The Mofeng Ointment Formulation (MOF, 摩风膏方) is recorded in the Dunhuang manuscript Unnamed Treatise on the Pulse (《亡名氏脉经》) and stands as a representative ancient external therapeutic formula in traditional Chinese medicine (TCM). Known for dispelling wind, activating blood circulation, warming the meridians, and relieving pain, it is particularly effective in treating rheumatic arthralgia. Through literature research, this paper systematically reviews the composition, preparation techniques, and clinical application characteristics of MOF. In recent years, advancements in modern pharmaceutical technology have led to the development of various innovative ointment-based formulations derived from the traditional recipe, such as Dunhuang Xiaoding Ointment (敦煌消定膏), Dunhuang Xiaozhong and Zhentong Ointment (Patch) (敦煌消肿镇痛膏/贴), Xiaoding Ointment (消定膏) gel patch, and Xiaoding Ointment (消定膏) cataplasm. These innovations explore pathways for transforming classical prescriptions into modern applications, providing a theoretical basis for the external TCM treatment of pain-related and orthopedic conditions.
2.Trends in incidence and mortality of esophageal cancer in cancer registration areas of Anhui Province from 2014 to 2020

Objective:
To investigate the trends in incidence and mortality of esophageal cancer in cancer registration areas of Anhui Province from 2014 to 2020, so as to provide the basis for formulating prevention and control measures.
Methods:
The incidence and mortality data of esophageal cancer in Anhui Province from 2014 to 2020 was collected through the Cancer Registry in Anhui Province. The crude incidence and crude mortality were calculated. The Chinese population-standardized rate was standardized using the age structure of the standard population from the Fifth National Population Census in 2000. The trends in incidence and mortality of esophageal cancer were analyzed using the average annual percent change (APPC), stratified by genders, urban/rural areas, and ages.
Results:
In Anhui Province, the rank of esophageal cancer incidence dropped from the third in 2014 to the sixth in 2020. Concurrently, the crude incidence and Chinese population-standardized incidence declined from 28.74/100 000 and 20.74/100 000 to 19.23/100 000 and 10.59/100 000, respectively (AAPC=-5.846%, -9.658%, both P<0.05). The mortality rank remained stable at the fourth in 2014 and 2020, while the crude mortality and Chinese population-standardized mortality decreased from 19.96/100 000 and 14.09/100 000 to 16.00/100 000 and 8.41/100 000, respectively (AAPC=-3.542%, -7.784%, both P<0.05). The Chinese population-standardized incidence (AAPC=-9.682%, -9.188%, -6.175% and -12.575%, all P<0.05) and Chinese population-standardized mortality (AAPC=-7.734%. -7.447%. -5.366% and -10.209%, all P<0.05) showed declining trends in males, females, urban, and rural areas, respectively. From 2014 to 2020 in Anhui Province, the crude incidence and mortality of esophageal cancer generally increased with age. However, significant declining trends were observed in crude incidence (AAPC=-12.779%, -11.701%, -11.955% and -5.751%, all P<0.05) and crude mortality (AAPC=-12.255%, -11.120%, -10.985% and -5.751%, all P<0.05) for the age groups of 40-<50, 50-<60, 60-<70, 70-<80 years. A significant declining trend in crude incidence was also seen in the ≥80 years group (APPC=-6.334%, P<0.05), but the trend in crude mortality was no statistically significant (P>0.05).
Conclusion
In registration areas of Anhui Province, the incidence and mortality of esophageal cancer exhibited a declining trend from 2014 to 2020, calling for focused attention on the middle-aged and elderly population and enhanced health behaviors such as tobacco and alcohol control.
3.Case analysis of occupational disease diagnosis appraisal in Shanghai from 2012 to 2024

