1.Effectiveness and safety of sequential therapy with parathyroid hormone analogues and bisphosphonates for osteoporosis:a meta-analysis

OBJECTIVE To evaluate the effectiveness and safety of sequential therapy with parathyroid hormone analogues and bisphosphonates for osteoporosis. METHODS PubMed, Embase, the Cochrane Library, Web of Science, China National Knowledge Infrastructure, VIP, Wanfang data, and SinoMed were searched in both English and Chinese databases from their inception to March 25, 2024. Two researchers independently conducted literature searches, screening, and data extraction. Review Manager 5.4 software was used for the meta-analysis. Subgroup analyses and sensitivity analyses were performed based on different medication sequences in the treatment group to account for potential sources of heterogeneity. RESULTS A total of 7 randomized controlled trials involving 2 461 participants were included, with 1 215 in the treatment group and 1 246 in the control group. The meta-analysis results showed that the treatment group using sequential therapy with parathyroid hormone analogues and bisphosphonates had superior effects on improving bone mineral density at the lumbar spine [SMD=0.90, 95%CI (0.44, 1.35), P<0.001], total hip [SMD=0.68, 95%CI (0.14, 1.21), P=0.01], and femoral neck [SMD=0.45, 95%CI (0.04, 0.86), P=0.03] compared to the control group. It also significantly outperformed the control group in reducing the incidence of fractures post- treatment [OR=0.72, 95%CI (0.54, 0.97), P=0.03].significant difference was noted in the incidence of adverse reactions between the two groups [OR=1.21, 95%CI (0.99, 1.46), P=0.06]. Subgroup analysis based on intervention measures in the treatment group showed that switching from bisphosphonates to parathyroid hormone analogues [SMD=0.56, 95%CI (0.09, 1.03), P=0.02] or switching from parathyroid hormone analogues to bisphosphonates [SMD=0.97, 95%CI (0.49, 1.46), P<0.001] both significantly potentiated lumbar spine bone mineral density compared to the control group. Switching from bisphosphonates to parathyroid hormone analogues also significantly promoted total hip bone mineral density compared to the control group [SMD=0.66, 95%CI (0.18, 1.13), P=0.007]. Sensitivity analysis indicated that the results of this study were robust. CONCLUSIONS Sequential therapy with parathyroid hormone analogues and bisphosphonates can be recommended as an effective treatment for patients with osteoporosis, with good safety profiles. The medication sequences should be individually adjusted based on the patient’s particular situation and the different responses of various skeletal sites.
2.Correlation between geriatric nutritional risk index and adverse events in elderly hemodialysis patients

Objective To explore the relationship between geriatric nutritional risk index(GNRI)and adverse outcomes in elderly patients undergoing maintenance hemodialysis(MHD).Methods A prospective cohort trial was conducted on 337 MHD patients aged ≥60 years in hemodialysis centers of 11 hospitals in Beijing from April to June 2017.Their baseline data were collected,and they were divided into non-malnutrition(GNRI≥98,226 cases),mild malnutrition(92≤GNRI<98,81 cases),and major malnutrition groups(GNRI<92,30 cases).All of them were followed up until June 2018.The endpoint events were all-cause mortality and cardiovascular disease(CVD)mortality.Kaplan-Meier survival analysis was used to compare the cumulative survival rate among the 3 groups.Multivariate Cox regression model was employed to analyze the relationship of GNRI with all-cause and CVD mortality.Results The mild and major malnutrition groups had significantly lower BMI,serum albumin level and GNRI(P<0.01).During the median follow-up of 52(4.4-52.0)weeks,56(16.6%)patients died of all-cause death and 25(44.6%)of CVD death.Kaplan-Meier survival curve showed significant differences in all-cause mortality(x2=30.484,P<0.01)and CVD mortality(x2=22.398,P<0.01)in the 3 groups.Multivariate Cox regression analysis indicated that,as a continuous variable,elevated GNRI was a protective factor for all-cause mortality(HR=0.910,95%CI:0.870-0.952,P=0.000)and CVD mortality(HR=0.895,95%CI:0.852-0.940,P=0.000),and as a categorical variable,mild and major malnutri-tion were independently correlated with all-cause and CVD mortality(P<0.05).Conclusion GNRI is an independent risk factor for all-cause and CVD mortality in elderly MHD patients.Mo-nitoring the nutritional status using GNRI can predict the risk of adverse prognosis.
3.Meta-analysis of the correlation between ocular and neurological manifestations in 43 Chinese children with sialidosis type I

