1.Analysis of the characteristics of injury deaths and its disease burden in Taizhou City, Zhejiang Province from 2009 to 2022

ObjectiveTo investigate the characteristics of injury deaths and its disease burden in Taizhou City from 2009 to 2022, and to provide a basis for the prevention and control of injury. MethodsBased on the injury death surveillance data of Taizhou City from 2009 to 2022, the age-specific and sex-specific mortality rates, as well as the standardized mortality rates after adjusting China’s standardized population age of residents in Taizhou City were calculated. The crude mortality rate, standardized mortality rate, average years of life lost(AYLL), potential years of life lost (PYLL) and potential years of life lost rate (PYLLR) were calculated using Excel 2013 software. Joinpoint 4.2 software was used to estimate the annual percentage change (APC) and analyze the trends of injury mortality and PYLLR from 2009 to 2022. ResultsFrom 2009 to 2022, the standardized mortality rate of injuries in Taizhou City showed a decreasing trend (APC=-4.876%, P<0.001), with a mortality rate of 64.38/100 000 and a standardized mortality rate of 66.68/100 000. There was a statistically significant difference in injury deaths by genders (χ2=7 520.153, P<0.001). From 2009 to 2022, the PYLL and AYLL caused by injuries in Taizhou City were 587 518 person years and 21.91 years, respectively, with a PYLLR of 7.72%. The PYLLR of injuries showed a decreasing trend (APC=-7.454%, P<0.001), in addition, the PYLLR in urban(APC=-7.123%), rural areas (APC=-10.193%), males (APC=-7.595%) and females (APC=-7.046%) all showed a decreasing trend, all differences were statistically significant(all P<0.001). The top five causes of injury deaths leading to PYLL were traffic accident, drowning, accidental fall, suicide and accidental poisoning. ConclusionIn the last decade, injury has been a major cause leading to premature death among residents in Taizhou, with a higher mortality rate in males than that in females, and different injury characteristics caused by different types of injuries. It is necessary to take targeted injury prevention and intervention measures for different key population groups to effectively reduce the disease burden caused by injury deaths.
2.Characteristics of injury cases in Longhua District

Objective:
To understand the characteristics of injury cases in Longhua District, Shenzhen City, Guangdong Province from 2021 to 2024, so as to provide the evidence for the development of injury prevention and control measures.
Methods:
The data of injury cases in the first visit due to injury in the sentinel hospitals of Longhua District from 2021 to 2024 were collected from the Shenzhen Injury Surveillance System. The time, cause, place, activity, intention, nature, position, severity, and outcome of injury were described.
Results:
From 2021 to 2024, a total of 167 524 injury cases were reported in Longhua District, with a male-to-female ratio of 1.89∶1. The incidence of injuries was higher in cases aged 30-<45 years (49 957 cases, 29.82%). Injuries mainly occurred from July to August (31 272 cases, 18.67%). The main cause of injury was falls (52 048 cases, 31.07%). Injuries mainly occurred at home (64 110 cases, 38.27%). Leisure activities were the main activities when injuries occurred (79 008 cases, 47.16%). Most of the injuries were unintentional (159 173 cases, 95.02%). The main type of injury was contusion/abrasion (71 900 cases, 42.92%). The main injury site was upper limb (64 247 cases, 38.35%). Most injuries were mild (131 369 cases, 78.42%). The main injury outcome was discharge after treatment, 160 882 cases (96.04%). The second cause of male injury was blunt force injury (30 140 cases, 27.49%), and the second cause of female injury was animal injury (14 648 cases, 25.31%). Fall was the leading cause of injury in people aged 0-<15 years and ≥65 years, and blunt force injury was the leading cause of injury in people aged 15-<65 years. The second place for male injuries was industrial and construction places (23 722 cases, 21.64%), and for female injuries was school/public places (9 644 cases, 16.66%). The first place for injuries in people aged 45-<65 years was in industrial and construction places. The proportions of fractures, moderate injuries, and hospitalizations increased with age (all P<0.05).
Conclusions
The main injury cases in Longhua District were males and people aged 30-<45 years. July and August were a period of high risk for injuries. People aged 0-<15 years and ≥65 years were the high-risk groups of falls. More attention should be paid to the fracture risk in the elderly.
3.Discovery and proof-of-concept study of a novel highly selective sigma-1 receptor agonist for antipsychotic drug development.

