1.Status quo and influencing factors of postnatal sense of security in patients with hypertensive disorder complicating pregnancy

Objective:To explore the status quo and influencing factors of postnatal sense of security in patients with hypertensive disorder complicating pregnancy.Methods:This study was a cross-sectional study. Using the convenience sampling method, a total of 215 patients with hypertensive disorder complicating pregnancy and their spouses who gave birth in Women's Hospital of Nanjing Medical University from December 2022 to May 2023 were selected as the research objects. The general data questionnaire, Parents' Postnatal Sense of Security (PPSS), Ego-Resiliency Scale (ERS) and Perceived Social Support Scale (PSSS) were used to investigate them. Multiple linear regression analyses were used to explore the factors influencing postpartum security in patients with hypertensive disorders complicating pregnancy.Results:A total of 215 questionnaires were sent out in this study, and 202 were effectively collected, with an effective recovery rate of 94.0%. The total score of PPSS maternal version for 202 patients with hypertensive disorder complicating pregnancy was (59.32±6.81), while the total score of PPSS paternal version for spouses was (41.28±5.24). The results of multiple linear regression analysis showed that the number of antenatal examinations accompanied by family members, the effect of postpartum blood pressure control, the current state of mother and child, postnatal sense of security of spouse, ego-resiliency and perceived social support were the influencing factors of postnatal sense of security in patients with hypertensive disorder complicating pregnancy ( P<0.05) . Conclusions:The postnatal sense of security in patients with hypertensive disorder complicating pregnancy is at a medium high level. Medical personnel should focus on patients with hypertensive disorder complicating pregnancy who are accompanied by their family members less times, poor postpartum blood pressure control and postpartum separation from mother and child, providing them with targeted clinical care to improve their postnatal sense of security.
2.Severity and multiple indexes of jaundice in ABO hemolytic disease of fetus and newborn: a comparative analysis

【Objective】 To explore the correlation between the severity of jaundice in ABO hemolytic disease of fetus and newborn (HDFN) and multiple indicators, in order to achieve accurate diagnosis, timely treatment, and prevent complications in ABO HDFN. 【Methods】 From March 2020 to February 2023, a total of 283 ABO HDFN in our hospital were classified into mild, moderate and severe groups based on total bilirubin levels. The differences in basic data, relevant laboratory indicators and the agglutination intensity of the three hemolysis tests were analyzed and compared. 【Results】 In the severe group, 75% (12/16) were male infants, which was significantly higher compared to 37.5% (57/152) in the moderate group (P<0.05). The values (mg/dL) of transcutaneous jaundice in the mild, moderate and severe groups were:forehead 9.88±1.93 vs 12.34±2.01 vs 15.56±2.69, face 10.25±2.27 vs 13.28±2.32 vs 15.99±2.86, chest 9.67±2.16 vs 12.51±2.11 vs 15.33±2.36, respectively. The values(µmol/L) of direct bilirubin (DBIL) and indirect bilirubin (IBIL) were 9.87±2.06 vs 11.90±2.59 vs 16.12±4.73, and 159.36±37.55 vs 252.98±30.52 vs 353.76±55.68, respectively, and the differences between the mild, moderate and severe groups were statistically significant (all P<0.05) . The creatine kinase (CK) values(unit/L) of mild, moderate and severe groups were 664.15±498.26 vs 500.51 ±451.63 vs 402.63±224.27, and the difference between the mild group and the other two groups was statistically significant (P<0.05) . There was no statistically significant difference in the agglutination intensity of the three hemolysis tests among HDFN with different severity of jaundice. 【Conclusion】 Clinicians can predict the severity of jaundice in light of gender, transcutaneous jaundice, DBIL, IBIL and CK of ABO HDFN, for further and graded treatment, so as to avoid the sequelae or even life-threatening consequences caused by ABO HDFN.
3.Effect of comprehensive intervention on executive dysfunction in children with attention deficit and hyperactivity disorder aged 7-14 years

