1.Study on the Diagnostic Value of Serum NPASDP-4 and MBP Level Expression with Cognitive Dysfunction and Severity in Parkinson's Disease Patients

Objective To explore the diagnostic value of serum neuronal Per-Arnt-Sim domain protein 4(NPASDP-4)and myelin basic protein(MBP)expression in patients with Parkinson's disease in relation to cognitive impairment(CI)and severity.Methods Selected and 138 Parkinson's disease patients admitted to the 909th Hospital of the Joint Logistics Support Force of the People's Liberation Army of China as the Parkinson's disease group,and 69 healthy people in the physical examination center of the hospital were in the healthy control group.Patients with Parkinson's disease were divided into normal cognitive function group(n=55),mild CI group(n=51)and dementia group(n=32)according to whether CI occurred and its severity.General data of subjects was collected,the serum levels of NPASDP-4 and MBP were detected by ELISA,correlation analysis was adopted by Spearman rank correlation or Pearson linear correlation,diagnostic value was analyzed by ROC curve,and influencing factors were analyzed by multivariate Logistic regression.Results Compared with the healthy control group,the levels of serum NPASDP-4(6.75±0.48 ng/ml vs 2.38±0.31 ng/ml)and MBP(8.34±0.65 μg/L vs 3.54±0.42 μg/L)in the Parkinson's disease group were increased with statistical significance(r=68.751,55.761,all P<0.05).There were significant differences in H-Y stage among the normal cognitive function group,mild CI group and dementia group(x2=7.788,P<0.05).Compared with the group with normal cognitive function(47.92±11.63 score),the mild CI group(50.78±13.69 score)and the dementia group(41.95±10.36 score)showed an increase in UPDRS-Ⅲ scores,and the differences were statistically significant(H=6.672,all P<0.05).In normal cognitive function group,mild CI group and dementia group,the course of disease,and serum NPASDP-4(5.89±0.40,6.83±0.55,8.12±0.54 ng/ml)and MBP(6.65±0.56,8.94±0.69,10.27±0.70μg/L)levels were significantly increased(H=207.950,355.594,allP<0.05),while MMSE score(28.47±0.94,24.51±1.35,17.09±2.57 score),MoCA score(27.45±1.03,20.18±1.92,11.75±2.53 score)and GPCOG total score(13.47±0.69,10.25±1.04,8.97±0.82 score)were significantly decreased,and the differences were statistically significant(H=515.005,775.933,327.584,all P<0.05),respectively.The serum levels of NPASDP-4 and MBP in Parkinson's disease patients were significantly positively correlated with the course of disease(r=0.316,0.358),H-Y stage(r=0.345,0.384)and UPDRS-Ⅲ score(r=0.371,0.396),and significantly negatively correlated with MMSE score(r=-0.468,-0.517),MoCA score(r=-0.504,-0.569)and GPCOG total score(r=-0.527,-0.538)(all P<0.05),respectivey.The areas under the curve(AUC)of the serum levels of NPASDP-4,MBP and their combination in diagnosing of Parkinson's disease were 0.850,0.930 and 0.960,respectively.The AUC of the serum levels of NPASDP-4 and MBP and their combination in diagnosing the severity of CI in patients with Parkinson's disease were 0.866,0.803 and 0.933,respectively.H-Y stage metaphase[OR(95%CI):4.725(1.742~12.814)],H-Y stage advanced[OR(95%CI):5.083(1.919~13.464)],UPDRS-Ⅲ score[OR(95%CI):3.257(1.464~7.246)],NPASDP-4[OR(95%CI):5.324(1.516~18.701)]and MBP[OR(95%CI):5.769(2.459~13.533)]were the influential factors for CI in patients with Parkinson's disease(all P<0.05).NPASDP-4[OR(95%CI):4.768(2.382~9.543)]and MBP[OR(95%CI);5.846(3.141~10.882)]were the influential factors for the severity of CI in patients with Parkinson's disease(all P<0.05).Conclusion The serum levels of NPASDP-4 and MBP in patients with Parkinson's disease were high,and they were closely related to CI and its severity,which may have certain clinical diagnostic value.
2.Management of anticoagulant therapy in a patient with complex venous thromboembolism complicated with acute renal insufficiency

