1.Protective effect of Shengxian decoction and the decoction of single herb component against myocardial injury induced by hypoxia/reoxygenation

		                        			Objective To study the protective effect of Shengxian decoction and the single herb decoction against myocardial injury induced by hypoxia/reoxygenation. Methods The H9c2 cells were cultured to establish hypoxia/reoxygenation model. Rats were divided into 8 groups: normal control group, hypoxia/reoxygenation group (model group) and treated groups (Shengxian decoction and the single herb decoction). The apoptotic rate of cardiomyocytes, the activity of reactive oxygen species (ROS) and intracellular calcium concentration (Ca2+) were measured. Results Compared with hypoxia/reoxygenation group, the apoptosis rate, ROS activity and intracellular Ca2+ concentration were significantly lower in all treated groups (P<0.05). The ROS activity and intracellular Ca2+ concentration was decreased by 41.37% and 15.20% in Shengxian decoction group compared to the model group. Conclusion Shengxian decoction and the single herb decoction had protective effect on myocardial injury induced by hypoxia/reoxygenation.
		                        		
		                        		
		                        		
		                        	
2.Fine Setting of the Use Rules for Proton Pump Inhibitors in Outpatient Prescription Pre-review of Our Hospital

		                        			OBJECTIVE: To improve the proper use of proton pump inhibitors (PPIs) and develop prescription pre-review. METHODS: Among “Hospital Self-Maintenance Rule Base” of “Rational use of drug” in our hospital, according to the application of PPIs in our hospital, the dosage of PPIs under different indications, drug combination, age range and taboo level for children were refined to promote the prescription pre-review. After refinement, the qualified rate of PPIs prescription post-evaluation among outpatient prescriptions, successful rate of pharmacist intervention during pre-prescription review and unqualified prescription rate of each item for 8 PPIs (including “interaction” “clinical diagnosis does not conform to medication” “repeated administration” “inappropriate usage”) were analyzed statistically from Jan. to Aug. 2018. The effects of fined setting were evaluated. RESULTS: From Jan. 2018 to Aug. 2018, the qualified rate of PPIs prescription post-evaluation gradually increased from 81.94% to 95.10%; successful rate of pharmacist intervention during pre-prescription review increased from 32.75% to 91.01%; the rate of unqualified prescriptions for 4 items decreased significantly month by month. CONCLUSIONS: The fine setting of the rules for the use of proton pump inhibitors in our hospital has effectively carried out the pre-examination of outpatient prescriptions and promoted the rational use of PPIs.
		                        		
		                        		
		                        		
		                        	
3.Problems and Countermeasures of Promoting the Operation of the Pre-prescription Review System in Our Hospital

		                        			OBJECTIVE: To promote the implementation of the pre-prescription review work, and to ensure the rational drug use of patients. METHODS: With the idea of PDCA (Plan, Do, Check, Action) cycle management, the phased improvement of three PDCA cycles was gradually implemented in the operation of the pre-prescription review system, aiming at the establishment of the working mode of the prescription review work, the improvement of the rules of knowledge base in the review system and the improvement of the ability of pharmacists to review prescriptions. The operation results of the system were evaluated by comparing the irrational prescription rate of outpatient pharmacies before and after the operation of pre-prescription review system. RESULTS: Through adopting the prescription review mode of “rigid” and “flexible” interception, regular revision of knowledge base rules, regular training and examination of prescription pharmacists, pre-prescription review system operated smoothly, and the pre-prescription review work was carried out in the process of continuous improvement. In the three PDCA cycles, the irrational rate of prescriptions decreased significantly, such as after the first PDCA cycle, the irrational rate of TCM outpatient prescriptions decreased from 22.0% (1 393/6 332) in Jan. 2017 to 7.4% (416/5 627) in Jun. 2017; after the second PDCA cycle, the irrational rate of outpatient prescriptions in hospital decreased from 4.87% (5 244/107 691) in Mar. 2018 to 2.21% (2 219/100 412) in Aug. 2018. After the third PDCA cycle, the percentage of over-treatment course prescriptions in total prescriptions decreased from 16.97% (15 728/92 684) in Jun. 2018 to 5.55% (5 394/97 275) in Sept. 2018. CONCLUSIONS: The pre-prescription review system can effectively intercept and interfere with irrational prescriptions, and PDCA cycle management can effectively promote the operation of the pre-prescription review work.
		                        		
		                        		
		                        		
		                        	
4.Epidemiology of fungal infections in China.

