1.Analysis of subjective visual vertical test results in patients with benign paroxysmal positional vertigo at different head deflection angles

Objective To analyze the clinical significance of subjective visual vertical (SVV) tests at different head deflection angles in assessing utricle function in patients with benign paroxysmal positional vertigo (BPPV). Methods A total of 61 BPPV patients who were treated at the Hearing Impairment and Vertigo Diagnosis and Treatment Center of Otolaryngology Head and Neck Surgery, Xinhua Hospital, Shanghai Jiao Tong University School of Medicine from August 2022 to May 2023 were retrospectively included, and 29 healthy adults were selected as controls. SVV tests were performed on all research subjects at different head deflection angles: upright head (0°), left head 45° (L45°), right head 45° (R45°). The test results between the two groups were compared. Results SVV absolute value at R45° in BPPV group was lower than that in the control group (P=0.003); there was no significant difference in SVV values at 0° and L45° between the two groups. There was no statistical difference in SVV values at different head deflection angles between the control group and the left BPPV group. SVV absolute value at R45° in right BPPV group was lower than that in the control group (P<0.001); there was no statistical difference in SVV values at 0° and L45° between the two groups. Conclusions SVV test can provide subjective information about the utricle, and SVV tests at different head deflection angles can fine-tune evaluate the function of the utricle in BPPV patients.
2.Attenuation of Airway Inflammation and Airway Remodeling in Ovalbumin Asthmatic Rats by Artemisinin through PI3K/Akt Signaling Pathway

ObjectiveTo investigate the molecular mechanism of action of artemisinin in attenuating asthmatic airway inflammation and airway remodeling through the phosphoinositide 3-kinase(PI3K)/protein kinase B(Akt) signaling pathway. MethodFifty male SD rats were randomly divided into blank group, model group, and low-dose, medium-dose, and high-dose groups of artemisinin, with 10 rats in each group. The ovalbumin (OVA)-induced asthma model of the rats was established, and after successful modeling, the blank group and model group received tail vein injection of 1.0 mL·kg-1 normal saline, while the low-dose, medium-dose, and high-dose groups of artemisinin received tail vein injection of 12.5, 25, and 50 mg·kg-1 artemisinin daily for seven days. Airway resistance was measured by the acetylcholine chloride method. Cell number and species changes in the alveolar lavage fluid of each group were determined by flow cytometry. Morphological changes in airway endothelial tissue were determined by the hematoxylin-eosin (HE) staining method. Apoptosis was determined by CytoTox 96 method, and enzyme-linked immunosorbent assay (ELISA) method was used to determine the NOD-like receptor thermal protein domain associated protein 3 (NLRP3) inflammasome, interleukin-1β (IL-1β), and interleukin-10 (IL-10) expression. Western blot method was used to detect the (p)-PI3K/p-Akt level in the alveolar bronchial tissue of each group. ResultCompared with the blank group, the total number of cells, total number of macrophages, total number of eosinophils, total number of lymphocytes, and total number of neutrophils were significantly higher in the model group (P<0.05). HE staining showed that the airway mucosa of the rats had obvious edema, and a large number of inflammatory cells were infiltrated (P<0.05). The rate of apoptosis was significantly higher (P<0.05), and the levels of the inflammasome NLRP3, IL-1β, and IL-10 increased significantly (P<0.05). p-PI3K/p-Akt level increased significantly (P<0.05). Compared with the model group, the total number of cells, total number of macrophages, total number of eosinophils, total number of lymphocytes, and total number of neutrophils were significantly decreased after the intervention of artemisinin at low, medium, and high concentrations (P<0.05). HE staining showed that the degree of edema of the airway mucosa of the rats was reduced, and the area of the inflammatory cell infiltration was drastically reduced (P<0.05). The apoptosis rate was significantly reduced (P<0.05), and the levels of the inflammasome NLRP3, IL-1β, and IL-10 decreased significantly (P<0.05). p-PI3K/p-Akt level decreased significantly (P<0.05). ConclusionArtemisinin significantly inhibits NLRP3 inflammasome activation, reduces cellular pyroptosis and inflammatory cell expression, and attenuates airway inflammatory manifestations and airway remodeling in asthmatic rats, which may be related to the regulation of p-PI3K/p-Akt, and the results may provide laboratory insights and basis for the treatment of bronchial asthma with artemisinin.
3.Personalized treatment of well-differentiated gastric neuroendocrine tumors based on clinicopathological classification and grading: A multicenter retrospective study