Objective:
To analyze the improvement of executive function in children with attention deficit hyperactivity disorder (ADHD) aged 7-14 years with comprehensive intervention and drug therapy alone, to provide a basis for intervention research to improve ADHD.
Methods:
A total of 80 children with ADHD treated in the Third People s Hospital of Ganzhou from January 2021 to June 2022 were randomly divided into intervention group and control group. The intervention group received drug and comprehensive intervention therapy, and sensory integration training once a week for 60 to 90 min each for 12 weeks, and conduct related training for caregivers and school teachers, the control group received only drug therapy. The changes of executive function were assessed by the stroop color word association test and the Wisconsin Card Sorting Test (WCST) after 12 weeks of intervention.
Results:
After intervention, the results of the Stroop color word test in the intervention group (3.25±0.98, 4.92±1.40, 10.17±1.28) showed statistically significant differences ( t=12.94, 15.36, 26.34 , P <0.01) compared with those before intervention (6.47±1.92, 8.35±1.25, 16.55±1.57). There were also statistically significant differences ( t=6.76, 15.01, 16.15, P <0.01) in the control group ( 3.95 ±1.01, 5.45±1.15, 12.35±0.86) compared to those before intervention (6.17±1.87, 8.10±1.03, 16.02±1.38). Before intervention, the number of perseverative errors, non perseverative errors, and completed categories by WCST in the intervention group were (47.77±4.50, 35.50±2.37, 3.97±1.07), and in the control group were (46.45±7.34, 34.87±2.29, 3.70±1.11). After intervention, those of the intervention group and control group were (31.42±2.01, 24.75±2.05, 5.05±1.13) and (32.82±2.57, 25.55±1.04, 4.25±1.48), respectively. There were significant differences in the two groups before and after intervention ( t =21.93, 22.27 , -10.37; 10.84, 26.81, -6.90, P <0.01). After intervention, there were significant differences in the number of Stroop color word test errors, perseverative errors and non perseverative errors in WCST between the two groups ( t=-2.94, 2.29, -9.07, -2.35 , -2.06, P <0.05).
Conclusion
Through training for children and the therapy model of comprehensive intervention could significantly improve the executive function of children for a certain extent.
4.Relationship between serum complement C1q/tumor necrosis factor related protein 3 level and carotid atherosclerosis in patients with diabetes mellitus complicated with fatty liver

Objective:To investigate the correlation between serum complement C1q/tumor necrosis factor associated protein 3 (CTRP3) and carotid atherosclerosis in patients with type 2 diabetes mellitus (T2DM) and nonalcoholic fatty liver disease (NAFLD).Methods:From January 2018 to December 2019, 111 T2DM patients hospitalized in the Endocrinology Department of Nantong Third People ′s Hospital Affiliated to Nantong University, and 30 healthy physical examiners in the physical examination center of Nantong Third People 's Hospital Affiliated to Nantong University in the same period were selected. Thirty cases of healthy physical examination were the control group, 111 cases of T2DM were divided into 52 cases of T2DM group and 59 cases of T2DM+NAFLD group according to whether they were combined with NAFLD. The cross-sectional study method was used to collect the relevant clinical data of three groups. The comparison data between multiple groups conformed to the normal distribution and the variance was uniform. One way ANOVA was used. SNK- q test was used for pairwise comparison, χ2 test for qualitative data comparison. The correlation between carotid intima-media thickness (IMT) and influencing factors was analyzed by partial correlation analysis, and the influencing factors of carotid IMT were analyzed by multi factor linear regression. Results:In the control group, T2DM group and T2DM+NAFLD group, body mass index (BMI) (23.65±2.81), (25.52±3.12), (24.90±2.94) kg/m 2,systolic blood pressure (119.43±15.81), (130.63±10.20), (139.37±14.11) mmHg, diastolic blood pressure (72.93±9.74), (73.40±9.44), (77.97±10.00) mmHg, and fasting blood glucose (5.12±0.77), (9.78±1.37), (9.24±1.46) mmol/L,glycosylated hemoglobin (HbA1c) (4.87±1.43)%, (7.99±1.10)%, (8.56±1.29)%,homeostasis model assessment of insulin resistance (HOMA-IR)(1.56±0.37),(2.80±1.00), (3.47±0.94), high density lipoprotein cholesterol (HDL-C) (1.52±0.34),(1.23±0.31), (1.22±0.31) mmol/L,low density lipoprotein cholesterol (LDL-C) (2.41±0.53), (2.73±0.61), (2.93±0.59) mmol/L, CTRP3 (292.93±68.54), (241.69±61.01), (150.80±56.67) μg/L, the difference between groups were statistically significant ( F=3.712,23.023,4.074,134.285,90.818,47.105,10.139,7.941,60.035,all P<0.05). Pairwise comparison shows that the systolic blood pressure, diastolic blood pressure, HbA1c and HOMA-IR in T2DM+NAFLD group were higher than those in control group and T2DM group,and CTRP3 was lower than those in control group and T2DM group, the difference was statistically significant (all P<0.05). BMI, fasting blood glucose, HbA1c, HOMA-IR, HDL-C and LDL-C in T2DM group were higher than those in the control group, CTRP3 was lower than that in the control group (all P<0.05). In the control group, T2DM group and T2DM+NAFLD group, IMT were (0.75±0.13), (1.11±0.17) and (1.25±0.15) cm; Crouse scores were (1.28±0.97), (3.22±1.42) and (4.54±1.22); the plaque detection rates 16.7%(5/30), 65.4%(34/52) and 78.0%(46/59), and there were significant differences between the two groups ( F=105.941,67.063, χ2=32.108, all P<0.001). There were significant differences between the two groups (all P<0.05). T2DM+NAFLD group was the highest, followed by T2DM group, and the control group was the lowest. Partial correlation analysis showed that carotid IMT was positively correlated with systolic blood pressure, fasting blood glucose, HbA1c, HOMA-IR, triglyceride and LDL-C ( r=0.356, 0.572, 0.575, 0.620, 0.172, 0.291, all P<0.05), and negatively correlated with HDL-C and CTRP3 ( r=-0.335, -0.675, all P<0.001). Multivariate linear regression analysis showed that HbA1c, HDL-C and ctrp3 were the influencing factors of carotid atherosclerosis ( t=2.621, -3.764, -7.280, all P<0.05) Conclusion:Serum CTRP3 is associated with carotid atherosclerosis in T2DM patients with NAFLD,and may have a protective effect on vascular lesions in T2DM patients with NAFLD.
5.Comparison of different approaches to fascia iliaca compartment block for postoperative analgesia in elderly patients undergoing total hip arthroplasty

