Chinese Journal of Tissue Engineering Research 2025;29(1):52-57
BACKGROUND:Ferroptosis is a mode of programmed cell death distinct from apoptosis, necrosis, and other novel cellular deaths, which occurs mainly due to accumulated lipid peroxidation.Ferroptosis has been shown to be involved in the pathological process following subarachnoid hemorrhage.Baicalein, serving as an adept sequestered of iron, evinces its prowess by quelling lipid peroxidative cascades.Nonetheless, the enigma lingers as to whether baicalein possesses the capacity to ameliorate neuronal ferroptosis, elicited in the wake of early brain injury after subarachnoid hemorrhage. OBJECTIVE:To investigate the effect and mechanism of baicalein on neuronal ferroptosis after subarachnoid hemorrhage. METHODS:Primary neuronal cells were extracted from C57BL/6L fetal mice at 16-17 days of gestation.Hemoglobin was used to stimulate primary neuronal cells to simulate an in vitro subarachnoid hemorrhage model. The viability of primary neuronal cells treated with baicalein at concentrations of 5,15,25,50,and 100 μmol/L for 24 hours was detected by CCK-8 assay to determine the optimal concentration of baicalein.Primary neuronal cells were divided into control group, hemoglobin group, and hemoglobin+baicalein group. The levels of reactive oxygen species and malondialdehyde in cells were detected by kits. The mRNA expressions of ferroptosis-related markers PTGS2,SLC7A11,and glutathione peroxidase 4 were detected by RT-PCR.The primary neuronal cells were further divided into control group,SLC7A11 inhibitor Erastin group, hemoglobin group, hemoglobin+baicalein group, and hemoglobin+baicalein+Erastin group. The expression of the ferroptosis related markers SLC7A11 and glutathione peroxidase 4 was detected by western blot assay. RESULTS AND CONCLUSION:(1)Baicalein(25 μmol/L)was selected as the following experimental concentration.(2)Compared with the hemoglobin group, the level of malondialdehyde and the level of reactive oxygen species were significantly decreased(P<0.05)in the hemoglobin+baicalein group.(3)Compared with the hemoglobin group, the mRNA expression of PTGS2 significantly decreased, and the mRNA expression of SLC7A11 and glutathione peroxidase 4 significantly increased(P<0.000 1)in the hemoglobin+baicalein group.(4)SLC7A11 inhibitor Erastin could reverse the baicalin-improved ferroptosis effect to a certain extent(P<0.05).(5)The results showed that baicalein could alleviate the ferroptosis of neuronal cells after subarachnoid hemorrhage through the SLC7A11/GPX4 pathway.

BACKGROUND:The pituitary gland is an important endocrine organ in the body.Certain diseases can cause damage to the pituitary gland, such as pituitary adenoma and abnormal hormone secretion.Pituitary stem cells, due to their self-renewal and multi-directional differentiation potential, are expected to become a new therapeutic approach for repairing damaged pituitary glands. OBJECTIVE:To isolate and culture pituitary stem cells using the suspension cell ball culture method and identify their proliferation and differentiation ability. METHODS:Pituitary stem cells were isolated and cultured from the pituitary gland of newborn New Zealand white rabbits using the suspension cell ball culture method, and their morphological characteristics were observed.Immunofluorescence cytochemistry was used to detect the expression of pituitary stem cell markers SOX2 and Nestin.EdU labeling method was utilized to detect the proliferative ability of pituitary stem cells. After adherent and induced differentiation, the hormone levels in the culture medium were detected by ELISA. RESULTS AND CONCLUSION:Pituitary stem cell spheres could be successfully isolated by the suspension cell ball culture method, with strong proliferative ability.Positive expression of stem cell-specific markers SOX2 and Nestin was found in the cultured cells. After induction and differentiation, adrenocorticotropic hormone, thyroid hormone, growth hormone, luteinizing hormone,follicle-stimulating hormone, and prolactin levels significantly increased in the medium(P<0.001),with strong differentiation ability.

