1.Determination of multi-component content and analysis of differential components in Aurantii Fructus Immaturus from different origins

OBJECTIVE To determine the contents of 15 components in Aurantii Fructus Immaturus from different origins (Citrus aurantium, C. junos, C. aurantium Linn., C. sinensis Osb., C. sinensis), screen differential components, and provide references for the quality evaluation of Aurantii Fructus Immaturus. METHODS HPLC method was adopted to determine the contents of synephrine, N-methyltyramine, 5,7-dihydroxychromone-7-neohesperidoside, neoponcirin, narirutin, naringin, hesperidin, neohesperidin, naringenin, hesperetin, sinensetin, nobiletin, tangeretin, 5-demethylnobiletin, and auraptene in 46 batches of Aurantii Fructus Immaturus from different origins. The determination was performed on Waters Symmetry C18 column with mobile phase consisted of acetonitrile-0.1% formic acid (gradient elution) at the flow rate of 1.0 mL/min; column temperature was set at 40 ℃ , detection wavelength was 284 nm, and sample injection volume was 5 μL. The differences between different origins of Aurantii Fructus Immaturus were analyzed by cluster analysis, principal component analysis (PCA) and orthogonal partial least squares-discriminant analysis (OPLS-DA), and differential components were screened. RESULTS The linear relationships of the aforementioned 15 components were all good within the tested mass concentration ranges (all r>0.999 0). The RSDs for precision, stability (24 h), and repeatability tests were all less than 2.00%. The average recovery rate ranged from 91.1% to 103.9% (all RSDs<3.10%). Cluster analysis, PCA, and OPLS-DA revealed that C. sinensis Osb. and C. sinensis were clustered into one category, while C. aurantium,C. junos and C. aurantium Linn. were clustered into another category. The variable importance projection values for neohesperidin, auraptene, naringin, neoponcirin, tangeretin, hesperidin, sinensetin, and 5,7-dihydroxychromone-7-neohesperidoside were all greater than 1. CONCLUSIONS In this study, the contents of 15 components in Aurantii Fructus Immaturus from different origins are determined, and 8 differential components, including neohesperidin, auraptene, naringin, and neoponcirin, are screened out.
2.Risk Factors and a Prediction Model for Frequent Acute Exacerbations of Chronic Obstructive Pulmonary Disease

Objective To identify the risk factors of patients with frequent acute exacerbations of chro-nic obstructive pulmonary disease(AECOPD)and construct a prediction model based on the clinical data,provi-ding a theoretical basis for the clinical prevention and treatment.Methods A total of 25 638 COPD patients ad-mitted to the Department of Respiratory and Critical Care Medicine,the Third People's Hospital of Chengdu from January 1,2013 to May 1,2023 were selected.Among them,11 315 patients were included according to the inclusion and exclusion criteria,and their clinical characteristics were analyzed.Multivariate Logistic regression was carried out to identify the risk factors for frequent AECOPD.A nomogram model was utilized to quantify the risk of acute exacerbation,and the performance of the prediction model was assessed based on the area under the receiver operating characteristic(ROC)curve.Results In the patients with frequent AECOPD,male percentage(P<0.001),age(P<0.001),urban residence(P<0.001),smoking(P<0.001),length of stay(P<0.001),total cost(P<0.001),antibiotic cost(P<0.001),diabetes(P=0.003),respiratory failure(P<0.001),heart disease(P<0.001),application of systemic glucocorticoids(P<0.001),white blood cell count(P<0.001),neutrophil percentage(P<0.001),C-reactive protein(P<0.001),total cholesterol(P<0.001),and brain natriuretic peptide(BNP)(P<0.001)were all higher than those in the patients with infre-quent AECOPD.Multivariate Logistic regression analysis revealed that age,urban residence,smoking,diabe-tes,heart disease,Pseudomonas aeruginosa infection,application of systemic glucocorticoids,antibiotics,re-spiratory failure,and elevated white blood cell count,total cholesterol,and BNP were independent risk factors for hospitalization due to frequent AECOPD.A nomogram model of hospitalization due to frequent AECOPD was constructed according to risk factors.The ROC curve was established to evaluate the performance of the model,which showed the area under the ROC curve of 0.899(95%CI=0.892-0.905),the sensitivity of 85.30%,and the specificity of 79.80%.Conclusions Frequent AECOPD is associated with smoking,heart disease,ap-plication of systemic glucocorticoids,Pseudomonas aeruginosa infection,age,low body mass index,and elevat-ed BNP.Predicting the risks of hospitalization due to frequent AECOPD by the established model can provide the-oretical support for the treatment and risk factor management of the patients.
3.Study on the feasibility of low-dose chest CT combined with deep learning reconstruction algorithm in the diagnosis of pediatric mycoplasma pneumonia

