1.A prediction model for high-risk cardiovascular disease among residents aged 35 to 75 years

Objective:
To establish a prediction model for high-risk cardiovascular disease (CVD) among residents aged 35 to 75 years, so as to provide the basis for improving CVD prevention and control measures.
Methods:
Permanent residents aged 35 to 75 years were selected from Dongcheng District, Beijing Municipality using the stratified random sampling method from 2018 to 2023. Demographic information, lifestyle, waist circumference and blood biochemical indicators were collected through questionnaire surveys, physical examinations and laboratory tests. Influencing factors for high-risk CVD among residents aged 35 to 75 years were identified using a multivariable logistic regression model, and a prediction model for high-risk CVD was established. The predictive effect was evaluated using the receiver operating characteristic (ROC) curve.
Results:
A total of 6 968 individuals were surveyed, including 2 821 males (40.49%) and 4 147 females (59.51%), and had a mean age of (59.92±9.33) years. There were 1 155 high-risk CVD population, with a detection rate of 16.58%. Multivariable logistic regression analysis showed that gender, age, smoking, central obesity, systolic blood pressure, fasting blood glucose, triglyceride and low-density lipoprotein cholesterol were influencing factors for high-risk CVD among residents aged 35 to 75 years (all P<0.05). The area under the ROC curve of the established prediction model was 0.849 (95%CI: 0.834-0.863), with a sensitivity of 0.693 and a specificity of 0.863, indicating good discrimination.
Conclusion
The model constructed by eight factors including demographic characteristics, lifestyle and blood biochemical indicators has good predictive value for high-risk CVD among residents aged 35 to 75 years.

Objective:To compare the imaging quality and metabolic quantitative parameters of pulmonary nodules between Q. Flex whole information five-dimensional (5D) and conventional three-dimensional (3D) PET/CT imaging for clinical evaluation.Methods:Fifty-four patients (30 males, 24 females, age: 60(42, 75) years; 78 solid pulmonary nodules (maximum diameter≤3 cm) with abnormal uptake of 18F-FDG) from Tianjin Cancer Hospital Airport Hospital between June 2022 and August 2022 were enrolled in this retrospective study. All patients underwent 5D scanning and 3D, 5D reconstruction. Image quality scores, signal-to-noise ratio (SNR), SUV max, SUV mean and metabolic tumor volume (MTV) of pulmonary nodules of 5D group and 3D group were evaluated and compared with χ2 test and Wilcoxon signed rank test. Correlation of quantitative parameters between 2 groups were analyzed by using Spearman rank correlation analysis. Results:Thirty-five of 78(45%) pulmonary nodules with image quality score≥4 were found in 5D group, which were more than those in 3D group (22/78(28%); χ2=4.67, P=0.031). Meanwhile, SNR, SUV max, SUV mean, and MTV were significantly positively correlated between the 2 groups ( rs values: 0.86, 0.86, 0.85, and 0.95, all P<0.001). SNR, SUV max and SUV mean of pulmonary nodules in 5D group were significantly higher than those in 3D group, which were 37.46(18.42, 62.00) vs 32.72(16.97, 54.76) ( z=-4.07, P<0.001), 9.71(5.48, 13.82) vs 8.96(4.82, 12.63) ( z=-3.05, P<0.001) and 6.30(3.39, 8.94) vs 5.61(2.99, 7.63)( z=-4.07, P<0.001) respectively. MTV of pulmonary nodules in 5D group was significantly lower than that in 3D group, which was 1.72(0.66, 2.74) cm 3vs 1.98(1.06, 4.63) cm 3 ( z=-7.13, P<0.001). Quantitative parameters of lower lung field and nodules with maximum diameters of >10 mm and ≤20 mm based on 5D scanning changed most significantly compared with those based on 3D scanning ( z values: from -5.23 to -2.48, all P<0.05). Conclusion:Q. Flex 5D PET significantly improves the quantitative accuracy of SUV and MTV of pulmonary nodules, and the improvement of image quality is substantial without increasing the radiation dose, which has clinical practical value.
5.1470nm laser transurethral modified bladder neck incision for the treatment of female bladder neck obstruction

