1.Expressions of Mast Cell Tryptase and Brain Natriuretic Peptide in Myocardium of Sudden Death due to Hypersensitivity and Coronary Atherosclerotic Heart Disease

Objective To explore the value of mast cell tryptase and brain natriuretic peptide(BNP)in the differential diagnostic of sudden death due to hypersensitivity and coronary atherosclerotic heart dis-ease.Methods Totally 30 myocardial samples were collected from the autopsy cases in the Department of Forensic Pathology, Shanxi Medical University during 2010—2015. All samples were divided into three groups:death of craniocerebral injury group, sudden death of hypersensitivity group and sudden death of coronary atherosclerotic heart disease group, 10 cases in each group. Mast cell tryptase and BNP in myocardium were detected by immunofluorescence staining and Western Blotting.Results Immunofluo-rescence staining showed that the positive staining mast cell tryptase appeared in myocardium of sudden death of hypersensitivity group and coronary atherosclerotic heart disease group. Among the three groups, the expression of mast cell tryptase showed significantly differences through pairwise comparison(P<0.05); The expression level of BNP in sudden death of coronary atherosclerotic heart disease group were significantly higher than the sudden death of hypersensitivity group and death of craniocerebral injury group(P<0.05). The difference of the expression level of BNP between the sudden death of hypersensitivity group and the death of craniocerebral injury group had no statistical significance(P>0.05).Conclusion The combined detection of the mast cell tryptase and BNP in myocardium is expected to provide help for the forensic differential diagnosis of sudden death due to hypersensitivity and coronary atherosclerotic heart disease.
2.Diagnostic Significance of BAT in Anaphylaxis to Non-ionic Contrast Media

Objective To investigate the diagnostic significance of basophil activation test (B A T) in ana-phylaxis to non-ionic contrast media through testing the content of CD 63, m ast cell-carboxypeptidase A 3 (M C-CPA 3), and term inal com plem ent com plex SC5b-9 of the individuals by testing their levels in the norm al im m une group and the anaphylaxis groups to β-lactam drugs and non-ionic contrast media. Methods The CD 63 expression of basophilic granulocyte in blood w as detected by flow cytom etry. The levels of M C-CPA 3 in blood serum and SC5b-9 in blood plasm a w ere detected by ELISA . Results The CD 63 expression of basophilic granulocyte in blood, the levels of M C-CPA 3 and SC5b-9 of anaphylaxis to non-ionic contrast media and β-lactam drugs w ere significantly higher than that in norm al im m une group (P<0.05). Conclusion There is activation of basophilic granulocytes, m ast cells and com plem ent system in anaphylaxis to non-ionic contrast media. 3.Relative Expression of Indicators for Wound Age Estimation in Forensic Pathology

Objective In order to understand which kind of function genes play an important role for es-timating wound age, the variation of difference genes’ mRNA expression were compared after injury. Methods T he mRNA expression levels of seven candidate genes (ICAM-1, NF-κB, MX2, MT1, MT2, sTnI, and Cox6c) were analyzed in contused rat skeletal muscle at different time points using real-time fluorescent quantitative PC R (R T-qPC R ). T he rawC t values were normalized relative to that of RPL32 mRNA , and converted to standard C t values. A t each time point after injury, the standard deviations (SD ) of the standard C t values were calculated by SPSS. Results T he expression trends of the seven genes were all found to be related to wound age, but there were lower variation coefficients and greater reliability of sTnI and Cox6c when compared with other genes. Conclusion T he genes encoding struc-tural proteins or proteins that performbasic functions can be suitable for wound age estimation.
4.Analysis of 105 Incarcerated Inmate’s Death

Objective To analyze the characteristics in the incarcerated inm ate’s death, investigate the m ain cause of death of the incarcerated inm ate and provide som e inform ation for forensic investigation. Methods The cases from the forensic m edical center of Shanxi Medical U niversity from 2005 to 2013 were selected. The statistical analysis w as perform ed by using the incarcerated inm ate’s gender, age, cause of death, m anner of death, and disease as the m arkers. Results There were 100 men, 5 w omen in the 105 incarcerated inm ates;the age range w as from 16 to 65 years;Inm ates were mostly died of nat-ural diseases, m ainly in the respiratory and cardiovascular diseases;the m ain unnatural death w as suicide w ith a rate of 54.5%. Conclusion Atpresent, most incarcerated inm ate’s death are due to natural dis-eases. The prison should im prove incarcerated inm ate’s lives, w ork and health care conditions, and strengthen supervision of law enforcement.
5.Clinical significance of high sensitive C-reactive protein and immune function in children with Mycoplasma pneu-moniae pneumonia

