1.Clinical analysis of 61 cases of deep neck infection

Objective:To explore the clinical experience in care for deep neck infection (DNI) and to analyze the factors influencing surgical treatment outcome.Methods:A retrospective analysis was conducted on 61 patients with DNI admitted to the Department of Otorhinolaryngology Head and Neck Surgery of Yantai Yuhuangding Hospital from March 2013 to April 2019, including 33 males and 28 females, aged from 6 months to 81 years with a median age of 49 years. Patients were divided into two groups, 31 cases with localized infection (neck abscess) in Group A and 30 cases with diffuse infection (neck necrotizing fasciitis) in Group B. Patients in two groups were compared for sex, age, hospital stay, diabetes, tracheostomy, drainage methods, mediastinal infection, and pathogenic bacteria. The influence of different drainage methods on hospital stay in Group A was analyzed. SPSS 25.0 software was used for statistical analysis.Results:Of the 61 patients, 45 patients underwent surgical incision and drainage (21 cases in Group A and 24 cases in Group B), 23 patients underwent ultrasound-guided fine needle aspiration (UG-FNA) and catheter drainage (10 cases in Group A and 13 cases in Group B), and 7 patients in Group B were treated with both drainage methods. Pathogens were cultured in 31 cases (50.82%). There were significant differences in hospital stay, drainage method and mediastinal infection (χ 2 values were 26.890, 8.687 and 6.035, respectively, P<0.05), but no significant difference was found in sex, age, diabetes, tracheotomy and pathogenic bacteria (χ 2 values were 0.157, 3.685, 2.434, 3.631 and 0.807, respectively, P>0.05) between the two groups. There was no significant difference in hospital stay between two drainage methods in patients in Group A (χ 2=1.560, P>0.05). Conclusions:There were significant differences in hospital stay, drainage method and mediastinal infection between patients with localized infection and diffuse infection, as diffuse infection is often associated with serious complications. UG-FNA and catheter drainage is an optional method for the treatment of localized infection, with minimal invasion and no influence on hospital stay.
2.Effect of autophagy in curcumin enhancing chemosensitivity of LoVo cells to irinotecan

Objective To study the role of autophagy in the curcumin (Cur) enhancing the chemosensitivity of the colorectal cancer LoVo cells to irinotecan (CPT-11). Methods The LoVo cells were separated into 4 groups:the control group,the Cur group,the CPT-11 group and the Mix group.Western blot assay was applied to determine the expression of autophagy-related protein LC3-Ⅱ and P62. LC3-Ⅱ was detected by immunofluorescence method. Transmission electron microscopy was used to detect the autophagosome. CCK-8 assay was applied to detect the cell growth inhibitory rate of LoVo cells ,which were pre-treated with autophagy inhibitor 3-MA before 24-h treatment of Cur and/or CPT-11. Western blot assay was applied to detect the expression of LC3- Ⅱ and P62. Results The mixed treatment significantly increased the expression of LC3-Ⅱwhile decreased P62 expression,compared with the Cur and CPT-11 treatments. The autophagosomes in cells of Mix group were significantly more than those in cells of the Cur and Cpt group. Pre-treatment of 3-MA significantly reduced cell growth rate in the Cur group,the Cpt group and the Mix group. Pre-treatment of 3-MA significantly decreased the expression of LC3-Ⅱ,while increased P62 expression,compared with the Mix group. Conclution Combinations of curcumin and CPT-11 could enhance the chemosensitivity of LoVo cells to CPT-11,which might be related to the activation of autophagy.
4.Research on Research Trend of Tripterygium Hypoglaucum based on Bibliometric Method

This article was aimed to study the latest research progress of Tripterygium hypoglaucum, in order to pro-vide references for further research on T. hypoglaucum. CNKI Chinese Journal Full-text Database was used as data source. Bibliometric analysis was conducted on T. hypoglaucum research articles issued from 1915~2013. Bibliometric method was used in the analysis on the change trend among years, research institutes, publication dates, research per-sonnels and research topics of the articles with the built-in analysis tools and network analysis tools. The results indi-cated that T. hypoglaucum research was still in the development stage. And there is still plenty of room for growth in aspects including resources, material basis, manufacturing pharmacy and so on.
5.The clinical study about levetiracetam on cognitive function and emotional influence in the patients with partial epilepsy

Objective To investigate the clinical influence about levetiracetam on cognitive function and emotional influence in the patients with partial epilepsy.Methods A total of 62 patients with partial epilepsy were divided into carbamazepine group (30 cases) and levetiracetam group (32 cases) by random digits table method,carbamazepine group was treated by carbamazepine while levetiracetam group was treated by levetiracetam.The patients were assessed before treatment and 4,8,12,16 weeks after treatment by the Montreal cognitive assessment scale (MoCA),self rating anxiety scale (SAS) and self rating depression scale (SDS).Results There was no significant difference in MoCA score between two groups before treatment and 4 weeks after treatment (P > 0.05).MoCA score at 8,12,16 weeks after treatment in levetiracetam group was better than that in carbamazepine group [(22.6 ± 2.1) scores vs.(20.8 ± 2.6) scores,(23.5±2.7) scoresvs.(21.3± 2.8) scores,(24.6±4.7) scoresvs.(21.2±3.0) scores],the difference was statistically significant (P < 0.05).There was no significant difference between SAS score,SDS score between two groups before and after treatment (P > 0.05).Conclusion In the process of treating the patients with partial epilepsy by drug,levetiracetam is superior to carbamazepine on the improvement of cognitive function,but the mood improvement is not obvious.
6.The investigation on the correlation between plasma D-dimer with delayed encephalopathy in patients with acute carbon monoxide poisioning

