1.Clinical Comprehensive Evaluation of Oral Anticoagulants Based on A Quick Guideline for Drug Evaluation and Selection in Chinese Medical Institutions(the Second Edition)

Objective A comprehensive evaluation of oral anticoagulants(OACs)was conducted using the A Quick Guideline for Drug Evaluation and Selection in Chinese Medical Institutions(the Second Edition),to provide a reference for drug selection and clinical medication decisions in medical institutions.Methods Evaluation evidence was collected,and the drugs included in the evaluation were quantified on four dimensions of clinical properties(efficiency and safety),pharmaceutical properties,economy and others.Results All oral anticoagulants included in the evaluation had a score of 70 or higher in the comprehensive evaluation,while warfarin had the highest overall score.Clinical properties and pharmacologic properties were identified as the core attributes for drug selection evaluation.When considering only these factors,edoxaban received the highest score.Conclusion OACs are the preferred option for patients requiring long-term anticoagulation therapy.Various OACs offer distinct clinical advantages.Utilizing the Guidelines(Second Edition)for oral anticoagulant selection and evaluation can offer visual evidence for drug selection and promote the scientific,rational,and safe use of drugs in clinical management.
2.Analysis of the current status and regulatory effectiveness of daylighting and artificial lighting in primary and secondary school classrooms in Shanghai

Objective:
To understand the current situation and regulatory effectiveness of daylighting and artificial lighting in primary and secondary school classrooms in Shanghai, so as to provide a basis for enhancing the visual environment of school classrooms.
Methods:
From April 2021 to December 2023, the daylighting and artificial lighting conditions of classrooms in 1 735 regular primary and secondary schools currently in operation in Shanghai were monitored, and the qualified rate of each indicator was calculated. The Chisquare test or Fisher exact probability method were used to compare the differences in qualification rates across different educational stages, regions, school type, both before and after the implementation of regulatory measures. The regulatory measures included convene interview, propaganda and education, supervision order, supervisory opinion paper, rectification requests and offenses and punishment.
Results:
The qualified rate of daylighting and artificial lighting in primary and secondary school classrooms was 30.1%, with a qualified daylighting rate of 85.6% and a qualified artificial lighting rate of 32.9%. There was no statistically significant differences in the qualified rate of daylighting and artificial lighting in primary and secondary schools (32.4%,28.1%;χ2=3.76,P>0.05). However, statistically significant differences were found in the qualified rate of daylighting and artificial lighting in urban and rural school classrooms (32.6%,26.7%), as well as in public and private schools (31.4%, 20.6%) (χ2=6.99,9.92,P<0.05). Following the implementation of regulatory measures, the qualified rate of classroom daylighting and artificial lighting improved from 30.1% to 83.2%, while the respective qualified rates of daylighting and artificial lighting increased from 85.6% to 91.1% and 32.9% to 90.5%. Compared to the preimplementation period, the qualified rate of classroom daylighting and artificial lighting, as well as the respective rates of daylighting and artificial lighting, all showed statistically significant differences after the implementation of regulatory measures (χ2=995.29,25.34,1 219.87,P<0.01).
Conclusions
Effective regulatory measures can promote enhanced classroom daylighting and artificial lighting. Attention should be paid to improving classroom artificial lighting, in order to provide students with enhanced visual environment.
3.Construction and evaluation of a nomogram prediction model of atherogenesis risk in patients with type 2 diabetes mellitus

