1.Compatibility Effect and Mechanism of Astragali Radix-Curcumae Rhizoma on Antitumor: A Review

Malignant tumor is a serious and difficult disease threatening human health, which has a high morbidity and mortality rate worldwide. Traditional Chinese medicine has unique advantages in improving the therapeutic effect of malignant tumors and alleviating adverse reactions. Traditional Chinese medicine believes that Qi deficiency and blood stasis are important pathogeneses in the development of malignant tumors, and the method of supplementing Qi and activating blood is an effective strategy for treating malignant tumors. Astragali Radix, sweet in taste and warm in nature, has effects of tonifying Qi and rising Yang, strengthening the exterior and reducing sweat, promoting fluid and nourishing blood. Curcumae Rhizoma, acrid and bitter in taste and warm in nature, has the effects of promoting Qi and breaking blood stasis, eliminating mass, and relieving pain. Astragali Radix-Curcumae Rhizoma, as the classic herb pair of invigorating Qi and activating blood, has a clear effect on inhibiting tumor growth and metastasis. Studies have shown that Astragali Radix-Curcumae Rhizoma contains astragalus polysaccharide, astragaloside, calycosin, formononetin, curcumin, β-elemene, curcumenol, curcumenone, curcumendione, gemacrone, and other anti-tumor active ingredients. It can significantly inhibit the occurrence and development of liver cancer, colorectal cancer, gastric cancer, lung cancer, ovarian cancer, cervical cancer, breast cancer, and other cancers and has the advantages of superposition effect, synergistic complementarity, and increased dissolution compared with single herb and monomer of Chinese traditional herbs and has been widely valued in the field of TCM anti-cancer. Its anti-tumor mechanism includes inhibition of tumor cell proliferation, promotion of tumor cell apoptosis and autophagy, anti-invasion and metastasis, regulation of immune function, and enhancement of anti-tumor drug sensitivity. By combining Chinese and foreign literature, the compatibility effect and anti-tumor mechanism of Astragali Radix-Curcumae Rhizoma were summarized, and then scientific compatibility of these two herbs was expounded, in order to provide a useful reference for clinical application and future research of Astragali Radix-Curcumae Rhizoma.
2.Cancer cell membrane-coated bacterial ghosts for highly efficient paclitaxel delivery against metastatic lung cancer.

Chemotherapy is one of the major approaches for the treatment of metastatic lung cancer, although it is limited by the low tumor delivery efficacy of anticancer drugs. Bacterial therapy is emerging for cancer treatment due to its high immune stimulation effect; however, excessively generated immunogenicity will cause serious inflammatory response syndrome. Here, we prepared cancer cell membrane-coated liposomal paclitaxel-loaded bacterial ghosts (LP@BG@CCM) by layer-by-layer encapsulation for the treatment of metastatic lung cancer. The preparation processes were simple, only involving film formation, electroporation, and pore extrusion. LP@BG@CCM owned much higher 4T1 cancer cell toxicity than LP@BG due to its faster fusion with cancer cells. In the 4T1 breast cancer metastatic lung cancer mouse models, the remarkably higher lung targeting of intravenously injected LP@BG@CCM was observed with the almost normalized lung appearance, the reduced lung weight, the clear lung tissue structure, and the enhanced cancer cell apoptosis compared to its precursors. Moreover, several major immune factors were improved after administration of LP@BG@CCM, including the CD4+/CD8a+ T cells in the spleen and the TNF-α, IFN-γ, and IL-4 in the lung. LP@BG@CCM exhibits the optimal synergistic chemo-immunotherapy, which is a promising medication for the treatment of metastatic lung cancer.
3.The Influence of Shoutai Wan (寿胎丸) on Lactic Acid Content at the Maternal-Fetal Interface and Expression of Immune-Related Factors in Recurrent Miscarriage Model Mice

