1.Efficacy and safety of sacral neuromodulation in the treatment of refractory interstitial cystitis/bladder pain syndrome

Objective: To summarize the long-term efficacy and safety of sacral neuromodulation (SNM) in treating refractory interstitial cystitis/bladder pain syndrome (IC/BPS),so as to provide clinical evidence for this therapeutic approach. Methods: This study retrospectively analyzed the data of 27 patients with refractory IC/BPS treated at our hospital during Jan.2015 and Dec.2021.All patients underwent SNM Ⅱ.The therapeutic effects were evaluated with Global Response Assessment (GRA).The preoperative and postoperative daily urinary frequency,nocturia frequency,functional bladder capacity,O'Leary-Sant score,pelvic pain,urgency and frequency (PUF) score,quality of life (QoL) score,and visual analogue scale (VAS) score were compared. Results: During the follow-up of (55.55±16.33) months,12 patients showed no symptom relief (GRA≤4),while 15 experienced symptom relief (GRA>4),resulting in an efficacy rate of 55.56% (15/27).The average daily urination frequency decreased from (28.74±13.84) to (24.74±16.33) times,the O'Leary-Sant score decreased from (30.19±5.35) to (25.63±9.34),PUF score decreased from (25.63±5.34) to (22.04±8.29),QoL score decreased from (5.67±0.55) to (4.33±1.57),and VAS score decreased from (8.04±1.91) to (5.33±3.09),the differences were statistically significant (P<0.05). No complications such as incision infections or pain at the implant site were observed. Conclusion: SNM is safe and effective for refractory IC/BPS,but there is a risk of long-term failure.Therefore,patients should be discreetly selected.
2.Establishment of a duplex real-time PCR method for differentiation of African swine fever virus I177L gene-deleted strains

African swine fever virus(ASFV)I177L gene deletion vaccine is one of the key directions of African swine fever(ASF)live attenuated vaccine research and development.In order to effec-tively distinguish between the wild-type ASFV strain and the I177L gene-deleted strain,specific primers and probes were designed based on ASFV B646L and I177L genes,respectively.After screening and optimization,a duplex real-time PCR method was developed that can simultaneously detect these two genes.The results showed that ASFV B646L and I177L genes were detected spe-cifically and simultaneously by the method developed without cross-reactions with porcine circovir-us type 2,Seneca virus A,classical swine fever virus,foot-and-mouth disease virus,porcine respira-tory and reproductive syndrome virus.The detection limits of the duplex real-time PCR for recom-binant plasmids pUC57-B646L and pUC57-I177L were 1×103 copies/mL.The intra-and inter-as-say coefficients of variation were less than 4%,respectively.Detection of 122 pork and pork prod-ucts using the duplex real-time PCR developed and the real-time PCR recommended by WOAH showed that the coincidence rates of the two methods for B646L gene detection was 100%with two amplification curves appeared in the positive results of the established methods.The method established in this study can be used for the detection of ASFV I177L gene deletion strains,which provides technical support for ASF surveillance and epidemiological investigation.
3.Transcriptome Sequencing Reveals the Potential Mechanisms of Modified Electroconvulsive Therapy in Schizophrenia

Objective:
Schizophrenia (SCZ) is one of the most common and severe mental disorders. Modified electroconvulsive therapy (MECT) is the most effective therapy for all kinds of SCZ, and the underlying molecular mechanism remains unclear. This study is aim to detect the molecule mechanism by constructing the transcriptome dataset from SCZ patients treated with MECT and health controls (HCs).
Methods:
Transcriptome sequencing was performed on blood samples of 8 SCZ (BECT: before MECT; AECT: after MECT) and 8 HCs, weighted gene co-expression network analysis (WGCNA) was used to cluster the different expression genes, enrichment and protein-protein interaction (PPI) enrichment analysis were used to detect the related pathways.
Results:
Three gene modules (black, blue and turquoise) were significantly associated with MECT, enrichment analysis found that the long-term potentiation pathway was associated with MECT. PPI enrichment p-value of black, blue, turquoise module are 0.00127, <1×10-16 and 1.09×10-13, respectively. At the same time, EP300 is a key node in the PPI for genes in black module, which got from the transcriptome sequencing data.
Conclusion
It is suggested that the long-term potentiation pathways were associated with biological mechanism of MECT.
4.The similarities and differences between air-charged catheters and water-filled catheters in the application of pressure-flow study