OBJECTIVE To provide a reference for the safe use of drugs in patients with complex venous thromboembolism (VTE) and acute renal insufficiency. METHODS Clinical pharmacists participated in the management of anticoagulant therapy for a patient with complex VTE complicated with acute renal insufficiency, and evaluated the patient as high-risk thrombosis and bleeding based on their medical history, laboratory test results, etc.; combined with the complexity of thrombosis and renal insufficiency, clinical pharmacists suggested that enoxaparin sodium should be used in the acute stage of thrombosis (5 to 21 days after onset), and then warfarin should be adopted for oral anticoagulation treatment. Because the patient’s anticoagulation was not up to the standard (the target range of the international normalized ratio was 2-3), clinical pharmacists suggested increasing the warfarin dose, detecting the warfarin metabolism genotype, and adjusting the warfarin dose according to the genotype; at the same time, clinical pharmacists developed an anticoagulation monitoring plan to ensure the safety of anticoagulation treatment. RESULTS Doctors had adopted all the recommendations of clinical pharmacists. The patient did not experience adverse events such as bleeding or worsening of thromboembolism during anticoagulation in the hospital. When the anticoagulation met the standards, the patient was allowed to be discharged with medication. CONCLUSIONS By participating in the anticoagulation treatment management of patients with complex VTE and acute renal insufficiency, clinical pharmacists have assisted doctors in formulating personalized anticoagulation plans to promote the compliance with the anticoagulation treatment standard and ensure the safety and effectiveness of medication for patients.
3.Simultaneous Determination of 7 Active Components in Mori Australis Cortex and Mori Cortex from Different Sources by HPLC

OBJECTIVE:To establish a meth od for the simultaneous determination of 7 active components in Mori Australis Cortex and Mori Cortex from different sources in Chongqing area ,so as to provide reference for improving the quality control standards of Mori Australis Cortex and Mori Cortex and comparing the equivalence of their quality. METHODS :HPLC method was used to determine the contents of neochlorogenic acid ,mulberroside A ,chlorogenic acid ,astragalin,kaempferol,morusin and isoquercetin in 58 batches of Mori Australis Cortex and Mori Cortex. The chromatographic column was Diamonsil C 18 with mobile phase consisted of 0.1% formic acid solution-acetonitrile (gradient elution ) at the flow rate of 1.0 mL/min. The detection wavelength was 280 nm,column temperature was 30 ℃,and the injection volume was 10 μL. Using SPSS 22.0 software, independent sample t-test,principal component analysis and cluster analysis were used to analyze the content difference of the above-mentioned 7 active components in Mori Australis Cortex and Mori Cortex. RESULTS :There was a good linear relationship between the peak area and the concentration of the above 7 active components (r≥0.999 0). The RSDs of precision ,stability(24 h),repeatability,durability and recovery were less than 3%. The average contents of neochlorogenic acid ,mulberroside A , chlorogenic acid , astragalin, kaempferol, morusin and 023-58576130。E-mail:1025473978@qq.com isoquercetin in Mori Australis Cortex were 0.304,22.462, 1.730,1.308,1.593,2.842 and 0.657 mg/g,respectively. Those of Mori Cortex were 0.305,22.995,2.486,2.438, 2.916,4.158 and 1.264 mg/g,respectively. The results of independent sample t-test showed that only the content of kaempferol in the above 7 active components of Mori Australis Cortex and Mori Cortex had significant difference (P<0.05). The results of principal component analysis and cluster analysis showed that there was no significant difference in the contents of above 7 active components between Mori Australis Cortex and Mori Cortex. CONCLUSIONS:The established HPLC method is simple ,sensitive and accurate ,which can provide a reference for improving the quality control standard of Mori Australis Cortex and Mori Cortex. Mori Australis Cortex and Mori Cortex have certain quality equivalence in main active components ,and the Mori Australis Cortex from M. australis and M. cathayana can be used as a substitute for the Mori Cortex.
4.Prevalence and clinical characteristics of Kawasaki disease with arthritis in children

