1.Screening and functional analysis of differentially expressed long non-coding RNA in the liver of mice infected with Schistosoma japonicum during the chronic pathogenic stage

Objective To screen differentially expressed long non-coding RNAs (lncRNAs) in the liver of mice infected with Schistosoma japonicum during the chronic pathogenic stage and identify their functions, so as to provide insights into unravelling the role of lncRNAs in S. japonicum infection-induced liver disorders. Methods Twenty 6-week-old C57BL/6 mice were randomly divided into two groups, of 10 animals each group. Each mouse in the experimental group was infected with (15 ± 2) S. japonicum cercariae via the abdomen for modeling chronic S. japonicum infection in mice, and distilled water served as controls. All mice were sacrificed 70 days post-infection, and mouse liver specimens were sampled for RNA extraction and library construction. All libraries were sequenced on the Illumina NovaSeq 6000 sequencing platform. Data cleaning was performed using the fastp software, and reference genome alignment and gene expression (FPKM) calculation were performed using the HISAT2 software. Potential lncRNA sequences were predicted using the software CNIC, CPC, Pfam, and PLEK, and potential lncRNAs were screened. Differentially expressed lncRNAs were screened with the DESeq2 software and subjected to Gene Ontology (GO) and Kyoto Encyclopedia of Genes and Genomes (KEGG) enrichment analyses to identify biological processes and metabolic pathways involved in target genes of differentially expressed lncRNAs. Results A total of 333 potential lncRNAs were screened, and 67 were identified as differentially expressed lncRNAs, including 49 up-regulated and 18 down-regulated lncRNAs. A total of 53 target genes were predicted for differentially expressed lncRNAs. GO enrichment analysis showed that these target genes were mainly enriched in biological process and molecular function, among which Sema7a, Arrb1, and Ccl21b genes may be hub target genes for positive regulation of extracellular regulated protein kinase 1 (ERK1) and ERK2 cascades and may participate in the regulation of collagen expression. KEGG enrichment analysis showed that the target genes of differentially expressed lncRNAs were mainly enriched in cytokine-cytokine receptor interaction, viral protein interactions with cytokines and cytokine receptors, chemokine signaling pathway, and nuclear factor kappa-B (NF-κB) signaling pathway. Conclusions This study identifies differentially expressed lncRNAs and functional enrichment of their target genes in the liver of mice during the chronic pathogenic stage of S. japonicum infection. Up-regulated lncRNAs may affect biological processes of ERK1/2 cascades and chemokine signaling pathways via target genes Sema7a, Arrb1, and Ccl21b, thereby affecting collagen expression and inflammatory signal pathways, ultimately affecting the development of liver disorders.
2.Characterization the response of Chlamydomonas reinhardtii serine/threonine protein kinase mutant to blue light.

In order to investigate the molecular mechanism of silk/threonine protein kinase (STK)-mediated blue light response in the algal Chlamydomonas reinhardtii, phenotype identification and transcriptome analysis were conducted for C. reinhardtii STK mutant strain crstk11 (with an AphvIII box reverse insertion in stk11 gene coding region) under blue light stress. Phenotypic examination showed that under normal light (white light), there was a slight difference in growth and pigment contents between the wild-type strain CC5325 and the mutant strain crstk11. Blue light inhibited the growth and chlorophyll synthesis in crstk11 cells, but significantly promoted the accumulation of carotenoids in crstk11. Transcriptome analysis showed that 860 differential expression genes (DEG) (559 up-regulated and 301 down-regulated) were detected in mutant (STK4) vs. wild type (WT4) upon treatment under high intensity blue light for 4 days. After being treated under high intensity blue light for 8 days, a total of 1 088 DEGs (468 upregulated and 620 downregulated) were obtained in STK8 vs. WT8. KEGG enrichment analysis revealed that compared to CC5325, the crstk11 blue light responsive genes were mainly involved in catalytic activity of intracellular photosynthesis, carbon metabolism, and pigment synthesis. Among them, upregulated genes included psaA, psaB, and psaC, psbA, psbB, psbC, psbD, psbH, and L, petA, petB, and petD, as well as genes encoding ATP synthase α, β and c subunits. Downregulated genes included petF and petJ. The present study uncovered that the protein kinase CrSTK11 of C. reinhardtii may participate in the blue light response of algal cells by mediating photosynthesis as well as pigment and carbon metabolism, providing new knowledge for in-depth analysis of the mechanism of light stress resistance in the algae.