Journal of Central South University(Medical Sciences) 2024;49(6):961-971
Objective:Compared with long-term renal replacement therapy, kidney transplantation is the ideal treatment for end-stage renal disease(ESRD),significantly extending patient life and improving quality of life.Kidney transplant patients need to adhere to lifelong immunosuppressive medication regimens, but their medication adherence is generally poor compared with other organ transplant recipients.Medication adherence is closely related to medication literacy and psychological status, yet related studies are limited. This study aims to investigate the current status of medication adherence, inner strength, and medication literacy in kidney transplant patients, analyze the relationships among these 3 factors, and explore the mediating role of inner strength in the relationship between medication literacy and medication adherence. Methods:A cross-sectional survey was conducted from March to October 2023 involving 421 patients aged≥18 years who visited kidney transplantation outpatient clinics at 4 tertiary hospitals in Hunan Province. The inner strength, medication literacy, and medication adherence of kidney transplant patients were investigated using the Inner Strength Scale(ISS),the Chinese version of the Medication Literacy Assessment in Spanish and English(MedLitRxSE),and the Chinese version of the Morisky Medication Adherence Scale-8(C-MMAS-8),respectively.Univariate analysis was performed to examine the effects of demographic and clinical data on medication adherence.Correlation analysis was conducted to explore the relationships among medication literacy, medication adherence, and inner strength.Significant variables from univariate and correlation analyses were further analyzed using multiple linear regression, and the mediating effect of inner strength was explored. Results:Among the 421 questionnaires collected,408 were valid, with an effective rate of 96.91%.The scores of C-MMAS-8,MedLitRxSE,and ISS were 6.64±1.16,100.63±14.67,and 8.47±4.03,respectively.Among the 408 patients, only 86(21.08%)patients had a high level of medication adherence, whereas 230(56.37%)patients had a medium level of medication adherence, and 92(22.55%)patients had poor medication adherence.Univariate analysis indicated that the kidney transplant patients'age, marital status, education levels, years since their kidney transplant operation, number of hospitalizations after the kidney transplant, and adverse drug reactions showed significant differences in medication adherence(all P<0.05).Correlation analysis showed that inner strength positively correlated with both medication literacy(r=0.183,P<0.001)and medication adherence(r=0.201,P<0.00l).Additionally,there was a positive correlation between medication adherence and medication literacy(r=0.236,P<0.00l).Inner strength accounted for 13.22%of the total effect in the mediating role between medication literacy and medication adherence. Conclusion:The level of medication adherence among kidney transplant patients needs improvement, and targeted intervention measures are essential.Inner strength mediates the relationship between medication literacy and medication adherence in these patients.Healthcare professionals should focus on enhancing medication literacy and supporting patients'inner strength to improve medication adherence.

Chinese Journal of Trauma 2024;40(9):769-780
Hip fracture in the elderly is characterized by high incidence, high disability rate, and high mortality and has been recognized as a public health issue threatening their health. Surgery is the preferred choice for the treatment of elderly patients with hip fracture. However, lower extremity deep venous thrombosis (DVT) has an extremely high incidence rate during the perioperative period, and may significantly increase the risk of patients′ death once it progresses to pulmonary embolism. In response to this issue, the clinical guidelines and expert consensuses all emphasize active application of comprehensive preventive measures, including basic prevention, physical prevention, and pharmacological prevention. In this prevention system, basic prevention is the basis of physical and pharmacological prevention. However, there is a lack of unified and definite recommendations for basic preventive measures in clinical practice. To this end, the Orthopedic Nursing Professional Committee of the Chinese Nursing Association and Nursing Department of the Orthopedic Branch of the China International Exchange and Promotive Association for Medical and Health Care organized relevant nursing experts to formulate Expert consensus on perioperative basic prevention for lower extremity deep venous thrombosis in elderly patients with hip fracture ( version 2024) . A total of 10 recommendations were proposed, aiming to standardize the basic preventive measures for lower extremity DVT in elderly patients with hip fractures during the perioperative period and promote their subsequent rehabilitation.