Background::The incidence of well-differentiated gastric neuroendocrine tumors (G-NET) is increasing annually, and while they have a good prognosis and low mortality rate, their high recurrence rate makes treatment options controversial. This study aims to determine the relationship between individualized treatment plans and the recurrence of G-NET.Methods::We performed a multicenter, retrospective study of 94 patients with highly differentiated G-NET and treated at Peking Union Medical College Hospital, Yantai Yuhuangding Hospital, and Beijing Zhong-Neng-Jian Hospital from November 2015 to September 2023. Risk factors for recurrence of G-NETs were investigated using chi-squared test and multifactorial logistic regression analysis.Results::After a median follow-up of 49 months, the overall recurrence rate among the 94 G-NET patients was 14% (13/94). The recurrence rates of endoscopic mucosal resection (EMR), endoscopic submucosal dissection (ESD), somatostatin analog (SSA) therapy, and surgery were 43% (6/14), 10% (5/49), 5% (1/22), and 11% (1/9), respectively. Post-treatment recurrence rates were significantly different ( P = 0.014) among four treatments (EMR, ESD, SSA, and surgery), and further subgroup comparisons revealed lower recurrence rates in the ESD and SSA groups than in the EMR group. From the second month onward, SSA therapy considerably reduced the gastrin levels from 1081.0 (571.5, 2472.8) pg/mL to 461.5 (255.3, 795.0) pg/mL ( Z = -3.521, P <0.001). Both chi-squared test and multifactorial logistic regression analysis suggested that among the clinicopathological parameters studied, only the pre-treatment gastrin level ( P = 0.018 and 0.005) and the type of treatment ( P = 0.014 and 0.017) were significantly associated with G-NET recurrence. Conclusions::Individualized treatment strategies may reduce the risk of relapse after G-NET treatment. Long-term SSA therapy may be a secure and efficacious treatment option for type 1 G-NET with more than six lesions, and it substantially decreases the incidence of post-treatment recurrence.

Hip fracture in the elderly is characterized by high incidence, high disability rate, and high mortality and has been recognized as a public health issue threatening their health. Surgery is the preferred choice for the treatment of elderly patients with hip fracture. However, lower extremity deep venous thrombosis (DVT) has an extremely high incidence rate during the perioperative period, and may significantly increase the risk of patients′ death once it progresses to pulmonary embolism. In response to this issue, the clinical guidelines and expert consensuses all emphasize active application of comprehensive preventive measures, including basic prevention, physical prevention, and pharmacological prevention. In this prevention system, basic prevention is the basis of physical and pharmacological prevention. However,there is a lack of unified and definite recommendations for basic preventive measures in clinical practice. To this end, the Orthopedic Nursing Professional Committee of the Chinese Nursing Association and Nursing Department of the Orthopedic Branch of the China International Exchange and Promotive Association for Medical and Health Care organized relevant nursing experts to formulate Expert consensus on perioperative basic prevention for lower extremity deep venous thrombosis in elderly patients with hip fracture ( version 2024) . A total of 10 recommendations were proposed, aiming to standardize the basic preventive measures for lower extremity DVT in elderly patients with hip fractures during the perioperative period and promote their subsequent rehabilitation.
5.Expression and clinical significance of cullin 4A in cholangiocarcinoma