Objectives To detect the clinical significance of high sensitive C-reactive protein (hs-CRP) and immune function in children with Mycoplasma pneumoniae pneumonia (MPP). Methods 103 children with MPP, 47 cases of systemic inflammatory response syndrome (SIRS group), 56 cases of non-systemic inflammatory response syndrome (non-SIRS group) were recruited. 26 healthy children served as the control group. ELISA was used to detect the level of serum hs-CRP, immune in-dexes, IgG, IgA, and IgM, Cellular immune CD3+, CD4+, CD8+, CD4+/CD8+. Results The level of serum hs-CRP、IgG、IgM and CD8+in control group were significantly lower than those in non-SIRS group and SIRS group (P<0.05). The level of IgA、CD3+, CD4+, CD4+/CD8+in control group were significantly higher than those in non-SIRS group and SIRS group (P<0.05). The level of serum hs-CRP、IgG in SIRS group were significantly higher than those in non-SIRS group (P<0.05). The level of IgA、CD3+、CD4+、CD4+/CD8+in SIRS group were significantly lower than those in non-SIRS group. There was no significant difference in non-SIRS group and SIRS group of IgM and CD8+(P>0.05). The level change of serum hs-CRP were positively related with IgG (r=0.66,P=0.001) and were negatively related with IgA、CD4+、CD4+/CD8+(r=0.79, 0.67, 0.82, P all were<0.05) in chil-dren with MPP. Conclusion Children with MPP have Immunity function( including humoral immunity and cellular immunity) disorder which is related to the disease status. The level of hs-CRP could be an anpation index for the severity and immune func-tion of the children with MPP.
6.Bcl-2 and Bax expression in mice with collagen-induced arthritis following heterogenic umbilical cord blood stem cell transplantation

BACKGROUND: The occurrenceand development of rheumatoid arthritis were strongly associated with unbalance proliferation and apoptosis of synovial cells and lymphocytes. Some synovial cell apoptosis was abnormal. OBJECTIVE: To observe effects of heterogenic double umbilical cord blood stem cell transplantation on Bcl-2 and Bax expression in mice with type Ⅱcollagen-induced arthritis. METHODS: C57BL/6(H-2b) mice were induced with Freund's complete adjuvant and type Ⅱcollagen to establish mouse models of type Ⅱcollagen-induced arthritis.At 2days after secondary immunity incubation, mice were injected with saline via tail vein in the model and normal control groups. Umbilical cord blood hemopoietic stem cells were injected into mouse tail vein in the UBSCs transplantation groups (single dose:2×106/50 g; double dose: 1×106/50 g each, totally 2×106/50 g). Mice were intragastrically administrated methotrexate in the methotrexate positive control group, 0.017 5 g/kg once, once every 5 days, totally six times. RESULTS AND CONCLUSION: Joint tissue below knee and elbow was obtained at42 days following transplantation. Histopathology displayed that smooth and glossy articular surface, no inflammatory cell infiltration in the synovial layer and normal chondrocytes in the normal control group. Hyperplasia, a lot of inflammatory cell infiltration and damaged cartilage surface were visible in the model group. Slight hyperplasia and a little inflammatory cell infiltration were detectable in the methotrexate positive controlgroup and single UBSCs transplantation group. Double UBSCs transplantation group exhibited smooth and glossy articular cartilage surface, no damage, a little inflammatory cell infiltration. Immunohistochemistry demonstrated that Bcl-2 and Bax expression was lower in the double UBSCs transplantation group compared with single UBSCs transplantation group (P < 0.05). Compared with normal control group, no significant difference was detected (P > 0.05). Results suggest that double umbilical cord blood stem cells can induce synovial cell apoptosis in mice with type Ⅱcollagen-induced arthritis in a certain number and action time, and protect synovial membrane against damage.

Method The experimental amniotic embolism model was established by injecting the amniotic fluid or placental extract-amniotic fluid through the rabbit's marginal vein of the ear. Objective Carry out the mast cell tryptase (MCT) granule count in throat, lung and small intestine mast cells in persons died of anaphylactic shock.Method Control group: 10 cases died from severe brain injury. Research group:15 cases died from anaphylactic shock(group A)and 8 cases died of amniotic fluid embolism (group B). H & E staining and immunohistochemical staining were employed and the MCT granule count performed on the throat, lung and small intestine tissue.Results There was marked increase of MCT granule count in the throat (average MCT-positive number of group A 28.48 and of group B 24.35),lung (average MCT-positive number of group A 46.98 and of group B 43.50) and small intestine (average MCT-positive number of group A 48.23 and of group B 42.72) in specimens of group A and group B as compared with the control group (average number in throat 7.97,lung 12.94 and small intestine 20.25).Conclusion Tryptase granular count of MCT increased in throat, lung and small intestine tissue in persons died from anaphylactic shock and amniotic fluid embolism.
9.PATHOLOGICAL CHANGES OF MYOCARDIUM OF THE EXPERIMENTAL RABBITS BLEEDING TO DEATH AND SHOCK

Transmission electron microscopy, histochemstry, immunohistochemistry and morphometry were used to study ultrastructural changes, succinic dehydrogenase (SDH) activity and myoglobin (Mb )content of the rabbit's myocardium bleeding to death and to hemorrhagic shock. The results were as follows: the myocardium showed different degrees of ultrastructural changes; various degrees of decreasing of SDH activity and deletion of Mb. These changes gradually become more serious with lapse of time after shock. The mechanism of the changes and the significance in forensic medicine are discussed.