Objective To investigate the treatment of female bladder neck obstruction by 1470nm laser transurethral modified enlarged female bladder neck obstruction(FBNO).Methods The clinical data of 34 patients with FBNO from January 2019 to November 2021 were retrospectively analyzed.The patient underwent a 1470nm laser transurethral modified enlarged bladder neck incision.The 1470nm laser was used to vaporise the bladder neck at 12 o'clock(lithotomy),and the incision site was expanded along the bladder neck to 9 o'clock and 3 o'clock to form a semi-circular surgical wound.The patients were followed up for complications,scored form of Bristol female lower urinary tract symptoms questionnaire(BFLUTS-SF)urination symptom subscale,quality of life(QoL)score and the maximum urinary flow rate(Qmax),detrusor pressure at maximum flow rate(PdetQmax),post-void residual(PVR)were reviewed at 1,4,and 10 months after operation.Results After 10 months of follow-up,the subjective indexes of BFLUTS-SF and QoL scores were significantly improved compared with those before operation(P<0.001),and the objective indexes of Qmax,PdetQmax,and PVR were significantly improved compared with those before operation(P<0.001).Two patients had mild urgency urinary incontinence and urinary tract infection symptoms half a month after operation,and the symptoms were improved after anti-infection and pelvic floor rehabilitation treatment.During the follow-up period,there were no complications such as vesicovaginal fistula,stress urinary incontinence,or recurrent bladder neck obstruction.Conclusion 1470nm laser transurethral modified enlarged bladder neck resection can effectively relieve bladder neck obstruction without significant postoperative complications,with high safety and good patient satisfaction.
6.Herbal Textual Research on Pyrrosiae Folium in Famous Classical Formulas

This article systematically analyzes the historical evolution of the name, origin, producing area, quality evaluation, harvesting and processing, clinical efficacy of Pyrrosiae Folium by consulting the ancient materia medica, medical books and prescription books, combined with modern literature, in order to provide a reference for the development of famous classical formulas containing this herb. After herbal textual research, it was found that the names of Pyrrosiae Folium in the past dynasties were mostly derived from its color, shape and efficacy. And there were other nicknames such as Shizhe, Shipi and Shilan. Song, Yuan dynasties and before the period, the main origin of Pyrrosiae Folium was Pyrrosia petiolosa, in the Ming dynasty, the main origins were P. petiolosa and P. sheareri, during the Qing dynasty to the present, the main origins were P. sheareri, P. petiolosa and P. lingua. Anciently, the respected Dao-di production area of Pyrrosiae Folium was the area of Lianyungang city, Jiangsu province. In modern times, Anhui and Zhejiang provinces are the main producing areas of P. sheareri, Fujian and Taiwan provinces are the main producing areas of P. lingua, and Guizhou and Hubei provinces are the main producing areas of P. petiolosa. In ancient and modern times, Pyrrosiae Folium with large leaves and thick texture is considered to be the best, the medicinal part is the leaves, and the harvesting and processing methods recorded in the past dynasties were mainly shade-drying after harvesting in the February and July of the lunar calendar, while the modern ones are mostly harvested throughout the year. The processing methods of the past dynasties mainly included removing fuzz by scraping, lightly roasted, frying, fat-fried. However, in modern times, it is mostly used the raw products as a medicine after cleaning, cutting and drying. In ancient times, Pyrrosiae Folium was thought to have a neutral nature with slightly sweet and bitter taste, while in modern times, it is thought to have a slightly cold nature with slightly sweet and bitter taste, and the main effects in ancient and modern times are diuretic, clearing lung-heat, hemostasis and so on. Based on the research results, it is suggested that P. sheareri, P. petiolosa and P. lingua can be used as the medicinal base, processing method can be according to the requirements of formulas, and if the processing requirements are not indicated, the raw products can be selected as the medicine.
7.Clinical features of acute pancreatitis in pregnancy and related risk factors

