1.Application of liver hanging maneuver in anterior approach for isolated complete liver caudate lobectomy.

OBJECTIVE:
To explore the technique and effect of liver hanging maneuver in anterior approach for isolated complete liver caudate lobectomy.
METHODS:
We recruited 17 patients with liver caudate lobe tumor (13 primary hepatocellular carcinoma, 3 cholangiocarcinoma and 1 liver metastasis from colorectal cancer). Isolated complete caudate lobectomy with liver hanging maneuver was performed in 17 patients.
RESULTS:
All 17 patients were successfully received the above-mentioned operation. The operative time was 166-427 (211.5 ± 20.1) min and the intraoperative blood loss was 372-1 208 (472.7 ± 83.6) mL. There was no operative death. The survival rates of follow up for 1, 3 and 5 years were 76.5%, 52.9% and 23.5%, respectively.
CONCLUSION
Liver hanging maneuver for isolated complete resection of the caudate lobe is an ideal approach for liver neoplasms resection.

2.Influence of BRAF interference on SW579 cell lines in thyroid cancer.

OBJECTIVE:
To determine the influence of v-raf murine sarcoma viral oncogene homolog B1 (BRAF) interference on SW579 cell lines in thyroid cancer.
METHODS:
We designed 2 pairs of siRNA interference sequences, transfected them into SW579 cell line with liposome, and detected the interference with RT-PCR method. For successfully interfered cell lines, the changes in cell proliferation, cell cycle, and the expression of related proteins in mitogen-activated protein signal-regulated kinase /extracellular signal-regulated kinase (MEK/ ERK) signal pathway were detected.
RESULTS:
After 2 pairs of siRNA transfection, the expressions of BRAF mRNA and protein of SW579 cell lines were significantly inhibited (P<0.01). The proliferation was inhibited, the cell cycle was changed, G1/S phase increased, and MEK/ERK signal pathway was inhibited.
CONCLUSION
Inhibition of growth and proliferation of SW579 cell lines by BRAF may be functioned by de-activating MEK/ERK signal pathway.

3.BRAF mutation and papillary thyroid cancer.

OBJECTIVE:
To explore the association bewteen BRAF mutations and papillary thyroid cancer.
METHODS:
BRAF gene mutations in 73 cases of papillary thyroid cancer and 16 cases of thyroid gland benign tumor were detected by PCR and sequencing technology.
RESULTS:
We found 42 instances of BRAF mutations in 73 cases of thyroid cancer. However, no mutation was found in tissues from benign thyroid tumors. There were significant correlations between BRAF mutation and lymph node metastasis, and between BRAF mutation and the clinical stage of papillary thyroid cancer (P<0.05).
CONCLUSION
The mutation of BRAF is associated with the pathogenesis, lymph node metastasis, and clinical stage of papillary thyroid cancer.

4.Expression of BRAF and its extracellular signal-regulated kinase 1/2 signal pathway in papillary thyroid cancer.

OBJECTIVE:
To determine the association between activity of BRAF and mitogen-activated protein/ extracellular signal-regulated kinase kinase (MEK) / extracellular signal-regulated kinase (ERK) signal pathway in papillary thyroid cancer and its mechanism.
METHODS:
We collected the clinical data and blood samples from 73 cases of papillary thyroid cancer and another 16 cases of benign thyroid gland tumor, and detected the expression of rat sarcoma (RAS), BRAF, MEK1/2, and ERK1/2 in all tumor specimens and benign thyroid tissues with immunohistochemistry and Western blot.
RESULTS:
The expression of RAS, BRAF, pMEK1/2, and pERK1/2 protein in papillary thyroid cancer tissues was higher than those in the benign thyroid tissues(P<0.05 or P<0.01). The expression of RAS, BRAF, MEK1/2, and ERK1/2 was associated with the tumor size, the lymph node metastasis, and the clinical stage of papillary thyroid cancer(P<0.05 or P<0.01).
CONCLUSION
The expression of RAS, BRAF, pMEK1/2, and pERK1/2 is associated with the pathogenesis, the lymph node metastasis, and the clinical stage of papillary thyroid cancer. The MEK/ERK signaling pathway may be activated by BRAF in papillary thyroid cancer.