Objective:To investigate the correlation between macular cherry red spot (CS) and severity of neurological manifestations in Chinese children with sialidosis (SD) type I.Methods:A evidence-based medical study. "China", "Sialidosis" and "Sialidoses" were used as Chinese and English search terms. The literature was searched in CNKI, Wanfang and PubMed. The cases were all from China and matched the diagnostic criteria. According to the presence or absence of CS in the fundus, the SD children were divided into a group with CS (+) and a group without CS (-), and the correlation between the occurrence of ocular CS and neurological manifestations was compared with meta-analysis by RevMan 5.3 software.Results:Sixty-eight studies were initially retrieved according to the search strategy, and 17 studies were finally included, and 5 studies with CS+ and CS- were meta-analyzed. Among the 43 patients, 28 were male and 15 were female, with a median age of 12 years. Visual impairment was observed in 37 cases (90.2%, 37/41, 2 cases not recorded), and CS was present in 24 cases (55.8%, 24/43). The most common neurological manifestation was myoclonus (97.7%, 42/43), followed by cerebellar ataxia (95.1%, 39/41, 2 cases not recorded) and seizures (91.4%, 32/35, 8 cases not recorded). Pathogenic NEU1 gene mutations were detected in 42 cases and one case was undocumented. The incidence of seizure in group CS+ (100%, 20/20) was higher than that in group CS- (80%, 12/15). Meta-analysis showed that there was no statistically significant difference between the incidence of myoclonus or ataxia [relative risk ( RR)=1.13, 95% confidence interval ( CI) 0.79-1.63, P=0.49] and seizure ( RR=1.13, 95% CI 0.84-2.06, P=0.24) among the children in the CS+ and CS- groups. Conclusions:The incidence of ocular CS in Chinese children with type I SD was 55.8%. There was no correlation with neurological manifestations, however the incidence of seizure was significantly higher in patients with CS than in others without CS.
4.Effect of Macrophage Migration Inhibitory Factor on Liver Inflammation in Mice with Acute Liver Failure

Objective To investigate the role of macrophage migration inhibitory factor(MIF)on liver inflammation in mice with a-cute liver failure(ALF).Methods Eighteen male mice were randomly divided into normal group,model group and MIF antagonist ISO-1 group.D-galactose combined with lipopolysaccharide were used to induce ALF model.The serum levels of alanine aminotrans-ferase(ALT)、aspartate aminotransferase(AST)and total bilirubin(TBIL)were detected.The pathological changes of liver were ob-served by HE staining and the apoptosis of liver cells was observed by Tunel staining.The levels of MIF,CXC chemokine receptor 2(CX-CR2)and tumor necrosis factor-α(TNF-α)in mouse liver tissues were detected by enzyme-linked immunosorbent assay(ELISA)and the expression of MIF was observed by immunofluorescence.Results Compared with the normal group,the model group showed seri-ous liver tissue structure damage,increased hepatocyte apoptosis and decreased liver function.ISO-1 reduced liver tissue damage and hepatocyte apoptosis.The expression levels of MIF,CXCR2 and TNF-α in liver tissue of mice in model group was significantly in-creased,while ISO-1 could significantly reduce the expression levels of MIF,CXCR2 and TNF-α,with statistical significance(P<0.05).Conclusion The expression of MIF is significantly increased in ALF,and inhibition of MIF can improve liver tissue injury and the expression of inflammatory factors,indicating that MIF plays an important role in ALF.
5.Auxiliary diagnosis and prognosis evaluation of KIF4A, RAD51AP1 and CDKN3 in esophageal cancer

