As a threat to human health, steroid-induced osteonecrosis of femur head is a common refractory orthopedic disease mainly caused by glucocorticoids, with poor prognosis and unclear pathogenesis. Osteogenesis-associated signaling pathways play an important role in bone formation. Glucocorticoid-induced abnormal activation and transport of these signaling pathways lead to abnormal differentiation of bone marrow mesenchymal stem cells, dysfunction of bone metabolism, and osteogenesis disorders, which may be the main reasons for the occurrence and development of steroid-induced osteonecrosis of femur head. Bone formation and remodeling need the participation of bone marrow mesenchymal stem cells, which are stem cells characterized by continuous self-renewal and differentiation. The key to strengthening bone remodeling is to improve the osteogenic differentiation capacity, which is the key point to inhibit bone resorption and prevent bone marrow mesenchymal stem cells from differentiating into osteoclasts. Traditional Chinese medicine (TCM) has been used in the treatment of osteonecrosis in ancient times. It is recorded in the Treasury of Words on Materia Medica (《本草汇编》) that "The deficiency in the lower energizer cannot be tonified without Eucommiae Cortexz. The soreness in lower legs cannot be alleviated without Eucommiae Cortex...The pain in the waist and knee cannot be relieved without Eucommiae Cortex...Tonifying liver and invigorating kidney, Eucommiae Cortex is an essential medicine." This indicates that ancient physicians have already begun to use the liver-tonifying, kidney-invigorating, and sinew-bone-strengthening effects of Eucommiae Cortex for the treatment of osteonecrosis. As the national support for the development of TCM strengthens, increasing studies have been conducted on the TCM prevention and treatment of steroid-induced osteonecrosis of femur head. Studies have suggested that Chinese medicinal herbs can exert a positive effect on the differentiation of bone marrow mesenchymal stem cells by affecting targeted signaling molecules, and promote osteogenesis and bone defect repair, thus combating the occurrence and development of steroid-induced osteonecrosis of femur head. The regulation of osteogenic signaling pathway by Chinese medicines to prevent steroid-induced osteonecrosis of femoral head has become a hot research topic. This article reviews the studies about the prevention and treatment of steroid-induced osteonecrosis of femur head with the active components in Chinese medicinal herbs by regulating osteogenic signaling pathways. We then explore the mechanism of the active components in promoting the differentiation of bone marrow mesenchymal stem cells into osteoblasts and inhibiting their differentiation into osteoclasts to facilitate bone formation, aiming to provide a reference for the further study of treating steroid-induced osteonecrosis of femoral head with Chinese medicinal herbs.

Objective:To explore the relationship between the levels of serum complement C3 and C4 and the degree of renal pathological injury in patients with IgA nephropathy (IgAN).Methods:It was a retrospective study. The clinical and pathological data of patients with primary IgAN diagnosed by renal biopsy in the Department of Nephrology of the Second People's Hospital of Qujing City, Yunnan Province from December 1, 2019 to December 31, 2022 were collected. According to the IgAN Oxford classification criteria, the patients were divided into mild renal pathological injury group (mild group, <3 pathologic types) and severe renal pathological injury group (severe group, ≥3 pathological types). The levels of serum C3 and C4 and other clinical data were compared between the two groups. Spearman correlation method was used to analyze the correlation between serum C3, C4 levels and estimated glomerular filtration rate (eGFR) during renal biopsy.Multivariate logistic regression model was used to analyze the influencing factors of the pathological injury degree in IgAN patients and the forest map depicted the effect of risk factors. Results:A total of 164 IgAN patients were included in the study, including 77 males (47.0%), aged (35.5±12.9) years old. There were 60 patients in the mild group and 104 patients in the severe group. Compared with the mild group, the patients in the severe group were older, had higher levels of serum C4, serum uric acid, low density lipoprotein cholesterol and 24 h urinary protein, higher proportions of hypertension, glucocorticoids/immunosuppressant therapy, C3 deposition in renal tissues and microscopic hematuria, and had lower hemoglobin and serum C3 level (all P<0.05). The results of Spearman correlation analysis showed that the level of serum C3 was positively correlated with eGFR ( r=0.303, P<0.001), and the level of serum C4 was negatively correlated with eGFR ( r=-0.238, P=0.002). Multivariate logistic regression analysis results showed that serum C3 (every 0.01 g/L increase, OR=0.976, 95% CI 0.957-0.996, P=0.018), serum C4 (every 0.01 g/L increase, OR=1.091, 95% CI 1.020-1.166, P=0.011), hemoglobin ( OR=0.969, 95% CI 0.950-0.988, P=0.002), and serum uric acid ( OR=1.005, 95% CI 1.001-1.009, P=0.012) were independent related factors of renal pathological damage (severe injury /mild injury) in IgAN patients. Conclusions:Serum C3 and C4 are independent related factors of the severity of renal pathological injury in IgAN patients.