Objective:To explore the feasibility of 60 kV low-dose scanning technique on chest combined with ClearInfinity deep learning reconstruction algorithm in the diagnosis of pediatric mycoplasma pneumonia.Methods:A total of 132 pediatric patients,who admitted to Taihe Hospital Affiliated to Hubei Medical College and were diagnosed as mycoplasma pneumonia,were selected,and all of them underwent computed tomography(CT)scans on chest.They were randomly divided into routine dose group(66 cases),low dose ClearView and ClearInfinity group(66 cases).In the routine dose group,the tube voltage of CT scan on chest was 100 kV,and 50%ClearView iterative algorithm was adopted in this group.The tube voltage of CT scan on chest was 60kV in low dose ClearView and ClearInfinity group,and 50%ClearView iterative algorithm and 50%ClearInfinity deep learning reconstruction algorithm were used respectively to conduct reconstruction.The difference of radiation dose among the three groups was compared.The CT values and standard deviation(SD)values,signal-to-noise ratio(SNR)and contrast-to-noise ratio(CNR)of the region of interest(ROI)of the images of 3 groups were measured and calculated.The images were subjectively evaluated by two diagnostic attending physicians with 10 years of work experience,and the Kappa test was adopted to analyze the consistency of the score results.Results:Compared with the routine dose group,the volume CT dose index(CTDIvol)values,dose-length product(DLP)values and effective radiation dose(ED)values of the low dose ClearView group and ClearInfinity group reduced respectively by 87.58%,87.24%and 88.00%,and the differences were statistically significant(t=4 584.07,63.73,61.27,P<0.01).The noise values of left and right lung of the routine dose group were significantly lower than those of the low dose ClearView group,while were significantly higher than those of ClearInfinity group,and the differences were significant(Z=-9.912,-7.013,P<0.01),and the difference of them between low dose ClearView group and ClearInfinity group was significant(Z=-9.912,P<0.01).The SNR and CNR of left and right lung of low dose ClearView group were significantly lower than those of the routine dose group,with statistically significant(t=-34.810,5.522,P<0.01),while these of the low dose ClearInfinity group were significant higher than them of the routine dose group(t=3.544,-8.674,P<0.05),respectively.The two attending physicians had favorable consistency in the subjective evaluation for images(Kappa>0.75,P<0.01).The subjective score of the routine dose group was not significantly different with that of the low dose ClearInfinity group(P>0.05),but was significantly better than that of the low dose ClearView group(Z=-6.425,P<0.01).Conclusion:For pediatric patients with mycoplasma pneumonia,the 60 kV low dose CT on chest combined with ClearInfinity deep learning reconstruction algorithm can ensure image quality on the premise of reducing radiation,which can ensure the diagnostic effect.
4.Diagnostic analysis of seven subtypes of mature small B-cell lymphoma involving the bone marrow and peripheral blood

Objective:To investigate the diagnosis and differential diagnosis methods of chronic mature small B-cell lymphoma involving the bone marrow and peripheral blood.Methods:The clinical data of 27 patients with mature small B-cell lymphoma involving the bone marrow and peripheral blood (seven subtypes phase IV) who received treatment in the Kunshan Third People's Hospital from February 2015 to June 2021 were retrospectively analyzed. The application value of different detection methods in the diagnosis of mature small B-cell lymphoma involving the bone marrow and peripheral blood was analyzed.Results:The majority of patients' peripheral blood was mainly characterized by an increase in the ratio or absolute value of lymphocytes. In terms of cell morphology, mature lymphocytes were mainly small to medium in size. A few bone marrow smears or peripheral blood smears show characteristic changes in cell morphology. Flow cytometry results showed that among the cohort of 15 patients presenting CD5 expression, 11 patients had chronic lymphocytic leukemia, 1 patient had mantle cell lymphoma, 1 patient had splenic diffuse red pulp small B-cell lymphoma, and 2 patients had B-cell chronic lymphoproliferative disorders (unclassified). Among 12 patients presenting no CD5 expression, 8 had Waldenstr?m's macroglobulinemia, 3 had splenic marginal zone lymphoma, and 1 had follicular lymphoma. Among the 2 patients presenting CD5-CD10 expression, 1 patient had follicular lymphoma, and 1 patient had Waldenstr?m's macroglobulinemia. One patient with splenic diffuse red pulp small B-cell lymphoma expressed CD5, CD11c, and CD103 in addition to pan-B-cell markers, while BRAF V600E mutation detection and immunohistochemical staining for tartrate-resistant acid phosphatase and annexin-1A showed negative expression.Conclusion:This type of lymphoproliferative disease is a general term for lymphoma that has various different molecular and biological characteristics. Its diagnosis and differential diagnosis need to comprehensively consider the clinical characteristics of the patient, relevant laboratory tests, cell morphology, flow cytometry detection results, reasonable use of fluorescence in situ hybridization, molecular biology, special chemistry, and bone marrow immunohistochemistry. In a few cases, diagnosis of the lymphoproliferative disease still relies on non-bone marrow involvement and tissue biopsy.
5.Clinical application of adjustable traction skin stretchers in repair of wounds at the lower leg, foot and ankle