Objective To analyze the risk factors influencing the occurrence of atherosclerosis in patients with type 2 diabetes, and to construct and evaluate a nomogram prediction model. Methods Multivariate logistic regression was used to analyze the risk factors of atherosclerosis in type 2 diabetes mellitus, and R software was used to build a nomogram prediction model. The accuracy and clinical validity of the model were verified by using H-L fit curve, area under ROC curve and calibration curve. Results The prevalence rate of atherosclerosis was 56.37%. Independent risk factors for atherosclerosis in type 2 diabetes mellitus (P<0.05) were body weight (OR=1.42,P<0.05), glycated serum protein (OR=1.35, P<0.05), lactate dehydrogenase (OR=1.17, P<0.05), alkaline phosphatase (OR=0.79, P<0.05), hyperlipidemia (OR=2.30, P<0.05), stroke (OR=4.20, P<0.05), coronary heart disease (OR=64.54, P<0.05), lower extremity artery disease (OR=24.52, P<0.05), and other endocrine diseases (OR=1.65 , P<0.05). The area under ROC curve was 0.91, the slope of the calibration curve was close to 1, and the H-L fit curve χ2=3.11. The internal verification result of the constructed nomogram prediction model was P=0.93. External verification of patients in the test set showed that the area under ROC curve was 0.91, indicating good differentiation and accuracy of the model. Conclusion The prediction model established by using the risk factors screened in this study has a high accuracy and differentiation, and medical staff can take effective prevention measures according to the individual factors of patients.
4.Impact of Yuyetang on Cognitive Impairment of Rats with Type 2 Diabetes Mellitus Based on Regulation of Ghrelin Level

ObjectiveTo observe the regulatory effect of Yuyetang on Ghrelin level in rats with type 2 diabetes mellitus (T2DM)-induced cognitive impairment (DCI) and explore the pathway in the prevention and treatment of DCI. MethodThe T2DM model was induced in rats by intraperitoneal injection of streptozotocin (STZ) combined with the high-fat and high-sugar diet (STZ). The model rats were divided into model group, metformin group (200 mg·kg-1), and low-,medium-, and high-dose Yuyetang groups(4.575,9.15, 18.3 g·kg-1)according to the blood glucose, with 10 rats in each group. A normal group was also set up. The rats were administered with corresponding drugs by gavage for 30 days, and the body weight and blood glucose of the rats in each group were observed and recorded. After drug intervention, the learning and memory abilities of rats were tested by the Morris water maze. After the test, the whole brains of rats were sampled for hematoxylin-eosin (HE) staining to observe the pathological changes in the hippocampal CA1 region, and the expression of Ghrelin in gastric tissues and hippocampal CA1 region was detected by immunohistochemistry. ResultCompared with the normal group , the model group showed increased blood glucose(P<0.01),reduced body weight(P<0.01),prolonged escape latency(P<0.05,P<0.01), shortened retention time and movement distance in the target area,decreased number of platform crossings(P<0.01), abnormal morphology and structure of cells with disordered arrangement and reduced number in the hippocampal CA1 region, and decreased expression of Ghrelin in the serum,hippocampal CA1 region, and gastric tissues(P<0.05, P<0.01). Compared with the model group, the medium- and high-dose Yuyetang groups showed increased body weight, while all Yuyetang groups showed reduced blood glucose(P<0.01), shortened escape latency (P<0.05), prolonged retention time and movement distance in the target area,increased platform crossings (P<0.05, P<0.01), improved morphology and structure of cells, increased number of normal cell in the hippocampal CA1 region, and elevated Ghrelin levels in the serum, gastric tissues, and hippocampal CA1 region(P<0.05, P<0.01). ConclusionYuyetang can effectively improve the cognitive ability of DCI rats, and its mechanism may be related to the regulation of Ghrelin levels in the serum, hippocampal CA1 region, and gastric tissues.
5.Sensitivity and specificity of nucleic acid testing in close contacts of COVID-19 cases in Guangzhou