Objective:We compare the consistency, similarities and differences of operating procedures, data and conclusions of air-charged catheters(ACC) and water-filled catheters(WFC), as simultaneously using ACC and WFC in pressure-flow study(PFS).Methods:This study was a prospective, synchronously controlled study, including eligible patients who underwent PFS in the Department of Urology, Beijing Chaoyang Hospital from January 2021 to March 2021. Inclusion criteria: ① Patients need PFS for lower urinary tract symptoms like frequency of urination, urgent urination, urinary incontinence and dysuria; ② Age over 18 years old. Exclusion criteria: ① Unable to complete or cooperate during the urodynamic test; ② Patients with severe urethral stricture or acute stage urinary tract infection; ③ Pregnant women. The bladder pressure was measured continuously by using a 7FDR T-DOC ?AC three-chamber bladder pressure tube, which linked to ACC sensor and improved WFC pressure conduction module. At the same time, 7FA T-DOC ?AC single-lumen rectal pressure tube and 7F Labori-CAT411 double-lumen water sac abdominal pressure tube was used to measure the ACC and WFC rectum pressure, respectively. We recorded the Pdet, Pves and Pabd measured by ACC and WFC, at the point of initial sitting position, bladder filling at 100 ml, 150 ml, 200 ml, cough, Q max, maximum Pdet and the end of urination, and compared the mean values, differences, and consistencies of our data. Result:A total of 63 patients (26 female, 37 male) were included in this study, with an average age of 59.19 years (25-86 years old). During bladder filling phase, the mean values of Pves measured by ACC and WFC were 30.78/24.67cmH 2O (initial sitting position), 29.79/25.13cmH 2O (100 ml), 30.87/25.90cmH 2O (150 ml) and 30.95/26.17cmH 2O(200 ml), respectively, the mean value of Pabd were 30.03/24.17cmH 2O (initial sitting position), 28.81/21.78cmH 2O (100ml), 28.89/21.38cmH 2O (150ml), 28.44/21.60cmH 2O (200ml), respectively, and were significantly different at each sampling point ( P<0.01). During urination period, no significant differences were found in data( P>0.05), and the data measured with ACC and WFC system have good consistency. There were significant differences in Pves(mean 57.30/49.95 cmH 2O, respectively) and Pdet(mean 54.21/43.10 cmH 2O, respectively) between ACC and WFC in cough ( P<0.01), but there was a strong linear correlation between these data between two systems(R 2=0.792 in Pves and 0.756 in Pabd). Bland-Altman analysis showed that detrusor pressure at the maximum urine flow rate maintained good consistency between ACC and WFC, which 95% CI was -13.9 cmH 2O to 15.8 cmH 2O. Conclusions:In PFS, although the ACC measurement values (Pves and Pabd) during the filling phase are higher than those WFC readings, but the absolute measurement difference is small, so there is no practical meaning in clinical practice. There was no significant difference in detrusor pressure measured during voiding phase, which indicated that the urodynamic judgment and clinical conclusions of the two systems are highly consistent in judgment of the detrusor contractility and the bladder outlet obstruction.

6.How to improve one-time yield in blood plasma separation

【Objective】 To analyze the factors affecting "one-time yield" in blood plasma separation (BPS) and explore the methods to improve the rate of it. 【Methods】 The rate of "one-time yield" in BPS of 2018 was calculated and the influencing factors were analyzed. Such influencing factors as air release after leukodepletion, poor performance of leukodepletion filter, selection of blood bag barrel, cup filling method, standing time after centrifugation, separation method were improved in 2019, and the difference of the "one-time plasma yield" rate of fresh frozen plasma/frozen plasma and the centrifugal damage before and after the improvement were compared. 【Results】 After the improvement, the rate of "one-time yield" of fresh frozen and frozen plasma increased to 89% and 80%, respectively, which was significantly increased as compared with that before(P<0.05); the centrifugal damage rate decreased to 0.01%, showed no statistical significance compared with that before. 【Conclusion】 With accurate analysis of the factors affecting "one-time plasma yield", the improved method proved to be effective, the rate of "one-time yield" has been improved significantly, and the blood quality has been guaranteed.
7.Nurses' Perceptions of Factors Influencing Elder Self-neglect: A Qualitative Study

