1.Convenience of “Loco-check” Combination in Quick Screening of Latent Preliminary Group of Locomotive Syndrome by Measurement of Handgrip Strength: Secondary Analysis of the Previous Report

As a screening tool for detecting latent pre-locomotive syndrome (latent pre-LS) in women over the age of 40, measuring handgrip strength with a cut-off value of 26 kg was proposed in a previous report. However, this screening method missed 22% of latent pre-LS. It would be beneficial to screen almost persons with latent pre-LS in community pharmacies. In this study, it was investigated whether screening using the combination of measuring handgrip strength and the questionnaire, “Loco-check,” which was proposed by the Japanese Orthopaedic Association, improved the detection of latent pre-LS in the same group mentioned above. Combining only one of the “Loco-check” questions, “I cannot put on a pair of socks while standing on one leg,” with the measurement of handgrip strength with the cut-off value of 26 kg, the detection of latent pre-LS was increased to 90.2%. The odds ratio was 9.72 in logistic regression analysis. Using the combination of the measurement of handgrip strength and the response to one question is both rapid and convenient. Therefore, in this study, this screening combination is proposed to be a useful tool in community pharmacies for detecting early latent pre-LS.
2.Associations between self-rated health and daily physical activity, sports participation, sedentary behaviour and sleep among the general population of Japanese junior high school and high school students

The purpose of this cross-sectional study was to examine associations between self-rated health (SRH) and frequency of overall daily moderate-to-vigorous physical activity (MVPA), compliance with sedentary behaviour (SB), and sleep, sports participation, and active transportation to school among junior high school and high school students, using data of a random sampling survey in Japan. Participants were Japanese junior high school and high school students (437 boys and 398 girls). We analyzed data from the 2019 SSF National Sports-Life Survey of Children and Young People, which used the frequency of MVPA, screen time, sleep duration, organized sports participation and active transportation to school. The highest MVPA frequency and participation of sports were associated with better SRH adjusted age and annual family income (odds ratio (OR) 3.01; 95% CI, 1.30–6.93; OR=5.80; 3.08-10.93 for boys; OR=4.93; 2.22-10.96; OR=2.75; 1.59-4.75 for girls). The association with highest MVPA frequency was slightly increased after adjusting for meeting the SB for girls (OR=5.26; 2.23-12.43). Meeting SB was associated with better SRH after adjusting for meeting the frequency of MVPA among girls (OR=2.91; 1.49–5.69). These findings show that frequency of MVPA is important to better SRH in junior high school and high school students, independent of SB. In addition, SB is also important for girls, independent of MVPA. Thus, gender should be taken into account in order to better SRH in Japanese junior high school and high school students, as the factors associated with subjective health differ between boys and girls.
3.Correlates associated with exercise participation in mothers with primary school children

The aim of the present study was to examine the factors associated with exercise participation in mothers with primary school children. A cross-sectional study of 315 mothers of primary school children was completed. Participants were recruited from fourteen elementary schools in which their children were enrolled. Exercise participation and associated factors including demographic, psychological, behavioral, and environment factors were assessed by questionnaire. After controlling for age, body mass index and sociodemographic variables, results of logistic regression indicated that holding a full-time job (OR=0.15 (0.05-0.44), p < 0.01) and high frequency of cleaning (OR=0.30 (0.14-0.65), p < 0.01) were significantly negatively correlated with exercise participation. Moreover, high self-efficacy of exercise (OR=3.11 (1.66-5.84), p < 0.01) and car driving (OR=2.68 (1.57-4.56), p < 0.01) were significantly positively associated with exercise participation. The findings of this study suggest that employment status, psychological and behavioral factors were related with exercise participation in mothers with primary school children.
4.Relationship between adherence to WHO “24-Hour Movement Guidelines for the Early Years” and motor skills or cognitive function in preschool children: SUNRISE pilot study