Objective To explore the expression of cullin 4A (CUL4A) in cholangiocarcinoma tissues and its clinical significance.Methods Primary fresh cholangiocarcinoma tissues (n =35) and normal bile duct tissues (n =15) from patients who underwent curative surgery in Binzhou People's Hospital of Shandong Province from February 2011 to December 2013 were collected.The expressions of CUL4A mRNA were detected by quantitative real-time PCR (qRT-PCR).Then,cholangiocarcinoma tissues (n =72) and normal bile duct tissues (n =36) from patients who underwent curative surgery in Binzhou People's Hospital of Shandong Province from January 2008 to January 2014 were collected.The expressions of CUL4A protein were tested by immunohistochemistry.The relationships between the expression of CUL4A and the patients' clinicopathologic features and prognosis were analyzed.Results The expression of CUL4A mRNA in cholangiocarcinoma tissues was obviously higher than that in normal bile duct tissues (3.876 ±0.975 vs.1.216 ±0.265),and the difference was statistically significant (t =12.23,P < 0.001).The positive rates of CUL4A protein in cholangiocarcinoma and normal bile duct tissues were 70.8% (51/72) and 2.8% (1/36) respectively,and the difference was statistically significant (x2 =44.524,P < 0.001).The expression of CUL4A protein in cholangiocarcinoma was related to the differentiated degree (x2 =4.341,P =0.037),neural invasion (x2 =8.326,P =0.004),TNM stage (x2 =7.745,P =0.005),lymph node metastasis (x2 =3.869,P =0.049) and vascular invasion (x2 =5.555,P =0.018).Survival analysis results showed that the median survival time of CUL4A positive patients was significantly shorter than that of CUL4A negative patients (32.40 months vs.51.30 months),and the difference was statistically significant (x2 =6.561,P =0.011).Conclusion The expression of CUL4A in cholangiocarcinoma tissues is higher than that in normal bile tissues.CUL4A plays an important role in the process of tumor invasion and metastasis and indicates poor prognosis,which may become a potential target for the diagnosis and therapy of the cholangiocarcinoma.
6.Compatibility Stability of Salvianolic Acid for Injection Combined with 8 Kinds of Common Solvents

OBJECTIVE:To investigate the compatibility stability of Salvianolic acid for injection (SAFI) combined with 8 kinds of common solvents. METHODS:Referring to package inserts,SAFI was collected and combined with 0.9% Sodium chlo-ride injection,5% Glucose injection,10% Glucose injection,Compound sodium chloride injection,Mannitol injection,Glycerol fructose and sodium chloride injection,Sodium lactate Ringer's injection,Glucose and sodium chloride injection,250 mL each re-spectively,and then sealed in the dark at the room temperature. The appearance of mixtures were observed,pH value,the number of insoluble particles,maximal absorption wavelength and maximal absorbance were detected,and the contents of salvianolic acid B,rosmarinic acid,lithospermic acid and salvianolic acid Y in mixtures were determined by HPLC at 0,1,2,4,8 h after mix-ing. RESULTS:Under above condition,no obvious change was found in appearance or pH values of the mixtures within 8 h. Maxi-mal absorption wavelength ranged 284.5-286.0 nm. After mixed with Mannitol injection,the number of particles≥10μm(1-8 h af-ter mixing)and particles ≥25 μm(4-8 h after mixing)exceeded the scope of Chinese Pharmacopoeia(2015 edition);the maxi-mal absorbance changed significantly(RSD=9.17%,n=5);the relative content of salvianolic acid B,rosmarinic acid,lithosper-mic acid and salvianolic acid Y decreased by more than 10%(RSD=14.65%,6.45%,8.97%,12.49%,n=5);after mixed with Sodium lactate Ringer's injection,the relative content of rosmarinic acid and lithospermic acid changed greatly (RSD=14.57%, 7.28%,n=5);after mixed with 5% Glucose injection(4-8 h after mixing)and Glycerol fructose and sodium chloride injection(8 h after mixing),the relative content of rosmarinic acid were less than 90%(RSD=6.30%,4.86%,n=5);and the number of particles ≥25μm exceeded the scope of phamcopoeia after mixing with Glycerol fructose and sodium injection(0 h). The number of insoluble particles in other mixtures were in line with the standard of pharmacopoeia;maximal absorbance had no significant change(RSD<5%,n=5),and the relative content change of analytes were all less than 10%. CONCLUSIONS:Clinical appli-cation of SAFI combined with Mannitol injection,Sodium lactate Ringe's injection and Glycerol fructose and sodium injecrion should be avoided. After mixed with 5% Glucose injection,SAFI should be used within 4 h. SAFI can be compatible with 0.9%Sodium chloride injection,10% Glucose injection,Compound sodium chloride injection and Glucose and sodium chloride injec-tion.

