1.Investigation and analysis of the current situation of occupational stress of radiation workers in China

		                        			Objective To investigate and analyze the occupational stress levels and influencing factors among radiation workers in China, and provide a reference for alleviating occupational stress and promoting mental health. Methods Using the general situation questionnaire, Effort-Reward Imbalance questionnaire, and radiation protection knowledge questionnaire, a convenience sampling method was adopted to investigate the occupational stress of 243 radiation workers in Liaoning, Fujian, Guangdong, and Xinjiang provinces. The independent samples t-test, one-way analysis of variance, chi-square test, and binary logistic regression were used to analyze the influencing factors. Results The average score of Effort-Reward Imbalance was 0.97 ± 0.22, and 100 (41.15%) radiation workers had occupational stress. There were significant differences in the detection rate of occupational stress among radiation workers of different ages, working years in radiation positions, monthly incomes, daily sleep durations, and daily working hours (P < 0.05). Logistic regression analysis identified daily working hours as a factor contributing to occupational stress. Conclusion The occupational stress among radiation workers in China is relatively severe. It is recommended to pay attention to the associated risks and implement targeted intervention measures to reduce the impact of occupational stress.
		                        		
		                        		
		                        		
		                        	
2.Monte Carlo simulation analysis of the design of a medical electron linear accelerator maze

		                        			Objective To analyze the impact of different maze structures of medical electron linear accelerator on the radiation level at the entrance of the treatment room. Methods The Monte Carlo simulation method was used to compare the radiation field distribution in two accelerator rooms with different maze widths and to conduct a detailed analysis of the radiation levels. Results The radiation level at the entrance of the accelerator room with a wide maze was significantly higher than that at the entrance of a compact maze. The neutron radiation level at the entrance of the narrow maze model decreased by 38.6% compared to the wide maze model, and the level of X-ray radiation also showed a declining trend. Conclusion When designing medical electron linear accelerators, a compact maze structure should be adopted to reduce the setting of unnecessary space, thereby lowering the radiation level at the entrance of the treatment room and enhancing the effectiveness of radiation protection.
		                        		
		                        		
		                        		
		                        	
3.Analysis of the current situation of occupational stress of radiation workers in typical nuclear power plants

		                        			Objective To investigate the current situation of occupational stress among radiation workers in nuclear power plants in China, to explore the factors influencing occupational stress, and to provide a reference for improving occupational stress. Methods A convenient sampling method was adopted to investigate the occupational stress of 100 radiation workers in two nuclear power plants in Fujian and Liaoning, using the general situation questionnaire, effort-reward imbalance questionnaire, and the radiation protection knowledge questionnaire. Independent samples t-test, one-way analysis of variance, chi-square test, and binary logistic regression analysis were used to examine the current situation of occupational stress and the influencing factors. Results The average value of effort-reward imbalance was 0.99 ± 0.15 and 47 (47%) radiation workers had occupational stress. There were significant differences in the detection rate of occupational stress among radiation workers with different ages, years of radiation work, monthly incomes, sleep durations (day), and exercise habits (P < 0.05). Logistic regression analysis showed that monthly income, sleep duration (day), and exercise habits were the influencing factors for occupational stress. Conclusion The occupational stress of radiation workers in nuclear power plants is a serious issue influenced by many factors. Attention should be given to this issue, and targeted interventions should be implemented to improve the social and working environment of this occupational group and reduce the level of occupational stress.
		                        		
		                        		
		                        		
		                        	
4.Research status of occupational stress among radiation workers in China

		                        			In recent years, occupational stress has become a focus of public attention in public health in China and abroad. Radiation workers are exposed to ionizing radiation in their daily work, which further exacerbates their levels of occupational stress. This not only affects their physical and mental health, but also increases the potential for radiation incidents or accidents. Based on the literature on occupational stress among radiation workers in China throughout the years, this article provides a summary, conclusion, and systematical analysis of the research status of occupational stress among radiation workers, with a detailed sorting of the data. 5.Observation on the therapeutic effect of LPE combined with semi whole blood replacement for postoperative anemia caused by infection