4.Relationship between poliovirus receptor and prognosis of gastric cancer patients
International Journal of Biomedical Engineering 2021;44(6):460-464,469
Objective:To explore the prognostic value of poliovirus receptor(PVR) in patients with gastric cancer. Methods:Bioinformatics methods were used to analyze the expression of PVR mRNA in gastric cancer tissues and adjacent normal tissues and its relationship with prognosis. The clinicopathological data of 73 gastric cancer patients who underwent surgical treatment were retrospectively analyzed. Immunohistochemical method was used to detect the expression level of PVR in gastric cancer tissue and adjacent normal tissue, and its relationship with the clinicopathological characteristics of gastric cancer patients was analyzed. Results:The expression level of PVR mRNA in gastric cancer tissues was higher than that in adjacent normal tissues ( P<0.05). The expression of PVR is related to the clinical prognosis of gastric cancer patients, and the higher the expression of PVR, the lower the overall survival rate ( P=0.011) and the disease-free survival rate ( P=0.032) of gastric cancer patients. The results of immunohistochemical staining showed that PVR was highly expressed in gastric cancer tissue, mainly in the cytoplasm, while it was not expressed or lowly expressed in normal gastric tissue. High expression of PVR was associated with tumor size ( P=0.001) and recurrence 3-years after surgery ( P=0.013). Conclusions:PVR is overexpressed in gastric cancer tissue, and gastric cancer patients with high expression of PVR have a worse prognosis. The level of PVR has great value for the prognosis evaluation of gastric cancer patients.
5.Expression and clinical significance of myocyte enhancer factor 2C in patients with gastric cancer
International Journal of Biomedical Engineering 2020;43(5):349-353
Objective:To investigate the expression of myocyte enhancement factor 2C (MEF2C) protein in gastric cancer, and to study its relationship with pathology and prognosis. Methods:Using bioinformatics methods, the expression of MEF2C in gastric cancer patients and its relationship with overall survival were analyzed in the TCGA database. Sixty-six patients with gastric cancer were selected as the research object. The expression of MEF2C in gastric cancer was analyzed by immunohistochemical staining of postoperative gastric cancer tissue samples. By analyzing the expression of MEF2C in gastric cancer, combined with pathological information and postoperative follow-up information, the effect of MEF2C on the progression of gastric cancer was studied. Results:The high expression of MEF2C in gastric cancer tissue was related to tumor size and tumor recurrence (all P<0.05), but not related to tumor grade and lymph node metastasis (all P>0.05). Conclusions:The high expression of MEF2C protein can be used as a predictor for poor prognosis of gastric cancer.

There were 17 males and 9 females, aged 54.15±8.60 (range 41-68) years. The index level included C 2, 3 1 case, C 3, 4 3 cases, C 4,5 9 cases, C 5, 6 6 cases, C 6, 7 7 cases. Twelve cases underwent anterior cervical decompression with Zero-p intervertebral fusion and fixation (Zero-p group), while 14 cases underwent anterior cervical decompression with titanium plate combined with cage fusion and fixation (titanium plate group). The following parameters, including operative duration, intraoperative blood loss, Japanese Orthopaedic Association (JOA) score, visual analogue score (VAS), neck disability index (NDI), dysphagia Bazaz grade, bone graft fusion Eck grade, C 2-C 7 Cobb angle, and related complications, were compared between the two groups. Results:The operation was performed successfully in all the patients. The patients were followed up for averagely 33.38±21.26 (range 12-71) months. The operation duration was 95.83±5.47 (range 89-105) min in the Zero-p group, which was shorter than 121.28±8.24 (range 106-131) min in the titanium plate group. The Bazaz classification of dysphagia in the Zero-p group was superior to the titanium plate group at 1 month after operation ( W=126.00, P=0.022). Neither group had dysphagia 3 months after surgery. The JOA score increased from preoperative 9.50±1.31 to 14.33±0.78, and VAS decreased from 5.33±1.67 to 0.83±0.72 in the Zero-p group. The NDI decreased from 43.62%±9.31% to 14.99%±3.26%, and C 2-C 7 Cobb changed from 8.26°±2.92° to 14.80°±4.18° in the Zero-p group. The JOA score increased from preoperative 9.14±1.79 to 14.71±0.73, and VAS decreased from 5.43±1.55 to 1.43±0.76 in the titanium plate group. The NDI decreased from 43.76%±8.47% to 14.22%±4.59%, and C 2-C 7 Cobb changed from 5.53°±9.04° to 14.68°±6.89° in the titanium plate group. No complication, such as screw loosening or breakage or esophageal injury, occurred during the follow-up. Conclusion:Both methods can achieve good clinical effects in treating symptomatic ASD and can restore the physiological curvature of the cervical spine to a certain extent. Furthermore, the advantages of Zero-p intervertebral fusion include shorter operation duration, reducing soft tissue injury and less postoperative dysphagia.
