1.Establishment of a standardized management model for postoperative anti-osteoporosis medication in patients with brittle fractures

OBJECTIVE To investigate the establishment and promotion of a new standardized management model for anti- osteoporosis medication after fragility fracture surgery by resident clinical pharmacists, and provide references for resident pharmacists to carry out clinical pharmaceutical services. METHODS From July 2023 to March 2024,595 post-brittle fracture surgery patients were enrolled. Using the PDCA (plan-do-check-act) cycle,resident clinical pharmacists identified issues and conducted investigations in clinical practice. Through integrating clinical pharmacist intervention services before, during and after treatment, a medication treatment pathway was developed, thereby establishing a standardized management model for anti- osteoporosis treatment following fragility fracture surgery. Leveraging the National Brittle Fracture Big Data Platform (under the National Clinical Research Center for Orthopedics and Sports Rehabilitation), a dedicated data module was constructed, providing big data support to evaluate the efficacy of this pharmaceutical care model. RESULTS Continuous PDCA cycle driven improvements significantly increased the proportion of osteoporosis diagnosis (from 9% before intervention to 81%) and proportion of drug treatment (from 4% to 75%).The proportions of bone density and bone metabolism testing also rose markedly,positively impacting long-term patient outcomes. CONCLUSIONS The establishment of a standardized management model for anti- osteoporosis treatment following fragility fracture surgery by resident clinical pharmacists has enhanced clinicians’ diagnostic and therapeutic capabilities for osteoporosis, ensures rational medication use in osteoporosis patients, and demonstrates significant potential for widespread adoption and application.
2.Recreational use of electronic products among high school students in Shanxi Province

Objective:
To investigate the recreational use status of electronic products among high school students in Shanxi Province and the influencing factors for excessive use, so as to provide insights into the promotion of rational use of electronic products among high school students.
Methods:
The high school students from 117 schools in Shanxi Province were selected using the stratified random sampling method, and basic information, lifestyle behaviors and recreational use of electronic products were collected using questionnaire surveys. The prevalence of excessive recreational use of electronic products was analyzed, and the factors affecting excessive recreational use of electronic products among high school students were analyzed using a multivariable logistic regression model.
Results:
A total of 13 804 valid questionnaires were recoverd, with an effective rate of 98.32%. There were 6 634 males (48.06%) and 7 170 females (51.94%), with a median age of 17.00 (interquartile range, 1.00) years. There were 7 024 students in Grade One (50.88%) and 6 780 students in Grade Two (49.12%). The prevalence of recreational use of electronic products was 14.18% (1 958 cases). Multivariable logistic regression analysis showed that males (OR=1.461, 95%CI: 1.325-1.611), students in Grade Two (OR=1.720, 95%CI: 1.559-1.897), students whose parents had below high school education (OR=1.391, 95%CI: 1.156-1.674), students without parental support (OR=1.281, 95%CI: 1.078-1.523), students not living on campus (OR=1.142, 95%CI: 1.026-1.271), students without myopia (OR=1.121, 95%CI: 1.008-1.248), and students with sufficient sleep (OR=1.162, 95%CI: 1.054-1.281) had a higher risk of excessive recreational use of electronic products.
Conclusion
The prevalence of excessive recreational use of electronic products among high school students in Shanxi Province was relatively high, which was related to gender, grade, parental education, parental attitudes, boarding status, myopia and sleep quality.
3.Establishment and evaluation of a streptozotocin-induced diabetic encephalopathy rat model