Objective:To evaluate the clinical effects of adjustable traction skin stretchers used in repair of wounds at the lower leg, foot and ankle.Methods:A retrospective study was performed to analyze the clinical data of 56 patients who had been treated for skin defects at the lower leg, foot and ankle from August 2016 to September 2022 at The First Affiliated Hospital of Zhengzhou University, Honghui Hospital, Affiliated to Xi'an Jiaotong University Medical College, The First Affiliated Hospital of Henan Polytechnic University, and Yunnan Zhongde Orthopedic Hospital. There were 35 males and 21 females, aged (39.9±18.7) years. There were 43 traumatic wounds, 3 burns, 6 inflammatory wounds, 3 relief incisions due to osteofascial compartment syndrome, and 1 scar. The areas of skin defect ranged from 2.5 cm × 2.0 cm to 20.0 cm × 10.0 cm. The duration of wounds was (8.6±7.8) d. All the wounds were repaired with adjustable traction skin stretchers. The row-hook type of skin stretchers was used in 28 cases, the single-rod type in 20 cases, the single-rod type combined with an external fixator in 5 cases, and a combination of the row-hook type and the single-rod type in 3 cases.The time for wound traction closure, color of wound skin margin, skin swelling around the wound, functional recovery of affected limb and complications were recorded.Results:The time from skin stretching to wound closure was (7.8±3.8) d in the 56 patients. The color of wound skin edge after stretching was normal in 16 cases, dark red in 38 cases, and dark in 2 cases; the skin swelling around the wound was degree 1 in 21 cases, degree 2 in 33 cases, and degree 3 in 2 cases. The 56 patients were followed up for (8.9±4.1) months. Primary wound closure was achieved in 48 patients, and secondary wound closure in 8 patients after repair with an autologous skin graft. Partial skin necrosis occurred due to tension blisters after skin stretching in 2 patients, one of whom was repaired with an autologous skin graft and the other of whom by dressing change. Deep bone infection recurred in 2 patients whose wounds healed after their bone defects were repaired using Ilizarov technique of bone transfer. In the 56 patients, the muscle strength of the lower extremity beyond the wound was recovered to normal, and the range of motion of the joints adjacent to the wound also recovered to normal.Conclusion:In repair of wounds at the lower leg, foot and ankle, adjustable traction skin stretchers can lead to fine clinical effects and limited complications, because the stretchers can control the tension of skin digitally and precisely.
6.Development and application syndromic surveillance and early warning system in border area in Yunnan Province.

Objective: To establish a dynamic syndromic surveillance system in the border areas of Yunnan Province based on information technology, evaluate its effectiveness and timeliness in the response to common communicable disease epidemics and improve the communicable disease prevention and control in border areas. Methods: Three border counties were selected for full coverage as study areas, and dynamic surveillance for 14 symptoms and 6 syndromes were conducted in medical institutions, the daily collection of information about students' school absence in primary schools and febrile illness in inbound people at border ports were conducted in these counties from January 2016 to February 2018 to establish an early warning system based on mobile phone and computer platform for a field experimental study. Results: With syndromes of rash, influenza-like illness and the numbers of primary school absence, the most common communicable disease events, such as hand foot and mouth disease, influenza and chickenpox, can be identified 1-5 days in advance by using EARS-3C and Kulldorff time-space scanning models with high sensitivity and specificity. The system is easy to use with strong security and feasibility. All the information and the warning alerts are released in the form of interactive charts and visual maps, which can facilitate the timely response. Conclusions: This system is highly effective and easy to operate in the detection of possible outbreaks of common communicable diseases in border areas in real time, so the timely and effective intervention can be conducted to reduce the risk of local and cross-border communicable disease outbreaks. It has practical application value.

