Objective:To analyze the early volume characteristics of patients with severe cardiogenic shock treated with veno-arterial extracorporeal membrane oxygenation (VA-ECMO) and the relationship between their early volume and the prognosis. Methods:This study reviewed patients of Emergency Medical University , treated with VA-ECMO and screened the patients with severe cardiogenic shock and VA-ECMO running more than 72 h for further study. The basic condition of the patients was recorded, and the fluid balance in the first 72 h was analyzed. The patients were grouped according to their fluid balance in the first 72 h. The gender, age, survival rate, continuous renal replacement therapy (CRRT) rate, intra-aortic balloon pump (IABP) rate, and invasive mechanical ventilation rate were compared between the two groups, and the relative risk to the prognosis was calculated. The prognosis was compared between the two groups. Results:Totally 77 patients with severe cardiogenic shock were enrolled. Forty-one cases survived, with an overall survival rate of 53.2%. The volume balance at 48-72 h and the total volume balance at the first 72 h were different between the survival and dead groups. Compared with the positive balance group, patients in the negative balance group were less likely to receive CRRT or invasive mechanical ventilation during the first 72 h. Patients in the negative balance group during the first 72 h had a better survival rate, and their relative risk of survival was 1.81 (95% confidence interval: 1.101, 2.985). However, there was no significant difference in survival rate according to every 24 h fluid balance. Conclusions:Patients with severe cardiogenic shock treated with VA-ECMO who had negative total volume balance during the first 72 h are more likely to survive and less likely to require CRRT or invasive mechanical ventilation.

Objective:To explore the predictive factors of renal replacement therapy (RRT) in extracorporeal membrane oxygenation (ECMO) patients. Methods:The clinical data of 68 ECMO patients treated at Emergence Department of Jiangsu Provincial Hospital from January 2015 to December 2018 were retrospectively analyzed. Vasoactive-inotropic score (VIS) was used to assess the usage of vasoactive-inotropic drugs on day 1, 2 and 3 of ECMO (24 h VIS, 48 h VIS, and 72 h VIS). According to received RRT or not, patients were divided into the RRT group and non-RRT group. Age, gender, weight, VIS, presence of cardiac arrest before ECMO, ECMO mode, and ECMO treatment time were compared. Logistic regression analysis was used to identify predictive factors for RRT in ECMO patients. Results:Of the enrolled patients, 73.5% of ECMO patients received RRT. The mean age, 24 h VIS, ECMO failure and mortality of the RRT group were significantly higher than those of the non-RRT group ( P <0.05). The use of RRT was 87.8% in elderly ECMO patients (> cutoff age of 38.5 years). According to the cutoff value of 24 h VIS (33.75), ECMO patients were divided into the high VIS group and low VIS group. The rates of RRT and mortality were both exceeded 90% in the high VIS group, which was significantly higher than that of the low VIS group ( P <0.05). Logistic regression analysis showed that age ( OR=1.223) and 24 h VIS ( OR=1.033) were predictive factors of RRT in ECMO patients ( P <0.05). Conclusions:Age and 24 h VIS show the predictive value for RRT in ECMO patients.

Objective:To analyze the characteristics of malaria epidemic situation before and after malaria elimination in Qiandongnan Prefecture, and to provide the basis for establishment of effective strategies and measures to consolidate the achievements of malaria prevention and control. Methods:The data of malaria cases in 16 counties (cities) of Qiandongnan Prefecture from 2005 to 2018 were collected, and descriptive epidemiological method was used to analyze the infection rate of Plasmodium among local residents and floating population before (2005-2011) and after (2012-2018) elimination of malaria, and the characteristics of population distribution, seasonal distribution, species of Plasmodium and types of malaria vectors were analyzed. Results:Before elimination of malaria, total of 1 412 cases of malaria were reported, among those cases, 1 361 cases were local cases, accounting for 96.39% of the total cases. After elimination of malaria, total of 17 cases were reported, all of them were imported cases. After comparison of malaria cases before and after the elimination, the proportion of people aged from 18 to 60 was 70.54% (996/1 412) before the elimination, all 17 imported cases were 18-60 years old after the elimination, and the proportion of children/students decreased from 24.65% (348/1 412) before the elimination to 0 after the elimination. The peak incidence of malaria cases before the elimination was from June to October, and cases occurred every month. After the elimination, the imported cases were sporadic. Plasmodium vivax was the main species of Plasmodium before the elimination (98.58%, 1 392/1 412), and Plasmodium falciparum was mainly imported after the elimination (70.59%, 12/17). Before and after the elimination, Anopheles sinensis, the malaria vector, was the dominant population, but no distribution of Anopheles minimus and Anopheles anthropophagus was found after 2015. Conclusions:After the elimination of malaria in Qiandongnan Prefecture, there is a risk of local malaria cases caused by imported cases. It is suggested that local authorities should focus on the treatment of suspected malaria cases and vector surveillance of overseas returnees in the future.