Objective:To analyze the sensitivity and specificity of SARS-CoV-2 nucleic acid testing in 20 348 close contacts of COVID-19 cases in different prevention and control stages in Guangzhou and to provide scientific evidence for optimizing epidemic response strategies.Methods:A total of 20 348 close contacts of COVID-19 cases in Guangzhou were traced between February 21 and September 22,2020. All the close contacts were tested for the nucleic acid of SARS-CoV-2. The sensitivity and specificity of nucleic acid testing and diagnosis in the different prevention and control stages were compared.Results:In 20 348 close contacts, 12 462 were males (61.24%), the median ( P 25, P 75) of age of them was 31.0 years (23.0,43.0), the median number ( P 25, P 75) of nucleic acid testing for them was 2.0 (1.0,3.0), and the median ( P 25, P 75) of their quarantine days was 12.0 (8.0,13.0) days, respectively. A total of 256 COVID-19 cases were confirmed in the close contacts after seven nucleic acid tests. In the 1 st, 2 nd, 3 rd and 7 th nucleic acid testing, the sensitivity and specificity were 69.14% and 99.99% (177 cases confirmed), 89.84% and 99.99% (230 cases confirmed), 97.27% and 99.99% (249 cases confirmed), and 100.00% and 99.98%, respectively. In the three stages of COVID-19 prevention and control in China: domestic case stage, imported case stage, and imported case associated local epidemic stage, the sensitivity of the 1 st nucleic acid testing was 70.68%, 68.00% and 67.35%, and the specificity was 99.98%, 100.00% and 100.00%, respectively. Conclusions:The sensitivity of nucleic acid testing in the close contacts at the different stages were consistent with slight decrease, which might be related to the increased proportion of asymptomatic infections in the late stage of epidemic prevention and control with COVID-19 in Guangzhou. It is suggested to give three nucleic acid tests to improve the sensitivity and reduce false negative risk.
6.Efficacy and Safety of Ticagrelor Versus Clopidogrel in the Treatment of ACS Patients in East Asia :A Meta- analysis

OBJECTIVE:To syst ematically evaluate the effectiveness and safety of ticagrelor versus clopidogrel in the treatment of acute coronary syndrome (ACS)patients in East Asia ,and to provide evidence-based references for clinical drug use. METHODS:Retrieved from Cochrane Library ,PubMed,Embase,CNKI,Wanfang database ,etc.,randomized controlled trials (RCTs)about ticagrelor (trial group )versus clopidogrel (control group )in the treatment of ACS patients in east Asia were collected. After literature screening and data extraction ,the quality of included literatures was evaluated by using biasrisk evaluation tool recommended by Co chrane system evaluation manual 5.1.0,and Meta-analysis was performed by using Rev Man 5.3 statistical software. RESULTS :A total of 5 RCTs were included ,with a total of 4 511 cases. Meta-analysis showed that the incidence of major adverse cardiovascular events [OR =0.85,95%CI(0.68,1.04),P=0.12],the incidence of death from cardiovascular causes [OR =0.76,95%CI(0.57,1.03),P=0.08] and the incidence of stroke [OR =0.77,95%CI(0.48,1.24),P=0.28], without statistical significance. The incidence of major bleeding events [OR =1.54,95%CI(1.19,1.99),P=0.001] and minor bleeding events [OR =1.80,95% CI(1.40,2.32),P<0.000 01] in trial group were significantly higher than control group. CONCLUSIONS:Ticagrelor is comparable to clopidogrel in reduce the major adverse cardiovascular events,death from cardiovascular causes and stroke in ACS patients in East Asian ,but it can increase the risk of major and minor bleeding events.
7.Application and Practice of Drug Quantitative Scoring Table in the Temporary Drug Purchase of Our Hospital

OBJECTIVE: To standardize the management of temporary drug purchase, and to provide reference for drug selection in Hospital Pharmaceutical Administration and Drug Treatment Committee (Pharmaceutical Association). METHODS: Clinical pharmacists set up drug quantitative scoring table according to the 10 attributes of drugs as effectiveness, safety, economy, etc. 15 temporary purchased drugs submitted by departments in Oct. 2018 were graded according to the rules of the scoring table, and the evaluation results were fed back to Pharmaceutical Association. A retrospective evaluation of 20 temporary purchased drugs which were discussed at the Pharmaceutical Association from Jul. to Sept. 2018 was made according to previous approval model without using drug quantitative scoring table. The effect of pre-intervention was evaluated after using drug quantitative scoring table. RESULTS: Among 15 temporary purchased drugs, 2 of them scored below 60, and the unqualified rate was 13.3%. It suggested that 2 drugs could not be discussed at the meeting. Among 20 temporary purchased drugs that have been discussed at the meeting, 9 of them scored below 60, with the unqualified rate of 45.0%, suggesting that 9 drugs may have wasted the workload of the Pharmaceutical Association. CONCLUSIONS: Drug quantitative scoring table can play a pre-intervention role in the scoring of temporary purchased drugs to a certain extent. At the same time, the table can also be used as a relevant reference for hospital drug evaluation. It is helpful to optimize hospital drug use list and improve the level of rational drug use in clinic.
8.T-cell receptor-engineered T cells for cancer treatment: current status and future directions.