Objective To compare the efficacy of ultrasound-guided fascia iliaca compartment block(FICB)above inguinal ligament and longitudinal inguinal FICB for postoperative analgesia in elderly patients undergoing total hip arthroplasty(THA).Methods Sixty American Society of Anesthesiologists physical statusⅠ orⅡ patients of both sexes,aged 65-76 yr,with body mass index of 19-26 kg/m2,scheduled for elective unilateral THA,were divided into 2 groups(n=30 each)using a random number ta-ble method: FICB above inguinal ligament group(S group)or longitudinal inguinal FICB group(G group).After the end of surgery,patients received ultrasound-guided FICB using a short-axis in-plane technique in S group and longitudinal inguinal FICB using a long-axis in-plane technique in G group,and patients received 0.4%ropivacaine as a loading dose of 40 ml,followed by continuous infusion of 0.2%ropivacaine 5 ml/h for 48 h.Ultrasound imaging time,puncture injection time and operating time of FICB were recorded.The efficacy of nerve block,effective pressing times,cumulative consumption of ropiva-caine,satisfaction with analgesia,and development of related complications were recorded at 6,12,24,36,48 and 72 h after surgery(T1-6).Results Compared with group G,the requirement for rescue anal-gesia with dezocine was significantly decreased,the effective pressing times at T3-5 and cumulative consump-tion of ropivacaine at T3,4 were reduced,and the success rate of obturator nerve block was increased at T1-6 in group S(P<0.05).There were no significant differences in the success rate of the femoral nerve and lat-eral femoral cutaneous nerve block,satisfaction with analgesia and development of related complications be-tween the two groups(P>0.05).Conclusion Ultrasound-guided FICB above inguinal ligament increases the success rate of the obturator nerve block,provides better efficacy for postoperative analgesia and is more helpful for early postoperative functional exercise and recovery than the longitudinal inguinal FICB in the pa-tients undergoing THA.

OBJECTIVE To exlpore the prevention and therapy methods of nasal complications after transsphenoidal pituitary adenomasectomy.METHODS 129 cases of pituitary adenomas underwent transsphenoidal surgery.The postoperative nasal complications were observed and disposed.RESULTS A total of 26 cases(20.1%) of nasal complications appeared in all 129 cases of pituitary adenomas underwent transsphenoidal surgery,including nasal hemorrhage 6 cases(4.8%),eerebrospinal fluid rhinorrhea 9 cases(6.9%),sphenoid sinusitis 3 cases(2.3%),atrophy rhinitis 2 cases(1.6%),olfactory dysfunction 2 cases(1.6%),nasal septum perforation 1 case(0.8%),and nasal adhesion 3 cases(2.3%).They were all cured after nasal endoscopic regular cavity clean,nasal hormone,nasal irrigation.CONCLUSION The nasal complications after transsphenoidal pituitary adenomas ectomy surgery should be timely disposed,which can effectively improve the patients' clinical symptoms.
8.Drinking water arsenic species in endemic areas of endemic arsenicosis in Jilin Province