5.Surgical treatment for complex pediatric retroperitoneal tumors.

OBJECTIVE:
To explore and summarize the experience of surgical treatment for primary retroperitoneal tumor (PRT)in children.
METHODS:
Clinical data of 17 patients with PRT treated from January 2001 to January 2008 were retrospectively analyzed, including image examination, pathologic examination and surgical procedure.
RESULTS:
Seventeen patients underwent complete resection; 8 benign PRT, and 9 malignant PRT were diagnosed by operation and postoperative pathologic examination. Vascular surgery was done on 11 patients, 6 cases of multi-visceral resection, 1 vascular transplant, and 1 multi-visceral resection. Two patients had recurrent malignant PRT.
CONCLUSION
For pediatric complex retroperitoneal tumors, complete resection can reduce the recurrence and improve the long-term survival.

6.Study of hepatocellular cancer markers

Objective HCC is one of many malignant tumors. The HCC markers are most urgently necdad. But the currently available markers lack sensitivity and specificity. A number of novel candidate markers have recently been introduced. Methods We performed a review of the literature of traditional and novel serum markers for hepatocellular cancer. Results Several promising new HCC markers have rencently been identified. They include single protein, complex proteomics features, and tumor-specific autoantibodies.The excitement about the new markers is tempered by the realization that none of them have yet met the most stringent criteria defined by the Early Detection Research Network (EDRN). Conclusion A new generation of HCC serum markers awaits validation in properly controlled clinical studies.
7.Study the antitumor mechanisms of extract of centipede on HCC Bel-7404 cell line

Objective To investigate the cell cycle and mechanisms of HCC Bel-7404 cell line by treatment of extract of centipede ( ECP). Method Bel-7404 cell line was cultured in vitro. ECP was applied to the interference of the growth of Bel-7404 with different drug concentration. Cell cycles and apoptosis ratios of ECP group were detected by flow cytometry (FCM). To investigate the expression of apop-tosis related genes XIAP and Bax, we also detect HCC Bel-7404 cell line after 48 hours treated with ECP by immunohistochemical method. Result The results of FCM displayed that ECP in treatment group mainly retard cell cycle phase in G0/G1 after 48 hours. The ratios of G0/ G1 .proliferation index (PI) and apoptosis ratios were significantly different between treatment group and control group when the concentrations were different. It was also significantly different between treatment group and control group when the same concentration of ECP was used. It showed that XIAP gene expression decreased gradually while Bax gene expression increased with the increase of ECP concentration, and these were statistical significance in contrast to control group ( P <0.05). Conclusion ECP mainly retarded cell cycle phase of Bel-7404 in G0/G1, suppressed the proliferation, and could induce it to apoptosis. The inhibitory effect was time - concentration dependent. The mechanisms were due to promote Bax gene and inhibit XIAP gene expression in HCC Bel-7404 cell line.

Objective To study the expression of hMLH1, hMSH2 and their association with clinico-pathological characteristics in hepatocellular carcinoma(HCC). Methods The expression of hMLH1 and hMSH2 in 37 HCC tissues was detected by SABC immunohistochemistry. 10.Emergency orthotopic liver transplantation for acute hepatic failure:a report of 8 cases

Objective To study the efficacy of emergency orthotopic liver transplantation(EOLT) for acute(hepatic) failure(ALT).Methods A retrospective review was undertaken on the clinical data of 8 patients undergoing emergency liver transplantation for ALT.Results The 8 patients completely regained consciousness in 12 to 72 hours after operation.No case developed central nervous complications.One case of severe(hepatitis) complicated by acute renal failure died of respiratory infection and ARDS on postoperative day 7.One case who refused to take medication died from chronic rejection 12 months after operation.One case was(complicated) by bile duct stricture and biliary sludge at 14 months postoperatively and survived for 18 months.Four of the other 5 cases were followed up for 17 months and 1 cases for 14 months,and thir quality of life was excellent.3 of them have returned to work.Conclusions Emergency orthotopic liver thansplantation is an effective means to treat ALF.Intensive care and effective treatment preoperatively are pre-requisite(conditions) to ensure the success of EOLT.