Infectious bone defect is bone defect with infection or as a result of treatment of bone infection. It requires surgical intervention, and the treatment processes are complex and long, which include bone infection control,bone defect repair and even complex soft tissue reconstructions in some cases. Failure to achieve the goals in any step may lead to the failure of the overall treatment. Therefore, infectious bone defect has been a worldwide challenge in the field of orthopedics. Conventionally, sequestrectomy, bone grafting, bone transport, and systemic/local antibiotic treatment are standard therapies. Radical debridement remains one of the cornerstones for the management of bone infection. However, the scale of debridement and the timing and method of bone defect reconstruction remain controversial. With the clinical application of induced membrane technique, effective infection control and rapid bone reconstruction have been achieved in the management of infectious bone defect. The induced membrane technique has attracted more interests and attention, but the lack of understanding the basic principles of infection control and technical details may hamper the clinical outcomes of induced membrane technique and complications can possibly occur. Therefore, the Chinese Orthopedic Association organized domestic orthopedic experts to formulate An evidence-based clinical guideline for the treatment of infectious bone defect with induced membrane technique ( version 2023) according to the evidence-based method and put forward recommendations on infectious bone defect from the aspects of precise diagnosis, preoperative evaluation, operation procedure, postoperative management and rehabilitation, so as to provide useful references for the treatment of infectious bone defect with induced membrane technique.
4.Summary of the best evidence for prevention strategies of exercise-related hypoglycemia in type 1 diabetes mellitus children after discharge

Objective:To retrieve and analyze evidence on exercise-related hypoglycemia prevention in children with type 1 diabetes mellitus (T1DM) and summarize the best evidence.Methods:BMJ Best Practice, UpToDate, NICE, JBI, SIGN, Cochrane Library, RNAO, IDF, ADA, Web of Science, PubMed, Embase, Medlive, Wanfang database, China National Knowledge Infrastructure (CNKI) and China Biology Medicine disc (CBM) were searched by computer for guidelines, evidence summary, recommended practice, systematic review, clinical decision-making and expert consensus on the prevention of exercise-related hypoglycemia in children with T1DM in the recent 6 years (2015-2020) .Results:A total of 13 papers were included, including 6 guidelines, 3 expert consensus papers and 4 clinical decision papers. Finally, 32 pieces of best evidence were formed, including 7 aspects such as characteristics of exercise-related hypoglycemia, the prevention strategies of hypoglycemia before, during and after exercise, and nocturnal hypoglycemia, hypoglycemia prevention strategies and health education.Conclusions:The best evidence of exercise-related hypoglycemia prevention strategy for T1DM children selected in this study is comprehensive and specific, based on which nursing staff can conduct health education on exercise-related hypoglycemia prevention for out-of-hospital type 1 diabetes children and their parents.
5.Transcriptome analysis of signal transduction pathway involved in light inducing astaxanthin accumulation in Haematococcus pluvialis.

The unicellular green alga Haematococcus pluvialis is the best source of natural astaxanthin (AST) in the world due to its high content under stress conditions. Although high light (HL) can effectively induce AST biosynthesis, the specific mechanisms of light signal perception and transduction are unclear. In the current study, we used transcriptomic data of normal (N), high white light (W), and high blue light (B) to study the mechanisms of light inducing AST accumulation from the point of photoreceptors. The original data of 4.0 G, 3.8 G, and 3.6 G for N, W, and B were obtained, respectively, by the Illumina Hi-seq 2000 sequencing technology. Totally, 51 954 unigenes (at least 200 bp in length) were generated, of which, 20 537 unigenes were annotated into at least one database (NR, NT, KO, SwissProt, Pfam, GO, or KOG). There were 1 255 DEGs in the W vs N, 1 494 DEGs in the B vs N, and 1 008 DEGs in the both W vs N and B vs N. KEGG enrichment analysis revealed that photosynthesis, oxidative phosphorylation, carotenoid biosynthesis, fatty acids biosynthesis, DNA replication, nitrogen metabolism, and carbon metabolism were the significantly enriched pathways. Moreover, a large number of genes encoding photoreceptors and predicted interacting proteins were predicted in Haematococcus transcriptome data. These genes showed significant differences at transcriptional expression levels. In addition, 15 related DEGs were selected and tested by qRT-PCR and the results were significantly correlated with the transcriptome data. The above results indicate that the signal transduction pathway of "light signal - photoreceptors - interaction proteins - (interaction proteins - transcription factor/transcriptional regulator) - gene expression - AST accumulation" might play important roles in the regulation process, and provide reference for further understanding the transcriptional regulation mechanisms of AST accumulation under HL stress.

