Objective:To investigate the prevalence of uremic pruritus (UP) in patients with peritoneal dialysis and its association with life behaviors. Methods:A total of 255 patients with peritoneal dialysis were followed up in Peritoneal Dialysis Center of Hangzhou Hospital of Traditional Chinese Medicine from October to December 2023 were enrolled. The general clinical data, disease awareness, treatment, and lifestyle behaviors (smoking, drinking, diet, bathing, and exercising etc) were compared between patients with or without UP, and among patients with different severity of UP.Results:Among 255 peritoneal dialysis patients, there were 144 UP cases with a prevalence rate of 56.5%, including 55 cases with mild itching (38.2%), 61 cases with moderate itching (42.4%), and 28 cases with severe itching (19.4%). The awareness rate of UP was 22.2% (32/144), the treatment rate of UP was 12.5% (18/144). There were significant differences in bath frequency (χ 2=7.03), and using alkaline body wash (χ 2=6.47) between UP group and non-UP group ( P<0.05). The severity of UP was associated with old age (≥60 years, Z=-3.13), low educational levels (primary school or lower, Z=-3.56), less water drinking (<500 ml/d, H=18.01), more spicy food and meat ( H=9.34), bath more than 3 times per week ( H=10.96), and using alkaline body wash ( H=6.57) (all P<0.05). Conclusion:Peritoneal dialysis patients have a high incidence of UP and a low rate of knowledge and treatment, and it is necessary to strengthen the health education and to modify their life behaviors in order to reduce the incidence of UP.

Objective:To develop a candidate reference method for cortisol in human serum by isotope dilution liquid chromatography-tandem mass spectrometry (ID-LC/MS/MS).Methods:An isotope-labeled internal standard was added to samples, followed by alkalizing with sodium carbonate solution and liquid-liquid extraction with n-hexane/ethyl acetate. Isoelution with a methanol aqueous solution was employed for liquid chromatographic separation, while ESI in the positive ion and multiple reaction monitoring mode were used for mass spectrometry. The volume of sample, standard, and internal standard solutions were all controlled by weighing, and the results were calculated by bracketing method. The accuracy, precision, specificity, linearity, LOD and LOQ of this method were evaluated referring to the CLSI C62-A and C50-A guidelines and the domestic expert consensus documents. Results:The method demonstrated excellent accuracy and precision with the bias of ERM-DA192, ERM-DA193 and RELA 2021-2023 all being less than 2%. The recovery of added cortisol ranged from 98.2% to 101.1%. Both intra- and inter-assay imprecisions was <2%. The method was free from interference by structural analogue and showed a good linearity in the range of 25-1 600 nmol/L. The LOD and LOQ were 0.5 nmol/L and 1.0 nmol/L, respectively. A cortisol assay kit (chemiluminescence immunoassay) traced to this candidate reference method was used to determine 46 clinical serum samples concurrently, and the two methods exhibited good correlation. Conclusions:A candidate reference method for the determination of cortisol in serum was established, demonstrating high sensitivity, good repeatability and accuracy. This method can serve as a reference for the measurement traceability and accuracy evaluation of routine methods.

Objective:To investigate the clinicopathological features of fumarate hydratase (FH) deficiency uterine leiomyoma. Methods:The data of 38 patients with FH deficiency uterine leiomyoma were screened and analyzed. The expressions of FH, S-(2-succino)-cysteine (2SC), desmin, p16, p53, CD 10 and cell proliferation associated nuclear antigen (Ki-67) proteins were detected by immunohistochemistry, and their clinicopathological features were analyzed retrospectively. Results:(1) Clinical features: the median age of the patients was (42.5±7.4) years old. Twenty-one cases (55%) of them were myomas found in physical examination, and the median maximum diameter of the tumor was 6.0 cm (range: 5.0-7.5 cm); myomectomy was performed in 23 cases (61%), total hysterectomy with or without bilateral appendages in 15 cases (39%); laparoscopic surgery in 27 cases (71%), open surgery in 11 cases (29%); none of the patients had renal cell carcinoma. (2) Histological features: atypical nuclear cells were distributed locally or diffusely, eosinophilic nucleoli and intranuclear inclusion bodies could be seen, glass like globules could be seen in the cytoplasm, nuclear division was 0-4/10 high power field (HPF), and antler like blood vessels and pulmonary edema-like changes could be seen in the stroma. Among 38 patients with FH deficiency uterine leiomyoma, FH was negative in 37 cases (97%), and positive in 1 case (3%); 2SC, desmin, p16, p53, CD 10 and Ki-67 showed focal positive expression in 38 cases (100%), including 35 cases (92%) with Ki-67 index<10% and 3 cases (8%) with Ki-67 index ≥10%. (3) Follow-up: 4 cases (11%) recurred, and there was no death. There were significant differences in age, family history, distribution of atypical nuclei and mitosis number between recurrent group and non-recurrent group (all P<0.05). Conclusions:FH deficiency uterine leiomyoma is a rare tumor, which needs pathological examination, immunohistochemical examination and clinical history. Patients younger than 43 years old, with family history, histologically atypical diffuse nuclear distribution and mitotic number ≥3/10 HPF should be alert to the risk of recurrence.
