OBJECTIVE To study the improvement effect and mechanism of Gubenanshen decoction on menopausal syndrome model rats . METHODS Totally 70 rats were anesthetized and placed on the operating table for bilateral ovariectomy to induce menopausal syndrome model . Other 10 rats without modeling were taken as sham operation group . Sixty model rats were divided into model group ,Gengniananpian group (TCM positive control group ,0.56 g/kg),Estradiol valerate tablet group (chemical drug positive control group ,0.1 mg/kg),Gubenanshen decoction high -dose,medium-dose and low -dose groups (18.0,9.0,4.5 g/kg), with 10 rats in each group . Model group and sham operation group were given normal saline intragastrically ;administration groups were given relevant medicine intragastrically ,once a day ,for consecutive 4 weeks. After last administration ,the plasma contents of luteinizing hormone (LH),follicle stimulating growth hormone (FSH)and estradiol (E2)were determined .The indexes of uterus, adrenal gland ,spleen and thymus were measured ;pathological changes of rat uterus were observed ;mRNA expressions of kisspenptin 1(Kiss-1),inositol triphosphate receptor 1(IP3R1)and gonadotropin -releasing hormone (GnRH)in hypothalamic tissues were detected ,and the protein expressions of Kiss -1,ERK1/2 and p -ERK1/2 were also detected . RESULTS Compared with sham operation group ,the contents of LH and FSH in the plasma of rats were significantly increased in model group (P<0.05 or P<0.01). The contents of E 2,organ index ,mRNA expressions of Kiss -1,IP3R1 and GnRH and protein expressions of Kiss -1 and p-ERK1/2 were decreased significantly in hypothalamic tissues (P<0.05 or P<0.01). The glands of the uterus were reduced and the cells in the lamina propria were dense . Compared with model group ,above indexes of Gubenanshen decoction groups (except for the content of E 2 in the low -dose group )were reversed significantly (P<0.05 or P<0.01). The number of uterine glands increased, and single columnar epithelial cells were arranged orderly (except for the low -dose group ). CONCLUSIONS Gubenanshen decoction can increase mRNA expressions of Kiss -1,IP3R and GnRH and protein expressions of Kiss -1 and p -ERK1/2 in hypothalamic tissues of rats ,and activate GnRH secretion signaling pathways so as to improve the menopausal syndrome .

Objective To investigate the clinical manifestations and imaging features of dural arterio-venous fistula of super petrosal venous drainage and treatment. Methods From May 2013 to September 2014,9 patients with petrosal vein drained dural arteriovenous fistula at the Department of Neurosurgery, Xuanwu Hospital,Capital Medical University and the Department of Neurosurgery,Beijing Haidian Hospital were enrolled retrospectively. The patients were treated with endovascular embolization or microsurgery, and the MRI and DSA examinations were improved, and the scores of the modified Aminoff&Logue scale (ALS) were performed before and after treatment. Results In the 9 patients, there were 3 females and 6 males. They all had different degrees of limb sensory and motor abnormalities,7 of them also had urination and/or bowel disorders,4 had cranial nerve dysfunction, including hoarseness, bucking, hiccup, and paralysis. Six patients received embolization treatment,3 received microsurgery, and they all achieved anatomic cure. The preoperative ALS score was 6. 0 ± 2. 7,and the score at 3 months after procedure was 2. 8 ± 1. 7. There was significant difference between before and after treatment (t=4. 816,P<0. 05). Conclusions The petrosal vein drained dural arteriovenous fistula is a kind of rare cerebrovascular malformation. The lesion involves a wide range. The clinical manifestations are severe. Both endovascular embolization and microsurgery can achieve a more ideal therapeutic effect. If the vascular condition is permitted, the interventional embolization treatment should be preferred.