8.Biological activity evaluation of porous HA ceramics using NH4 HCO3/PVA as pore-creating agents.

Porous HA ceramics were prepared by using NH4 HCO3/PVA as pore-formed material along with biological glass as intensifier, and these ceramics were immersed in Locke's Physiological Saline and Simulate Body Fluid (SBF). The changes of phase composition, grain size and crystallinity of porous HA ceramics before and after immersion were investigated by X-Ray Diffraction (XRD) and Scanning Electron Microscopy (SEM). The biological activity was evaluated. The porous HA ceramics showed various degrees of decomposition after immersion in the two solution systems, but there was no evident change in respect to crystallinity. Besides, the impact of different degrees of solution systems on the change of grain size and planar preferred orientation was observed. The TCP phase of the ceramics immersed in Locke's Physiological Saline decomposed and there was no crystal growth on the surface of ceramics; however, the grain size of ceramics immersed in SBF became refined in certain degree and the surface of ceramics took on the new crystal growth.

9.Research on chromatographic fingerprint of fruits of Schisandra chinesis.

OBJECTIVETo study the chromatographic fingerprint of the fruits of Schisandra chinensis, and identify the peaks.
METHODAnalysis was performed at 30 degrees C on a Waters Symmetry C18 column (4.6 mm x 250 mm, 5 microm), eluted with acetonitrile-water gradient elution. The flow-rate was 1.0 mL x min(-1), and detection wavelength was 218 nm. The peaks in the chromatogram were identified by LC-MS.
RESULTThe fingerprint of the fruits of S. chinensis was established, and fifteen peaks of lignans were identified.
CONCLUSIONThe method was easy, reliable and could be used as a powerful tool for the further quality control of S. chinensis.

10.Navebine continuous intravenous infusion plus cisplatin in the treatment of advanced breast cancer

Objective To evaluate the efficacy and toxicity of navebine(NVB) given by continuous infusion plus cisplatin combination therapy in the treatment of advanced breast cancer.Methods 60 patients with advanced breast cancer were enrolled in this study.The type of his to pathologyis invasive ductal carcinoma.19 patients had no previous chemotherapy but 21 cases received chemotherapy containing ADM.26 patients had ≥2 metasta-ticsites.All the patients received vinorelbine according to the following doses chedule:10mg bolus followed by 10mg by continuous infusion over 24 hours every 5 days every 3 weeks.Results 60 patients were evaluable for response.A total of 131 cycles were administered.The lverall response rate was 41.6%(3 patients achieved a complete response and 22 patients achieved a partial response).The median over all survival was 35 weeks and the median duration of response was 19 weeks.The dose limiting toxicity was marrow-suppression.Neutropenia was in 43% patients(31% was grade Ⅲ and Ⅳ).Thrombo-cytopenia and anemia were mild.Conclusion A highe rsponse is obtained in advanced breast cancer treated by continuous intravenous NVB infusion with tolerable toxicity and deserve further revaluation.