Objective:To investigate the expression and its clinical significance of cydooxygenase-2(COX-2)and tumor necrosis factor-alpha(TNF-α)in mucosal tissues in elderly ulcerative colitis(UC)patients with arthritis. Methods:A total of 164 active(observation group)and 50 non-active(control group)UC patients with arthritis were admitted in our hospital from April 2016 to March 2019.Diagnostic criteria of UC and its severity were based on Consensus on the Norms for Diagnosis and Treatment of Inflammatory Bowel Disease in China(2007). The expressions of COX-2 and TNF-alpha in mucosal tissues were detected and compared between the two groups.Patients in the observation group were re-grouped according to the severity of ulcer lesion and Kellgren-Lawrence(K-L)score of arthritis.Based on severity of ulcer lesion and Kellgren-Lawrence(K-L)score of arthritis, the patients in observation group were re-grouped intoⅠ, Ⅱ, Ⅲ, Ⅳ subgroups, among those subgroups the expressions of COX-2 and TNF-alpha in mucosal tissues were compared. Results:The expressions of COX-2 and TNF-alpha in mucosal tissue were higher in the observation group than in the control group( P<0.05), and were increased along with the increased severity of UC and the K-L grade of arthritis( P<0.05). The differences in above expressions had statistical significance among patients with different severity of ulcer and different K-L grade of arthritis. The expressions of COX-2 and TNF-alpha in mucosal tissues were positively correlated with the severity of ulcer( r=0.491 and 0.627, P<0.05 or 0.01)and K-L grade of arthritis( r=0.438 and 0.597, P<0.05 or 0.01). Conclusions:The expressions of COX-2 and TNF-alpha in mucosal tissues are positively correlated with the severity of ulcer and K-L grade of arthritis in elderly UC patients with arthritis. The detection of COX-2 and TNF-alpha levels is helpful to evaluate the disease severity.

To compare the safety and efficacy between endoscopic submucosal dissection (ESD) and radical surgery (RS) for the treatment of large colorectal laterally spreading tumors (LST) larger than 50 mm in diameter.
Methods: From January 2011 to January 2016, a total of 82 patients were diagnosed as large LST without deep submucosal invasion (T1 SM2, ≥1 000 µm) in the Second Xiangya Hospital of Central South University. Among them, 52 patients were treated by ESD and the other 30 patients were treated by RS [laparoscopic-assisted colectomy (LAC)/open colectomy (OC)]. The clinic data were retrospectively analyzed and the en-bloc resection rate, en-bloc R0 resection rate, local recurrence, complication, procedure time and hospital stay were collected and analyzed.
Results: The lesion sizes were (5.80±1.20) cm and (5.53±0.69) cm in diameter for ESD and RS groups, respectively (P>0.05). En-bloc resection rates, en-bloc R0 resection rates and recurrence rates showed no significant difference between the ESD group and RS group (P>0.05). Complication rate of the ESD group (7.69%, 4/52) was much lower than that in the RS group (33.33%, 10/30; P<0.01). The ESD group also had a shorter hospital stay and operation time than the RS group (P<0.05).
Conclusion: ESD appears to be a safe, minimal invasive and effective strategy for treating large LST and it is obviously better than RS in the aspects of hospital stay, operation time and short-term complication.

Herald of Medicine 2014;(11):1442-1445
Objective To investigate the effects of radiotherapy combined with resveratrol( Res)on proliferation, invasion and apoptosis of hepatoma cell line Bel-7404. Methods Bel-7404 cell line was treated with Res(25 μmol·L-1 ) combined with radiotherapy, then divided into four groups( group 1:the control;group 2:radiation at dose of 2 Gy;group 3:radiation at dose of 4 Gy;group 4:radiation at dose of 6 Gy). Cell proliferation and invasion were detected by MTT assay. Cell apoptosis were observed by fluorescence microscopy. Expressions of MMP-2 and VEGF proteins were determined by Western blotting. Results Compared with the control group, cell proliferation and invasion were significantly inhibited, while cell apoptosis was increased in all radiation groups(P〈0. 05). Conclusion The sensitivity of hepatoma Bel-7404 cells to radiation can be enhanced by resveratrol. Radiation therapy combined with resveratrol can inhibit proliferation and invasion of hepatoma cells and increase the cell apoptosis, which may be related with the down-regulation of MMP-2 and VEGF proteins.