ObjectiveTo explore the possible mechanisms of Shoutai Wan (寿胎丸) in treating recurrent miscarriage (RSA) from the perspective of immune tolerance under the acidic microenvironment at the maternal-fetal interface. MethodsFemale CBA/J mice were randomly divided into normal group, model group, progesterone group, and Shoutai Wan group, with 15 mice in each group. The mice in the normal group and model group were given 0.2 ml distilled water by gavage each day, the Shoutai Wan group given Shoutai Wan decoction 0.15 g/(10 g·d) by gavage, the progesterone group given progesterone tablets 0.44 mg/(10 g·d) by gavage. After gavage for 14 days, the mice were cohabited. Female CBA/J mice in the normal group were mated with male BALB/c mice at a ratio of 2∶1, and female CBA/J mice in the other groups were mated with male DBA/2 mice at a ratio of 2∶1 to establish the RSA mouse model. Vaginal smears were taken from the female mice the next morning, and the appearance of a large number of spermatozoa and the presence of a vaginal plug were considered as the first day of pregnancy. After the appearance of the plug, the mice were continued to be administered according to the previous method until the 10th day of pregnancy. On the 10th day of pregnancy, maternal-fetal interface tissues were collected from each group of mice, and lactate dehydrogenase colorimetric method was used to detect lactate (LA) content; qPCR method and Western blot method were used to detect the expression of immune-related factors interleukin-4 (IL-4), interferon-gamma (IFN-γ), transforming growth factor beta 1 (TGF-β1), and forkhead box protein 3 (Foxp3) mRNA and protein; flow cytometry was used to detect the numbers of helper T lymphocyte 1 (Th1), helper T lymphocyte 2 (Th2), regulatory T cell (Treg), classical macrophage (M1), and alternative macrophage (M2). The bivariate Pearson test was used to analyze the correlation between LA content and the numbers of Th1, Th2, Treg, M1, and M2 cells, as well as the correlation between LA content and the expression of IL-4, IFN-γ, TGF-β1, Foxp3 protein, and mRNA. ResultsOn the 10th day of pregnancy, compared with the normal group, the LA content decreased in the model group, and the expression of IL-4, TGF-β1, Foxp3 protein and mRNA in the maternal-fetal interface tissues decreased, while the expression of IFN-γ protein and mRNA increased. The numbers of Th1 and M1 cells increased, while the numbers of Th2, Treg, and M2 cells decreased (P<0.05 or P<0.01). Compared with the model group, the LA content increased in the Shoutai Wan group and progesterone group. The expression of IL-4, TGF-β1, Foxp3 protein and mRNA in the maternal-fetal interface tissues increased, while the expression of IFN-γ protein and mRNA decreased. The numbers of Th1 and M1 cells decreased, while the numbers of Th2, Treg, and M2 cells increased (P<0.05 or P<0.01). The LA content was positively correlated with the numbers of Th2, Treg, and M2 cells, and the expression of IL-4, TGF-β1, Foxp3 protein, and mRNA (P<0.05 or P<0.01); the LA content was negatively correlated with the numbers of Th1, M1 cells, and the expression of IFN-γ protein and mRNA (P<0.05 or P<0.01). ConclusionShoutai Wan may improve immune tolerance by regulating the expression of immune-related factors in the acidic microenvironment at the maternal-fetal interface of RSA model mice, thereby exerting its role in preventing miscarriage.
4.Clinical analysis of retrograde distal perfusion via posterior tibial artery in femoral veno-arterial extracorporeal membrane oxygenation

Objective:To summarize and analyze the modalities and experience of retrograde distal perfusion with distal perfusion catheter (DPC) via cannulation of the posterior tibial artery in veno-arterial extracorporeal membrane oxygenation(V-A ECMO).Methods:The clinical data of 15 patients who were treated with V-A ECMO and underwent DPC placement via the posterior tibial artery in our hospital from January 2022 to June 2023 were retrospectively analyzed.Results:The V-A ECMO catheterization method in 15 patients was percutaneous puncture catheterization, and all of them underwent surgical incision to indwelling retrograde DPC through the posterior tibial artery: 6 cases of preventive catheterization, 9 cases of remedial catheterization, the success rate of one-time catheterization was 93.33%, and the type of catheter was mainly 6 F sheath (66.67%). There was no ALI in preventive catheterization, and one case of osteofascial compartment syndrome occurred in remedial catheterization, and the catheterization time was (20.73 ± 3.47) min.Conclusions:In V-A ECMO, placement of DPC via the posterior tibial artery for retrograde distal perfusion is perfectly feasible, and has a high success rate, which can prevent or treat lower extremity ischemia.
5.Discussion on Prescription Law of Wang Yinglin's Treatment for Pediatric Cough Based on Carma Algorithm and Complex Network