This study examined the relationship between meeting the World Health Organization’s (WHO) 24-Hour Movement Guidelines for the Early Years and motor skills and cognitive function in preschool children. Participants were 4-year-old boys and girls in urban and rural areas (n=69). Physical activity was measured using a triaxial accelerometer (ActiGraph GT3X). Screen time and sleep duration were assessed via self-report by guardians. Meeting the 24-h movement guidelines was defined as: 10 to 13 h/night and nap of sleep, ≤1 h/day of sedentary screen time, and at least 180 min/day more than 1.5 METs. Motor skills were evaluated by the Ages & Stages Questionnaires, Third Edition (ASQ-3). Executive functions (shifting, visual-spatial working memory and inhibition) were evaluated by the Early Years Toolbox (Japanese translation). The prevalence of children meeting all three recommendations was 7.2% and 7.2% met none of the three recommendations. Children meeting physical activity recommendation had a better inhibition score compared to children meeting none of the recommendation (p=0.005). While, children not meeting the sleep recommendation had a better inhibition score compared to children meeting of the recommendation (p=0.042). In conclusion, meeting the physical activity or sleep recommendations were positively or negatively associated with the inhibition score. On the other hand, meeting none of the sedentary behaviour and the 3 recommendations was not associated with motor skills or cognitive function.
5.Physical activity in young children during outdoor and indoor free playing time and in physical education lessons in preschools

The primary aim of this study was to examine moderate-to-vigorous physical activity (MVPA) levels in preschool children during indoor and outdoor free playing time and during preschool physical education (PE) classes. The secondary aim was to determine which types of PE lessons are the best for increasing the percentage of MVPA. Physical activity during free playing time and in lessons was objectively evaluated with a triaxial accelerometer in preschool boys and girls (n = 281). Minutes of physical activity were categorized into physical activity ratios (PAR). MVPA was defined as PAR ≥3.0. Time tables and PE lesson types were recorded using logs kept by class teachers. The percentage of MVPA in outdoor free playing time (38.9±16.3%) was significantly higher than that of indoor free playing time (12.6±10.1%, p<0.001). On the other hand, there was no significant difference between the percentage in MVPA in outdoor free playing time and the percentage in PE lessons (n=94: 38.0±19.4%vs. 34.9±15.8%, ns). After adjustment for gender, age, body height and weight and preschool, there were not significantly different the percentage of MVPA among contents of PE lessons. In conclusion, children engaged in almost 30~40% MVPA during outdoor free playing time or PE. The children also engaged MVPA during indoor free playing time.
6.Establishment of evaluation methods for international comparison of daily physical activity and its factors in Japanese children

Past reports have reinforced global public health concerns related to physical inactivity. Recent global efforts to increase physical activity and decrease sedentary behaviors have been underway. The development and release of Report Cards on physical activity for children and youth have in many countries improved advocacy and social mobilization with the intent of increasing young people’s activity by influencing perceptions, priorities, policies, and practices. In 2016, 38 countries including Japan produced a cross-country comparison finding that showed higher physical activity and lower sedentary behavior in countries that reported poorer infrastructure, and lower physical activity and higher sedentary behavior in countries that reported better infrastructure. This suggests that autonomy to play, travel, chore requirements and/or fewer attractive sedentary pursuits, rather than infrastructure and structured activities, may facilitate higher levels of physical activity. Future nationally representative surveys on overall physical activity, active play, and community and the built environment are needed. The establishment of evaluation methods for the international comparison of daily physical activity and its factors in Japanese children and youth are needed.
7.Increased Cooperation with Dentistry by the Palliative Care Team

Recently, the palliative care team (PCT) at our hospital has included dentists. Among a total of 127 cancer patientsand required PCT intervention from 2009 to 2014, 17 patients (13.3%) had oral symptoms. Therefore, the PCT held discussions in order to determine the optimal way to treat each patient. Various symptoms, including oral pain, dry mouth, taste disturbance, furred tongue, excessive amounts of saliva, appetite loss, and trismus were treated by the dentists. As a result, the oral findings improved in all patients, while the oral symptoms improved in 16 of the 17 patients (94%). Thanks to the fact that dentists have joined the PCT, oral symptoms are effectively relieved, and PCT members now have an increased interest in oral cavity complications. Furthermore, conducting thorough examinations of the oral cavity by the PCT not only results in an improved QOL, but it has also increased the interest in the oral cavity on the part of the PCT. Therefore, more effective palliative care is expected to be achieved by promoting increased cooperation with more clinical departments.
8.Differing effects of aquatic movement in either deep or shallow water on the physiological responses among middle-aged, female and recreational synchronized swimmers