6.Evidence-based nursing practice for prevention of oral mucositis in children with hematopoietic stem cell transplantation

Objective:To apply the best evidence for prevention of oral mucositis in children with hematopoietic stem cell transplantation (HSCT) to clinical practice and evaluate its efficacy.Methods:Evidence-based nursing method was applied to obtain the best evidence, the evidence quality review indexes were established and the obstacle factors were analyzed, and then the action strategy was constructed. Using the convenient sampling method, a total of 60 children who were admitted to HSCT ward of Beijing Children's Hospital of Capital Medical University and 14 nurses working in HSCT ward were selected as the research objects from August 2019 to October 2020. The children admitted from August 2019 to February 2020 were included in the control group, and children admitted from March to October 2020 were included in the experimental group, with 30 cases in each group. Children in the control group received routine preventive measures for oral mucositis, while children in the experimental group received preventive measures for oral mucositis constructed based on the best evidence. Before and after the application of the evidence, the nurses' knowledge of preventing oral mucositis in children with HSCT was investigated through self-developed test papers. The incidence of oral mucositis and parents' satisfaction with prevention measures for oral mucositis were compared between the two groups.Results:Evidence-based nursing practice used 10 pieces of evidence. The incidence of oral mucositis in the experimental group was 50% (15/30) , lower than 83% (25/30) in the control group, and the difference was statistically significant (χ 2=7.50, P < 0.05) . The score of satisfaction with for preventive measures of oral mucositis of parents in the experimental group was (4.96±0.05) , which was higher than (3.65±1.60) of the control group, and the difference was statistically significant ( t=-9.69, P<0.01) . After the application of the evidence, the score of nurses' knowledge of prevention of oral mucositis was (98.22±6.65) , which was higher than (78.72±12.03) before the application, and the difference was statistically significant ( t=5.30, P<0.01) . Conclusions:Management of oral mucositis in children with HSCT through evidence-based nursing practice can effectively reduce the incidence of oral mucositis in children, improve parents' satisfaction with prevention measures for oral mucositis and nurses' knowledge of prevention of oral mucositis.

It is an important measure to establish an effective communication mechanism for filling in the front page of medical records to ensure the good operation of diagnosis-related groups. Taking cerebral infarction as an example, the authors carried out the pilot work of multidisciplinary cooperation mode based on disease types around its coding axis. The multi-disciplinary assistance model could provide a good platform for communication and learning among multiple disciplines, break the barriers between disciplines, improve the quality of the front page and medical record writing of clinicians, and improve the quality and efficiency of coders′ coding.
8.Clinical study of value of serum Klotho level of elderly donors in predicting postoperative renal graft function in recipients