		                        			Objective To observe the therapeutic effect of lymphatic plasma therapy(LPE)combined with semi whole blood exchange on patients with moderate to severe anemia caused by severe postoperative infection.Methods Thirty patients who developed severe postoperative infections with moderate to severe anemia and were treated with antibiotics for 48 to 72 hours but failed to meet the criteria were divided into an observation group(LPE combined with semi whole blood replacement surgery and antibiotic treatment,n = 15)and a control group(conventional antibiotic anti infection and blood transfusion treatment,n = 15).Evaluation indicators of infection and anemia between and within two groups before and after treatment,and the treatment effect were compared,respectively.Results There was no statistically significant difference in the observation indicators of infection and anemia between the two groups before treatment(P>0.05);After each treatment,there was a statistically sig-nificant difference in the evaluation and observation indicators of infection and anemia between the two groups(P<0.05);Observation group:There was a statistically significant difference in the observation and evaluation in-dicators of infection and anemia before and after treatment within the group(P<0.05);Control group:Compared within the group before and after treatment,there was no significant decrease in WBC,NEUT,and NEU observation indicators,and CRP and PCT observation indicators showed an upward trend,while RBC,HBG,and HCT did not show a significant increase.The difference was not statistically significant(P>0.05);Conclusions The com-bination of LPE and semi whole blood replacement surgery is superior to the conventional treatment regimen of anti-biotics alone for anti-infection and blood transfusion to improve anemia symptoms in the treatment of postoperative severe anemia.
		                        		
		                        		
		                        		
		                        	
6.Application of case-based learning combined with online teaching in standardized residency training of rheumatology and immunology

		                        			Objective:To investigate the effectiveness of case-based learning (CBL) combined with online teaching in standardized residency training of rheumatology and immunology.Methods:A total of 78 individuals who participated in standardized residency training in Department of Rheumatology and Immunology in our hospital from June 2019 to August 2020 were included and divided into observation group and control group. The individuals in the control group received traditional teaching, and those in the observation group received CBL combined with online teaching. The physicians receiving standardized residency training were evaluated by theoretical examination, clinical operation skill assessment, and instructor rating, and the degree of satisfaction with teaching, degree of satisfaction with teaching methods, and classroom learning atmosphere were also evaluated.Results:The observation group had a theoretical examination score of (94.10±2.01) and a clinical operation skill assessment score of (90.44±1.57), which were significantly higher than those of the control group ( P<0.05), and the observation group had a significantly better instructor rating (89.36±1.33) than the control group ( P<0.05). Compared with the control group, the observation group had significantly higher degree of satisfaction with teaching (3.79±0.41), degree of satisfaction with teaching methods (3.92±0.27), and evaluation of classroom learning atmosphere (3.90±0.31) ( P<0.05). Conclusion:CBL combined with online teaching can help to improve learning efficiency, stimulate the enthusiasm for learning, expand clinical thinking, promote the growth of teaching and learning, and form a virtuous cycle among trainees receiving standardized residency training, which holds promise for further exploration.
		                        		
		                        		
		                        		
		                        	
7.Minimally invasive repair of acute closed Achilles tendon rupture with two-way needle suture technique

		                        			Objective:To investigate the efficacy of two-way needle suture technique (TNST) in the minimally invasive repair of acute closed Achilles tendon rupture.Methods:From June 2019 to June 2021, 26 patients with acute closed Achilles tendon rupture were treated at Zhengzhou Orthopedic Hospital. They were 20 males and 6 females, with a mean age of 28 (23, 31) years. The rupture end was (4.2±1.3) cm away from the calcaneal insertion, and the interval from injury to operation 4.3 (2.0, 5.0) d. Preoperative MRI examinations revealed in all the patients closed Achilles tendon rupture which was to be repaired by TNST. The operation time, incision length, incidence of complications, ankle dorsiflexion and plantar flexion were recorded. The Arner-Lindholm scoring was used to evaluate the clinical efficacy.Results:The operation time was (20.0±5.0) min and the incision length (2.5±0.4) cm. Postoperatively, all incisions healed by the first stage, with no complications like incision infection, skin edge necrosis, deep vein thrombosis at lower limbs, injury to the sural nerve, or re-rupture of the Achilles tendon. All patients were followed up for (12.0±6.0) months. At the last follow-up, the patients walked normally, their incisions healed well, the continuity of the Achilles tendon was good by palpation, their heel lift was strong, and all their activities were restored to the levels before rupture of the Achilles tendon. The ankle dorsiflexion was 22.6°±3.7° and the plantar flexion 25.3°±3.7°, According to the Arner-Lindholm evaluation, the clinical efficacy was rated as excellent in 25 cases and as good in 1 case, giving an excellent and good rate of 100% (26/26).Conclusion:In the minimally invasive repair of acute closed Achilles tendon rupture, TNST shows the advantages of limited surgical invasion, a low incidence of postoperative complications, and reliable curative effects.
		                        		
		                        		
		                        		
		                        	
8.Measurement and analysis of 90Y radioactivity in urine of patients treated with 90Y resin microspheres

		                        			Objective:To evaluate 90Y activity concentration in renal excretions during the first 48 hours after being treated with 90Y resin microspheres seleceive internal radiation therapy(SIRI) and to provide advice on the management of patient excreta after surgery. Methods:After surgery, urine excreted from 3 patients during 0-24 h and 24-48 h was collected respectively, and the 90Y activity concentration in urine was tested and analyzed. Results:90Y radioctivity in the urine excreted from 3 patients after surgery was (1 266±258)kBq/GBq during 0-24 h and (140±106) kBq/GBq during 24-48 h, respectively, and 90Y activity concentration were (640±113) kBq/L during 0-24 h and (53±12) kBq/L during 24-48 h. Conclusions:90Y radioactivity in patient′s urine excreted at 1 d was about 10 times higher than that at 2 d. After surgery, patients can accelerate the reduction of free 90Y activity by increasing excretion. Urine excreted by the patients during hospitalization should be handled in accordance with the requirements of the national standard HJ 1188-2021 Radiation protection and safety requirements for nuclear medicine.
		                        		
