4.Death rate and life loss caused by injuries in Jinshan District, Shanghai in 1990-2023

Objective To analyze the death rate and life loss of injuries in Jinshan District of Shanghai from 1990 to 2023, and to provide a scientific basis for the formulation of injury prevention and control measures. Methods Excel 2010 was used to establish a database for injury death reports in Jinshan District from 1990 to 2023, and the crude mortality rate, standardized mortality rate, age-specific mortality rate, potential years of life lost (PYLL), rate of potential years of life lost (PYLLR) and standardized rate of potential years of life lost (SPYLLR) were calculated. Results From 1990 to 2023, the crude mortality rate due to injuries in Jinshan District was 52.25/100 000, while the standardized mortality rate was 36.95/100 000. Based on the standardized mortality rate, the top four causes of injury-related deaths were traffic accidents, falls, drowning, and suicide. Traffic accidents ranked first as the leading cause of death for both males (19.30/100 000) and females (8.41/100 000). The top cause of death for the 0-14 years old group was drowning, the 15-64 years old group was traffic accidents, and the 65 years old and above was falls. Over the 34-year period, according to SPYLLR, the top four causes of death for both genders were consistently traffic accidents, drowning, suicide, and falls. Conclusion From 1990 to 2023, traffic accidents, falls, drowning, and suicide have been the primary causes of injury-related deaths in Jinshan District, Shanghai. It is essential to develop targeted prevention and control strategies based on the specific characteristics of these injury-related fatalities.

6.Establishment and application of intravenous drug resolvent generation system and solvent incompatibility rules library

OBJECTIVE To standardize the use of sterile powder for injection redissolution solvent and intravenous drugs diluent solvent, and ensure the safety of clinical drug use. METHODS The intravenous drug resolvent generation system and the intravenous drug solvent incompatibility rules library were constructed and operated by prescription-checking pharmacist team in our hospital by formulating a list of intravenous drugs needed to be dissolved with sterilized water for injection and a contraindication list of intravenous solvent compatibility. The unreasonable medical orders reviewed and intervened in our hospital from January to April 2023 (before the implementation of the intravenous drug solvent incompatibility rules library) and from May to August 2023 (after the implementation) were analyzed statistically to compare the number of unreasonable solvent selection orders, interception rate, and the success rate of pharmacists’ interventions for unreasonable solvent selection orders before and after the implementation. RESULTS Before the implementation of the solvent incompatibility rules library, a total of 5 229 groups of medical orders with unreasonable solvent selection in our hospital were identified, among which there were 1 204 groups of seriously unreasonable medical orders, with interception rate of 23.03% and the success rate of pharmacists’ intervention of 15.90%. After the implementation of the solvent incompatibility rules library, the total number of medical orders with unreasonable solvent selection was 3 258 groups, among which there were 1 148 groups of seriously unreasonable medical orders, with interception rate of 35.24% and success rate of pharmacists’ intervention of 24.83%, being significantly higher than before implementation (P<0.01 or P<0.001). CONCLUSIONS The establishment and application of the intravenous drug resolvent generation system and the solvent incompatibility rules library can significantly increase the interception rate of unreasonable solvent selection orders and the success rate of pharmacists’ interventions, and ensure the safety of clinical drug use.
7.MiR-101-5p inhibits the proliferation and invasion of lung squamous cell carcinoma cells by targeting ATAD2