Objective To investigate the correlation between plasma D-dimer with delayed encephalopathy (DE) in patients with acute carbon monoxide poisioning(ACOP).Methods According to the severity of disease,100 patients with ACOP were divided into mild group (27 cases),moderate group (31 cases),severe group (42cases).And depending on whether DE occured,all patients were divided into DE group(20 cases) and no DE group (80 cases).4 venous blood samples were obtained separately from each subject in 1,7,14 days after their admission to hospital by using latex immunoturbidimetry.The levels of blood plasma D-dimer of patients in each group were detected by immune turbidimetry and analyzed,1th,7th and 14th day after admission.Results The plasma D-dimer level of the mild group at different time points was in the normal range,and the difference was not statistically significant(all P > 0.05) ;The plasma D-dimer level of the severe group and moderate group was significantly higher than that of the mild group(all P <0.01),and it reached a peak in one week(7 days),and showed a downward trend after two weeks(14 days).The DE all occurred in the moderate and severe group.The plasma D-dimer levels of the DE group were higher than those of no DE group from admission to the 7 th and 14 th day (all P < 0.01).Conclusion The dynamic monitoring plasma D-dimer is helpful in assessing to the severity and prognosis of ACOP,and could surpport positive clinical significance for the prevention and treatment of DE.
7.Swelling property of common hydrophilic polymers and their use in push-pull osmotic-pump tablets.

OBJECTIVETo investigate the feasibility of swelling hydrogel instead of polyethylene oxide as swelling polymer in push-layer of push-pull osmotically controlled-release tablets.
METHODThe swelling patterns of tablets made of pure polymers were studied by immerging the tablets into purified water and testing their size at different time points. The push-pull osmotic-pump tablets were prepared and their release patterns in vitro were studied and compared by their similar factor (f2).
RESULTTablets with different swelling materials all showed satisfying release pattern in vitro and their release ratio at 12 h were all above 80%.
CONCLUSIONWith its release rate and cumulative release percentage at 12 h, the mixture of HPMC K15M and CMCNa in ratio of 1:1 is the best choice instead of polyethylene oxide as swelling polymer in push-layer.

8.Blocking the posterier branch of lumbar nerves to treat lumbago and scelalgia caused by the injury of posterier branch of lumbar nerves

Objective To evaluate the effect of blocking the posterior branc h of lumbar nerves in treatment of the lower lumbago and scelalgia caused by the i njury of the posterior branch of lumbar nerves. Methods 560 cases with lower lum bago and scelagia caused by the injury of the posterior branch of lumbar nerves were randomized into the treating group and the contrast group. Follow-ups were done 2 to 3 months after the treatment. Then the effects of the treatment were evaluated. Results The effective rate of the treating group was 98. 0 %,and th e excellect and good rate was 83.0 %; the effective rate of the contrast group was 83.3 %,and the excellect and good rate was 64.9 %. 9.Digital Subtraction Angiography of Meningioma and Preoperative Embolotherapy

Objective To investigate the features of digital subtraction angiography(DSA) of meningioma and the application of therapy in preoperative embolization.Methods DSA in 36 cases with meningioma proved pathologically was performed.Of them,22 cases which the blood supply of tumor was mostly by external carotid artery were undergone external carotid artery embolization and surgical incision 3~10 days after embolization.Results Among 36 cases,the blood supply of tumor was only from external carotid artery in 9 cases,both of external and internal carotid artery supply in 25,and only internal carotid artery in 2.Among 22 cases undergone embolization of supplying artery the tumor stain absolutely disappeared in 16,evidently decreased in 6.22 cases undergone surgical incision after embolization,the tumors were wholly resected,average bleeding 150~500 ml during surgery.Conclusion The blood supply of meningioma can be only by external or internal carotid artery,or both.Preoperative cerebral angiographies and embolization are beneficial for the total resection of meningioma and reduction of bleeding during surery.
10.Interventional Treatment in Cancer of Rectum(Report of 36 Cases)

36 patients with cancer of rectum treated by infusion chemotherapy after selective inferior mesenteric arteriography were reported.All patients had good result.A- mong them 13 cases without embolization,showed vascular blockage,decrease in size of tu- mor on follow up angiographies and reduction of bleeding during operation.The authors be- lieved that arterial infusion chemotherapy not only can kill tumor cells hut also resulted in degenerative change of vascular endothelia,necrosis and vascular occlusion.Simultaneously it can also stimulate the tissue in the periphery of tumor to produce inflammatory cell infil- tration and fihroplasia,suppress the grwoth of tumor,prevent the spread of tumor cells and metastasis;thus prolong the survival time.Interventional treatment is of great value to can- cer of rectum.The indications were recommended.