Sigma-1 receptor (σ 1R) has become a focus point of drug discovery for central nervous system (CNS) diseases. A series of novel 1-phenylethan-1-one O-(2-aminoethyl) oxime derivatives were synthesized. In vitro biological evaluation led to the identification of 1a, 14a, 15d and 16d as the most high-affinity (K i < 4 nmol/L) and selective σ 1R agonists. Among these, 15d, the most metabolically stable derivative exhibited high selectivity for σ 1R in relation to σ 2R and 52 other human targets. In addition to low CYP450 inhibition and induction, 15d also exhibited high brain permeability and excellent oral bioavailability. Importantly, 15d demonstrated effective antipsychotic potency, particularly for alleviating negative symptoms and improving cognitive impairment in experimental animal models, both of which are major challenges for schizophrenia treatment. Moreover, 15d produced no significant extrapyramidal symptoms, exhibiting superior pharmacological profiles in relation to current antipsychotic drugs. Mechanistically, 15d inhibited GSK3β and enhanced prefrontal BDNF expression and excitatory synaptic transmission in pyramidal neurons. Collectively, these in vivo proof-of-concept findings provide substantial experimental evidence to demonstrate that modulating σ 1R represents a potential new therapeutic approach for schizophrenia. The novel chemical entity along with its favorable drug-like and pharmacological profile of 15d renders it a promising candidate for treating schizophrenia.
4.Clinical effect of ultrasound-guided microwave thermal ablation for treating benign thyroid nodules

Objective To evaluate the effectiveness and safety of ultrasound-guided microwave ablation technique in the treatment of benign thyroid nodules.Methods A total of 3985 nodules in 2913 patients with pathologically confirmed benign thyroid nodules during 2016-2023 were analyzed retrospectively.The one-time ablation success rate,incidence rate of postoperative complications and side effects were evaluated.For 1890 ablation nodules in 1449 patients with a median follow-up period of 12 months,the nodule volume re-duction rate (VRR),recurrence rate,re-ablation rate and technical effective ablation rate were evaluated in postoperative 1,3,6,12,24 months and more.Results The success rate of one-time ablation in all patients was 99.7% (3972/3985),the technical effective ablation rate was 88.6% (535/604),the recurrence rate was 3.1% (59/1890) and the re-ablation rate was 0.7% (13/1890).In the follow-up nodules,VRR in postopera-tive 1,3,6,12,24 months and more was (-17±430)% (n=727),(48±152)% (n=643),(72±38)% (n=573),(83±20)% (n=604),(88±18)% (n=279),respectively.In all patients,the incidence rate of major complications was 2.57%,the incidence rate of minor complications was 2.06%,and the incidence of side effects was 3.09%.Conclusion Ultrasound-guided microwave ablation of thyroid nodules is an effective and safe method for the treatment of benign thyroid nodules.
5.Analysis on early predictors of respiratory depression in patients with glufosinate poisoning

Objective:To investigate the early predictors of respiratory depression in patients with glufosinate poisoning, and provide reference for clinicians to make decisions.Methods:In March 2022, the clinical data of patients with glufosinate poisoning admitted to the intensive care unit of the Affiliated Xiangshan Hospital of Wenzhou Medical University from March 2018 to January 2022 were retrospectively analyzed. The patients were divided into respiratory depression group and non-respiratory depression group according to the occurrence of respiratory depression during hospitalization. The clinical data such as age, gender, past history, intake, initial treatment and laboratory examination were compared between the two groups. Multivariate logistic regression was used to analyze the predictors of respiratory depression in patients with glufosinate poisoning, and its predictive value was analyzed by receiver operating characteristic (ROC) curve.Results:A total of 34 patients with glufosinate poisoning were enrolled, including 13 patients in non-respiratory depression group and 21 patients in respiratory depression group. There were significant differences in intake, blood amylase and bicarbonate radical in arterial blood gas between the two groups ( P<0.05). Respiratory depression occurred at 6.5-48.0 h after ingestion, with a median of 15.0 (9.5, 24.0) h. Multivariate logistic regression analysis showed that the intake of glufosinate ( OR=1.440, 95% CI: 1.033-2.009, P=0.032) and bicarbonate radical in arterial blood gas ( OR=0.199, 95% CI: 0.040-0.994, P=0.049) were predictors of respiratory depression in patients with glufosinate poisoning, and the area under the curve (AUC) of ROC curves were 0.936 and 0.842. The optimal cut-off values were 15.0 g (sensitivity=95.2%, specificity=76.9%) and 17.6 mmol/L (sensitivity=71.4%, specificity=84.6%), respectively. Conclusion:The intake of glufosinate and bicarbonate radical in arterial blood gas have good prediction effects on the occurrence of respiratory depression in patients with glufosinate poisoning.