To investigate the expression of mRNA in esophageal cancer (ESCA) tissues and its potential and diagnostic and prognostic value by high-throughput sequencing data. Using the Cancer Genome Atlas Program (TCGA) database in USA by integrative bioinformatics analysis methods, the gene expression profiles and clinical data of 173 patients with ECSA were collected. The mRNA expression levels in ESCA tissue and para-cancerous tissue samples were analyzed using DESeq2, edgeR and limma to screen the differentially expressed genes (DEGs). DEGs-related protein network diagrams were drawn. GO and KEGG function enrichment analysis were performed and the hub genes were screened and the survival analysis of hub genes was analyzed. Genes related to the prognosis of ESCA were selected and their prognostic value in ESCA was analyzed. Finally, the receiver operating characteristic curve was drawn to evaluate its diagnostic value. The results showed that using TCGA cancer data, a total of 620 up-regulated DEGs and 668 down-regulated DEGs with significant differential expression between ESCA and para-cancerous tissues were screened. DEGs were mainly involved in receptor complexes, ubiquitin ligase complexes, etc., playing GTPase activity, phospholipid binding, and other molecular functions, and participating in the regulation of intracellular substance transport, small molecule metabolism, and other biological processes. Protein functional enrichment analysis showed that these proteins were mainly enriched in the IL-17 signaling pathway, TNF signaling pathway, Toll-like receptor signaling pathway, Epstein-Barr virus infection, neutrophil extracellular trap formation, and other pathways involved in the formation and development process of ESCA. Survival analysis showed that the overall survival rate of ESCA patients with high expression of KIF4A, RAD51AP1, and CDKN3 was significantly shortened, and the difference was statistically significant ( P<0.05). Furthermore, the areas under the curve (AUC) of KIF4A, RAD51AP1, and CDKN3 for diagnosing esophageal cancer were 0.956, 0.951 and 0.979, respectively, with sensitivities and specificities both exceeding 80%. Additionally, ROC results of the combined diagnostic model of these three genes showed an AUC of 0.979, with sensitivities and specificities of 0.914 and 1, respectively. This indicates that KIF4A, RAD51AP1 and CDKN3 have individual or combined auxiliary diagnostic value for ESCA. In conclusion, KIF4A, RAD51AP1 and CDKN3 have high diagnostic efficiency for ESCA, and their increased expression is closely related to the prognosis, suggesting that these three genes could be used as auxiliary diagnostic and prognostic factors for ESCA.
6.Clinical research of the correlation between hyperkalemia and adverse outcome events in elderly patients with maintenance hemodialysis

Objective To explore the relationship between hyperkalemia and adverse outcomes in elderly maintenance hemodialysis(MHD)patients.Methods A retrospective cohort trial was conducted on 177 MHD patients aged≥60 years in the hemodialysis center of Beijing Shijitan Hospital,Capital Medical University.Baseline data were collected.The mean age was(77.28±14.25)years,among them 109 cases were males(61.6%).According to the peak serum potassium within 4 months,patients were divided into the K+<5.0 mmol/L group(38 cases,21.5%),the 5.0-5.5 mmol/L group(42 cases,23.7%)and the K+≥5.5 mmol/L group(97 cases,54.8%).Based on the frequency of hyperkalemia within 4 months,patients were divided into the 0,1-2 and 3-4 frequency groups.All of patients were followed up for 1 year.The end point events were all-cause mortality and cardiovascular disease(CVD)mortality.Kaplan-Meier survival analysis was used to compare the cumulative survival rate between the 3 groups.A multivariate Cox regression model was employed to analyze the relationship between the degree and frequency of hyperkalemia with all-cause and CVD mortality.Results The mean follow-up time of 177 patients was 12(1-12)months,20(11.3%)patients died of all-cause death and 14(7.9%)of CVD death.Kaplan Meier survival curve showed cardiovascular mortality rate and all-cause mortality were significantly increased in the K+5.0-5.5 mmol/L group and the K+≥5.5 mmol/L group(both P<0.05).Multivariate Cox regression analysis indicated that,as a continuous variable,peak potassium levels were independently associated with an increased risk of all-cause death and CVD death(P<0.001).As a categorical variable,the all-cause mortality risk and CVD mortality risk were significantly higher in both the K+5.0-5.5 mmol/L group and the K+≥5.5 mmol/L group than those in the K+<5.0 mmol/L group(P<0.05),and those were significantly higher in the 1-2 and 3-4 frequency groups than those in the 0 frequency group of hyperkalemia(P<0.05).Conclusion Serum potassium>5.0 mmol/L in elderly MHD patients is an independent risk factor for all-cause and CVD mortality.It is recommended to adjust the upper limit of the normal range of serum potassium to 5.0 mmol/L.