Objective:To investigate the incidence and clinical characteristics of Kawasaki disease complicated with arthritis, and explore the relationship with coronary artery disease.Methods:Patients diagnosed with Kawasaki disease at Xiamen Children′s Hospital from January 2015 to June 2020 were included in this study.They were divided into the arthritis group( n=53) and the non-arthritis group( n=401), depending on whether complicated with arthritis.Demographic, clinical symptoms, and laboratory results were retrospectively analyzed. Results:A total of 454 children were included in this study with 53 cases acomplicated with arthritis.There were 32 male cases and 21 female cases.The average age of arthritis group was(5.89±1.35) y, which was older than non-arthritis group[(4.28±1.25) y, P=0.026]. Among the 53 cases of arthritis group, 36 cases (67.92%)of small jiont arthritis, 14 cases(26.41%)of coxitis, ten cases(18.87%)of carpitis, eight cases(15.09%)of gonitis, four cases(7.55%)of anconitis, and three cases(5.66%) of ankle arthritis were involved.There was a statistic difference in the prevalence of intravenous immunogloblin(IVIG)resistant between arthritis group and non-arthritis group(14 cases, 26.14% vs.43 cases, 10.72%, P=0.002). The inflammatory markers(CRP, TNF-α, IL-6) of the arthritis group were significantly higher than those in the non-arthritis group, and the differences were statistically significant( P<0.05, respectively). The incidence of coronary artery disease in the arthritis group(60.38%, 32/53) was higher than that in the non-arthritis group(52.37%, 210/401), but the difference was not statistically significant( P>0.05). Conclusion:Kawasaki disease with arthritis in children is self-limited, with no sequelae.Patients in the arthritis group have a higher rate of IVIG resistance and higher levels of inflammatory markers, but no significant difference in the incidence of coronary artery disease compared with those without arthritis.
5.Effects of enhanced recovery nursing pathway in perioperative management of patients with hepatic echinococcosis

Objective:To explore the effect of enhanced recovery nursing pathway in the enhanced recovery after surgery of perioperative patients with hepatic echinococcosis.Methods:From November 2017 to February 2019, simple random sampling was used to select 160 hepatic echinococcosis patients with surgery admitted to Shannan People's Hospital of Tibet Autonomous Region as the research object. Patients were randomly divided into experimental group and control group, 80 cases in each group. Patients in control group received general surgical routine nursing, and experimental group carried out enhanced recovery nursing pathway on the basis of routine nursing. The postoperative rehabilitation indicators, pain scores and postoperative complications were compared between the two groups.Results:The time of first exhaust time and defecation in experimental group was (3.77±3.05) h and (23.81±17.29) h, which was earlier than (13.27±8.58) h and (37.79±18.23) h in control group, and the differences were statistically significant ( t=4.265, 4.975; P<0.01) . The pain scores of experimental group at 2, 4, 8, and 24 hours after surgery were (1.08±1.04) , (1.61±0.85) , (1.13±0.70) and (0.26±0.41) lower than those [ (2.41±1.20) , (2.94±0.96) , (2.94±0.96) , (1.33±0.90) ]of control group, and the differences were statistically significant ( t=7.501, 9.250, 13.650, -8.720; P<0.01) . There were 2 cases (2.50%) of lung infection in experimental group and 13 cases (16.25%) in control group. The incidence in experimental group was lower than that in control group, and the difference was statistically significant (χ 2=8.901, P<0.01) . Conclusions:The standardized nursing model based on the enhanced recovery nursing pathway is conducive to the postoperative rehabilitation of patients with hepatic echinococcosis, shortens the hospital stay, and reduces postoperative lung infection complications, which is worthy of further promotion.
6.Effect of transfusion convalescent recovery plasma in patients with coronavirus disease 2019.