Objective To explore the prescription law of Professor Wang Yinglin for treating pediatric cough based on the Carma algorithm and complex network.Methods The prescriptions of children with cough as the chief complaint who were treated by Professor Wang in the outpatient department from November 2022 to May 2023 were taken as the research object.Carma algorithm and complex network were used to analyze the main prescriptions of Professor Wang for treating children's cough,and explore the prescription law of Professor Wang for treating children's cough.Results A total of 420 cases were included,with an average age of 6.5 years old.Among them,there were 158 males and 262 females,involving 420 prescriptions,97 kinds of Chinese materia medica,a total frequency of 4 665,and 37 drugs with a frequency of use>20.By analyzing the drug combination derived from Carma analysis of algorithms and clinical verification,it was found that Professor Wang commonly used two drug combinations to treat children's cough:Poria-Exocarpium Citri Rubrum,Scrophulariae Radix-Imperatae Rhizoma,Peucedani Radix-Cynanchi Stauntonii Rhizoma,Perillae Fructus-Asteris Radix,Saposhnikoviae Radix-Liquidambaris Fructus;three medicine combination:Perillae Fructus-Asteris Radix-Semen Lepidii,Poria-Cablin Potchouli Herb-Exocarpium Citri Rubrum,Magnoliae Flos-Saposhnikoviae Radix-Liquidambaris Fructus;the combination of four drugs included Poria-Scrophulariae Radix-Exocarpium Citri Rubrum-Imperatae Rhizoma,Poria-Adenophorae Radix-Exocarpium Citri Rubrum-Scrophulariae Radix;five medicine combinations:Poria-Scrophulariae Radix-Exocarpium Citri Rubrum-Imperatae Rhizoma-Adenophorae Radix,Poria-Scrophulariae Radix-Exocarpium Citri Rubrum-Imperatae Rhizoma-Cablin Potchouli Herb;six medicine combinations:Poria-Scrophulariae Radix-Exocarpium Citri Rubrum-Imperatae Rhizoma-Adenophorae Radix-Folium Eriobotryae,Poria-Scrophulariae Radix-Exocarpium Citri Rubrum-Imperatae Rhizoma-Adenophorae Radix-Isatidis Radix,Poria-Scrophulariae Radix-Exocarpium Citri Rubrum-Imperatae Rhizoma-Cablin Potchouli Herb-Saposhnikoviae Radix,Folium Eriobotryae-Perillae Fructus-Asteris Radix-Semen Lepidii-Peucedani Radix-Cynanchi Stauntonii Rhizoma,Glehniae Radix-Crataegi Fructus-Stemonae Radix-Bulbus Lilii-Bulbus Fritillariae Cirrhosae-Ophiopogonis Radix.Complex network analysis found that the core drugs were:Adenophorae Radix,Poria,Exocarpium Citri Rubrum,Scrophulariae Radix,Imperatae Rhizoma,Folium Eriobotryae,Bulbus Fritillariae Thunbergii,Isatidis Radix,Peucedani Radix,Cynanchi Stauntonii Rhizoma,Stemonae Radix,Bambusae Concretio Silicea,Cablin Potchouli Herb.Five core prescriptions were obtained by multi-scale backbone network analysis.Conclusion Professor Wang's treatment of pediatric cough varies depending on the medical history,symptoms,and location of the disease,with different prescriptions.New diseases are often considered based on pathogenic factors,with phlegm heat as the main treatment,and the efficacy is mostly achieved by purging the lungs and resolving phlegm;phlegm heat gradually subsides,and residual pathogens are not cleared.The main approach is to eliminate residual pathogens and replenish qi and yin;long term illness mainly focuses on supplementing qi and nourishing yin.
6.Expression levels of complement regulatory proteins CD55 and CD59 in the peripheral blood of patients with Stevens-Johnson syndrome/toxic epidermal necrolysis