T-cell receptor (TCR)-engineered T cells are a novel option for adoptive cell therapy used for the treatment of several advanced forms of cancer. Work using TCR-engineered T cells began more than two decades ago, with numerous preclinical studies showing that such cells could mediate tumor lysis and eradication. The success of these trials provided the foundation for clinical trials, including recent clinical successes using TCR-engineered T cells to target New York esophageal squamous cell carcinoma (NY-ESO-1). These successes demonstrate the potential of this approach to treat cancer. In this review, we provide a perspective on the current and future applications of TCR-engineered T cells for the treatment of cancer. Our summary focuses on TCR activation and both pre-clinical and clinical applications of TCR-engineered T cells. We also discuss how to enhance the function of TCR-engineered T cells and prolong their longevity in the tumor microenvironment.

9.Thoracic vertebroplasty guided by a self-designed sight

Objective To investigate the clinical efficacy of our self-designed sight used to guide thoracic vertebroplasty.Methods A retrospective analysis was conducted of the 52 patients (70 thoracic vertebrae) who had undergone percutaneous vertebroplasty (PVP) (n =36)or percataneous kyphoplasty (PKP) (n =34) of T1-T4 vertebral bodies at Department of Orthopaedics,The First Affiliated Hospital to Zhengzhou University form August 2012 to October 2013.The operation time,intraoperative bleeding,intraoperative radiation by C-arm roentgenography,and visual analogue scale (VAS) were compared between the patients whose surgery had been guided by our self-designed sight and those whose surgery had been not.Results All the patients were followed up for 18 to 36 months (average,22.3 months).Compared with those who had not used the sight,the patients who had used the sight incurred significantly shorter operation time (16.5 ± 3.2 min versus 26.5 ± 3.7 min),significantly less intraoperative bleeding (2.8 ± 1.3 mL versus 6.3 ± 1.7 mL) and significantly less radiation by C-arm roentgenography (5.6 ± 3.3 times versus 9.4 ± 3.1 times) (P <0.05).The VAS scores at postoperative 3 days and final follow-up were significantly decreased than the preoperative values in all the patients (P < 0.05).There were no significant differences between the patients who had used the sight and those who had not in the the VAS scores at postoperative 3 days or final follow-up (P > 0.05).Conclusions The upper thoracic PVP or PKP guided by our self-designed sight has theadvantages of short operation time,less intraoperative bleeding and less frequency of C-arm radiation.The sight is suitable for various vertebroplasties.
10.Evidence summary for prevention and management of implantable venous access port occlusion in adult patients

Objective To retrieve,appraise and summarize the available evidence on management and prevention of implantable venous access port occlusion in adult patients.Methods We searched the BMJ best practice,UpToDate,Cochrane Library,Joanna Briggs Institute Library,Registered Nurses' Association of Ontario,National Guideline Clearinghouse,PubMed,EMbase,CNKI and CBM to collect literatures including guidelines,evidence summary,best practice information sheet,recommended practice,systematic review and consensus.Results Five references including one guideline,one systematic review,two evidence summaries,and one expert consensus were included.A total of eight items of best evidence were summarized with regard to syringe size,flush volume,flush technique and frequency of administration,choose of needleless connectors,management of mechanical occlusion,drug/mineral precipitation,and thrombotic occlusion.Conclusion Healthcare workers should regulate the standard of venous access port flushing and locking and manage catheter occlusion in a timely manner,to avoid adverse incidents like interruptions in treatment,bacteremia,and venous thrombosis.