【Objective】 To investigate the molecular mechanism of microRNA-101-5p (miR-101-5p) affecting the proliferation and invasion of lung squamous cell carcinoma (LUSC) cells. 【Methods】 We downloaded the miRNA mature expression data and total RNA sequencing data of TCGA-LUSC from TCGA database to identify differentially expressed genes. The signal pathway enriched in ATAD2 was analyzed. The mRNA expressions of miR-101-5p and ATAD2 in the LUSC cells were detected by qRT-PCR. The effects of miR-101-5p on the proliferation and invasion of LUSC cells were detected by MTT assay, cloning assay, and invasion assay. The effects of ATAD2 on the cell cycle of LUSC cells were detected by flow cytometry. Western blotting was used to detect the expression of ATAD2 protein. Double luciferase experiment was used to verify whether miR-101-5p could bind to ATAD2 target. Finally, we detected the changes in the proliferation, cloning and invasion ability of LUSC cells by co-transfection with oe-ATAD2 and miR-101-5p mimic, and further explored whether miR-101-5p could regulate the biological function of LUSC cells through ATAD2. 【Results】 The miR-101-5p was significantly downregulated in LUSC tissues and cells. Overexpression of miR-101-5p could significantly inhibit the proliferation and invasion of LUSC cells. ATAD2, its downstream regulatory target gene, was significantly upregulated in LUSC, and miR-101-5p and ATAD2 expressions were inversely correlated. GSEA enrichment results showed that ATAD2 was significantly enriched in the cell cycle signal pathway. The double luciferase experiment proved that miR-101-5p targeted ATAD2, and the recovery experiment showed that miR-101-5p regulated the proliferation and invasion of LUSC cells by targeting ATAD2. 【Conclusion】 In this study, we found that miR-101-5p had low expression in LUSC, and that miR-101-5p decreased the proliferation and invasion of LUSC cells by targeted inhibition of ATAD2.
8.Prognostic value of combined preoperative MRI and postoperative pathological assessment of lymph node metastasis in rectal cancer patients

Objective To investigate the value of combining preoperative magnetic resonance imaging(MRI)and postoperative pathological assessment of lymph node metastasis in predicting overall survival in rectal cancer patients.Methods This retrospective study collected clinical,pathological and image information of 2610 patients histopathologically confirmed with rectal adenocarcinoma at the Sixth Affiliated Hospital of Sun Yat-sen University between January 2016 and December 2021.All patients underwent MRI scans and were divided into three groups according to lymph node status assessed by preoperative MRI(MRIN)and postoperative pathology(PN):MRIN+but PN-(MRIN+group),PN+but MRIN-(PN+group),MRIN+and PN+(MRI-PN+group).Inverse probability weighting(IPW)was used to adjust for confounding factors.Kaplan-Meier curves were used to estimate overall survival and log-rank tests were used to compare the difference.Univariate Cox regression models were used to analyze the correlation between tumor characteristics and overall survival,and bidirectional stepwise Cox regression models were used to identify independent risk factors for overall survival.Results The MRI-PN+group showed higher tumor staging,more frequent perineural invasion,more distant metastases,and a higher risk of death compared to the P N+group and MRIN+group(all P<0.05).Kaplan-Meier curves showed that the 3-year survival rates for the MRIN+group,PN+group,and MRI-PN+group were 90.5%,79.1%,and 76.4%,respectively;the 5-year survival rates were 85.7%,71.5%,and 59.2%,respectively.Stepwise Cox regression showed that age,tumor location,carcino-embryonic antigen,carbohydrate antigen 19-9,lymph nodes number,pathological tumor stage,lymphovascular invasion,perineural invasion,distant metastasis,neoadjuvant therapy and adjuvant therapy,and MRI-pathology lymph node status were independent risk factors for overall survival in rectal cancer(all P<0.05).Conclusion Evaluating the lymph node status by combining preoperative MRI and postoperative pathology helps predict overall survival in rectal cancer patients more accurately.
9.Effect of inferior vena cava respiratory variability-guided fluid therapy after laparoscopic hepatectomy: a randomized controlled clinical trial.

BACKGROUND:
After major liver resection, the volume status of patients is still undetermined. However, few concerns have been raised about postoperative fluid management. We aimed to compare gut function recovery and short-term prognosis of the patients after laparoscopic liver resection (LLR) with or without inferior vena cava (IVC) respiratory variability-directed fluid therapy in the anesthesia intensive care unit (AICU).
METHODS:
This randomized controlled clinical trial enrolled 70 patients undergoing LLR. The IVC respiratory variability was used to optimize fluid management of the intervention group in AICU, while the standard practice of fluid management was used for the control group. The primary outcome was the time to flatus after surgery. The secondary outcomes included other indicators of gut function recovery after surgery, postoperative length of hospital stay (LOS), liver and kidney function, the severity of oxidative stress, and the incidence of severe complications associated with hepatectomy.
RESULTS:
Compared with patients receiving standard fluid management, patients in the intervention group had a shorter time to anal exhaust after surgery (1.5 ± 0.6 days vs. 2.0 ± 0.8 days) and lower C-reactive protein activity (21.4 [95% confidence interval (CI): 11.9-36.7] mg/L vs. 44.8 [95%CI: 26.9-63.1] mg/L) 24 h after surgery. There were no significant differences in the time to defecation, serum concentrations of D -lactic acid, malondialdehyde, renal function, and frequency of severe postoperative complications as well as the LOS between the groups.
CONCLUSION:
Postoperative IVC respiratory variability-directed fluid therapy in AICU was facilitated in bowel movement but elicited a negligible beneficial effect on the short-term prognosis of patients undergoing LLR.

