADE-DWI was significantly smaller than that of RESOLVE ( Z=3.31, P=0.001). Conclusions:Compared with RESOLVE, TGSE-BLADE-DWI is superior in depicting the anatomical structures, decreasing susceptibility artifacts and geometric distortions at the sellar region and effectively improves the image quality of DWI, which has great value in clinical applications.

Myoclonus dystonia syndrome (MDS) is an inherited movement disorder, and most MDS-related mutations have so far been found in the ε-sarcoglycan (SGCE) coding gene. By generating SGCE-knockout (KO) and human 237 C > T mutation knock-in (KI) mice, we showed here that both KO and KI mice exerted typical movement defects similar to those of MDS patients. SGCE promoted filopodia development in vitro and inhibited excitatory synapse formation both in vivo and in vitro. Loss of function of SGCE leading to excessive excitatory synapses that may ultimately contribute to MDS pathology. Indeed, using a zebrafish MDS model, we found that among 1700 screened chemical compounds, Vigabatrin was the most potent in readily reversing MDS symptoms of mouse disease models. Our study strengthens the notion that mutations of SGCE lead to MDS and most likely, SGCE functions to brake synaptogenesis in the CNS.
4. Effect of Hei Xiaoyaosan on Expressions of APP,PERK in Hippocampus Area of Alzheimer's Dementia Mice

Objective: To study the effect of Hei Xiaoyaosan on endoplasmic reticulum stress in hippocampal neurons of Alzheimer's disease (AD) model mice, including behavioral, histopathology and amyloid precursor protein (APP), protein kinase endoplasmic reticulum kinase (PERK) expressions. Method: The 42 4-month-old SPF-grade double transgenic (APP/PS1) mice were randomly divided into the high-dose group and the low-dose group, the donepezil hydrochloride group and the model group, and 10 C57BL mice of the same age were used as the blank group. Firstly, they were adapted to the environment for one week. After 2 months of treatment with different drug interventions, Morris water maze behavior was used to test the learning and memory abilities of each group of mice. After 1 month of treatment, histopathological changes in the hippocampus of each group of mice were observed by light microscopy. The expressions of APP, PERK protein and mRNA in the endoplasmic reticulum of hippocampus were detected by immunohistochemistry (IHC) and real-time fluorescent quantitative polymerase chain reaction (Real-time PCR). Result: After drug intervention, compared with the blank group, the escape latency of the AD model group was significantly prolonged (P<0.01), the neurons in the hippocampus of the model group were severely damaged, and the expressions of APP and PERK in the model group were significantly increased (P<0.01). In the comparison model group, the escape latency of the mice in each treatment group was significantly shortened (P<0.05,P<0.01), the degree of neuronal damage in the hippocampus of each treatment group was alleviated, and the positive expressions of APP and PERK in each treatment group were weakened (P<0.05). Conclusion: Hei Xiaoyaosan can significantly improve the learning and memory abilities of AD mice, which may be related to the reduction of the excessive stress response of endoplasmic reticulum to alleviate cognitive impairment in AD mice.
5. Effect of Hei Xiaoyaosan on Expressions of Aβ1-42, GSK-3β, NEP, IDE in Hippocampus Area of Alzheimer's Dementia Mice

