1.Analysis of soil-transmitted helminthiasis surveillance in China from 2017 to 2022

Objective To analyze the current epidemic status of soil-transmitted helminthiasis (STHs) in China, providing reference data for the prevention and control of soil-transmitted helminthiasis. Methods Surveillance data on soil-transmitted helminthiasis in China from 2017 to 2022 were collected and statistically analyzed to investigate the infection status, infection trends, and spatial autocorrelation. Results From 2017 to 2022, the average annual infection rate (AAIR) of soil-transmitted helminthiasis was 1.12% (25 123/2 251 632), with hookworm, roundworm, and whipworm having AAIRs of 0.70%, 0.26%, and 0.20% (15 827 cases, 5 836 cases and 4 586 cases), respectively. The infection rate of soil-transmitted helminths decreased annually by an average of 17.16% (AAPC =-17.16, P=0.007), with annual decreases in the infection rates of hookworm, roundworm, and whipworm being 14.70%, 22.13%, and 20.56%, respectively (AAPC=-14.70, P=0.025; AAPC=-22.13, P=0.015; AAPC =-20.56, P=0.029). The spatial distribution of hookworm, roundworm, and whipworm exhibited spatial positive correlation (Moran's I=0.09, P=0.121; Moran's I=0.46, P=0.001; Moran's I=0.23, P=0.013), with roundworm showing the highest spatial aggregation. The AAIRs of males and females were 1.03% (11 308/1 100 016) and 1.20% (13 851/1 149 286), respectively, with statistical significance (χ2=154.19, P<0.001). The highest AAIR was 1.85% (10 949/590 621) in individuals over 60 years old, with statistical significance across different age groups (χ2=4426.44, P<0.001). Hookworm, roundworm, and whipworm were mainly mild infections. The average annual positive rates of soil ascaris eggs and hookworms were 4.06% (649/15 984) and 2.45%(391/15 984), respectively, with statistical significance (χ2=66.16,P<0.001). Conclusions The infection rate of soil-transmitted helminthiasis in China is relatively low and shows a downward trend. Yet, there are local high-prevalence areas, with the elderly and females being the risk-prone populations. Comprehensive measures including expanding the monitoring scope, innovating monitoring technology, and promoting the rural toilet revolution are necessary to block the spread of soil-transmitted helminthiasis further.
2.Incidence trends and spatiotemporal distribution characteristics of dengue fever in China

Abstract: Objective To analyze the incidence trends and spatiotemporal distribution characteristics of dengue fever in China based on the data from the China Disease Control Information System and the "National Statutory Infectious Disease Epidemic Overview" published by the National Health Commission of the People's Republic of China, and to provide a scientific basis for the prevention and control of dengue fever epidemics. Methods The incidence rates and monthly case numbers of dengue fever in China from 2015 to 2023 were collected. Provinces were classified according to the severity of the epidemic and the distribution status of the Aedes vector. The incidence trends, seasonal distribution, and spatial aggregation of dengue fever in China were statistically analyzed. Results The top five provinces with the highest average annual incidence rates from 2015 to 2023 were Yunnan (6.16/100 000), Guangdong (1.70/100 000), Hainan (1.13/100 000), Fujian (1.13/100 000), and Chongqing (0.74/100 000). The average annual incidence rate of dengue fever in China increased by 70.79% from 2015 to 2019 (AAPC=70.79, P=0.045). In Class Ⅰ regions, the incidence of Guangxi and Hainan showed a linear regression trend, with average annual increases of 844.73% (AAPC=844.73, P=0.021) and 516.51% (AAPC=516.51, P=0.013), respectively. In Class Ⅱ regions, except for Shanghai, the incidence of other provinces showed a linear regression trend, with Jiangxi having the highest average annual increase of 610.16% (AAPC=610.16, P=0.021), followed by Chongqing at 345.12% (AAPC=345.12, P=0.038). In Class Ⅲ regions, the incidence of Hebei, Shanxi, and Liaoning had a linear regression trend, with average annual increases of 80.67% (AAPC=80.67, P=0.028), 202.31% (AAPC=202.31, P=0.001), and 70.19% (AAPC=70.19, P=0.031), respectively. The epidemic of dengue fever showed no obvious seasonality in 2021, strict seasonality in 2022, and strong seasonality in the rest of the years. The epidemic peak days were mostly concentrated in the middle and late September to early October of each year, and the epidemic peak periods were mainly concentrated in August-early November. From 2015 to 2018, the incidence rate had no spatial aggregation nationwide, and the incidence rate in 2019 had spatial aggregation nationwide. In 2015-2019 and 2023, there were no high-low gathering areas in the country, and high-high gathering areas appeared in 2018 (Fujian, Hainan) and 2019 (Guangxi). Conclusions The situation of dengue fever prevention and control in China is grim. High-risk epidemic areas of dengue fever will no longer be limited to Class Ⅰ provinces, and the risk of expansion to Class Ⅱ provinces is increasing.
3.SWI/SNF Complex Gene Mutations Promote the Liver Metastasis of Non-small Cell Lung Cancer Cells in NSI Mice.