Objective To identify the morphology of arsenic in drinking water,by detecting arsenic in the drinking water within the endemic areas so as to determine the causes of arsenic through different forms of arsenic exposure.Methods The arsenic poisoning area and the high arsenic arsenic area in Taonan City and Tongyu County of Jilin Province were selected as the survey sites.The drinking water samples were collected,and the arsenic content and different arsenic species in the water samples were measured and analyze the relationship between well depth and water arsenic content.Resets A total of 161 arsenic water samples were tested,mainly in the form of inorganic arsenic;As5+ concentration was 0.004 to 0.226 mg/L,the median was 0.053 mg/L;the As3+ concentration was 0.004 to 0.309 mg/L,the median was 0.057 mg/L.Total arsenic content was in the range of 0.009 to 0.509 mg/L,the median was 0.100 mg/L.Monomethylated arsenic (MMA) was detected in 1 water sample with the content of 0.005 mg/L,dimethyl arsine (DMA) was detected in 1 water sample with the content of 0.014 mg/L.Totally 101 wells were surveyed with the depth of 13 to 75 meters.Totally 94 water samples had the water arsenic level of more than 0.05 mg/L,and most of them were detected in the well with the depth of more than 50 meters,which was accounted for 85.1% (80/94).Conclusions Arsenic mainly exists in the form of inorganic arsenic in drinking water,organic arsenic is only found in water at low concentrations.Excessive water arsenic is mainly distributed in wells deeper.
9.Differential diagnosis in bloodstream infection with Trichosporon asahii and Geotrichum capitatum

Objective To provide reference for establishing diagnosis and differential diagnosis methods of rare yeast-like fungal bloodstream infection for clinical microbiology laboratory.Methods Trichosporon asahii (T.asa-hii)and Geotrichum capitatum (G.capitatum)bloodstream infection was diagnosed and differentially diagnosed through clinical data analysis,morphological examination,biochemical reactions,and molecular biology technology. Results Two types of yeast-like fungal bloodstream infections in case 1 and case 2 both occurred in leukemia agranulocytosis phase after chemotherapy,such infections were serious and highly similar.The cultivated colonies on blood agar plates of case 1 and case 2 were performed gram stain and microscopic examination.Hyphae,arthro-spores and microconidia were visible in the former,thickness of hyphae branches and length of arthrospores were different,most presented rectangular and barrel shape;the latter can be seen hyphae with transparent septum bro-ken up into arthrospores,presented rectangular shape,did not produce blastoconidia.Identification with API 20C AUX showed that they were T.asahii and G.capitatum.The PCR product sequences were compared with NCBI, suggesting that T.asahii and G.capitatum were at sexual stage.Conclusion Comprehensive application of a varie-ty of technical methods is helpful for improving the diagnosis accuracy of bloodstream infection with yeast-like fungi, identifying Trichosporon and Geotrichum to the species level may help physicians to understand such rare fungal in-fection,choose antifungal agents rationally,and improve clinical prognosis.
10.Modified nasal septum root mucoperiosteum incision of pituitary adenomas via endonasal transsphenoidal approach

Objective To explore the modified nasal septum root mucoperiosteum incision of pituitary adenomas via endonasal transsphenoidal approach,and reduce the incision-related postoperative complications as rhinorrhagia,septumperforation and dysosmia.Methods Sixteen patients with pituitary adenomas,admitted to and underwent modified nasal septum root mucoperiosteum incision of pituitary adenomas via endonasal transsphenoidal approach in our hospital from January 2013 and June 2013,were chosen in our study.The postoperative nasal complications were observed.The clinical data of them were analyzed retrospectively.Results Of all the 16 patients,total or subtotal removal rate was 93.8%.Rhinorrhagia or septumperforation did not appear.Dysosmia was not appeared in 15 patients with normal olfactory function;one patient had preoperative olfactory dysfunction,without getting worsen three months after surgery.Conclusion The reformative nasoseptal root mucoperiosteum incision of transsphenoidal surgery for pituitary adenomas can maintain the integrality of sphenopalatine artery and nasoseptal olfactory mucosa,and decrease the incidence of postoperative nasal complications,such as rhinorrhagia,septumperforation and dysosmia.