OBJECTIVES:
To evaluate curative effects of coronavirus disease 2019 (COVID-19) patients by the transfusion of other convalescent plasma.
METHODS:
Retrospective analysis of the clinical data of 18 patients with severe and critical COVID-19, who were hospitalized in the ICU of Xianghu Branch of the First Affiliated Hospital of Nanchang University from February 1 to March 15, 2020. Patients were subdivided into an experimental group (=6, who had transfused the plasma) and an observation group (=12, who had no plasma transfusion). Basic clinical data and prognosis indexes of these two groups were compared. Moreover, for the experimental group, the dynamic changes of blood oxygen saturation before and after the transfusion, the changes of lymphocyte absolute value 48 hours after the transfusion, and the detection of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) nucleic acid were analyzed.
RESULTS:
There were no significant differences in age, gender, blood type and other basic clinical data between the two groups (all >0.05).There were no significant differences in ventilator machine weaning time, extracorporeal membrane oxygenation (ECMO) weaning time, body temperature recovery to normal time, and hospitalization days between these two groups (all >0.05). For the experimental group, before, during and after the convalescent plasma transfusion, the blood oxygen saturation of all 6 patients at all time (1, 6, 8, 12, 24, 36, and 48 h) was more than 90%, and there was no significant fluctuation. There were 3 patients whose absolute value of lymphocyte was increased 48 hours after the transfusion, and the remaining was decreased. There were 5 patients whose SARS-CoV-2 nucleic acid detection turned negative 48 hours after the transfusion, accounting for 83.3%.
CONCLUSIONS
Transfusion of convalescent plasma will not affect outcomesof COVID-19 patients, which can neutralize SARS-CoV-2 in patients and reduce the loading capacity of SARS-CoV-2.

7.Current status of treatment of primary hepatocellular carcinoma in the elderly population

With the global aging, the incidence rate of HCC in the elderly population is gradually increasing , and thus it has become a global health issue.With reference to relevant literature, this article analyzes the clinical features of elderly HCC patients and reviews the current status of therapies and clinical outcomes .The review of clinical outcomes is mainly based on the comparison of therapeutic outcome between elderly and non -elderly HCC patients in many countries and regions in the world .This article points out that elderly HCC patients have their own clinical features, and since elderly HCC patients often have other diseases , such as diabetes, renal failure, and lung and cardiovascular diseases, there is an increase in the risk of treatment, but they often have positive treatment outcomes .With the deepening of research and the improvement of various treatment methods , the therapeutic outcome of elderly HCC patients will also be improved .Therefore,positive attitude and strategies should be adopted in the clinical treatment of elderly HCC patients .
8.A case-control study on the risk factors of mixed hemorrhoid

Objective To investigate major risk factors of mixed hemorrhoid,and provide scientific basis for primary prevention of the disease.Methods A hospital-based 1:1 matched case-control study method was adopted.A total of 341 patients who were diagnosed with mixed hemorrhoid in the Department of Anorectal Diseases,the Affiliated Hospital of Chengdu University of Traditional Chinese Medicine from June 2015 to February 2016 was recruited as case group.Another 341 non-mixed-hemorrhoid patients with the same gender proportion and age difference within 5 years old were taken as control group.Self-designed questionnaires were applied to face-to-face interview these subjects.The questionnaires included occupational factors,living habits,defecation related factors,disease history,reproductive history,family history and basic knowledge of mixed hemorrhoid.SPSS 20.0 software was used for single-factor analysis.Results It turned out that the factors related to occurrence of hemorrhoid were as followed:education level(x2 =15.431),working position(x2 =18.078),duration of single working position(OR=3.345),alcohol intake(OR=3.269),smoking(OR=1.852),spicy food intake(OR =2.409),less physical exercise(OR =1.522),defecation posture(OR =1.750),defecation time(x2 =7.516),defecation frequency(x2 =8.405),stool shape(x2 =8.004),obesity(OR=1.618),reproductive history(OR=2.211) and hemorrhoid family history of first-degree relatives(OR=1.763);The correlation intension (OR value) between anal fissure,anal pruritus and mixed hemorrhoid was 0.564 and 2.714 respectively;While tea drinking,perianal abscess or anal fistula were in no relation to mixed hemorrhoid onset.Conclusion The onset of mixed hemorrhoid is resulted from joint efforts of occupational factors,living habits,defecation habits and reproductive history,etc.Anal fissure,history of anal pruritus and preanal eczema are closely related to mixed hemorrhoid,and genetic factors may get involved as well.
9.Adipogenic capacity of CD54+/CD54- adipose-derived stem cells