Objective: To observe the effect of Hei Xiaoyaosan on expressions of β-amyloid 1-42 peptide(Aβ1-42),glycogen synthase kinase-3β(GSK-3β),neprilysin(NEP),insulin-degrading enzyme(IDE) in the hippocampus area of Alzheimer's dementia mice. Method: After weighing, 42 APP/PSI bivalent transgenic mice were randomly divided into 4 groups:10 mice in the model group, 10 mice in the positive drug control group, 11 mice in the high-dose Hei Xiaoyaosan group, and 11 mice in the low-dose Hei Xiaoyaosan group; 10 wild C57BL/6J mice of the same age and strain were used for negative control group. Drugs were administered to mice by gavage once a day for 12 weeks. Then the behavior of all the mice were detected by Morris water maze, the morphological changes in hippocampal neurons were observed by hematoxylineosin(HE) staining, the expressions of Aβ1-42, GSK-3β, NEP and IDE proteins in hippocampus were detected by immunohistochemistry. Result: After 3 months of treatment, compared with negative control groups, the average escaping latency periods prolonged significantly, and the number of cross-platform was decreased significantly in model group (P<0.01), the expressions of Aβ1-42 and GSK-3β proteins in model mice hippocampus were significantly increased (P<0.01), the expressions of NEP and IDE proteins in model mice hippocampus were significantly decreased (P<0.01), suggesting serious damage of hippocampal nerve cells in model group mice according to HE staining; compared with the model group, the escape latency of drug groups were significantly shortened, and the number of crossing platforms was significantly increased (P<0.05), the expressions of Aβ1-42 and GSK-3β proteins in the hippocampus of drug groups were significantly decreased (P<0.01), the expressions of NEP and IDE proteins in the hippocampus of drug groups were significantly increased (P<0.05,P<0.01), suggesting the alleviation in the damage of hippocampal nerve cells in drug groups. Conclusion: Hei Xiaoyaosan can significantly improve the learning and memory abilities of AD mice, which may be related to the reduction of cognitive impairment in AD mice by regulating abnormal deposition and degradating Aβ in the hippocampus.
6.Preparation and Quality Evaluation of Jinlei Capsule

OBJECTIVE: To prepare Jinlei capsule and evaluate its quality. METHODS: Wet granulation was adopted. The molding technology of Jinlei capsule content granule was optimized with drug-excipient ratio, excipient ratio and ethanol volume fraction as factors, using comprehensive score of particle forming rate, moisture content and fluidity as the evaluation index. According to 2015 edition of Chinese Pharmacopoeia (part Ⅳ), the characters, moisture, volume difference, disintegration time limit were checked. Qualitative identification of Gentianopsis paludosa and Lysimachia christinale were analysed by TLC, and quantitative analysis of luteolin and kaempferol were analysed by HPLC. RESULTS: The optimal molding technology of Jinlei capsule content granule was that the maltodextrin and the micro-silica gel are mixed as a mixed auxiliary material according to 10 ∶ 1 and then mixed with the drug powder by 1 ∶ 0.5, using the wet granulation method with 90% ethanol as wetting agent. The characters, moisture, volume difference and disintegration time limit of Jinlei capsule were in line with Chinese Pharmacopoeia. TLC showed the same color spots on the corresponding positions of the reference chromatogram. The linear range of luteolin and kaempferol were 4.4-88.0 μg/mL and 9.6-96.0 μg/mL (all r=0.999 9). RSD of precision (n=6), reproducibility (n=6) and stability (18 h, n=7) tests were all lower than 2.5%. The average recoveries were 95.74% and 99.77% (RSD=1.50%, 2.72%, n=6); the content of them were 2.52, 0.34 mg/g. CONCLUSIONS: The optimal molding technology of Jinlei capsule is stable and feasible; prepared Jinlei capsule in controllable in quality.
7.Association between frailty and risk of postoperative delirium:a systematic review

Objective To systematically evaluate the association between frailty and risk of postoperative delirium.Methods Systematic review of literature was conducted using eight electronic databases:PubMed,Embase,CENTRAL,Web of Science,CNKI,CBM,VIP and Wanfang Data,and prospective cohort studies about association between frailty and postoperative delirium published before March 2017 were included.Two authors independently screened the literature,extracted the data,and assessed the quality using NOS Scale,and meta-analysis was conducted by RevMan 5.3 software.Results A total of eight studies involving 846 patients were included in this review.Meta-analysis showed that:frailty was associated with higher risk of postoperative delirium [OR=3.63,95%CI (2.06,6.40),P<0.001].Subgroup analysis showed that:①Frailty assessment tool:Fried frailty criteria and other frailty assessment were associated with increased risk of postoperative delirium[OR=5.81,95%CI(3.54,9.77),P<0.001],[OR=1.76,95%CI(1.06,2.92),P=0.03].②Age:frailty patients aged 60~74 had increased risk of postoperative delirium [OR=5.05,95%CI (3.14,8.12),P<0.001],but for patients aged ≥ 75,frailty wasn't associated with postoperative delirium [0R=1.73,95%CI (0.99,3.00),P=0.05].③Type of surgery:for cardiovascular and non-cardiovascular surgery patients,frailty was associated with increased risk of postoperative delirium [OR=3.40,95%CI (1.64,7.05),P<0.001],[OR=4.95,95%CI (2.41,10.16),P<0.001].Conclusion Frailty can increase the risk of postoperative delirium.In consideration of quantity and quality of included studies,the conclusion needs to be validated by multi-centered prospective cohort studies with large sample size.
8.Influence of different cultivation measures on the content of three free carbohydrates of Codonopsis Radix.