7.Epidemiological characteristics of injury-induced deaths among the residents in Taizhou City, Zhejiang Province, 2009‒2022

ObjectiveTo analyze the mortality rate and the changing trends of injury in Taizhou City from 2009 to 2022, so as to provide a reference for developing injury intervention strategies. MethodsSurveillance data on injury deaths of the registered residents in Taizhou from 2009 to 2022 were used, and descriptive statistics and χ2 test were employed to analyze the mortality rates and differences by age group, gender, and region. Joinpoint regression analysis was used to analyze the trends and calculate the annual percentage change(APC). ResultsFrom 2009 to 2022, a total of 53 707 injury deaths were recorded in Taizhou, with a mortality rate of 64.38/105 and a standardized mortality rate of 66.68/105. The injury mortality rate showed a decreasing trend (APC=-1.30%, P<0.05). The top five causes of death were accidental falls, traffic accidents, drowning, suicide, and accidental poisoning, all showing a decreasing trend (APC=-3.30%, -7.65%, -2.77%, -5.78%, and -7.82%, respectively, all P <0.05). The leading causes of death for those under 15 years old, 15‒64 years old, and 65 years old and above were drowning, traffic accidents, and accidental falls, respectively. There was no statistical difference in the mortality rate between urban and rural areas (χ2=3.81, P=0.05), but there was a statistical difference between genders (χ2=7 520.15, P<0.01). ConclusionIn recent years, injury deaths among the residents in Taizhou have been decreasing. Efforts should be made to strengthen the prevention and control of injuries such as drowning in children, traffic accidents in middle-aged and young people, and accidental falls in the elderly.
8.Contemporary Evidence Summary of Strategies for Weaning From Extracorporeal Membrane Oxygenation in Adult Patients

Objectives:To retrieve,evaluate and summarize the contemporary evidence of strategies for weaning from extracorporeal membrane oxygenation(ECMO)of adult patients,and to provide evidence-based reference for clinical practice. Methods:The Web of Science,Embase,Cochrane Library,PubMed,Wanfang Database,CNKI,VIP website,SinoMed,BMJ Best Practice,National Institute for Health and Care Excellence,Joanna Briggs Institute Library,UpToDate and the website of Agency for Healthcare Research and Quality,Society of Critical Care Medicine,American Association of Critical-Care Nurses,European Society of Intensive Care Medicine and Extracorporeal Life Support Organization were researched to collect the literature related to randomized controlled trials,systematic reviews,guidelines,evidence summaries,expert consensuses and clinical decisions in this field.The time limit for the retrieval is from the inception of databases until July 2023. Results:A total of 13 related literature were retrieved,including 4 guidelines,4 expert consensuses,3 clinical decisions and 2 system reviews.Totally 42 evidences were formulated based on retrieved literature,including adequately accessing the ability of gas exchange before weaning from veno-venous ECMO(V-V ECMO)and withdrawing from veno-arterial ECMO(V-A ECMO)as soon as possible when patients's heart function has recovered,involving six aspects such as team composition,anticoagulation measures,assessment before weaning,weaning implementation,cannula and wound management and quality measures. Conclusions:It is suggested to build a professional ECMO team based on the actual hospital situation,to follow the contemporary evidence to standardize the weaning process of patients from ECMO to ensure the patients'safety and improve the outcomes.
9.Core competencies of thoracic surgery specialist nurses: a scoping review