10.Establishment of a modified BISAP scoring system and its clinical significance in the early diagnosis and prognostic evaluation of severe acute pancreatitis

ObjectiveTo establish a modified BISAP scoring system, and to investigate the value of the BISAP scoring system versus the modified BISAP scoring system in assessing the severity and condition of acute pancreatitis (AP). MethodsFor the establishment of the new scoring system, a retrospective analysis was performed for the clinical data of 1 033 patients with AP who were admitted to Third Xiangya hospital of central South University from January 2019 to December 2021, and according to the revised Atlanta classification, they were divided into mild acute pancreatitis (MAP) group with 827 patients and severe acute pancreatitis (SAP) group with 206 patients. The two groups were compared in terms of clinical features, laboratory markers, and imaging data. A binary logistic regression analysis was performed for the statistically significant indicators to screen for the independent risk factors for SAP. The receiver operating characteristic (ROC) curve was used to obtain the optimal cut-off value corresponding to the maximum Youden index for each independent risk factor, and a score of 0 or 1 was assigned depending on different situations, which was integrated into the BISAP scoring system to establish a modified BISAP scoring system. For the validation of the new scoring system, a retrospective analysis was performed for the clinical data of 473 patients with AP who were admitted to Third Xiangya hospital of central South University from January 2017 to December 2018. BISAP score and modified BISAP score were determined for each patient, and the area under the ROC curve (AUC) was used to compare the value of the two scoring systems in predicting the severity and prognosis of AP. The chi-square test or the Fisher’s exact test was used for comparison of categorical data between two groups, and the independent-samples t test and the Mann-Whitney U test were used for comparison of continuous data between two groups. ResultsFor the establishment of the new scoring system, there were significant differences between the MAP group and the SAP group in mode of admission, length of hospital stay, ICU admission rate, number of deaths, underlying diseases, and incidence rate of complications (all P<0.05). The binary logistic regression analysis showed that body temperature, neutrophil-to-lymphocyte ratio (NLR), C-reactive protein (CRP), albumin, triglycerides, D-dimer, fibrinogen, and MCTSI score were independent risk factors for SAP (all P<0.05). The ROC curve analysis showed that CRP (AUC=0.921), NLR (AUC=0.798), D-dimer (AUC=0.768), and MCTSI score (AUC=0.931) had a good predictive value for SAP, and the combination of these four indicators had an AUC of 0.976 and showed a significantly higher diagnostic efficiency than each indicator alone or the combination of two or three indicators (all P<0.05). For the validation of the new scoring system, a total of 473 patients were enrolled, with 408 in the MAP group and 65 in the SAP group, and there were significant differences between the two groups in mode of admission, length of hospital stay, ICU admission rate, number of deaths, and incidence rate of complications (all P<0.05). The modified BISAP score was better than the BISAP score in predicting SAP (AUC: 0.972 vs 0.887, P<0.05), with an optimal cut-off value of >3 points. The modified BISAP score also had a relatively high value in predicting the mortality of AP patients (AUC=0.910), but there was no significant difference between the modified BISAP score and the BISAP scoring system (AUC: 0.910 vs 0.896, P=0.707). ConclusionThe modified BISAP score is better than the BISAP score in predicting the severity of AP and has a relatively high value in predicting the mortality of AP patients, giving a more accurate, objective, and early assessment of the condition of AP patients.
