Objective:To evaluate the relationship between hippocampal receptor-interacting protein kinase-1 (RIPK1) and nucleotide-binding oligomerization domain-like receptor containing pyrin domain 3 (NLRP3) inflammasomes in postoperative neurocognitive dysfunction of aged rats with chronic knee arthritis pain.Methods:Sixty-four healthy male Sprague-Dawley rats, aged 18 months, weighing 500-550 g, were divided into 4 groups ( n=16 each) using a random number table method: chronic knee arthritis pain group (group P), chronic knee arthritis pain+ operation group (group PS), RIPK1 inhibitor necrostatin-1+ chronic knee arthritis pain+ operation group (group NPS), and DMSO+ chronic knee arthritis pain+ operation group (group DPS). The knee arthritis model was prepared by intra-articular injection of monosodium iodoacetate (MIA) 1 mg into the left knee joint, and 12 weeks later exploratory laparotomy was performed under sevoflurane anesthesia. Necrostatin-1 6.25 mg/kg and the equal volume of DMSO were intraperitoneally injected at 1 h before operation in NPS group and DPS group, respectively. Thermal pain threshold was measured at 1 week before MIA injection and 6 and 12 weeks after MIA injection. Morris water maze test was used to evaluate the cognitive function at 7 days after surgery. Hippocampal tissues were obtained for microscopic examination of the pathological changes (after HE staining) and for determination of the expression of RIPK1, phosphorylated RIPK1 (p-RIPK1), NLRP3, activated cysteine-aspartic protease caspase-1 (cl-caspase-1), apoptosis-associated speck-like protein containing a CARD (ASC) (by Western blot) and contents of interleukin-1beta (IL-1β) and IL-18 (by enzyme-linked immunosorbent assay). Results:Thermal pain threshold was significantly decreased at 6 and 12 weeks after MIA injection as compared with that before injection ( P<0.05), and there was no significant difference in thermal pain threshold among the four groups ( P>0.05). Compared with P group, the escape latency was significantly prolonged, the time of staying at the original platform quadrant was shortened, the number of crossing the original platform was reduced, the expression of RIPK1, p-RIPK1, NLRP3, cl-caspase-1 and ASC was up-regulated, and the contents of IL-1β and IL-18 were increased ( P<0.05), and pathological changes of hippocampal neurons were marked in PS group, DPS group and NPS group. Compared with PS group and DPS group, the escape latency was significantly shortened, the time of staying at the original platform quadrant was prolonged, the number of crossing the original platform was increased, the expression of RIPK1, p-RIPK1, NLRP3, cl-caspase-1 and ASC was down-regulated, the contents of IL-1β and IL-18 were decreased ( P<0.05), and pathological changes of hippocampal neurons were significantly attenuated in NPS group. Conclusions:Postoperative hippocampal RIPK1 function is enhanced in aged rats with chronic knee arthritis pain, which then activates NLRP3 inflammasomes, triggering neuroinflammation, and this process may be involved in the mechanism of postoperative neurocognitive dysfunction.

Parkinson's disease (PD) is a progressive chronic neurodegenerative disorder with a complex pathogenesis involving oxidative stress, neuroinflammation, mitochondrial dysfunction, and other factors. Currently, the clinical treatment of PD mainly includes levodopa, dopamine receptor agonists, monoamine oxidase B inhibitors, catechol-O-methyltransferase inhibitors, and anticholinergic drugs, but there is a lack of disease-modif g therapies that can definitively improve disease progression. According to the understanding of traditional Chinese medicine (TCM), PD is characterized by asthenia in origin and sthenia in superficiality. It is primarily caused by liver-kidney Yin deficiency, Qi-blood insufficiency, and closely related to wind, fire, phlegm, and blood stasis. Numerous clinical practices have shown that TCM has significant clinical value in the prevention and treatment of PD, the management of motor and non-motor symptoms, and the neuroprotection of dopaminergic neurons. The underlying mechanisms of TCM include antioxidative stress, anti-neuroinflammation, and regulation of mitochondrial dysfunction. This article categorized and summarized the pathogenesis of PD, systematically elucidated the pharmacological actions and molecular mechanisms of TCM monomer extracts and compounds in the prevention and treatment of PD, and provided the latest clinical research progress, aiming to provide references for the development and clinical use of TCM for PD.

Objectives To develop a multi-stage and multi-epitope vaccine, which consists of epitopes from the early secretory and latency-associated antigens of Mycobacterium tuberculosis (MTB). Methods The B-cell, cytotoxic T-lymphocyte (CTL) and helper T-lymphocyte (HTL) epitopes of 12 proteins were predicted using an immunoinformatics. The epitopes with antigenicity, without cytotoxicity and sensitization, were further screened to construct the multi-epitope vaccine. Furthermore, the proposed vaccine underwent physicochemical properties analysis and secondary structure prediction as well as 3D structure modeling, refinement and validation. Then the refined model was docked with TLR4. Finally, an immune simulation of the vaccine was carried out. Results The proposed vaccine, which consists of 12 B-cell, 11 CTL and 12 HTL epitopes, had a flexible and stable globular conformation as well as a thermostable and hydrophilic structure. A stable interaction of the vaccine with TLR4 was confirmed by molecular docking. The efficiency of the candidate vaccine to trigger effective cellular and humoral immune responses was assessed by immune simulation. Conclusion A multi-stage multi-epitope MTB vaccine construction strategy based on immunoinformatics is proposed, which is expected to prevent both active and latent MTB infection.

Journal of Clinical Surgery 2023;31(12):1185-1190
Objective To investigate the expression, function and prognostic value of Ras-related C3 botulinum toxin substrate 2(RAC2)in breast cancer. Methods The expression profile of RAC2 mRNA was investigated in the TCGA-BRCA cohort, the prognostic value was analyzed using the Kaplan-Meier algorithm, the potential molecular function was investigated by gene set enrichment analysis, the relationship between RAC2 and immune infiltration was analyzed by Estimate and Cibersort algorithms, and the association between Rac2 and mutation spectrum was evaluated by maftools software package. The RNA-seq software package"pRRophetic"was used to predict the effect of chemotherapy drugs. In the validation cohort,Immunohistochemistry(IHC)staining was performed on tumor pathological sections of patients.Kaplan-Meier survival analysis and cox regression analysis were used to analyze the relationship between RAC2 and the prognosis of breast cancer. Results Bioinformatics analysis showed that the expression of RAC2 was up-regulated in estrogen receptor(ER)positive BC.In the Pam50 molecular subtype of BC,its expression decreased in the order of normal-like group, triple negative group,HER2 overexpression group,Luminal A group and Luminal B group.BC patients with high RAC2 expression had better overall survival(OS)and recurrence-free survival(RFS).The results of the validation cohort confirmed that RAC2 can be used as an independent predictor of BC prognosis.RAC2 was involved in the process of immune infiltration of tumor cells, and its high expression was mainly related to the mutations of CCDC168,VPS13C and AHNAK genes. Its up-regulation increased the drug sensitivity of doxorubicin, cisplatin, paclitaxel and gemcitabine. In terms of metabolism,it was negatively correlated with glycolysis and pentose phosphate pathway(PPP),and positively correlated with fatty acid oxidation(FAO)and glutamine metabolism. Conclusion High expression of RAC2 is associated with a good prognosis of BC and plays a role in the metabolism and immune infiltration.Further study on the molecular mechanism of RAC2 may provide new strategies for the prognosis and treatment of BC.
