1.Clinical Observation of Treating Ulcerative Colitis with Acupuncture at Front Mu Points Combined with Oral Mesalazine

Objective To observe the clinical efficacy of acupuncture at Front Mu point in the treatment of ulcerative colitis.Methods Sixty patients with ulcerative colitis treated at the Traditional Chinese Medicine Hospital in Kunming from August 2022 to June 2023 were collected.Using a random number table method,30 cases were assigned to each of the control group and the combined group.The treatment method involved administering oral mesalazine to the control group for a continuous period of 8 weeks,while the combined group received both oral mesalazine and acupuncture at front Mu points.The clinical efficacy,colonoscopy results score(Baron score),and colonic mucosal healing score(Geboes)before and after treatment were compared.Follow-up was conducted at 3 months to calculate the recurrence rate in the combination and control groups.Results The total effective rate in the combination group was higher than that in the control group(P<0.05),with rates of 93.33%and 67.67%,respectively.After treatment,the disease activity index,Baron score,and Geboes score decreased compared to before treatment(P<0.05),and the combination group had a lower disease activity index,Baron score,and Geboes score than the control group after treatment(P<0.05).Comparing the recurrence rates at 3 months post-treatment,the combination group was lower than the control group.Conclusion Acupuncture at Front Mu Point can significantly improve the clinical symptoms of ulcerative colitis,reduce the recurrence rate compared to patients in the control group,and is safe and reliable without serious adverse reactions.
2.Value of Three-dimensional Rectal Intraluminal Ultrasound Combined with Couplant Contrast for Surgical Guidance of Perianal Necrotising Fasciitis

Objective To explore the application value of Three-Dimensional rectal cavity ultrasound combined with contrast agent imaging in necrotizing fasciitis of the anal region.Methods Before surgery,standard three-dimensional rectal cavity ultrasound examinations(referred to as the conventional group)and contrast agent imaging examinations(referred to as the imaging group)were conducted for 40 patients clinically diagnosed with anal region necrotizing fasciitis.Separate observations were made for the primary lesion,as well as for the depth and superficial necrosis of the fascia,and injuries to the anal sphincter muscle.Comparative analysis with surgical results was undertaken to assess the diagnostic sensitivity of both the conventional and imaging groups.Results In comparing the conventional group with the imaging group,the rates of primary lesion visibility rose significantly from 70%to 97.5%,deep fascial necrosis visibility increased from 50%to 88.8%,superficial fascia visibility improved from 70%to 100%,and the visibility of anal sphincter muscle injury escalated from 62.5%to 97.2%,all demonstrating statistical significance at P<0.05.Conclusions Three-dimensional rectal cavity ultrasound combined with contrast agent imaging exhibits significantly enhanced accuracy in identifying primary lesions associated with perianal necrotizing fasciitis,as well as the necrosis affecting deep and superficial fascia,in contrast to conventional three-dimensional rectal cavity ultrasound.This advancement offers more precise guidance for clinicians in devising surgical plans,thereby augmenting the success rate of surgical interventions.
3.The relationship between the expression of serum GP3 and CHI3L1 and the degree of liver fibrosis and pathological changes in patients with hepatitis

Objective Exploring the relationship between changes in serum Golgi apparatus transmem-brane glycoprotein 73(GP73)and chitosanase 3-like protein 1(CHI3L1)levels and liver fibrosis and lesion sever-ity in patients with hepatitis B(CHB).Methods Using a case-control study,80 patients diagnosed with CHB infection and developing liver fibrosis in the Infectious Disease Department of Changsha First Hospital from June 2020 to June 2023 were selected as the liver fibrosis group,while 120 patients diagnosed with CHB infection but not developing liver fibrosis in the Infectious Disease Department of our hospital were selected as the control group.The serum GP3,CHI3L1,liver function,and fibrosis indicators of the two groups of patients were compared,and the liver fibrosis group was divided into mild according to Scheuer system standards Perform stratified analysis on patients with significant liver fibrosis.Results The serum GP3 and CHI3L1 levels in the liver fibrosis group were significantly higher than those in the control group,with statistical significance(P<0.05);The ROC curves were plotted using serum GP3,CHI3L1,and GP3+CHI3L1,respectively.The sensitivity for diagnosing liver fibrosis in CHB patients was 62.81%,60.94%,and 96.33%,with specificity of 80.66%,80.05%,and 75.30%.The AUC values under the curves were 0.792,0.756,and 0.908,respectively;The levels of ALT,AST,HA,LN,PC III NP,C IV,and CG in the liver fibrosis group were higher than those in the control group,and the PLT measurement values were lower than those in the control group,with statistical significance(P<0.05);49 patients with moderate to severe liver fibrosis(33 in S2 phase and 16 in S3 phase)and 31 patients with mild liver fibrosis(all in S1 phase)had significantly higher serum GP3 and CHI3L1 levels than mild patients,with statistical significance(P<0.05);The levels of DBIL,ALT,AST,HA,LN,PC III NP,C IV,and CG in patients with moderate to severe liver fibrosis were higher than those in the mild group,and the PLT measurement values were lower than those in the mild group,with statistical significance(P<0.05).Conclusion The serum GP3 and CHI3L1 levels in CHB patients with liver fibrosis are significantly elevated,and there is a certain correlation with the degree of liver fibro-sis.The combination of these two indicators is beneficial for diagnosing liver fibrosis in CHB patients.
4.Analysis of Material Basis of Famous Classical Formula Baoyuantang Based on UPLC-Q-TOF-MS/MS