8.Influence of remaining coronal tooth structure and fiber post location on the fracture resistance of restored endodontically treated maxillary premolars

120 extracted maxillary first premolars were endodontically treated and randomly divided into 5 groups(n=24).The teeth in group A had 4 walls of coronal tooth structure,in group B,C,D and E had only 3 walls,missing the palatal,buccal,mesial and distal wall,respectively.The teeth in group A0-E0(n=6)were restored without post,in group A1-E1(n=6)with buccal post,in group A2-E2(n=6)with palatal post,in group A3-E3(n=6)with buccal and palatal post,respectively.The fracture resistance of group A was higher than that of B,C,D and E groups(P<0.05).The fracture resistance of fiber posts placed in the buccal root canal was higher than that in the palatal root canal(P<0.05).The 360° complete ferrule can provide the best fracture resistance,When the ferrule is not complete,it is recommended to place buccal fiber post for repair.

The medical equipment industry is an important foundation of the medical and health services,which is related to the life and health of people,as well as the overall situation of high-quality development.China has continuously issued important industrial policy documents,such as the"14th Five Year"Plan for Development of the Medical Equipment Industry and the"Action Plan for High-Quality Development of the Medical Equipment Industry(2023-2025)".The positive results have been achieved in the innovative development of medical equipment industry,which included emerging high-end medical equipment,key components of accelerating breakthrough,accelerated pace in application and demonstration,appeared vitality in the collaborative innovation of medical-industry,and continuous improvement of support capabilities for equipment.This article sorted out and summarized the achievements of innovation and development of China's medical equipment in the new era,and analyzed the existing problems,and looked forward to future development,and proposed suggestion on the basis of visual angle of formulating and executing the"14th Five Year"Plan for Development of the Medical Equipment Industry.
10.Releasing Fresh Method of Gypsum Fibrosum by Compatibility: A New Interpretation Based on Analysis of Traditional Chinese Medicine Classical Prescriptions

The idea that Gypsum Fibrosum can release flesh has a long history, but its clinical popularization, application, and in-depth discussion are insufficient due to the problems of mineral drug dissolution and limited research means. Through the analysis of releasing flesh method (RFM), this paper concluded that the RFM-targeted pathogenesis was deficiency of body fluid and disturbance in the exterior, and the core connotation of RFM of Gypsum Fibrosum was clearing heat to restore fluid and expelling pathogenic factors in the exterior. Based on the ancient and modern application of RFM and modern research on coordination chemistry, this paper enriched the connotation of RFM of Gypsum Fibrosum from compatibility with traditional Chinese medicine (TCM) classical prescriptions recorded in the Treatise on Cold Damage (《伤寒论》) and Synopsis of the Golden Chamber (《金匮要略》) as examples. Specifically, in addition to muscle discomfort symptoms after external contraction, RFM of Gypsum Fibrosum could also affect the function of internal organs, thereby treating internal injuries and miscellaneous diseases. Moreover, a new idea, RFM of Gypsum Fibrosum by compatibility, was proposed, that the fresh-releasing effect of Gypsum Fibrosum could be markedly exerted through compatibility with Ephedrae Herba, Puerariae Lobatae Radix, Stephaniae Tetrandrae Radix, Anemarrhenae Rhizoma, Ginseng Radix et Rhizoma, Paeoniae Radix Alba, Bambusae Caulis in Taenias, Rehmanniae Radix, Ophiopogonis Radix, and Polygonati Odorati Rhizoma by adjusting the expelling and heat-clearing intensity. A clinical case was presented to provide a reference for the clinical application of Gypsum Fibrosum and a direction for the study of drug mechanism of compatibility.
