1.Clinical Observation of Modified Huanglian Wendantang in Treatment of Cardiovascular Risk Factors in Patients with Metabolic Syndrome Under Guidance of Treating Disease before Its Onset

ObjectiveTo observe the clinical effect of modified Huanglian Wendantang on cardiovascular risk factors in patients with metabolic syndrome under the guidance of treating disease before its onset. MethodsA total of 82 patients with metabolic syndrome treated in the First Affiliated Hospital of Heilongjiang University of Chinese Medicine from July 2023 to July 2024 were selected and allocated into an observation group (41 cases) and a control group (41 cases) by the random number table method. The control group received routine treatment, and the observation group was treated with modified Huanglian Wendantang on the basis of routine treatment. Both groups were treated for 8 weeks. The therapeutic effects on TCM symptoms after treatment in the two groups were evaluated. The levels of obesity degree indicators, blood pressure indicators, glucose and lipid metabolism indicators, inflammatory factors, and vascular endothelial function indicators before and after treatment in the two groups were measured, and the treatment safety was evaluated. ResultsAfter treatment, the total response rate of TCM symptoms in the observation group was 97.56% (40/41), which was higher than that (87.80%, 36/41) in the control group (χ2=5.205, P<0.05). After treatment, both groups showed declines (P<0.05) in systolic blood pressure (SBD), diastolic blood pressure (DBP), triglyceride (TG), total cholesterol (TC), low density lipoprotein cholesterol (LDL-C), fasting blood glucose, 2-hour postprandial blood glucose (2 h PG), glycosylated hemoglobin (HbA1c), fasting insulin (FINS), Homeostatic Model Assessment for Insulin Resistance (HOMA-IR), body mass index (BMI), waist circumference (WC), waist-to-hip ratio (WHR), leptin (LEP), interleukin-6 (IL-6), tumor necrosis factor-α (TNF-α), endothelin-1 (ET-1), and inducible nitric oxide synthase (iNOS). Moreover, the declines in the observation group were more obvious than those in the control group (P<0.05, P<0.01). After treatment, both groups showed elevated levels of high density lipoprotein cholesterol (HDL-C), adiponectin (ADP), nitric oxide (NO), and endothelial nitric oxide synthase (eNOS) (P<0.05), and the above indexes in the observation group were higher than those in the control group (P<0.01). ConclusionUnder the guidance of the thought of treating disease before its onset, modified Huanglian Wendantang was used to treat patients with metabolic syndrome. The decoction improved the clinical efficacy by ameliorating IR to improve insulin sensitivity, reducing inflammation, and protecting the vascular endothelial function. It inhibits cardiovascular risk factors without inducing adverse reactions, being worthy of clinical application and promotion.
2.Cause Analysis and Solution Strategy of Poor Solubility of Dioscoreae Rhizoma Formula Granules

Dioscoreae Rhizoma formula granules are made from decoction pieces by decocting, extracting, separating, concentrating, drying and granulating, which have the advantages of simple dispensing, convenient use and easy to take without decoction. However, because Dioscoreae Rhizoma is rich in starch and mucus components, its extract powder and formula granules are poorly soluble and difficult to dissolve or disperse completely within 5 min, and the insoluble material is difficult to dissolve completely even after 24 h in water, which affects the quality evaluation of the formula granules and medication psychology of patients. Therefore, by studying the dissolution process and mechanism of Dioscoreae Rhizoma extract and its formula granules, it was found that the special chemical composition of Dioscoreae Rhizoma, the denaturation of starch and its compounding with protein and other substances during the high temperature extraction process, and the contraction of coating membrane during the spray drying process were combined to form the special microstructure of coating membrane covering starch granules, and it is the root cause of poor solubility of Dioscoreae Rhizoma formula granules. Based on the research on the structure, property and function of the powder, this paper proposed a technical strategy to improve the solubility of Dioscoreae Rhizoma formula granules by powder modification process, and experimentally demonstrated that the modified Dioscoreae Rhizoma formula granules could completely dissolve within 2 min, which solved the technical problem and could provide reference for the improvement of solubility of other similar varieties, and promote the high-quality development of traditional Chinese medicine formula granule industry.
3.Study on chemical constituents and in vitro antioxidant activity of ethanol extract of Sabia parviflora