ObjectiveA rapid method for identification of chemical constituents in Baoyuantang reference sample was established in order to clarify the material basis of this formula. MethodBased on ultra-performance liquid chromatography-quadrupole-time-of-flight mass spectrometry(UPLC-Q-TOF-MS/MS) and self-established database information, the chemical components in Baoyuantang were systematically characterized and identified. The chromatography was performed on a Waters ACQUITY UPLC HSS T3 column(2.1 mm×100 mm, 1.8 μm) with mobile phase of 0.1% formic acid aqueous solution(A)-0.1% formic acid acetonitrile solution(B) for gradient elution(0-3 min, 2%-19%B; 3-8 min, 19%B; 8-8.1 min, 19%-22%B; 8.1-14 min, 22%-29%B; 14-16 min, 29%B; 16-32 min, 29%-45%B; 32-32.1 min, 45%-90%B; 32.1-35 min, 90%-95%B; 35-36 min, 95%-98%B; 36-37 min, 98%-2%B; 37-40 min, 2%B). Based on electrospray ionization(ESI), continuum data format was collected in both positive and negative ion modes with a scanning range of m/z 50-1 500. Chemical constituents in the decoction of Baoyuantang were systematically analyzed by UNIFI 1.9.4 software matching, control comparison, The Encyclopedia of Traditional Chinese Medicine(ETCM) database search and literature reports. ResultA total of 229 components were identified under negative ion mode and 181 under positive ion mode, with a total of 322 components after removing duplicates, including 116 triterpene saponins, 66 flavonoids, 19 organic acids, 6 gingerphenols, 6 gingerols, 5 gingerones, 10 amino acids, 7 saccharides, 5 coumarins and 82 other components. Among them, 83, 141, 39, 35 and 38 components were attributed to Ginseng Radix et Rhizoma, Glycyrrhizae Radix et Rhizoma, Astragali Radix, Cinnamomi Cortex and Zingiberis Rhizoma Recens, respectively. ConclusionIn this study, the rapid characterization and identification of multi-class components in Baoyuantang was realized, and it was confirmed that the material basis of this formula was mainly triterpenoid saponins, flavonoids, gingerols and organic acids, and the chemical composition was attributed and analyzed, which provided a reference for the subsequent quality control research.
5.Research progress in biological characteristics of dental pulp stem cells and their applications in treatment of corneal and retinal diseases

Dental pulp stem cells(DPSCs)are a type of mesenchymal stem cells with broad application potential.Due to their multipotent differentiation capabilities and ease of access,DPSCs have become a focus of research in the field of ophthalmology.In recent years,DPSCs have shown the potential clinical applications in the treatment of corneal epithelial injuries and retinal degenerative diseases.DPSCs can promote the corneal epithelial regeneration and reconstruction by differentiating into the corneal epithelial cells and inhibiting the M1 macrophages.Additionally,DPSCs can differentiate into the retinal photoreceptor-like cells and retinal ganglion-like cells,replace original retinal neurons,secrete neurotrophic factors to mediate injury repair,promote retinal regeneration,and improve the original function of the retina.This article systematically reviews the relevant domestic and international literatures in recent years,discussing the biological characteristics of DPSCs and the research progress in their applieation in the treatment of corneal and retinal diseases,with the aim of providing insights and strategies for the study of DPSCs in translational medicine and ophthalmic-related diseases.
6.Mechanism of Flavonoids from Mulberry Leaves on Insulin Resistance and Inflammatory Response in Diabetic Mice