		                        			With the increasing number of immunocompromised hosts, the epidemiological characteristics of fungal infections have undergone enormous changes worldwide, including in China. In this paper, we reviewed the existing data on mycosis across China to summarize available epidemiological profiles. We found that the general incidence of superficial fungal infections in China has been stable, but the incidence of tinea capitis has decreased and the transmission route has changed. By contrast, the overall incidence of invasive fungal infections has continued to rise. The occurrence of candidemia caused by Candida species other than C. albicans and including some uncommon Candida species has increased recently in China. Infections caused by Aspergillus have also propagated in recent years, particularly with the emergence of azole-resistant Aspergillus fumigatus. An increasing trend of cryptococcosis has been noted in China, with Cryptococcus neoformans var. grubii ST 5 genotype isolates as the predominant pathogen. Retrospective studies have suggested that the epidemiological characteristics of Pneumocystis pneumonia in China may be similar to those in other developing countries. Endemic fungal infections, such as sporotrichosis in Northeastern China, must arouse research, diagnostic, and treatment vigilance. Currently, the epidemiological data on mycosis in China are variable and fragmentary. 5.A magnetic resonance spectroscopy study on cerebellar nuclei in patients with insomnia disorder

		                        			Objective The objective of this study was to utilize proton magnetic resonance spectroscopy (1H-MRS) to assess metabolites in cerebellar nuclei in unmedicated patients with insomnia disorder. Methods 1H-MRS was performed on cerebellar nuclei in 23 unmedicated patients with insomnia disorder (insomnia group) and 18 normal sleepers (control group). N-acetylaspartate (NAA), choline-containing compound (Cho) and creatine (Cr) were measured and the ratios of NAA/Cr and Cho/Cr were determined.Pittsburgh Sleep Quality Index (PSQI) and Insomnia Severity Index (ISI) were used to assess the subjective sleep quality and insomnia severity of all subjects, while State-Trait Anxiety Inventory (STAI) and Beck Depression Inventory (BDI) were used to assess the levels of anxiety and depression of all subjects. Sleep parameters of all subjects were measured by polysomnography (PSG). Results Mean NAA/Cr ratio of right cerebellar nuclei in insomnia group was significantly lower than that in control group (1.72±0.37 vs. 2.03±0.50, t=2.280, P=0.028). Mean NAA/Cr ratio of right cerebellar nuclei was significantly higher than that of left cerebellar nuclei within control group (2.03±0.50 vs. 1.68±0.21, t=3.386, P=0.004). There was no significant difference with regard to NAA/Cr ratio between bilateral cerebellar nuclei within insomnia group (t=1.416, P=0.171). Across all subjects, PSQI global scores (r=-0.369, P=0.018), and sleep latency (r=-0.437, P=0.004) and number of awakenings after sleep onset (r=-0.432, P=0.005) measured by PSG were negatively correlated with NAA/Cr ratios of right cerebellar nuclei, while percentages of stage 3 sleep (r=0.377,P=0.015) measured by PSG were positively correlated with NAA/Cr ratios of right cerebellar nuclei,respectively. Conclusion Patients with insomnia disorder have a hemispherically lateralized metabolic disturbance of NAA/Cr in right cerebellar nuclei,indicating that patients with insomnia disorder have neuronal damage in right cerebellar nuclei.
		                        		
		                        		
		                        		
		                        	
6.The mass spectrometry analysis of serum biomarker 32 kD protein in first episode-schizophrenia

		                        			Objective To analyze the constituent of the 32 kD protein band and its expression in schizophrenia se?rum. Methods Sixty schizophrenia patients and 58 health controls were recruited. The serum samples were collected and precipitated with 7%PEG. The sodium dodecyl sulfate polyacrylamide gel electrophoresis (SDS-PAGE) was used to ob?tain the abnormal 32 kD proteins band in patients. This protein band was cut and then analyzed using mass spectrometric technique. Results The 32 kD protein band was present in 38 schizophrenia patients but not in control and positive rate was 63.33%. The mass spectrometric analysis showed that 32 kD protein band contained 14 proteins ranging from 30 kD to 35 kD, including 6 high-frequency proteins (cDNA coded protein 1 and 2, Apolin protein A-1, Isoform 2 of ficolin-2, Complement factor H and clusterin) and 8 low-frequency proteins (IgG H chain, zinc-alphg-2-glycoprotein, fermitin,family apolin protein L-1, isoform 10 of collectin-1, purine nucleoside, anne xin and cDNA coded protein 3). Three cD?NA coded unknown proteins were highly similar to complement C4-B, β2-glycoprotein and erythrocyte band 7 integral membrane protein. 7.The diagnostic value of cytokeratin 19 fragment in non small-cell lung cancer

		                        			Objective To discuss the diagnostic value of cytokeratin 19 fragment(CYFRA21-1) in non small-cell lung cancer (NSCLC) .Methods The serum CYFRA21-1 levels were tested in 1 046 healthy adults(healthy group) ,244 benign pulmonary dis-ease patients(benign disease group) and 90 NSCLC patients(NSCLC group) .The results were analyzed statistically .Results There was not significantly difference between man and women in healthy group for CYFRA21-1 level(P>0 .05) .The CYFRA21-1 level in more than 60 years cases was obviously higher than that in less than 18-40 years group and >40-60 group .The upper limit of serum CYFRA21-1 was 4 .00 ng/mL according 95% confidence interval of benign disease group .A cut-off value of 4 .70 ng/mL in NSCLC group was suggested when compared with receiver operating characteristic curve(ROC curve) ,and its sensitivity and speci-ficity were 62 .1% and 92 .6% respectively .Conclusion The diagnostic value for serum CYFRA21-1 in NSCLC was 4 .70 ng/mL .
		                        		