Chinese Journal of Anesthesiology 2024;44(6):688-693
Objective:To evaluate the role of nuclear factor kappa B (NF-κB)/NOD-like receptor thermoprotein structural domain-associated protein 3 (NLRP3) signaling pathway in hippocampal microglia in dexmedetomidine-induced improvement in long-term cognitive function after multiple sevoflurane anesthesia in newborn mice. Methods:One hundred SPF healthy male C57BL/6 mice, aged 6 days, weighing 3-5 g, were divided into 5 groups ( n=20 each) by a random number table method: control group (group C), multiple sevoflurane anesthesia group (group S), multiple sevoflurane anesthesia+ NF-κB inhibitor pyrrolidine dithiocarboxylate group (group SP), multiple sevoflurane anesthesia + dexmedetomidine group (group SD), and multiple sevoflurane anesthesia + NF-κB agonist phorbol 12-myristate 13-acetate (PMA) + dexmedetomidine group (group SPD). Anesthesia was induced by inhalation of 3% sevoflurane and maintained by inhalation of 60% oxygen and 3% sevoflurane for 2 h for 3 consecutive days in S, SP, SD and SPD groups. At 30 min before each sevoflurane inhalation, pyrrolidine dithiocarbamate 100 mg/kg was intraperitoneally injected in group SP, and dexmedetomidine 20 μg/kg was intraperitoneally injected in group SD. NF-κB agonist PMA 15 μg/kg was intraperitoneally injected at 1 h before each sevoflurane inhalation, and 30 min later dexmedetomidine 20 μg/kg was intraperitoneally injected in group SPD. The mice were sacrificed at the age of 11 days, and the hippocampal tissues were isolated for determination of the expression of phosphorylated NF-κB p65 (p-NF-κB p65) and NLRP3 (by Western blot). The co-staining area of NLRP3 and microglia-specific ionized calcium binding adaptor molecule 1 (Iba-1) double positive cells (NLRP3 + -Iba-1 + cells) was calculated by immunofluorescence. The open field test and novel object recognition test were performed at 29 days of age, and the Morris water maze test was performed at 30-34 days of age. Results:There were no significant differences in each parameter of the open field test among the five groups ( P>0.05). Compared with group C, the expression of p-NF-κB p65 and NLRP3 in hippocampal tissues was significantly up-regulated, the co-staining area of NLRP3 + -Iba-1 + cells was increased, the percentage of novel object exploration and discrimination index were decreased, the escape latency was prolonged, and the frequency of crossing the original platform was reduced in group S ( P<0.05). Compared with group S, the expression of p-NF-κB p65 and NLRP3 in hippocampal tissues was significantly down-regulated, the co-staining area of NLRP3 + -Iba-1 + cells was reduced, the percentage of novel object exploration and discrimination index were increased, the escape latency was shortened, and the frequency of crossing the original platform was increased in SP, SD and SPD groups ( P<0.05). Compared with SD group, the expression of p-NF-κB p65 and NLRP3 was significantly up-regulated, the co-staining area of NLRP3 + -Iba-1 + cells was increased, the percentage of novel object exploration and discrimination index were decreased, the escape latency was prolonged, and the number of crossing the original platform location was decreased in SPD group ( P<0.05). Conclusions:The mechanism by which dexmedetomidine improves long-term cognitive function after multiple sevoflurane anesthesia may be related to inhibiting the activation of NF-κB/NLRP3 signaling pathway in the hippocampal microglia of newborn mice.

Chinese Journal of Nephrology 2024;40(5):367-378
Objective:To analyze the clinical characteristics of pediatric atypical hemolytic uremic syndrome (aHUS), and provide clinical experience for the diagnosis and treatment of aHUS in China. Methods:It was a single-center retrospective study. Fifteen aHUS children treated and having complete clinical data at Children's Hospital of Nanjing Medical University between December 31, 2017 and October 15, 2023 were enrolled to analyze the clinical features covering laboratory examinations, genetic testing results, and clinical manifestations. The children were classified based on genetic testing and complement factor H (CFH) antibody detection results to analyze the corresponding clinical characteristics. Results:Among the 15 aHUS patients. There were 8 males and 7 females. The onset age was 5.1 (0.7, 10.8) years old. All patients underwent genetic testing, with 9/15 of aHUS-related gene mutation, revealing 2 de novo mutations in complement factors-related genes. Among 11 patients screened for CFH antibody, 6 tested positive. C3 was detected in 14 patients , and C3 decreased in 9 patients. In laboratory examinations, there were notable decreases in red blood cell (RBC) count in 13 patients, platelet (PLT) count in 15 patients, hemoglobin (Hb) in 15 patients and estimated glomerular filtration rate (eGFR) in 14 patients. Blood urea nitrogen (BUN) and serum creatinine (Scr) were markedly elevated in 13 patients and 9 patients, respectively. Twelve patients exhibited elevated transaminase levels, and 14 patients exhibited elevated lactate dehydrogenase (LDH) levels. Clinically, 11 patients had triggers, and 4 patients had clear family histories. Common clinical features including anemia, thrombocytopenia, proteinuria and hematuria were in 15 patients. There were statistically significant differences in RBC count ( Z=-2.84, P=0.005), PLT count ( Z=-6.65, P<0.001), Hb ( t=-3.71, P=0.002), LDH ( Z=3.76, P=0.002), BUN ( Z=2.71, P=0.017), and eGFR ( Z=-3.65, P=0.003) before and after treatment except alanine transaminase, aspartate transaminase, Scr and complement C3 (all P>0.05). There were no significant differences in onset age, RBC count, PLT count, Hb, LDH, alanine transaminase, aspartate transaminase, Scr, BUN, eGFR, and C3 between aHUS-related gene mutation and non-mutation groups, and CFH antibody-positive and negative groups (all P>0.05). Conclusions:aHUS is marked by severity, and has diverse clinical manifestations. There are no significant differences in clinical presentation at admission between hereditary and acquired aHUS, highlighting the critical importance of genetic testing and complement-related factor detection in diagnosing aHUS etiology. The family history plays a supportive role in diagnosis of aHUS.