OBJECTIVE To separate and identif y the chemical constituen ts in 70% ethanol extract of Sabia parviflora ,and to preliminarily evaluate their in vitro antioxidant activity. METHODS The chemical constituents were separated and purified by silica gel,ODS reversed-phase silica gel ,Sephadex-LH20 column and preparative high performance liquid chromatography. The structures of the isolated compounds were identified by 1H-NMR,13C-NMR and ESI-MS. The in vitro antioxidant activities of the compounds were investigated by 2,2-diphenyl-1-picrylhydrazyl radical (DPPH·),2,2′-azino-bis(3-ethylbenzothiazoline-6- sulfonate)diammonium radical (ABST+)and hydroxyl radical (OH·). RESULTS A total of 9 compounds were isolated from the 70% ethanol extracts of S. parviflora . They were identified as rutin (1),diiononyl phthalate (2),dibutyl phthalate (3),vomifoliol (4),rhododendrol(5),quercetin-3-O-gentiobioside(6),narcissoside(7),kaempferol-3-O-rutinoside(8)and bonaroside (9). The in vitro antioxidant results showed that compound 1-9 showed certain in vitro antioxidant activity ,and the half scavenging concentrations of compound 1,6,7 and 8 to DPPH ·,ABST+,OH·were lower than 70 μg/mL. CONCLUSIONS Vomifoliol, rhododendrol and bonaroside are isolated from S. parviflora for the first time ,and rutin ,quercetin-3-O-gentiobioside,narcissoside and kaempferol- 3-O-rutinoside show good in vitro antioxidant activity.
4.Prenatal ultrasound diagnosis of fetal right aortic arch

Objective:To explore the prenatal ultrasound manifestations of fetal congenital right aortic arch and its diagnostic significance.Methods:The characteristics of prenatal ultrasound images (focusing on the three vessel trachea section) and clinical data of 128 cases of fetal congenital right aortic arch diagnosed in Dalian Women and Children′s Medical Center(Group) from January 2014 to January 2019 were analyzed retrospectively.Results:Among 128 cases of fetal congenital right aortic arch, 111 cases were right aortic arch and leftductus arteriosus with aberrant left subclavian artery (RAA-LDA-ALSA), 1 case was diagnosed as left aortic arch atresia of double aortic arch by operation after birth; 9 cases were right aortic arch and right ductus arteriosus (RAA-RDA); 6 cases were mirror-image right active aortic arch and left ductus arteriosus connected to descending aorta (RAA-BAMB-LDA-DAO); 2 cases were mirror-image right aortic arch and left ductus arteriosus connected to the left innominate artery (RAA-BAMB-LDA-LINA). The three vessels and trachea view (3VT) had characteristic sonographic features.If necessary, coronary section of the upper thoracic aorta and / or stic were added - HD live flow technology assisted the diagnosis of 3vt section.Conclusions:Three vessel trachea view is a sensitive and effective view for prenatal detection and diagnosis of congenital right aortic arch. Upper thoracic aorta coronal section and combined stic - HD live flow technology can make up for its shortcomings. Prenatal ultrasound diagnosis of right aortic arch hasguiding significance for prenatal prognosis consultation.
5.A case of Ⅴ-type hyperlipidemia pancreatitis in children

Hyperlipidemia pancreatitis in children is mostly genetic metabolic disease.The incidence of acute pancreatitis in children is only (3-13)/100 000, and pancreatitis caused by hyperlipidemia accounts for 9% of acute pancreatitis.A child suffering from V-type hyperlipidemia pancreatitis was admitted to the Children′s Hospital Affiliated to Nanjing Medical University in July 2019.The missense mutation at position c. 2770G>A of CFTR gene (nucleotide 2770 in coding region changed from guanine to adenine) in children was detected by gene sequencing, thus resulting in amino acid change p. D924N.It is extremely rare to report that CFTR gene mutation causes hereditary pancreatitis, and there is no literature report on c. 2770G>A site.This case is reported as follows, hoping to provide reference and inspiration for pediatricians.
6.Effect of ICX ? tablets on controlling the pollution of dental unit waterlines

Objective:To explore the effect of ICX ? tablets on controlling the pollution of dental unit waterlines. Methods:From September 30, 2018 to February 23, 2019, convenience sampling was used to select dental chair unit (DCU) with the dental pulp professional brand of A-dec which was newly put into use in a stomatology hospital for numbering. Using the method of random number table, four DCUs were selected and included in this study, including two in experimental group and two incontrol group. In control group, distilled water was added into two DCUs in dependent water storage tanks. In experimental group, two DCUs independent water storage tanks were added with distilled water and ICX ? tablets. From the first day of clinical use, water samples were collected continuously for 7 days, and the ICX ? tablets group was continuously sampled for 12 weeks for bacterial culture, and the number and qualified rate of colonies were counted. Results:A total of 280 water samples were collected.The number of colonies at the water outlet of the new DCU that used distilled water added with ICX ? tablets as dental unit waterlines was lower than that of distilled water group from the first day, and the difference was statistically significant ( P=0.007) . The qualified rate of colony number at the water outlet of the new DCU that used distilled water added with ICX ? tablets as dental unit waterlines was higher than that of distilled water group from the second day, and the difference was also statistically significant ( P=0.007) . Conclusions:ICX ? tablets can effectively control the pollution of dental unit waterlines and can keep the number of colonies in dental unit waterlines at a low level for a long time, but it still needs to be combined with regular monitoring and enhanced disinfection.
7.Monitoring of pollution conditions of the new dental unit waterlines