BACKGROUND:Animal models of diabetic encephalopathy that have been studied mainly include streptozotocin-induced model,high-sugar and high-fat diet-induced model and spontaneous animal model.Establishing a simple,easy,short-cycle,safe and effective model of diabetic encephalopathy can help to explore the subsequent pathogenesis and screen therapeutic drugs. OBJECTIVE:To further explore and evaluate the method of building diabetic encephalopathy rat models. METHODS:Twenty Sprague-Dawley rats were randomly divided into control(n=10)and model(n=10)groups.Rats in the model group were given a single injection of 45 mg/kg streptozotocin in the left lower abdominal cavity,and those in the control group were given the same amount of citrate buffer.During the experiment,the body mass,feed intake,water intake and blood glucose were measured.After 8 weeks,the glucose tolerance and oxidative stress levels were measured,and the pathological changes of brain tissue and the expression of apoptotic proteins were compared between groups. RESULTS AND CONCLUSION:Compared with the control group,the food intake,water intake,encephalization quotient,blood glucose and area under the blood glucose curve were significantly increased in the model group,while the body mass decreased significantly(P<0.01).Histopathological examination of the brain showed that compared with the control group,the number of surviving nerve cells was significantly reduced in the model group(P<0.01),with more significant pathological damage of nerve cells.Compared with the control group,the activities of serum superoxide dismutase,catalase and glutathione in the model group were significantly decreased(P<0.01),and the content of oxidative malondialdehyde was significantly increased(P<0.05).The expression levels of apoptosis-related proteins Bax and Caspase-3 in brain tissue increased in the model group compared with the control group,while the expression of Bcl-2 decreased(P<0.01).In conclusion,an 8-week injection of 45 mg/kg streptozotocin can cause obvious pathological damage to the brain tissue of diabetic rats,to successfully establish the rat model of diabetic encephalopathy.
4.Prevalence of cataracts and the coverage rate of cataract surgery in Ningxia region

Objective:To investigate the prevalence of cataracts, the surgical coverage, and postoperative visual acuity of adults in Ningxia.Methods:A cross-sectional study using multistage cluster random sampling was conducted.Ten survey sites in Ningxia were selected and the population aged 18 years and over was surveyed with questionnaire, height and weight measurements, visual acuity, intraocular pressure, fundus photography and slit-lamp examinations.Cataract prevalence and its influencing factors were analyzed.Cataract prevalence, surgical coverage and presenting visual acuity (PVA) and best corrected visual acuity (BCVA) after surgery were investigated in different age groups of the examined population.The study adhered to the Declaration of Helsinki and was approved by the Ethics Committee of the People's Hospital of Ningxia Hui Autonomous Region (No.[2023]-LL-010).Participants signed informed consent prior to the examination.Results:A total of 6 145 people should be examined, and 5 721 people were actually examined, with an examination rate of 93.10%.The study population consisted of 2 558 males, accounting for 44.71%, and 3 163 females, accounting for 55.28%, with ages ranging from 18 to 93 years old and an average age of (64.27±13.48) years.Among them, 1 180 patients diagnosed with cataract, with a cataract prevalence of 20.62%.The prevalence of cataract increased with age and decreased with education level, showing statistically significant differences ( χ2=1 091.32, 581.92; both at P<0.01).The prevalence of cataract was significantly higher among people with hypertension, diabetes mellitus, hyperlipidemia, and coronary heart disease than those without these diseases ( χ2=274.65, 118.15, 78.05, 182.71; all at P<0.01).Cataract surgery was performed in 245 cases in the cataract patient population, with a surgical coverage rate of 20.76%.Of the 245 cases, 229 cases were implanted with IOLs, with an implantation rate of 93.40%.The social burden rate of cataract blindness was 2.29%, and increased with age.Of the 339 eyes that underwent cataract surgery, 241 had a PVA≥0.3, accounting for 71.09%, and 272 had a BCVA≥0.3, accounting for 80.24%. Conclusions:In Ningxia, cataracts are still the main cause of vision impairment and blindness in the elderly, and the social burden rate of cataract blindness is high.Moreover, the coverage rate of cataract surgery is low, so both the coverage and quality of surgery need improvement.
5.Design and satisfaction investigation of continuing education program based on post competency for hospital pharmacists