		                        		
		                        		
		                        	
9.Radiological protection detection and dose assessment of selective internal radiotherapy with 90Y resin microspheres

		                        			Objective:To conduct radiation detection and dose assessment in selective internal radiotherapy with 90Y resin microspheres for the purpose of providing a reference for radiological protection. Methods:The dose rates from radiotherapy with 90Y resin microspheres were detected in the process of each operation at different distances from the body surface of patients the levels of dose to the persons concerned were compared with the relevant national regulations and standards. Results:The estimated dose rates were 1.12-454 μSv/h during 90Y resin microspheres dispensing and 2.06-58.2 μSv/h during surgical procedure. The dose rates at 0.5 h after surgery were 22.7-64.1 μSv/h at 5 cm and 0.82-2.55 μSv/h at 1 m from three patient′s body surface. Assuming treating 200 patients a year, the annual individual effective dose to the radiation workers was 0.12-1.03 mSv/year. The annual individual effective dose to the public, comforters and carers of patients was 0.02-0.24 mSv/year after release of a patient. Conclusions:During the treatment, nursing and release of patients, the radiation doses to workers, carers and the public are lower than the individual dose limit given in the GB18871-2002 basic standards for protection against ionizing radiation and for the safety of radiation sources and the management target value set by of the relevant medical institutions.
		                        		
		                        		
		                        		
		                        	
10.Periosteum-covered iliac crest autografts for treatment of severe osteochondral lesions of talus

		                        			Objective:To explore the clinical efficacy of periosteum-covered iliac crest autografts for treatment of severe osteochondral lesions of talus (OCLTs).Methods:A retrospective case series study was used to analyze the clinical data of 26 patients with severe OCLTs treated at Zhejiang Armed Police Corps Hospital from January 2013 to October 2019. There were 21 males and 5 females,aged 17-49 years [(36.3 ± 10.9)years]. All patients were treated using periosteum-covered iliac crest autografts. The visual analogue scale (VAS),American Orthopedic Foot and Ankle Society (AOFAS) ankle-hindfoot score and ankle joint range of motion (ROM) were assessed before operation,6 months after operation and at the last follow-up (≥ 12 months). The area of talus injury with MRI at the same level was recorded before operation and at the last follow-up. The healing of talus and joint surface was detected with CT at the last follow-up. The healing of the incision and osteotomy site and complications were observed.Results:All patients were followed for 12 to 22 months[(15.1 ± 3.2)months]. The VAS was (2.4 ± 0.9)points and (1.7 ± 0.6)points at postoperative 6 months and at the last follow-up,significantly lower than the preoperative (5.4 ± 1.2)points ( P < 0.01). Meanwhile,the VAS at the last follow-up was significantly lower than that at postoperative 6 months ( P < 0.01). The AOFAS ankle-hindfoot score was (71.7 ± 7.8)points and (87.8 ± 6.2) points at postoperative 6 months and at the last follow-up,significantly lower than the preoperative (66.5 ± 7.5) points ( P < 0.01). Meanwhile,the AOFAS ankle-hindfoot at the last follow-up was significantly lower than that at postoperative 6 months ( P < 0.01). The ankle ROM was (58.4 ± 5.5)° and (70.0 ± 4.9)° at postoperative 6 months and at the last follow-up,significantly improved when compared to the preoperative (42.3 ± 8.1)° ( P < 0.01). Meanwhile,the ankle ROM at the last follow-up was significantly improved when compared to that at postoperative 6 months ( P < 0.01). The area of talus injury with MRI at the same level was 0.67(0.55,0.89)cm 2 at the last follow-up,significantly improved when compared to preoperative 2.64(1.98,3.68)cm 2 ( P < 0.01). The transplantation had neither obvious defects nor joint surface steps based on CT findings. All surgical incisions were healed by first intention. There were no complications such as incision infection,skin necrosis,nonunion of osteotomy,malunion or severe ankle joint disorder except that 8 patients had residual local subchondral bone?marrow?edema-like?signal?and 2 patients showed delayed healing of medial malleolus osteotomy. Conclusion:For patients with severe OCLTs,periosteum-covered iliac crest autografts can effectively relieve ankle pain,improve ankle function,and reduce the area of injury.