Background Occupational disease diagnosis appraisal is an appeal procedure initiated when a party disagrees with the occupational disease diagnosis conclusion. It is a legal procedure to safeguard the health rights of employees and the legitimate rights and interests of employers. Objective To analyze the data of occupational disease diagnosis appraisal in Shanghai, identify existing problems, and provide suggestions for improving. Methods Statistical analysis was conducted on basic situation, characteristics of workers and employers, disease classification, and consistency between diagnosis and appraisal conclusions of all occupational disease diagnosis appraisal cases from 2012 to 2024. All diagnosis conclusions were named in accordance with the Classified Catalogue of Occupational Diseases included employers were classified according to the Measures for the Statistical Classification of Large, Medium, Small and Micro Enterprises" and the Notice on Adjusting the Provisions for Classifying Enterprise Registration Types; industry classification followed the Industrial Classification for National Economic Activities (GB/T 4754-2017). Results From 2012 to 2024, a total of 260 cases of occupational diseases diagnosis appraisal were closed. The main diagnosis were 138 cases of ear, nose, throat, and oral diseases (53.1%), chemical poisoning (18.8%), and pneumoconiosis and other respiratory diseases (16.2%). The proportion of ear, nose, throat, and oral diseases gradually increased (2022 excluded). The incidence of annual municipal and provincial cases decreased by 76.3% and 85.0% respectively (2024 vs 2012). The consistency rate of conclusions between appraisal and diagnosis gradually increased. The total consistency rates between final appraisals and diagnostic conclusions were 81.5%, and 79.7%, 87.8%, 100.0%, 85.0% for ear, nose, throat, and oral diseases, chemical poisoning, and pneumoconiosis and other respiratory system diseases, respectively. Specifically, the consistency rates were 80.1% and 80.0% for noise-induced deafness and chronic benzene poisoning. In terms of inconsistency reasons, "inconsistent interpretation of standards and excessive discretion in standard application" accounted for 43.8%, followed by "failure to correctly apply standards" (31.3%) and and "discrepancies in occupational exposure history recognition" (15.6%). Conclusion The revisions and improvements of the legal system for occupational disease diagnosis appraisal have played a positive role in improving the consistency of conclusions between appraisal and diagnosis of occupational diseases in Shanghai. It is suggested to further strengthening the publicity and training of occupational disease diagnosis standards to improve the quality of occupational disease diagnosis.
4.Effects of Line-Needling Method Combined with Distal Acupoint Selection Acupuncture on Waist and Lower Limb Circulation Temperature in Patients with Taiyang Meridian Type Lumbar Disc Herniation

Objective To observe the effects of line-needling method combined with distal acupoint selection on the temperature of waist and lower limb circulation in patients with Taiyang meridian type lumbar disc herniation(LDH)using infrared thermal imaging.Methods A total of 54 subjects were included,including 34 patients with Taiyang meridian type LDH and 20 healthy subjects.The patients were treated with lumbar line-needling method and distal acupuncture,leaving the needle for 30 min,once every other day,3 times a week for 4 weeks.Healthy subjects were not treated.The infrared thermal imaging of the waist and lower limbs of healthy subjects and patients before and after treatment was collected,and 4 areas(affected lumbar muscle area,thigh posterior side,popliteal cavity and calf posterior side)through which the meridians ran were selected to measure and record the body surface temperature.The difference of circulating temperature between healthy subjects and patients before and after treatment was compared and analyzed.The simplified McGill Pain Scale(SF-MPQ)score and Oswestry Disability Index(ODI)score were compared before and after treatment,and the clinical efficacy was evaluated.Results LDH patients with Taiyang meridian type had abnormal temperature along the meridian in the waist and lower limbs.Compared with healthy subjects,abnormal low-temperature areas could be seen in the meridian areas of the waist and lower limbs(P<0.05).After treatment,the temperature in the meridian area increased,and the SF-MPQ score and ODI score significantly decreased(P<0.05),with a total effective rate of 97.06%(33/34).Conclusion The combination of line-needling method and distal acupoint selection can significantly improve the clinical symptoms of LDH patients with Taiyang meridian type,and has a significant adjustment effect on the changes of lumbar and lower limb meridian temperature.
5.Analysis of the prognostic value of NLR in the treatment of PD-1 inhibitors in patients with HER2-negative advanced gastric cancer