BACKGROUND: Studies have shown that adipose-derived stem cells have pluripotent differentiation potential, but only 30%-40% of cells can differentiate into mature adipocytes with low adipogenic differentiation potential. Therefore, how to improve the adipogenic differentiation ability of adipose-derived stem cells is a key problem to be solved in the process of soft tissue regeneration. OBJECTIVE: To observe the relationship between the surface marker CD54 of rabbit adipose-derived stem cells and their adipogenic capacity, and to explore the adipogenic differentiation of CD54+/CD54- adipose-derived stem cells underthe same induction. METHODS: We successfully isolated and cultured the adipose-derived stem cells from inguinal subcutaneous fat pads (3 ml) of New Zealand white rabbits, aged 8-12 weeks, which were induced into multi-differentiation and used to detectsurface markers. We sorted the passage 3 adipose-derived stem cells by immunomagnetic beads and divided into two categories including CD54+ and CD54- adipose-derived stem cells. After 14 days of adipogenic induction, the cells in the two groups were subjected to oil red O staining and were compared by detecting the density of mature adipocytes and lipid droplet contenT.RESULTS AND CONCLUSION: The cultured adipose-derived stem cells possessed the characteristics of mesenchymal stem cells that could differentiate into mature adipocytes, osteoblasts and chondrocytes, with CD29, CD44, CD49d, CD54, CD73, CD90 and CD105 positive expression while CD31, CD34 and CD45 negative expression. Fourteen days after adipogenic induction, the density of mature adipocytes and the intracellular lipid droplet content in the CD54+ group were significantly higher than those in the CD54- group (P < 0.05). We also found that the mRNA expressions of PPARγ,ADD1, C/EBPα related to adipogenic differentiation in the CD54+ group were significantly higher than those in the CD54- group (P < 0.05). Taken together, CD54+ adipose-derived stem cells have excellent adipogenic differentiation capacity.
10.Community promotion of superficial needling plus club swing for post-stroke motion impairment of the shoulder joint

Objective: To investigate the community promotion feasibility of superficial needling plus club swing for post-stroke motion impairment of the shoulder joint. Methods:A total of 180 cases (duration <1.5 years) with post-stroke motion impairment of the shoulder joint were recruited from three community health centers in Changning District, 60 from each community. They were randomly allocated into an observation group (n=90) and a control group (n=90). Patients in both groups received standard internal and rehabilitation care. Patients in the observation group received additional superficial needling plus club swing. The visual analogue scale (VAS) was conducted before and 60 d after the treatment to evaluate the severity of shoulder pain. The active movement of the shoulder joint and activities of daily living (ADL) were also observed. Results:There were no between-group statistical differences before the treatment (allP>0.05). After a 60-day treatment, the shoulder pain severity, active range of motion of the shoulder joint and ADL in the observation group were significantly improved than those in the control group (allP<0.01). In addition, no adverse events were reported by participants in the observation group. Conclusion:Superficial needling plus club swing plays a positive role in improving post-stroke motion impairment of the shoulder joint. This safe, reliable and economical therapy has good patient compliance and is suitable for community promotion.