Objective To investigate the clinical features and maternal and fetal outcomes of acute pancreatitis in pregnancy(APIP)and the risk factors for disease aggravation,and to establish a predictive model.Methods A retrospective analysis was performed for 52 APIP patients who were admitted to Affiliated Hospital of Zunyi Medical University from January 2017 to December 2022,and according to disease severity,they were divided into mild acute pancreatitis(MAP)group with 32 patients,moderate-severe acute pancreatitis(MSAP)group with 8 patients,and severe acute pancreatitis(SAP)group with 12 patients.The logistic regression analysis was performed for the clinical data of each group,and the receiver operating characteristic(ROC)curves were plotted to assess the value of risk factors in predicting the severity of APIP.A one-way analysis of variance was used for comparison of normally distributed continuous data between multiple groups,and the least significant difference t-test was used for further comparision between two groups.The Kruskal-Wallis H test was used for comparison of non-normally distributed continuous data between multiple groups,and the Wilcoxon rank-sum test was used for further comparision between two groups;the chi-square test was used for comparison of categorical data between groups.Results Of all patients in terms of etiology,26(50%)had hyperlipidemic pancreatitis,20(38.4%)had biliary pancreatitis,and 6(11.5%)had idiopathic pancreatitis.In terms of gestational week,1 patient(1.9%)was in early pregnancy,25(48.1%)were in mid-pregnancy,and 26(50.0%)were in late pregnancy.A total of 10 patients(19.2%)had acute respiratory distress syndrome(ARDS),among whom 9(90%)required respiratory support.There were significant differences between the patients with different severities of APIP in aspartate aminotransferase,alanine aminotransferase,blood urea nitrogen,blood glucose,C-reactive protein(CRP),international normalized ratio(INR),pneumonia,ARDS,sepsis,hepatic insufficiency,and coagulation dysfunction(all P<0.05).The univariate analysis showed that the severity of APIP was associated with blood glucose,blood urea nitrogen,CRP,and pneumonia(all P<0.05),and pneumonia was a risk factor for the aggravation of APIP(odds ratio=18.938,95%confidence interval:1.020—351.747,P=0.048).CRP,blood glucose,blood urea nitrogen,and INR used in combination had a larger area under the ROC curve than each index used alone(0.954 vs 0.778/0.796/0.721/0.801).Conclusion Pneumonia is a risk factor for the aggravation of APIP,and the combination of CRP,blood glucose,blood urea nitrogen,and INR can be used to predict the severity of APIP.
8.Quality evaluation for Beidougen Formula Granules

AIM To evaluate the quality of Beidougen Formula Granules.METHODS Fifteen batches of standard decoctions and three batches of formula granules were prepared,after which paste rate and contents,transfer rates of magnoflorine,daurisoline,dauricine were determined.HPLC specific chromatograms were established,and cluster analysis was adopted in chemical pattern recognition.RESULTS For three batches of formula granules,the paste rates were 15.1%-16.6%,the contents of magnoflorine,daurisoline,dauricine were 18.93-19.39,9.42-9.60,6.79-6.85 mg/g with the transfer rates of 34.42%-35.25%,43.81%-44.65%,27.27%-27.51%from decoction pieces to formula granules,respectively,and there were seven characteristic peaks in the specific chromatograms with the similarities of more than 0.95,which demonstrated good consistence with those of standard decoctions and accorded with related limit requirements.Fifteen batches of standard decoctions were clustered into two types,and the medicinal materials produced from Jilin,Hebei,Shangdong could be used for the preparation of formula granules.CONCLUSION This reasonable and reliable method can provide references for the quality control and clinical application of Beidougen Formula Granules.

Its application in the monitoring of therapeutic drugs can effectively improve the quantitative accuracy and sensitivity, and formulate a personalized and optimal dosing plan for patients. However, this technology still faces some challenges, and automation, quality control, and quantitative traceability will be the future development direction.
10.Herbal Textual Research on Equiseti Hiemalis Herba in Famous Classical Formulas

In this paper, the name, origin, medicinal parts, producing area, harvesting, processing methods and efficacy of Equiseti Hiemalis Herba(EHH) in famous classical formulas were examined by reviewing related ancient and modern literature. Through textual research, Muzei was first appeared in Zhenyuan Guanglifang(《贞元广利方》), and used as a mainstream name by later generations. It is also known by other names, such as Cuocao and Bigancao. The main origin of ancient EHH was Equisetum hyemale, which was mixed with E. ramosissimum during the Qing dynasty. The medicinal part was the above-ground part of EHH. In ancient times, the genuine producing area was considered to be Qinzhou, which is now Tianshui city, Gansu. In modern times, EHH produced in Liaoning province is believed to be of higher quality. Currently, the main producing area of EHH circulating in the market is the northeast region in China. EHH with stems that are thick and long, a green color, a thick texture, and clearly visible edges and roughness, but without any easily separating joints being considered the best. The processing methods of the past dynasties mainly included filing, removing knots, stir-baked the crude drugs into black on outside and brown in inside, urine soaking, sun drying and shade drying. In modern times, the main processing method is to first moisturize the plant material, and then cut it into sections before drying. In terms of medicinal properties, EHH is considered by both ancient and modern medicine to have a neutral nature, a slightly sweet and bitter taste, and is non-toxic. Its primary therapeutic effects are related to treating eye diseases, intestinal wind bleeding and uterine bleeding. Based on the research, it is suggested that the dried above-ground part of E. hiemale be used in the development and utilization of famous classical formulas. For the processing requirements are not indicated, it is suggested using raw decoction pieces as medicine, and the processing method refers to the 2020 edition of Chinese Pharmacopoeia. If it is clearly stated that fried charcoal is required, it is recommended to refer to general requirements 0213 of the 2020 edition of Chinese Pharmacopoeia, if it is clearly stated that removing knots is required, it is recommended to follow the ancient method.