BACKGROUND:
The switch/sucrose nonfermentable chromatin-remodeling (SWI/SNF) complex is a pivotal chromatin remodeling complex, and the genomic alterations (GAs) of the SWI/SNF complex are observed in several cancer types, correlating with multiple biological features of tumor cells. However, their role in liver metastasis of non-small cell lung cancer (NSCLC) remains unclear. Our study aims to investigate the role and potential mechanisms underlying NSCLC liver metastasis induced by the GAs of SWI/SNF complex.
METHODS:
The GAs of SWI/SNF complex in NSCLC cell lines (H1299, H23 and H460) were identified by whole-exome sequencing (WES). ARID1A knockout H1299 cell was constructed with the CRISPR/Cas9 technology. The mouse model of liver metastasis from NSCLC was established to simulate lung cancer liver metastasis and observe the metastasis rate under different gene mutation conditions. RNA sequencing and Western blot were conducted for differential gene expression analysis. Immunohistochemistry (IHC) analysis was used to assess protein expression levels of SWI/SNF-regulated target molecules in mouse liver metastases.
RESULTS:
WES analysis revealed intracellular gene mutations. The animal experiments demonstrated a correlation between the GAs of SWI/SNF complex and a higher liver metastasis rate in immunodeficient mice. Transcriptome sequencing and Western blot analysis showed upregulated expression of ALDH1A1 and APOBEC3B in SWI/SNF-mut cells, particularly in ARID1A-deficient H460 and H1299 sgARID1A cells. IHC staining of mouse liver metastases further demonstrated elevated expression of ALDH1A1 in the H460 and H1299 sgARID1A group.
CONCLUSIONS
This study underscores the critical role of the GAs of SWI/SNF complex, such as ARID1A and SMARCA4, in promoting liver metastasis of lung cancer cells. The GAs of SWI/SNF complex may promote liver-specific metastasis by upregulating ALDH1A1 and APOBEC3B expression, providing novel insights into the molecular mechanisms underlying lung cancer liver metastasis.

4.Epidemiological investigation and etiological analysis on a case of human infection with avian influenza A (H3N8) virus in Changsha

Objective:To understand the epidemiological and clinical characteristics of human infection with avian influenza A(H3N8) virus and the molecular biological characteristics of the pathogen, and provide scientific evidence for the prevention and control of human infection with avian influenza A(H3N8) virus.Methods:An epidemiological investigation was conducted for a case of human infection with avian influenza A(H3N8) virus in Changsha in May 2022 to collect the information about exposure history, route of infection, onset and treatment, potential contacts and other possible exposures for a descriptive analysis. Nucleic acid detection and gene sequencing were used to detect the pathogen and analyze the genetic characteristics.Results:The case had a history of exposure to live poultry trading market 6 days before the onset of illness. Subtypes H3 and N8 of avian influenza virus were detected in live poultry markets. Deep gene sequencing showed that the virus had adaptive mutations in mammals, reduced sensitivity to alkamine agent, and no resistance mutations related to neuraminidase inhibitors and polymerase inhibitors were detected.Conclusion:The case was infected due to exposure to the live poultry market environment contaminated by avian influenza A(H3N8) virus, and no human to human transmission was found.
5.A multicenter study on human parainfluenza virus infections among children with community-acquired pneumonia from 2014 to 2020