Objective To explore the feasibility of serum Klotho level in the elderly donors to predict the renal graft function in the recipients. Methods Clinical data of 16 elderly donors and 27 recipients undergoing renal transplantation were collected. The general status of the recipients was observed. The levels of serum Klotho and serum creatinine (Scr) in the elderly donors were measured on the day of renal transplantation. The Scr levels in the recipients were measured at postoperative 1, 3 and 12 months respectively. The estimated glomerular filtration rate (eGFR) was calculated. The correlation between the serum Klotho level of the donors and postoperative graft function of the recipients was analyzed. Results The cold ischemia time during renal transplantation was (649±245) min. The incidence rate of delayed graft function (DGF) was 26%. The incidence rate of acute rejection was 7%. In the elderly donors, the serum Klotho level was 537 (245-793) pg/mL and the Scr level was (164±62) μmol/L. At postoperative 1, 3 and 12 months, the Scr levels in the recipients were (136±47), (132±43) and (133±46) μmol/L, respectively. The corresponding eGFR was (52±20), (52±19) and (53±21) mL/(min?1.73m2), respectively. The serum Klotho level in the elderly donors was negatively correlated with the renal graft function at postoperative 1 month in the recipients (
9.Application of plasma neutrophil gelatinase-associated lipocalin as an emerging biomarker in the early diagnosis of acute renal injury following renal transplantation

Objective To study the prognostic role of plasma neutrophil gelatinase-associated lipocalin (NGAL) early after renal transplantation.Methods A total of 37 kidney recipients were enrolled from Department of Organ Transplantation,Guangdong Second Provincial General Hospital within a 12-month period of time.Plasma NGAL was measured immediately before and at 6 and 12 h post-transplantation.Changes of serum creatinine were documented daily within the first week postoperation.Acute kidney injury (AKI)/graft rejection during the first week after transplantation was the outcome variable.Results The levels of serum NGAL in the 37 patients were (311.14 ± 102.69),(317.81 ± 107.28) and (312.16 ± 134.80) μg/L respectively immediately before and at 6 and 12 h post-transplantation.There was no significant difference in serum NGAL levels before and 6 h or 12 h after operation (P =0.70,and P =0.96).There were no significant differences in gender and age between the two groups (P =0.29,and P =0.20).There was significant difference in creatinine levels between the AKI group and the non-AKI group (P =0.002) and between pre-operation and 6 or 12 h postoperation.The preoperative levels of serum NGAL in AKI group and non-AKI group were (333.58 ± 116.30) and (300.36 ± 96.15) μg/L (P =0.36),and those were (383.3 ± 147.16) and (286.32 ± 65.97) μg/L (P<0.01) at 6 h,and (437.33 ± 164.16) and (252.08 ± 57.53) μg/L (P< 0.001) at 12 h after operation.The sensitivity and specificity of serumNGAL (317μg/L at 12 h after operation as the cutoff value) predicting AKI was 100% and 92% respectively,which was much better than that of serum creatinine at the corresponding time point (sensitivity =66.7%,and specificity =61.9%).Conclusion Plasma NGAL,particularly at 12 h after transplantation,is a very sensitive and specific biomarker for predicting AKI.
10.Obstetrical nurses′awareness and attitude to breastfeeding in patients with hepatitis B

Objective To understand obstetrical nurses′awareness and attitude to breastfeeding in patients with hepatitis B in level three class A hospitals in Beijing,so as to provide references for scientific guidance and increase of breastfeeding rate in this special population in the future.Methods Using convenience sampling method,questionnaires were delivered in 1 86 obstetrical nurses from 7 level three class A hospitals in Beijing,in order to investigate their awareness on whether patients with hepatitis B could feed infants with breast milk,and their attitude toward breastfeeding of patients with hepatitis B.Results A total of 1 86 obstetrical nurses were surveyed,of whom only 64.6% could correctly answer breastfeeding-related problems about patients with hepatitis B.82.3% of the surveyed obstetrical nurses falsely thought that level of HBV DNA titer in breast milk would affect breastfeeding.Only 29.6% of the surveyed nurses were aware of Clinical Guideline on Prevention of Mother-to-child Transmission.When surveying about their willingness,only 45.2% of the nurses supported the breastfeeding of patients with hepatitis B.Differences were statistically significant among nurses on their awareness and attitude toward whether newborn infants could be fed by breast milk from patients with hepatitis B after receiving immune block,and on whether the nurses would encourage puerperas with hypatitis B to feed their infants by breast milk when they were consulted by these puerperas.Conclusions Hospital and department managers should pay increasing attention to learning and awareness of new knowledge and new technologies of obstetrical nursing staff,by implementing continuing education,enhancing their acceptance of self-worth and their confidence in self knowledge level,so as to provide more scientific,accurate and effective health education for maternal and child safety.