Objective To explore the prognostic value of serum neutrophils/lymphocytes(NLR)for first-line treatment of patients with advanced gastric cancer using programmed cell death receptor 1(PD-1)inhibitors.Methods A total of 168 patients with advanced gastric cancer who were treated with immunotherapy combined with chemotherapy in the Fourth Hospital of Qinhuangdao from January 2018 to January 2021 were selected as study subjects,and the follow-up period was terminated at January 2023.The patients'data were collected,hematological and tumor markers before the combined treatment were analyzed,and the optimal cut-off value of NLR was calculated using X-tile software.The effect of NLR expression on the survival rate of patients with advanced gastric cancer was analysed by the Kaplan-Meier survival curve.Receiver operating curve(ROC)was used to analyze the predictive value of NLR in patients with advanced gastric cancer.The related factors affecting the disease progression of patients with advanced gastric cancer were screened combined with Cox proportional risk model.Results Among 168 patients,the optimal cut-off value of serum NLR before treatment was 2.41.Patients were divided into high NLR group(NLR>2.41,n=93)and low NLR group(NLR<2.41,n=75).NLR was related to tumor differentiation,distant metastasis,composite positive scores of PD-L1,carcinoembryonic antigen and cancer antigen 125(P<0.05);the effective rate in the low NLR group was significantly higher than that in the high NLR group(P<0.05);the median progression free survival(PFS)and the overall survival(OS)of patients in the low NLR group were both longer than those in the high NLR group(PFS:P=0.006;OS:P=0.023);ROC analysis showed that the area under the curve of NLR for the prognosis of advanced gastric cancer patients was 0.740,sensitivity was 81.50%,and specificity was 69.70%;in multivariate analysis,except initial NLR value,tumor differentiation degree and distant metastasis were also independent predictors of poor prognosis in patients with advanced gastric cancer(P<0.05).Conclusion Among patients with advanced gastric cancer who received first-line immunotherapy combined with chemotherapy,pretreatment NLR is correlated with efficacy and PFS/OS,and has high value in predicting the prognosis of immunotherapy for advanced gastric cancer.
6.Comparative analysis of inpatient medical services between secondary public and private general hospitals in Chengdu

Objective:To evaluate and compare the inpatient medical services of secondary public and private general hospitals by using disease risk adjustment model, and to explore the application of disease risk adjustment model in medical service evaluation of different ownership hospitals.Methods:Based on 1 032 865 front pages of medical records in Chengdu in 2017 and 2018, a regression model with mortality, average length of stay, total hospitalization expenses, medical service fees, drug costs and surgical consumables costs as dependent variables and related influencing factors as independent variables was established by using disease management intelligent analytic and evaluation system. The risk adjusted case mix index(ACMI) was calculated. The mortality, average length of stay, hospitalization expenses and other indicators were predicted. The ratio of observed value to expected value(O/E value) of each index in public and private secondary general hospitals was obtained and compared.Results:The ACMI value of secondary public general hospital was 4.63, slightly higher than that of private hospitals(4.55). The technical difficulty and resource consumption of the public hospitals were slightly higher than that of the private hospitals.From the O/E value, the management of disease mortality, medical service fees and inpatient drug costs of secondary public hospitals was generally good, and the O/E values of hospitalization expenses of each secondary private general hospital were quite different, and there was a possibility that the costs were unreasonable. The O/E value of surgical consumables cost in secondary public general hospital was 1.54, and there was room for improvement in cost management.Conclusions:The disease risk adjustment model fully considers the characteristics of different types and severity of diseases in different institutions, which can not be simply compared. Based on individual cases, it realizes the comparability of different ownership hospitals, and provides a new means for the evaluation of medical service ability and quality.
7.Practice of Standardizing Rational Use of Proton Pump Inhibitors in Neurosurgery Department of Our Hospital by PDCA Cycle Management

OBJECTIVE: To promote rational use of proton pump inhibitors (PPIs) during perioperative period. METHODS: PDCA (Plan, Do, Check, Action) cycle management was used, the irrational use of PPIs of 300 medical records in neurosurgery department of our hospital were collected. The reasons were analyzed, management target was formulated and measures were implemented. The effects of management were evaluated through comparing the rate of irrational drug use and ratio of irrational type of PPIs in 300 medical records of neurosurgery department during perioperative period after management. RESULTS: Through collecting related data to confirm risk factors of stress ulcer, establishing rationality evaluation criteria for perioperative prophylactic use of PPIs, conducting rational drug use training among medical staff, drawing up various management systems and strengthening supervision and management, the rate of irrational use of PPIs was decreased significantly in our hospital; the number of irrational drug use cases decreased from 240 before management to 156 after management, among which the rate of prophylactic drug use without indication decreased from 37.33% to 29.00% (P<0.05); the irrational dosage rate decreased from 11.33% to 6.33% (P<0.05); the rate of irrational dosing frequency dropped from 12.67% to 5.00% (P<0.01). CONCLUSIONS: PDCA cycle management of our hospital can standardize the prophylactic use of PPIs in neurosurgery department during perioperative period and promote rational use of PPIs.
8. Clinical characteristics of hepatic flare and efficacy of antiviral therapy in pregnant women with chronic hepatitis B virus infection