Objective:To detect expression levels of complement regulatory proteins CD55 and CD59 in the peripheral blood of patients with Stevens-Johnson syndrome (SJS) /toxic epidermal necrolysis (TEN), and to preliminarily analyze their potential roles in the occurrence of SJS/TEN.Methods:Hospitalized patients with SJS/TEN (SJS/TEN group) were collected from the Department of Dermatology of the First Affiliated Hospital of Anhui Medical University and the Second Affiliated Hospital of Anhui Medical University from December 2017 to December 2022. Meanwhile, patients with maculopapular exanthema (MPE) and healthy physical examinees were also collected and served as the mild group and healthy control group, respectively. Flow cytometry was performed to determine the proportions of CD4 + T lymphocytes and CD8 + T lymphocytes in peripheral blood mononuclear cells (PBMCs). The expression levels of inflammatory cytokines tumor necrosis factor (TNF) -α, interleukin (IL) -6, IL-17, IL-10, interferon (IFN) -γ, IL-2, and IL-4 were detected using flow cytometric bead array technology. The mRNA expression levels of CD55 and CD59 in PBMCs were detected by real-time fluorescence-based quantitative PCR (qRT-PCR). Flow cytometry was also performed to determine the protein expression of CD55 and CD59 on the surface of CD8 + T lymphocytes. Statistical analyses were carried out using one-way analysis of variance and Tukey's test. Results:Totally, 13 patients with SJS/TEN, 27 patients with MPE, and 40 healthy controls were collected. Among the SJS/TEN patients, there were 8 males and 5 females, with their age being 18 to 84 (47.15 ± 19.99) years, and disease duration being 7.74 ± 2.63 days. No significant differences were observed in the gender distribution or age among the 3 groups (both P > 0.05). The proportions of CD4 + T lymphocytes did not differ among the 3 groups ( F = 3.84, P = 0.051). The proportions of CD8 + T lymphocytes in the peripheral blood were significantly higher in the SJS/TEN group (25.60% ± 4.57%) than in the healthy control group (16.20% ± 6.28%; q = 4.59, P = 0.018). The expression levels of inflammatory cytokines TNF-α, IL-6, IL-10, IL-17, and IFN-γ were significantly higher in the SJS/TEN group than in the healthy control group and mild group (all P < 0.001). In addition, the mRNA expression of CD55 ( F = 9.46, P < 0.001) and CD59 in PBMCs ( F = 15.14, P < 0.001) was significantly lower in the SJS/TEN group than in the mild group and healthy control group. The protein expression levels of CD55 ( F = 51.51, P < 0.001) and CD59 ( F = 31.59, P < 0.001) on the surface of CD8 + T lymphocytes were also significantly lower in the SJS/TEN group than in the other two groups and the healthy control group, respectively. Conclusion:Complement regulatory proteins CD55 and CD59 were downregulated in SJS/TEN patients, which may be associated with the activation of CD8 + T lymphocytes and excessive inflammatory responses.
7.Evaluation of ICUs and weight of quality control indicators: an exploratory study based on Chinese ICU quality data from 2015 to 2020.

This study aimed to explore key quality control factors that affected the prognosis of intensive care unit (ICU) patients in Chinese mainland over six years (2015-2020). The data for this study were from 31 provincial and municipal hospitals (3425 hospital ICUs) and included 2 110 685 ICU patients, for a total of 27 607 376 ICU hospitalization days. We found that 15 initially established quality control indicators were good predictors of patient prognosis, including percentage of ICU patients out of all inpatients (%), percentage of ICU bed occupancy of total inpatient bed occupancy (%), percentage of all ICU inpatients with an APACHE II score ⩾15 (%), three-hour (surviving sepsis campaign) SSC bundle compliance (%), six-hour SSC bundle compliance (%), rate of microbe detection before antibiotics (%), percentage of drug deep venous thrombosis (DVT) prophylaxis (%), percentage of unplanned endotracheal extubations (%), percentage of patients reintubated within 48 hours (%), unplanned transfers to the ICU (%), 48-h ICU readmission rate (%), ventilator associated pneumonia (VAP) (per 1000 ventilator days), catheter related blood stream infection (CRBSI) (per 1000 catheter days), catheter-associated urinary tract infections (CAUTI) (per 1000 catheter days), in-hospital mortality (%). When exploratory factor analysis was applied, the 15 indicators were divided into 6 core elements that varied in weight regarding quality evaluation: nosocomial infection management (21.35%), compliance with the Surviving Sepsis Campaign guidelines (17.97%), ICU resources (17.46%), airway management (15.53%), prevention of deep-vein thrombosis (14.07%), and severity of patient condition (13.61%). Based on the different weights of the core elements associated with the 15 indicators, we developed an integrated quality scoring system defined as F score=21.35%xnosocomial infection management + 17.97%xcompliance with SSC guidelines + 17.46%×ICU resources + 15.53%×airway management + 14.07%×DVT prevention + 13.61%×severity of patient condition. This evidence-based quality scoring system will help in assessing the key elements of quality management and establish a foundation for further optimization of the quality control indicator system.