ObjectiveTo observe the glucose-lowering, insulin resistance-improving, and anti-inflammatory effects of flavonoids from mulberry leaves (FML) and explore their underlying mechanism. MethodMale db/db mice aged 6-7 weeks were randomly divided into a model group, a high-dose FML group (1.00 g·kg·d-1), and a low-dose FML group (0.50 g·kg-1·d-1). C57BL mice of the same age were assigned to the normal group. After six weeks of intervention, fasting blood glucose (FBG), serum fasting insulin levels (Fins), interleukin-6 (IL-6), tumor necrosis factor-α (TNF-α), free fatty acid (FFA), blood creatinine (SCr), blood urea nitrogen (BUN), and aspartate aminotransferase (AST) levels were measured, and the homeostasis model assessment of insulin resistance (HOMA-IR) was calculated. Superoxide dismutase (SOD), glutathione peroxidase (GSH-Px), and catalase activities in the liver were measured. Morphological changes in the liver were assessed by hematoxylin-eosin (HE) staining. The protein expression of cyclooxygenase-2 (COX-2), inducible nitric oxide synthase (iNOS), and nuclear factor-κB (NF-κB) in the liver was detected by Western blot. ResultCompared with the model group, the high-dose and low-dose FML groups showed significant reductions in FBG, Fins, HOMA-IR, IL-6, TNF-α, and FFA levels (P<0.05, P<0.01), and increased levels of SOD, GSH-Px, and catalase in the liver (P<0.05, P<0.01). HE staining of the liver in the FML groups showed improved arrangement of hepatocytes, reduced inflammatory cell infiltration, and alleviated cellular steatosis compared with the model group. The protein expression of COX-2, iNOS, and NF-κB in the liver significantly decreased in the FML groups as compared with that in the model group (P<0.05, P<0.01). ConclusionFML have glucose-lowering and insulin resistance-improving effect, which may be attributed to their regulation of the NF-κB pathway in the liver of diabetic mice, leading to the suppression of the release of COX-2, iNOS, and inflammatory cytokines, thereby improving the inflammatory state.

Parkinson's disease (PD) is a progressive chronic neurodegenerative disorder with a complex pathogenesis involving oxidative stress, neuroinflammation, mitochondrial dysfunction, and other factors. Currently, the clinical treatment of PD mainly includes levodopa, dopamine receptor agonists, monoamine oxidase B inhibitors, catechol-O-methyltransferase inhibitors, and anticholinergic drugs, but there is a lack of disease-modif g therapies that can definitively improve disease progression. According to the understanding of traditional Chinese medicine (TCM), PD is characterized by asthenia in origin and sthenia in superficiality. It is primarily caused by liver-kidney Yin deficiency, Qi-blood insufficiency, and closely related to wind, fire, phlegm, and blood stasis. Numerous clinical practices have shown that TCM has significant clinical value in the prevention and treatment of PD, the management of motor and non-motor symptoms, and the neuroprotection of dopaminergic neurons. The underlying mechanisms of TCM include antioxidative stress, anti-neuroinflammation, and regulation of mitochondrial dysfunction. This article categorized and summarized the pathogenesis of PD, systematically elucidated the pharmacological actions and molecular mechanisms of TCM monomer extracts and compounds in the prevention and treatment of PD, and provided the latest clinical research progress, aiming to provide references for the development and clinical use of TCM for PD.
8.Mechanism of Dendrobium huoshanense Polysaccharide Against Neuroinflammation in Parkinson's Disease Model: Based on NLRP3 Inflammasome