7.Bilateral hypertensive intracerebral hemorrhage in basal ganglia: a case report

Hypertensive intracerebral hemorrhage (ICH) is mostly single in basal ganglia, thalamus and pons. Simultaneous hemorrhage in other brain regions is relatively rare, accounting for only 5.6% of all hemorrhagic strokes, while bilateral symmetrical hemorrhage is extremely rare. A case of bilateral basal ganglia symmetrical hemorrhage is reported for clinical reference.
8.Characteristics and the first CD4+T lymphocyte counts of newly reported HIV-infected cases aged 50 and above in Dehong Prefecture of Yunnan Province during 2016 to 2021

ObjectiveTo investigate the factors that influence the first CD4+T lymphocyte counts in newly reported HIV-infected cases aged 50 and above in Dehong Prefecture of Yunnan Province during 2016 to 2021, and to understand the patient immune status and disease progression so as to provide scientific basis for HIV prevention and control strategies in the future. MethodsData was collected from the national HIV/AIDS information system. Multivariate logistic regression was used for the analysis of factors affecting the first CD4+T lymphocyte counts. ResultsA total of 642 cases of HIV infection were newly reported, among them, 571 cases had CD4+T lymphocyte counts and 200 cases (35.03%) had CD4+T lymphocyte counts <200 cells·μL-1. Patients who were in the 50-59 age group, male, divorced or widowed, and less educated were more likely to have CD4+T lymphocyte counts <200 cells·μL-1. Compared with active testing consultants, forced reeducation through labor or drug rehabilitation cases were less likely to have CD4+T lymphocyte counts <200 cells·μL-1. ConclusionThere is no obvious upward trend in newly reported HIV infected persons aged 50 years and above in Dehong Prefecture during 2016 to 2021. However, the situation of CD4+T lymphocyte counts <200 cells·μL-1 is still serious. Attention should be paid to the key groups: male, Chinese nationality, farmers, Han nationality, married or divorced, junior high school education or below, and heterosexual transmission. It is necessary to strengthen the intervention in people aged 50 and above and improve the detection efficiency.
9.Safety of intraarterial microguidewire electrocoagulation in aneurysms: an animal experimental study

Objective:To explore the efficacy and safety of intraarterial microguidewire electrocoagulation in arterial aneurysms.Methods:(1) SilverSpeed, a kind of microguidewire used in clinical intravascular treatment for intracranial aneurysms, was used to conduct in vitro electrolysis gas generation experiment with isolated arterial blood of anticoagulant New Zealand white rabbits as medium, and thrombus attachment on the surface of microguidewire was observed under scanning electron microscope. (2) Rabbit common carotid artery aneurysm models were established by using vein bag transplantation method, and divided into microguidewire electrocoagulation treatment groups ( n=40) and blank control group ( n=10). The number of closured tumor cavity and the quality of formed thrombus were observed after electrocoagulation simulation treatment with SilverSpeed microguidewire (charging at 6, 9, 12, 15, and 18 V voltage, respectively for 1, 3, 6, 9, 12, and 15 min). DSA was used to observe whether there was ruptured aneurysms or thrombosis of parent artery. Twelve h later, head MRI diffusion weighted sequence scan was performed to detect whether there were new cerebral ischemia foci in the distal cerebral blood supply area of the parent artery. DSA was performed again 6 months after surgery to observe whether the aneurysms recurred. Results:(1) Electrolytic gas generation experiment results showed that bubbles were generated after electrification of SilverSpeed microguidewire; the higher the voltage, the more severe the reaction. Scanning electron microscope showed that thrombus attached to the surface of the microguidewire after electrification in isolated blood; and the higher the voltage, the denser the thrombus. (2) Under the same charging time, the higher the voltage, the larger the number of closured tumor cavity in rabbits of the microguidewire electrocoagulation treatment groups. Under the same voltage, the longer the charging time, the better the quality of thrombosis. Ischemic events occurred only in the microguidewire electrocoagulation treatment group with voltage>9 V, and the charging duration was not associated with the incidence of embolic events. When the voltage was 15 V, 2 experimental rabbits died due to aneurysm rupture 3 min after electrification. When the voltage was 18 V, 4 experimental rabbits died of cardiac arrest 9 min after electrification, and another 2 rabbits died of aneurysm rupture 6 min after electrification.Conclusions:High voltage is the main cause of adverse events in the microguidewire electrocoagulation treatment of aneurysms. After setting the appropriate voltage, prolonging the electrification time can improve the electrocoagulation effect without increasing the safety risk.
10.A food poisoning incident suspected to be caused by proteus in a city of Dehong Prefecture,Yunnan Province

Objective To Investigate the cause of a food poisoning incident in a city in Dehong Prefecture, determine the scope of the incident, and to formulate effective prevention and control measures. Methods Field epidemiology and case-control study methods were used to formulate case definition; carry out case search; find suspicious food combined with clinical manifestations, dining history and other information; and collect samples from the suspicious food, cases and environment for laboratory testing. Results A total of 160 cases were found and the incidence rate was 26.02% (160/615). The main clinical manifestations were diarrhea, leukocytosis, vomiting, nausea and abdominal pain. The average incubation period was 10.89±5.09 h, with 2 h as the shortest and 28 h as the longest. The epidemiological curve suggested the point source exposure mode.