BACKGROUND:Bone cement has certain toxic effects on the human body. The probability of renovation is high after bone cement total hip arthroplasty. It is reported that the long-term effect of cementless total hip arthroplasty is apparently better than bone cement total hip arthroplasty, and can be renovated conveniently. OBJECTIVE:To investigate the clinical effect of cementless total hip arthroplasty on hip joint disease, and to compare the difference between unilateral replacement and bilateral replacement. METHODS: Clinical and folow-up data of 233 patients (280 hips), who were treated with cementless total hip arthroplasty in the Department of Orthopedics, Wuzhou Worker’s Hospital, Seventh Affiliated Hospital of Guangxi Medical University from July 2007 to December 2013, were retrospectively analyzed. According to the replacement program, they were divided into unilateral replacement group (n=186) and bilateral replacement group (n=47). Harris score of hip joint, visual analog scale score of thigh pain, the excelent and good rate of hip joint during final folow-up and complications were compared between the two groups before replacement, at 6, 12 and 24 months after replacement. RESULTS AND CONCLUSION:No significant difference in Harris scores was detected before replacement, at 6, 12 and 24 months after replacement (P > 0.05). Harris score was significantly higher at 6, 12 and 24 months after replacement compared with that before replacement in both groups (P < 0.05). No significant difference in the excelent and good rate was detected in the unilateral replacement group (87%) and the bilateral replacement group (86%) (P > 0.05). No significant difference in the visual analog scale score was seen before replacement, at 6, 12 and 24 months after replacement (P > 0.05). Visual analog scale scores were significantly lower at 6, 12 and 24 months after replacement than that before replacement in the two groups (P < 0.05). There was no significant difference in the incidence of complications after replacement in patients of both groups (P > 0.05). These findings confirm that the effects of cementless total hip arthroplasty for hip joint disease are evident, can effectively restore hip joint function. No significant difference was detected between unilateral replacement and bilateral replacement. Strict replacement operation and matching of prosthesis and medulary cavity can effectively reduce thigh pain after replacement.

Objective To study the risk factors of Pseudomonas aeruginosa (PA)infection and homology of PA isolated from nasopharyngeal carcinoma patients undergoing radiotherapy, and prevent and control the spread of in-fection.Methods Bacteria isolated from clinical specimens were identified by BD Phoenix automated microbiology system. Gene homology were analyzed with randomly amplified polymorphic DNA (RAPD)technique. Results Forty-nine strains of PA were mainly isolated from 43 nasopharyngeal carcinoma patients, the major specimens were nasopharyngeal swab (46.94% ),sputum(32.65% ),and oral secretion (10.20% ).All these strains were amplified 46 electrophore-sis diagrams, and 19 genotypes were identified. The highly homologous genotypes of type H and J strains were mainly isolated from patients in the second section (57.14% ,4/7)and fourth section (60.00% ,3/5)of radiotherapy department respectively. Conclusion Localized epidemic of highly homologous PA exists in different sections of ward, transfer of patients between different sections is the risk factor for homology PA infection/colonization. Gen-otyping technique such as RAPD for analyzing the homology of pathogenic bacteria in healthcare-associated infection has important value in preventing and control Ling infection spread.

Objective To clarify the mechanism of immediate early response gene 5 (ler5)transcription induced by radiation. Methods Deletant construction, site-specific mutagenesis, electrophoretic mobility shift assay (EMSA) and chromatin immunoprecipitation (ChIP) were used to forecast the promoter region, binding sites and transcription factors of Ier5 gene in HeLa cells. Results The promoter region of Ier5 gene might be in the region of Ier5 -8 deletant ( -408 - -238 bp).The Ier5 gene had two transcription factors of GCF and NFI,and GCF had two binding sites located in the region of - 388 - - 382 bp and - 274 - - 270 bp of Ier5 promoter. The binding site of NFI was located in - 362 --357 bp of Ier5 promoter. GCF could inhibit the expression of Ier5 gene and this inhibition was diminished when the radiation dose increased. In contrast, NFI increased the expression of Ier5.Conclusions The most possible region of Ier5 promoter is from -408 to - 238 bp which has two binding sites for the radiosensitivity transcription factors of GCF and NFI that could negatively and positively regulate the expression of Ier5 respectively.