Objective:To study changes of pollution conditions of the new dental unit waterlines.Methods:By means of random sampling, 7 new dental chair units of the same brand were selected. Of which, 5 dental chair units (DCU) were supplied with water after centralized treatment of municipal water and 2 dental chair units were supplied with independent water storage tanks. From the first day of clinical use, water samples of three-way syringes and high-speed dental handpieces and mouthwashes were continuously collected for bacterial culture and the number of colonies was counted.Results:A total of 343 water samples were collected. There was a statistically significant difference in the number of colonies in the water samples from the outlet of three-way syringes and high-speed dental handpieces after the waterlines was rinsed for 2 min before treatment ( P<0.01) . There was no statistically significant difference in the number of dental unit warterlines (DUWLs) colonies between the central water supply and the independent water storage tank before and after washing ( P> 0.05) . Waterline washing could significantly reduce the number of colonies in the waterlines ( P<0.01) , but there was no significant change in the number of colonies throughout the day ( P>0.05) . The number of colonies in the new DUWLs could exceed the recommended standard in the guideline on the second day of clinical use and the number of colonies increased significantly compared with that on the first day ( P<0.05) . Conclusions:In clinical work, no matter what kind of water supply method is used, in addition to waterline washing according to regulations, DUWLs should be disinfected daily.
8. The prognosis value of comprehensive geriatric assessment in elder patients with acute myeloid leukemia in a single center

Objective:
To evaluate the prognostic significance of comprehensive geriatric assessment (CGA) in Chinese elderly acute myeloid leukemia (AML) patients.
Methods:
73 AML patients over the age of 60 were enrolled. CGA stratification included the following 3 instrument assessment: activity of daily living (ADL) ; instrumental activity of daily living (IADL) ; comorbidity score according to the Modified cumulative illness rating score for geriatrics (MCIRS-G) . According to CGA and age, the enrolled patients were grouped into 'fit’, 'unfit’ and 'frail’ categories.
Results:
The median age of 73 elderly AML patients were 75 years old. According to CGA, 37 (50.1%) patients were classified as 'fit’, 14 (19.2%) as 'unfit’, and 22 (30.7%) as 'frail’. 33 (89.2%) patients in fit group received induction chemotherapy, or demethylation treatment, as 8 (57.9%) in unfit, 10 (45.5%) in frail. The overall response rate was 68.7%、62.5%, 75.0% in fit, unfit, and frail group, respectively (

Objective To evaluate the performance on comprehensive geriatric assessment (CGA) in elderly patients with acute myeloid leukemia (AML)and to investigate the differences between Eastern Cooperative Oncology Group (ECOG) performance status (PS) and CGA scoring.Methods Patients aged 60 years or over diagnosed with AML at Beijing Hospital from September 2008 to April 2018 were enrolled in this study.CGA was performed during staging procedures through application of age,activities of daily living(ADL)/instrumental activities of daily living(IADL)and the comorbidity score based on the cumulative illness rating scale for geriatrics(CIRS-G).According to CGA scores,patients were classified into a not-applicable group,an applicable group and a frail group.The difference between ECOG-PS and CGA scoring was compared.Results A total of 73 patients were enrolled in this study.There were 37 (50.7 %),14 (19.2 %) and 22 (30.1%) patients in the applicable,not-applicable and frail groups,respectively.Comorbidities were observed in the majority of patients,with cardiovascular disease,diabetes mellitus and hypertension among the most common disorders.In patients aged 60 to 64 years,16.7% (1/6) were considered as frail,and the percentage of frail patients increased to 36.4% (8/22)in patients aged 80 years and over.Moreover,functional impairment evaluated by CGA was observed in 36.5 % (19/52)of patients with ECOG-PS ≤ 1.Conclusions According to CGA scoring,approximately half of AML patients aged 60 years or over have functional impairment.Functional impairment is observed in some patients with ECOG-PS ≤1,indicating that ECOG-PS may underestimate functional impairment in elderly AML patients.
10.Prenatal diagnosis of two Turner syndrome fetuses with 46,X,i(X)(q10)

Objective To summarize the prenatal diagnosis and genetic counseling of Turner syndrome fetuses with 46,X,i(X)(q10).Methods Two gravidas admitted to the Obstetrics and Gynecology Hospital of Dalian were enrolled in this study.One gravida,who was admitted in October 2016,was classified as high risk of Down syndrome based on prenatal serologic screening and systematic ultrasonography,which found remarkably shorter humeri and femora than fetus of the same gestations.The other was suggested to be monosomy X after non-invasive prenatal testing and admitted in November 2017.Fluorescence in situ hybridization (FISH) and karyotyping were performed for prenatal diagnosis.Peripheral blood karyotyping was also offered to the two women and their partners.Results FISH test for amniotic fluid did not find numerical abnormality in 13,18,21,and sex chromosomes in these two fetuses.Karyotype analysis showed that the two fetuses were both 46,X,i(X) (q10),while their parents were normal.Both cases were terminated after genetic counseling.Conclusions Prenatal serological screening,systematic ultrasonography and non-invasive prenatal testing may help to identify Turner syndrome fetus of 46,X,i(X) (q10).Timely and accurate prenatal diagnosis may prevent the affected fetus from being born.