To provide a scientific evidence for the quality control of Codonopsis Radix, a method was established for determining the content of three free carbohydrates of Codonopsis Radix. The developed method showed good linearity. The calibration curves were linear within the range of 2.312 5-18.500 0 μg for sucrose, 1.500 0-12.000 0 μg for glucose, and 2.000 0-16.000 0 μg for fructose, resgectwely. The recoveries varied between 96.31%-101.8%. The method is simple, accurate and reproducible, and can be used for determining the content of sucrose, glucose and fructose of Codonopsis Radix. The results showed that different cultivation measures had an effect on the content of three free carbohydrates of Codonopsis Radix. According to the content of sucrose, using Zhuanggenling>not using Zhuanggenling. While, not pinching, shelving>not pinching, not shelving>pinching, not shelving>pinching, shelving. According to the content of glucose and fructose, not using Zhuanggenling>using Zhuanggenling. While, pinching, shelving>not pinching, not shelving>not pinching, shelving>pinching, not shelving. In consideration of the differences of sweetness and content of the three free carbohydrates in Codonopsis Radix, we recommend that the content of free carbohydrates could be considered as the marker to evaluate the quality of Codonopsis Radix.
9.Comparisons of the specific absorption rate in 3.0T MR brain examination

Objective To investigate the specific absorption rate (SAR)in 3.0T MR brain examination.Methods All 280 patients underwent 3 .0 MR brain examination.And the patients were divided into three groups:a group of 0-3 years old without hands intersection, inluding 40 cases through conventional MRI examination,40 cases through MRA,as well as 40 cases through DWI;a group of more than 20 years old without hands intersection,including 40 cases through conventional MRI examination,40 cases through MRA,as well as 40 cases through DWI;a group of more than 20 years old with hands intersection,consisting of 40 cases through MRA.The SAR values of each group were recorded,respectively.And the independent samples t test was used to compare statistical difference of different SAR values between groups.Results Compared the group of 0-3 years old without hands intersection and the group of more than 20 years old without hands intersection,the SAR values of all six sequences in front group were significantly higher than that those in latter one,but not exceed the safety standard of 3 w/kg in the United States FDA.There was no significant statistical differences in the SAR values of two groups which more than 20 years old with hands intersection or not (P>0.05).Conclusion The SAR values of each sequence in the group of 0-3 years old without hands intersection were significantly higher than those in the group of more than 20 years old without hands intersection,but in a safe and reasonable range.The brain scanning with two hands intersect of 20 years old and above,and there was no significant increase on the SAR values in the brain examination of the group of more than 20 years old with hands intersection.So it is of great significance to guide the daily work,settle doubts and improve success rates of the MR examination.
10.Influence of different cultivation measures on chemical quality of Codonopsis Radix.

To observe the influence of different cultivation measures on the chemical constituents of Codonopsis Radix and provide reference for its reasonable cultivation, Codonopsis Radix samples cultivated by different cultivation measures were collected from the planting base in Min county,and their quality were evaluated by establishing HPLC fingerprint and determining the content of lobetyolin and Codonopsis Radix polysaccharide. The results show that different cultivation measures have an effect on the quality of Codonopsis Radix and the contents of lobetyolin and Codonopsis Radix polysaccharide are obviously different. According to the content of lobetyolin, not using Zhuanggenling>using Zhuanggenling. While, not pinching, shelving>not pinching, not shelving>pinching, shelving>pinching, not shelving. According to the content of Codonopsis Radix polysaccharide, not using Zhuanggenling>using Zhuanggenling. While, not pinching, shelving>not pinching, not shelving>pinching, not shelving>pinching, shelving. Based on the chemical quality evaluation results, the appropriate cultivation measure of Codonopsis Radix is not using Zhuanggenling, not pinching and shelving.