Objective:To investigate the epidemiological and clinical characteristics of human parainfluenza viruses (HPIVs) infection among hospitalized children with community-acquired pneumonia (CAP) in China, and provide basic data for diagnosis, treatment and prevention of HPIVs infection.Methods:From November 2014 to February 2020, 5 448 hospitalized children with CAP were enrolled in 14 hospitals in 11 provinces and municipalities directly under the Central Government in southern China and northern China. Nasopharyngeal aspirates or throat swabs were collected, and the nucleic acids of 18 types respiratory viruses including HPIV1-4 were screened by suspension array technology. Demographic data and clinical information were collected for statistical analysis.Results:The total detection rate of HPIVs in 5 448 children with CAP was 8.83% (481/5 448), and the detection rate in males was higher than that in females (62.79% vs. 37.21%; χ2=0.000, P=0.992). The detection rate of HPIVs in 1~< 3 years age group was higher than that in other age groups, and the difference was statistically significant ( χ2=61.893, P<0.001). The detection rate of HPIVs in the northern region was higher than that in the southern region (9.02% vs 8.65%), but the difference was not statistically significant ( χ2=0.239, P=0.625). The prevalence of HPIV1-4 in northern and southern China was not completely same. HPIV1 was mainly prevalent in autumn in both northern and southern regions. HPIV2 was prevalent in summer in northern China, and the detection rate was low in southern China. HPIV3 reached its peak in both spring and summer in both northern and southern China, but its duration was longer in southern China than in northern China. HPIV4 is mainly popular in autumn in both southern China and northern China. Among 481 children infected with HPIVs, 58.42% (281/481) were infected with HPIV alone, and the main clinical manifestations were cough (90.75%) and fever (68.68%). Out of the HPIV-positive cases, 42.62% (205/481) were co-infected with another type of HPIV or a different virus, while 11.43% (55/481) had co-infections with two or more different viruses. HPIV3 was the most common type of co-infection with other viruses. HPIV3 infection accounted for the largest proportion (76.80%) in 47 HPIVs-positive children with severe pneumonia. Conclusions:HPIVs is one of the most important pathogens causing CAP in children in China, and children under 3 years of age are the main populations of HPIVs infection. The prevalence characteristics of all types of HPIVs in children in the north and south are not completely same. HPIV3 is the dominant type of HPIV infections and causes more severe diseases.
6.MicroRNA-200a Targets Cannabinoid Receptor 1 and Serotonin Transporter to Increase Visceral Hyperalgesia in Diarrhea-predominant Irritable Bowel Syndrome Rats

BACKGROUND/AIMS: MicroRNAs (miRNAs) were reported to be responsible for intestinal permeability in diarrhea-predominant irritable bowel syndrome (IBS-D) rats in our previous study. However, whether and how miRNAs regulate visceral hypersensitivity in IBS-D remains largely unknown. METHODS: We established the IBS-D rat model and evaluated it using the nociceptive visceral hypersensitivity test, myeloperoxidase activity assay, restraint stress-induced defecation, and electromyographic (EMG) activity. The distal colon was subjected to miRNA microarray analysis followed by isolation and culture of colonic epithelial cells (CECs). Bioinformatic analysis and further experiments, including dual luciferase assays, quantitative real-time polymerase chain reaction, western blot, and enzyme-linked immunosorbent assay, were used to detect the expression of miRNAs and how it regulates visceral hypersensitivity in IBS-D rats. RESULTS: The IBS-D rat model was successfully established. A total of 24 miRNAs were differentially expressed in the distal colon of IBS-D rats; 9 were upregulated and 15 were downregulated. Among them, the most significant upregulation was miR-200a, accompanied by downregulation of cannabinoid receptor 1 (CNR1) and serotonin transporter (SERT). MiR-200a mimic markedly inhibited the expression of CNR1/SERT. Bioinformatic analysis and luciferase assay confirmed that CNR1/SERT are direct targets of miR-200a. Rescue experiments that overexpressed CNR1/SERT significantly abolished the inhibitory effect of miR-200a on the IBS-D rats CECs. CONCLUSIONS: This study suggests that miR-200a could induce visceral hyperalgesia by targeting the downregulation of CNR1 and SERT, aggravating or leading to the development and progression of IBS-D. MiR-200a may be a regulator of visceral hypersensitivity, which provides potential targets for the treatment of IBS-D.

7.Diffusion tensor imaging of the central auditory system in the elderly

Objective:To evaluate the changes in the central auditory system of presbycusis by applying auditury brainstem response (ABR) and diffusion tensor imaging (DTI). Method:A total of twenty-five elderly subjects with expressed presbycusis, eight elderly subjects with normal hearing and fifteen healthy volunteers as young controls were enrolled in the study. Each participant was examined using ABR and DTI. ABR measures were obtained from all the subjects. The fractional anisotropy (FA) was measured at two brain regions of the auditory pathway-the inferior colliculus (IC) and Heschl's gyrus. Result:Higher FA values were recorded at the left side of the Heschl's gyrus in all three groups (P<0.05). There were no significant differences between the left and right side of the ABR results and FA values of inferior colliculus in the three groups (P>0.05). Aging increased ABR peak latencies, yet did not change the Ⅰ-Ⅴ interpeak latency interval. Comparing with the elderly subjects with normal hearing group, the wave Ⅴ latencies of the presbycusis group were prolonged (P<0.05). Comparing with the healthy control group, the FA values of the IC were reduced in both of the elderly subjects with normal hearing group and the presbycusis group (P<0.05). No statistically differences were observed between the presbycusis group and the elderly subjects with normal hearing group in the FA values of the IC (P>0.05). Significant differences between all groups were found in the FA values of the Heschl's gyrus, with higher values in the elderly subjects with normal hearing group than in the presbycusis group and even higher values in the healthy control group than in the elderly subjects with normal hearing group (P<0.05). Conclusion:The results indicated that the age-related microstructural changes exist in the central part of the auditory system, which are more obvious in the patients with presbycusis.
8.A model for evaluation of key measures for control of chikungunya fever outbreak in China