		                        		
		                        		
		                        	
8.Analysis of test results using residual reagents in electrochemiluminescence immunoassay analyzer

		                        			Objective To evaluate the detection efficiency of residual reagent used in electrochemiluminescence immunoassay an-alyzer .Methods Alpha fetoprotein (AFP) ,carcino-embryonic antigen(CEA) and total prostate specific antigen(TPSA) were de-tected by using remaining reagents .The precision and recovery rate of remaining reagents were analyzed .Referring to the document of NCCLS EP9-A2 ,8 clinic samples were detected everyday by residual reagent and new reagent respectively .The test results of to-tal 40 samples were recorded within 5 days .The test results derived from 2 kinds of reagents were analyzed comparatively and their bias was evaluated by using new reagent as a control method and residual reagent as experimental methods .Results The within-run and between-run coefficients of variation(CV) of the 3 items measured by residual reagents in low and high levels of quality control products met the related standard .The recovery rate was variable from 90% to 110% .The test rewults of the 2 kinds of reagents were positively correlated(r2 >0 .95) .Their anticipated biases were within allowed biases on the medical decision level of CEA ,AFP and T PSA .Conclusion Residual reagent of electrochemiluminescence immunoassay analyzer can meet the clinical practice needs , which also can ensure the quality of measurement and the reduction of the cost .
		                        		
		                        		
		                        		
		                        	
9.Fungal infection after a tragedy: a report of three cases of candidosis in a fire accident.

		                        			Patients who suffer severe burns are at increased risk for local and systemic infections. The incidence of fungal infections has increased in recent years, and these infections represent a major issue in burn intensive care units. Herein, we report three cases of fungal infection due to Candida species occurring in patients undergoing supportive therapy and antibiotic treatment during their hospitalization. Two of these patients were infected with Candida parapsilosis, and one was infected with Candida albicans. The risk factors for these patients' Candida infections were multiple and prolonged courses of antimicrobial treatment, steroid treatment, tracheal intubation and smoke inhalation. Susceptibility testing of nine antifungal compounds was performed, and the minimum inhibitory concentration (MIC) values of all isolated strains were lower than the breakpoint MIC value for resistance of the relevant drug. All three patients were cured by treatment with antifungal agents. Candida infection may occur 1 - 3 weeks after thermal injury, and the prompt recognition and treatment of such infections with antifungal therapies may result in decreased morbidity and mortality associated with these infections in burn patients.

10.Taxonomic analysis of cryptococcus species complex strain S8012 revealed Cryptococcus gattii with high heterogeneity on the genetics.

BACKGROUNDInitially, Cryptococcus (C.) neoformans was previously divided into two varieties comprising C. neoformans var. neoformans and C. neoformans var. gattii. Currently, taxonomic studies defined C. neoformans as C. species complex, which contains C. neoformans var. neoformans (serotype D), the hybrid isolates (serotype AD), C. neoformans var. grubii (serotype A) and C. gattii (serotypes B and C). However, Liao and his team once isolated a unique C. gattii isolate, namely strain S8012 with unique phenotype from cerebrospinal fluid (CSF) of a 43-year-old male patient in the Shanghai Changzheng Hospital and described as C. neoformans var. shanghaiensis in 1980s. The aim of this study was to explore the genetic background and polymorphism of Chinese clinical C. gattii isolates.
METHODSS8012 was analyzed as representative strain using the M13-polymerase chain reaction (PCR) fingerprinting pattern and multilocus sequence analysis including internal transcribed spacers of rDNA (ITS region), the intergenic spacer 1 regions (IGS1), RPB1, RPB2, CNLAC1, and TEF1 genes.
RESULTSThe PCR fingerprinting pattern results showed strain S8012 belonged to molecular types VGI, and phylogenetic analysis suggested strain S8012 was grouped into the cluster of C. gattii environmental isolates originated from Eucalyptus camaldulensis trees in Australia.
CONCLUSIONC. gattii isolates from Chinese patients expresses high polymorphism on the phenotype, and molecular type VGI isolates from China have a close genetic relationship with the C. gattii isolates from Australia.