Purpose:
Elder self-neglect is a global public health issue and should be taken seriously at large. Nurses,usually working directly with elderly patients, have a better understanding of what factors may causeelder self-neglect. In this qualitative study, we explored the influencing factors of elder self-neglect fromthe perception of nurses in the context of Chinese culture.
Methods:
Face-to-face, in-depth interviews were conducted from November 2018 to December 2018.Purposive sampling was used. Twenty one participants recruited from eight geriatric wards of a generalhospital located in Wuhan were interviewed. A content analysis of qualitative nature was performed toanalyze the data.
Results:
Our conceptual model illustrated the findings based on the three themes of the conflict betweenpersonal recognition and social judgment, the choice between current needs and individual beliefs, aswell as the compromise between insufficient abilities and limited resources.
Conclusion
Nurses together with family members and social workers can help older adults improvetheir awareness of self-neglect to bridge the gap with social judgment, learn to focus on their own needs,as well as seek as much support as possible. Nurses should also respect the autonomy and selfdeterminationof elder self-neglecters because self-neglect is related to older adults' values. Furthermore,larger studies are needed to quantitatively test and refine the model.

9.Comparison of single incision mini slings and tension-free vaginal tape obturator in the treatment of pure female stress urinary incontinence: results of 2-year investigation

Objective:To compare the clinical efficacy and safety of single incision mini slings (contasure needless, C-NDL) and tension-free vaginal tape obturator (TVT-O) in the treatment of pure female stress urinary incontinence(SUI).Methods:Clinical data of 107 female SUI patients who underwent C-NDL ( n=51) or TVT-O ( n=56) procedure during October 2013 and October 2017 in Department of Urology, Beijing Chaoyang Hosptial, Capital Medical University were enrolled.The age of the patients in the C-NDL and TVT-O group was (58.8±9.3) years (range: 43 to 81 years) vs.(56.9±11.4) years (range: 32 to 87 years), the cough leak point pressure was 70(20) cmH 2O ( M( Q R), 1 cmH 2O=0.098 kPa) vs. 80(25) cmH 2O. The pre- and peri-operative data, subjective cure rate, objective cure rate, quality of life, International Consultation on Incontinence Questionnaire-Short Form (ICIQ-SF), Incontinence Impact Questionnaire-Short Form (IIQ-7) and other complications were compared. The t test, Wilcoxon rank sum test, analysis of variance, χ 2 test andFisher exact test were applied to compare the indexes. Results:In the C-NDL and TVT-O group, the median operation time was 30(10) minutes vs. 40(30) minutes ( Z=-3.73, P<0.01). The postoperative follow-up period was 32.0(17.4) months vs.41.0(23.2)months. At the latest follow-up, the objective cure rate was 48/51 vs. 54/56 (χ 2=0.01, P=0.94), the subjective cure rate was 43/51 vs. 50/56 (χ 2=0.03, P=0.86), ICIQ-SF was 2.29±4.02 (range:0 to 13) vs. 1.71±3.67 (range:0 to 18), and IIQ-7 was 1.12±1.85 (range: 0 to 7) vs.0.70±1.70 (range: 0 to 6). The statistically significant difference in the postoperative visual pain analogue scale (VAS) were found between the two groups (0(1.5) vs.1.0(1.0), Z=3.41, P<0.01). Dysuria occurred in 2 patients in each group, vaginal pain occurred in 1 patient in the C-NDL group, groin pain occurred in 2 patients and urinary urgency occurred in 3 patients in the TVT-O group too. There were no significant difference between the complication rates of the 2 group (3/51 vs.7/51, P=0.325). Conclusion:Both C-NDL and TVT-O may be effective and reliable in the treatment of female SUI.
10.Development of the Self-neglect Questionnaire for the aged and its reliability and validity

Objective:To develop a Self-neglect Questionnaire for the aged in China and to test its reliability and validity so as to provide an effective assessment tool for differentiating self-neglect for the aged.Methods:The initial questionnaire was developed based on former results of qualitative research, literature review and the method of Delphi expert consultation. From April to May 2018, a total of 71 elderly inpatients were selected by convenience sampling at a ClassⅢ Grade A hospital in Wuhan to investigation with item analysis. From June to October 2018, by convenience sampling a total of 314 elderly inpatients were investigated formally at a Class Ⅲ Grade A hospital in Wuhan, and the questionnaire was tested in terms of reliability and validity.Results:Finally, the Self-neglect Questionnaire induding 7 common factors and 24 items was formed. The Cronbach's α of the questionnaire was 0.875, the split-half reliability coefficient was 0.709. In content validity, the item-content validity index (I- CVI) ranged from 0.895 to 1, and the average scale-content validity index (S- CVI/Ave) was 0.987. Exploratory factor analysis extracted 7 common factors with 69.391% for the cumulative variance contribution. Conclusions:The Self-neglect Questionnaire for the aged has good reliability and validity which can be an assessment tool for self-neglect of the aged.