Objective To analyze the transmission pattern of Chikungunya (CHIK) fever in community and evaluate the effectiveness of mosquito control, case isolation and other key control measures by using ordinary differential equation (ODE) model.Methods According to natural history of CHIK, an ODE model for the epidemiological anaysis of CHIK outbreak was established.The key parameters of the model were obtained by fitting the model with reported outbreak data of the first CHIK outbreak in China.Then the outbreak characteristics without intervention, the effectiveness of mosquito control and case isolation were simulated.Results Without intervention, an imported case would cause an outbreak in a community with population of 11 000, and cumulative case number would exceed 941 when the total attack rate was 8.55%.The results of our simulation revealed that the effectiveness of case isolation was not perfect enough when it was implemented alone.Although the number of cases could be decreased by case isolation, the duration of outbreak would not be shortened.Differently, the effectiveness of mosquito control was remarkable.In addition, the earlier the measure was implemented, the better the effectiveness would be.The effectiveness of mosquito control plus case isolation was same with mosquito control.Conclusion To control a CHIK outbreak, mosquito control is the most recommended measures.However, case isolation is also necessary as the supplementation of mosquito control.
9.Correlation of diffusion tensor imaging between the cerebral cortex and speech discrimination in presbycusis.

OBJECTIVE:
To investigate the relationship between pure-tone average (PTA), the fractional anisotropy (FA) of the auditory pathway, cognitive cortex and auditory cortex in presbycusis.
METHOD:
Twenty-five elderly subjects with presbycusis were participated in the study. PTA, speech discrimination abilities were evaluated in each subject. Diffusion tensor imaging (DTI) was applied to access the FA of the IC, the superior frontal gyrus and the Heschl's gyrus. Compare the difference between two sides of the values of FA in the three areas. Bivariate correlation analysis was performed to evaluate the effects of PTA and FA of the inferior colliculus (IC), the superior frontal gyrus and the Heschl's gyrus on speech discrimination abilities.
RESULT:
There were no significant differences between the left and right side of the inferior colliculus (P > 0.05). Higher FA values were recorded at the left side of the Heschl's gyrus and the superior frontal gyrus (P < 0.05). Both PTA and the FA of the superior frontal gyrus have a negative association with speech discrimination abilities (P < 0.01, P < 0.05), while the FA of the Heschl's gyrus has a positive association with speech discrimination abilities (P < 0.05).
CONCLUSION
Our findings indicated that the speech discrimination abilities of the elderly is not only related to the peripheral auditory function, but also to the central auditory and cognitive function.

10.Spontaneous brain activity in chronic smokers revealed by fractional amplitude of low frequency fluctuation analysis: a resting state functional magnetic resonance imaging study.

BACKGROUNDNicotine is primarily rsponsible for the highly addictive properties of cigarettes. Similar to other substances, nicotine dependence is related to many important brain regions, particular in mesolimbic reward circuit. This study was to further reveal the alteration of brain function activity during resting state in chronic smokers by fractional amplitude of low frequency fluctuation (fALFF) based on functional magnetic resonance imaging (fMRI), in order to provide the evidence of neurobiological mechanism of smoking.
METHODSThis case control study involved twenty healthy smokers and nineteen healthy nonsmokers recruited by advertisement. Sociodemographic, smoking related characteristics and fMRI images were collected and the data analyzed.
RESULTSCompared with nonsmokers, smokers showed fALFF increased significantly in the left middle occipital gyrus, left limbic lobe and left cerebellum posterior lobe but decreases in the right middle frontal gyrus, right superior temporal gyrus, right extra nuclear, left postcentral gyrus and left cerebellum anterior lobe (cluster size >100 voxels). Compared with light smokers (pack years ≤ 20), heavy smokers (pack years >20) showed fALFF increased significantly in the right superior temporal gyrus, right precentral gyrus, and right occipital lobe/cuneus but decreased in the right/left limbic lobe/cingulate gyrus, right/left frontal lobe/sub gyral, right/left cerebellum posterior lobe (cluster size >50 voxels). Compared with nonsevere nicotine dependent smokers (Fagerstrőm test for nicotine dependence, score ≤ 6), severe nicotine dependent smokers (score >6) showed fALFF increased significantly in the right/left middle frontal gyrus, right superior frontal gyrus and left inferior parietal lobule but decreased in the left limbic lobe/cingulate gyrus (cluster size >25 voxels).
CONCLUSIONSIn smokers during rest, the activity of addiction related regions were increased and the activity of smoking feeling, memory, related regions were also changed. The resting state activity changes in many regions were associated with the cumulative amount of nicotine intake and the severity of nicotine dependence.