ObjectiveTo explore the mechanism of Dendrobium huoshanense polysaccharide (DHP) against inflammatory damage of neurons in Parkinson's disease (PD) model. MethodSH-SY5Y cells were randomized into blank group, model group, and DHP group. The survival rate of cells was measured by thiazole blue(MTT) assay, and the levels of lactate dehydrogenase (LDH), reactive oxygen species (ROS), malondialdehyde (MDA), and superoxide dismutase (SOD) were measured by colorimetric analysis. BV-2 microglia were classified into blank group, model group, DHP group, and MCC950 group (positive control group), and enzyme-linked immunosorbent assay (ELISA) was applied to detect the levels of tumor necrosis factor-α (TNF-α), interleukin-1β (IL-1β), and interleukin-18 (IL-18). The expression of NOD-like receptor protein 3 (NLRP3), adaptor protein apoptosis-associated dot protein (ASC), cysteine aspartic protease-1 (Caspase-1), and IL-1β was measured by Western blot. A total of 50 C57BL/6 mice were randomized into blank group, model group, DHP low-dose (100 mg·kg-1) group, DHP equivalent-dose (350 mg·kg-1) group, and MCC950 group (positive control group), 10 mice in each group. The motor balance and coordination of C57BL/6 mice were observed by beam walking test, tail suspension test and rotarod test. The levels of Iba-1 and tyrosine hydroxylase (TH) were detected by immunofluorescence staining. The damage of dopaminergic neurons in the substantia nigra was detected by FJB staining. The levels of inflammatory factors such as IL-1β, IL-18, and TNF-α in mouse midbrain tissues were detected by ELISA and the protein levels of NLRP3, ASC, Caspase-1, and IL-1β protein were measured by Western blot. ResultCompared with the blank group, the SH-SY5Y model group showed decreased cell survival, increased levels of LDH, ROS, and MDA (P<0.05), and decreased levels of SOD (P<0.05). Compared with the model group, the DHP group demonstrated increased cell survival, decreased levels of LDH, ROS, and MDA (P<0.01), and increased level of SOD (P<0.01). Compared with the blank group, BV-2 model group had high levels of IL-1β, IL-18, and TNF-α (P<0.05) and high protein expression of NLRP3, Caspase-1, IL-1β, and ASC (P<0.05). Compared with the model group, DHP and MCC950 groups demonstrated low levels of IL-1β, IL-18, and TNF-α (P<0.01) and low protein expression of NLRP3, Caspase-1, IL-1β, and ASC (P<0.01). Compared with the blank group, the C57BL/6 model group displayed long time to pass the balance wood (P<0.05), short time spent on the rod in the rotarod test (P<0.05), high levels of IL-1β, IL-18, and TNF-α (P<0.05) and expression of Iba-1 in the midbrain substantia nigra (P<0.05), low TH expression (P<0.05), more positive neurons in the FJB staining (P<0.05), and high expression of NLRP3, Caspase-1, ASC, and IL-1β proteins (P < 0.05). Compared with the model group, the mice in the DHP and MCC950 groups had short time to pass the balance beam (P<0.01), long time spent on the rod (P<0.01), low levels of IL-1β, IL-18, and TNF-α (P<0.01), low Iba-1 expression in midbrain substantia nigra (P<0.01), high TH expression (P<0.01), and small number of positive neurons in the midbrain substantia nigra (P<0.01). The expression of NLRP3, ASC, and IL-1β proteins was lower in the MCC950 group (P<0.01), and the expression of NLRP3, ASC, Caspase-1 and IL-1β proteins was lower in the DHP equivalent-dose group (P<0.01) than in the model group. ConclusionDHP has anti-oxidative stress effect. It regulates the expression of NLRP3 inflammasome and inhibits the overactivation of microglia, thereby alleviating the neuroinflammatory injury in PD and exerting the neuroprotective effect.
9.Lysosomal membrane protein Sidt2 knockout induces apoptosis of human hepatocytes in vitro independent of the autophagy-lysosomal pathway.

OBJECTIVE:
To explore the regulatory mechanism of human hepatocyte apoptosis induced by lysosomal membrane protein Sidt2 knockout.
METHODS:
The Sidt2 knockout (Sidt2-/-) cell model was constructed in human hepatocyte HL7702 cells using Crispr-Cas9 technology.The protein levels of Sidt2 and key autophagy proteins LC3-II/I and P62 in the cell model were detected using Western blotting, and the formation of autophagosomes was observed with MDC staining.EdU incorporation assay and flow cytometry were performed to observe the effect of Sidt2 knockout on cell proliferation and apoptosis.The effect of chloroquine at the saturating concentration on autophagic flux, proliferation and apoptosis of Sidt2 knockout cells were observed.
RESULTS:
Sidt2-/- HL7702 cells were successfully constructed.Sidt2 knockout significantly inhibited the proliferation and increased apoptosis of the cells, causing also increased protein expressions of LC3-II/I and P62(P < 0.05) and increased number of autophagosomes.Autophagy of the cells reached a saturated state following treatment with 50 μmol/L chloroquine, and at this concentration, chloroquine significantly increased the expressions of LC3B and P62 in Sidt2-/- HL7702 cells.
CONCLUSION
Sidt2 gene knockout causes dysregulation of the autophagy pathway and induces apoptosis of HL7702 cells, and the latter effect is not mediated by inhibiting the autophagy-lysosomal pathway.