This study examined the physiological response to different water depths in recreational synchronized swimming. Nine middle-aged, female, recreational swimmers carried out the same team free routines in deep water (deep-water synchronized swimming: DWS) and shallow water (shallow-water synchronized swimming: SWS). Heart rate (HR) was measured continuously during each performance, combined with estimation of metabolic equivalent (MET) values using individual linear regression equations of HR-oxygen uptake. These equations were created using the results of 12 water activities. Blood lactate concentration and systolic blood pressure were measured at the 1/3 and 2/3 stages and immediately after each performance period. HR and estimated METs during DWS (mean±SD, 152.9±7.5 beats·min-1 and 7.3±1.2) were significantly higher than those measured during SWS (131.8±11.3 beats·min-1 and 5.7±0.8). Blood lactate concentration and systolic blood pressure measured immediately after DWS were significantly higher than those measured after SWS. With DWS, blood lactate concentration at the 2/3 stage and immediately after the performance were significantly higher than those measured at the 1/3 stage, whereas in SWS no significant difference was found in these values at any time period. In conclusion, the exercise intensity of DWS was high and SWS moderate. The predominant sources of energy may be phosphocreatine stores and aerobic metabolism during these performances. It is possible that glycolysis may also play an important role in energy requirements during the 2/3 stage and immediately after a DWS performance.
9.Relationship between outdoor playing time and moderate-to-vigorous physical activity for Japanese young children and correlates of physical activity

This study aimed to examine the relationship between daily outdoor playing time assessed using questionnaires for parents and daily moderate-to-vigorous physical activity (MVPA) using triaxial accelerometers for young children (213 boys: 5.8±0.6years and 193 girls: 5.9±0.6years). The study also examined the potential relationships between daily MVPA and exercise habits or familial and preschool factors which were assessed using questionnaires for parents. Daily MVPA was assessed using a triaxial accelerometer (ActivTracer, GMS) for 6 consecutive days, including weekdays and weekends. Outdoor playing time and percentage of outdoor playing time more than 60 min/day were 81±62 min/day and 64% for boys and 78±61 min/day and 62% for girls, respectively. Outdoor playing time was not significantly correlated with MVPA. The study found that no factors for MVPA, including exercise habits, were correlated with MVPA when adjusted for age, body height and log-body weight, except that MVPA for girls in kindergartens was higher than that in nursery schools. These findings suggest that the percentage of outdoor playing time more than 60 min/day was about 2/3 in young children. The questionnaire didn't reflect their MVPA. The factors used in the present study except for the preschool type for girls may not contribute to the time spent in MVPA in preschool children.
10.Relationship of exercise at preschool and out of school and daily physical activity to physical fitness in preschool children in the Kanto region: a cross-sectional study

This study sought to examine the potential relationships of physical education (PE) participation at preschool and sports club (SC) participation out of school and daily physical activity (PA) measured objectively using a triaxial accelerometer, with physical fitness in preschool children. Physical fitness testing was used to measure both health-related and skill-related parameters of fitness in 191 Japanese preschoolers in the Kanto region (94 boys and 97 girls, 5.6 ± 0.6 years). Daily PA was assessed using a triaxial accelerometer (ActivTracer, GMS) for 6 consecutive days, including weekdays and weekend days. PE and SC participation was assessed using questionnaires for preschool teachers or parents of subjects, respectively. All health-related and skill-related physical fitness and total Z-score were correlated with time spent with physical activity ratio (PAR)≧4 when adjusted for age, sex, body height and weight. In addition, greater grip strength was associated with PE participation at preschool, and 25 m run speed was associated with SC participation, also. However, 25 m run speed was negatively associated with PE participation. These findings suggest that daily PA may contribute to the development of both health-related and skill-related fitness in preschool children, although further research on the cause-effect relationship is needed. Moreover, participation in a SC may contribute to the development of running speed.