Objective:
To analyze the clinical characteristics of hepatic flare and evaluate efficacy of antiviral treatment in pregnant women with chronic HBV infection.
Methods:
A single-center, open-label, prospective study was conducted, and pregnant women with chronic HBV infection were enrolled. Liver function, HBV serum markers and HBV DNA of pregnant women with chronic HBV infection were reviewed during every 4 to 12 weeks of gestation period. The proportion and clinical characteristics of hepatitis flare during pregnancy were observed. Logistic regression analysis was used to predict hepatic flare in pregnant women with chronic HBV infection. Antiviral therapy with telbivudine (LdT) or tenofovir dipivoxil (TDF) was used to treat hepatic flare during pregnancy. Sequential entecavir (ETV) or TDF was applied after the delivery. Treatment course and drug withdrawal in pregnant women with hepatic flare was the same as those of the general patients with chronic hepatitis B. Liver function, HBV serum markers and HBV DNA were measured in pregnant women with hepatic flare at different time points (4, 12, 24 and 52 weeks). A t-test was used to compare the hepatic flare in pregnant women with and without hepatitis group. HBsAg and HBeAg were used to quantify the receiver operating characteristic (ROC) curve of pregnant women with hepatic flare during pregnancy. Area under the ROC curve was used to calculate the optimal cut-off value corresponding to the maximum sensitivity and specificity of the ROC curve.
Results:
Of the 220 pregnant women with chronic HBV infection, 55 (25%) had hepatitis flare during pregnancy and received antiviral treatment. Among the 55 women with hepatic flare during gestation, 47 (85.46%) had hepatic flare in the mid-second trimester (12-24 weeks); average peak value of alanine aminotransferase (ALT) was 220.62 U/L, and the average peak value of ALT in 32 cases (58.18%) of pregnant women with hepatic flare was between 2–5 × ULN. 9.Effects of audio-visual comprehensive training on cognitive impairment among breast cancer patients with chemotherapy

Objective? To explore the effects of audio-visual comprehensive training on cognitive impairment among breast cancer patients with chemotherapy. Methods? From July 2017 to October 2018, we selected 80 inpatients with chemotherapy in breast surgery as subjects by convenience sampling. All of the patients were divided into control group (n=40) and experimental group (n=40) with the lot-drawing method. Patients of two groups were all treated with the routine treatment and nursing of breast surgery, while experimental group also received audio-visual comprehensive training. The intervention effects were assessed with the Rivermead Behavioral Memory Test Second Edition (RBMT-Ⅱ). Results? After intervention, the scores of names, belongings, data, faces, delayed story, orientation, delayed route, messages and the total score of experimental group were higher than those before intervention; the score of immediate route was lower than that before intervention;the differences were all statistically significant (P< 0.05). After intervention, the total score and sub-test scores of RBMT-Ⅱof control group were lower than those before intervention with statistical differences (P< 0.05). After intervention, the scores of RBMT-Ⅱ of experimental group were higher than those of control group except for orientation and data with statistical differences (P< 0.05). Conclusions? Audio-visual comprehensive training can effectively improve cognitive impairment among breast cancer patients with chemotherapy which is simple and feasible, and can be promoted and applied.
10.Correlation of Family History of Gastric Cancer With Helicobacter pylori Infection and Pathological Changes of Gastric Mucosa in Patients With Chronic Gastritis

Background:Family history of gastric cancer is an important indicator of genetic susceptibility.Studies have shown that the family history of gastric cancer might have impact on Helicobacter pylori (Hp) infection and clinicopathological manifestations in patients with chronic gastritis.Aims:To investigate the correlation of family history of gastric cancer with Hp infection and pathological changes of gastric mucosa in patients with chronic gastritis.Methods:A total of 312 patients with chronic gastritis were enrolled to study the relationship between family history of gastric cancer and Hp infection,degree of gastric mucosal inflammation,activity of inflammation,and degree of atrophy and intestinal metaplasia.Results:In the 312 patients with chronic gastritis,165 (52.9%) had family history of gastric cancer,and 147 (47.1%) without family history of gastric cancer.Incidence of Hp infection,degree of gastric mucosal atrophy and intestinal metaplasia in patients with family history of gastric cancer were significantly higher than those without (P < 0.05),but no significant differences in degree and activity of inflammation were found between the two groups (P > 0.05).Conclusions:The incidence of Hp infection is higher in chronic gastritis patients with family history of gastric cancer.Family history of gastric cancer may aggravate gastric mucosal atrophy and intestinal metaplasia.