Objective:To identify the core competencies of thoracic surgery specialist nurses and provide a reference for training thoracic surgery specialist nurses.Methods:Following the methodological framework for scoping reviews proposed by the Joanna Briggs Institute, 9 databases were searched from inception to February 23, 2024.Results:A total of 17 papers were included, including 11 papers in Chinese and 6 papers in other languages. By analyzing and integrating the included literature, 6 themes, 15 sub-themes, and 59 entries were identified as the core competencies for thoracic surgery specialist nurses. The themes included direct nursing practice competencies, critical thinking competencies, professional development competencies, interpersonal competencies, managerial competencies, and ethical decision-making competencies.Conclusions:This study reviewed the core competencies of thoracic surgery specialist nurses based on publications worldwide. Combined with the national conditions of nursing development in China, we clarified the core competencies of thoracic surgery specialist nurses that align more closely with the clinical nursing practice in thoracic surgery. In the future, we suggest building a standardized training system based on these core competencies, systematically training thoracic surgery specialist nurses, addressing the challenges in developing core competencies of thoracic surgery specialist nurses, and further improving the quality of thoracic surgery nursing in China.
10.Respiratory virus infection and its influence on outcome in children with septic shock

Objective:To investigate respiratory virus infection in children with septic shock in pediatric care units (PICU) in China and its influence on clinical outcomes.Methods:The clinical data of children with septic shock in children′s PICU from January 2018 to December 2019 in 10 Chinese hospitals were retrospectively collected. They were divided into the pre-COVID-19 and post-COVID-19 groups according to the onset of disease, and the characteristics and composition of respiratory virus in the 2 groups were compared. Matching age, malignant underlying diseases, bacteria, fungi and other viruses, a new database was generated using 1∶1 propensity score matching method. The children were divided into the respiratory virus group and non-respiratory virus group according to the presence or absence of respiratory virus infection; their clinical characteristics, diagnosis, and treatment were compared by t-test, rank sum test and Chi-square test. The correlation between respiratory virus infection and the clinical outcomes was analyzed by logistic regression. Results:A total of 1 247 children with septic shock were included in the study, of them 748 were male; the age was 37 (11, 105) months. In the pre-and post-COVID-19 groups, there were 530 and 717 cases of septic shock, respectively; the positive rate of respiratory virus was 14.9% (79 cases) and 9.8% (70 cases); the seasonal distribution of septic shock was 28.9% (153/530) and 25.9% (185/717) in autumn, and 30.3% (161/530) and 28.3% (203/717) in winter, respectively, and the corresponding positive rates of respiratory viruses were 19.6% (30/153) and 15.7% (29/185) in autumn, and 21.1% (34/161) and 15.3% (31/203) in winter, respectively. The positive rates of influenza virus and adenovirus in the post-COVID-19 group were lower than those in the pre-COVID-19 group (2.1% (15/717) vs. 7.5% (40/530), and 0.7% (5/717) vs. 3.2% (17/530), χ2=21.51 and 11.08, respectively; all P<0.05). Rhinovirus virus were higher than those in the pre-Covid-19 group (1.7% (12/717) vs. 0.2% (1/530), χ2=6.51, P=0.011). After propensity score matching, there were 147 cases in both the respiratory virus group and the non-respiratory virus group. Rate of respiratory failure, acute respiratory distress, rate of disseminated coagulation dysfunction, and immunoglobulin usage of the respiratory virus group were higher than those of non-respiratory virus group (77.6% (114/147) vs. 59.2% (87/147), 17.7% (26/147) vs. 4.1% (6/147), 15.6% (25/147) vs. 4.1% (7/147), and 35.4% (52/147) vs. 21.4% (32/147); χ2=11.07, 14.02, 11.06 and 6.67, all P<0.05); and PICU hospitalization of the former was longer than that of the later (7 (3, 16) vs. 3 (1, 7)d, Z=5.01, P<0.001). Univariate logistic regression analysis showed that the presence of respiratory viral infection was associated with respiratory failure, disseminated coagulation dysfunction, the use of mechanical ventilation, and the use of immunoglobulin and anti-respiratory viral drugs ( OR=2.42, 0.22, 0.25, 0.56 and 1.12, all P<0.05). Conclusions:The composition of respiratory virus infection in children with septic shock is different between pre and post-COVID-19. Respiratory viral infection is associated with organ dysfunction in children with septic shock. Decreasing respiratory viral infection through respiratory protection may improve the clinical outcome of these children.