8.Diagnostic value of diffusion kurtosis imaging quantitative parameters in lymph node metastasis of rectal cancer

Objective:To investigate the diagnostic value of diffusion kurtosis imaging (DKI) quantitative parameters in lymph node metastasis of rectal cancer.Methods:The clinicopathological data of 79 patients with rectal cancers in Shanxi Province Cancer Hospital from November 2016 to March 2017 were retrospectively analyzed. All patients underwent routine magnetic resonance image (MRI) sequence and DKI sequence examinations before the operation. The tumor region of interest (ROI) was delineated by two radiologists. Matlab software was applied to calculate DKI quantitative parameters including apparent diffusion coefficient (ADC), mean diffusivity (MD) and mean kurtosis (MK) in two groups, respectively; and the consistency analysis was performed by using the interclass correlation coefficients (ICC). According to the results of postoperative pathology, all patients were divided into lymph node metastasis group and lymph node non-metastasis group; ADC, MD, MK of both groups were compared. The pathological diagnosis results were taken as the gold standard, receiver operating characteristic (ROC) curves of DKI quantitative parameters in the diagnosis of lymph node metastasis of rectal cancer were plotted, the area under the curve (AUC) was calculated, the optimal diagnostic threshold was determined based on the Yoden index, and the sensitivity and specificity were calculated.Results:The ICC of ADC, MD and MK calculated by two physicians were 0.934 (0.833-0.975), 0.963 (0.905-0.981) and 0.971 (0.949-0.991), respectively, showing a good inter-observer consistency. Among the 79 patients with rectal adenocarcinoma, 36 cases were in lymph node metastasis group and 43 cases were in lymph node non-metastasis group. MK value in lymph node metastasis group was higher than that in lymph node non-metastasis group, and the difference was statistically significant (0.97±0.08 vs. 0.89±0.09; t = -4.07, P < 0.001), while the ADC and MD values in lymph node metastasis group were lower than those in lymph node non-metastasis group, and the differences were not statistically significant (all P > 0.05). The AUC of MK value in the diagnosis of lymph node metastasis of rectal cancer was 0.735, and the corresponding sensitivity and specificity were 55.56% and 88.37%, respectively. Conclusions:DKI quantitative parameter MK has a certain diagnostic value in predicting lymph node metastasis of rectal cancer.
9.Biomechanical Study on Different fixation Methods of Cannulated Screws for Posterior Malleolus Fracture

Objective To study biomechanical effects of cannulated screws at different fixation angles on posterior malleolus fracture based on finite element method, so as to determine the best fixation method of cannulatedscrew. Methods The finite element model of ankle joint, including tibia, fibula, astragalus, corresponding cartilage and ligaments was reconstructed using CT images, and 1 / 2 posterior malleolus fracture model was established on the basis of verifying its validity. The biomechanical effects of cannulated screw fixation on posterior malleolus fracture fixation model were analyzed. Results Compared with 0°,5°,10°,20° fixation model, the 15° fixation model had the smallest displacement. The screw stress of 15° fixation model was lower than that of 5°, 10°, 20° fixation model, and higher than that of 0° fixation model. But when the screw fixation angle was 0°, the peak contact pressure of ankle joint was much larger than that of normal ankle joint, which was easy to cause traumatic osteoarthritis. Conclusions Cannulated screw is safe and effective for treating posterior malleolus fracture which is less than 1 / 2 fragment size. The displacement and stress of the model are different at different fixation angles of screws. When the fixation angle of screw is 15°, the biomechanical stability is the best, which can be used to guide clinical operation.
10.Plant prime editing technique: a new genome editing tool for plants.

The CRISPR/Cas9 based prime editing (PE) technique enables all 12 types of base substitutions and precise small DNA deletions or insertions without generating DNA double-strand breaks. Prime editing has been successfully applied in plants and plays important roles in plant precision breeding. Although plant prime editing (PPE) can substantially expand the scope and capabilities of precise genome editing in plants, its editing efficiency still needs to be further improved. Here, we review the development of PPE technique, and introduce structural composition, advantages and limitations of PPE. Strategies to improve the PPE editing efficiency, including the Tm-directed PBS length design, the RT template length, the dual-pegRNA strategy, the PlantPegDesigner website, and the strategies for optimizing the target proteins of PPE, were highlighted. Finally, the prospects of future development and application of PPE were discussed.