Objective:To explore a continuing education program of pharmacy suitable for hospital development based on the professional elements of post competency for hospital pharmacists.Methods:Under the guidance of the thinking of continuing medical education in China, the methods of literature analysis and behavioral event interview were used to screen for the typical indicators of post competency for hospital pharmacists, and a questionnaire survey was conducted using the Likert 5-level scale, with the questionnaire revised by the methods of item analysis (such as the correlation test and the t-test) and exploratory factor analysis based on principal component analysis and promax rotation. In addition, the confirmatory factor analysis was performed for the model fitting indexes such as RMSEA and CFI and the reliability and validity of the model, and a continuing education program was implemented for hospital pharmacists based on the revised questionnaire. Results:Based on the actual work conditions, a questionnaire survey was conducted for 37 elements of post competency for hospital pharmacists, and the item analysis and the exploratory and validation analyses obtained 4 factors covering 31 characteristic items, i.e., theoretical training, practice and application, teaching method, and type of expert. The revised questionnaire showed good reliability and validity, and the degree of satisfaction with this program achieved 97.87%(367/375).Conclusions:The continuing education program based on post competency indicators and the need of hospital pharmacists is accurate and effective and can help to continuously and effectively improve the professional technical level and pharmaceutical service of pharmacists.
6.The neuroprotective mechanism of RKIP signaling pathway expression on microglial polarization in cere-bral hemorrhage rat

Objective This study aimed to investigate the neuroprotective effect of microglia polarization mediated by Raf kinase inhibitor protein(RKIP)intracerebral hemorrhage(ICH)model.Methods Forty-eight adult male Sprague-Dawley(SD)rats were randomly divided into three groups:the Sham+Vector group,the ICH+Vector group,and the ICH+RKIP group,with 16 rats in each group.The collagenase ICH model was established in ICH+Vector group and ICH+RKIP group.Before operation and 1,3,5,and 7 days after operation,8 animals in each group were tested for behavior.Apoptosis of neurons was detected by flow cytometry.Seven days after ICH,the expressions of RKIP,p-p65,and TRAF6 around hematoma were analyzed by protein blot.Results Compared with ICH+Vector group,rats in ICH+RKIP group need less time to find the platform,spend longer time in the target quadrant,and significantly reduce the times of crossing the platform(P<0.05).The number of Nissl corpuscles in ICH+RKIP group was significantly higher than that in ICH+Vector group(P<0.05).In addition,the number of neuronal apoptosis in ICH+RKIP group was significantly lower than that in ICH+Vector group(P<0.05).Compared with Sham group,rats receiving ICH showed a gradual decrease in RKIP expression,and reached the lowest value on the 7th day(P<0.05).Seven days after ICH,the expression of RKIP protein in hematoma of rats in ICH+RKIP group was significantly higher than that in ICH+Vector group(P<0.05),and the expression of p-p65 and TRAF6 protein was significantly lower than that in ICH+Vector group(P<0.05).Compared with ICH+Vector group,the number of iNOS+Ibal1+cells in ICH+RKIP group decreased significantly(P<0.05),while the number of Arg-1+Ibal1+cells increased significantly(P<0.05).Conclusion Up-regulation of RKIP promotes functional recovery after ICH,and its mechanism involves inhibiting TRAF6/NF-κB signaling pathway.
7.Role and mechanism of RhoF-mediated Th17 polarization in development of acute pancreatitis

Objective To investigate the role and mechanism of Rho-related GTP-binding protein F(RhoF)-mediated Th17 polarization in the development of severe acute pancreatitis(SAP).Methods RhoF transgenic mice were randomly divided into control group(n = 10),SAP group(n = 10)and SAP+Y-27632 group(n = 10),and WT mice were randomly divided into control group(n = 10),SAP group(n = 10)and SAP+Y-27632 group(n = 10).SAP models were established in all groups except the control group.The SAP+Y-27632 group was infused with Y-27632 via tail vein after model establishment.T cells in mouse blood samples were isolated for RhoF,p-MYPT1 protein detection and ROCK activity determination,and the percentage of lymphoid CD4+T cells producing IL-17 was analyzed by flow cytometry.Results The expression of RhoF and p-MYPT1 was increased in T cells in the SAP group compared with that in the control group(P<0.01),and the level of RhoF was closely correlated with that of p-MYPT1(P = 0.018).The expression of RhoF protein level in T cells of mice in the RhoF group increased by approximately 30%compared to that in the WT group,and the level of spontaneous IL-17 production by CD4+ T cells was significantly increased(P = 0.003).Compared with WT mice,the histological score of pancreatic injury,the expression of RhoF and p-MYPT1 in T cells,the number of IL-17+ T cells and serum IL-17 level in RhoF transgenic mice were significantly increased(P<0.05).The histological score,RhoF and p-MYPT1 expression of T cells,IL-17+ T cell numbers and serum IL-17 level were significantly lower in the SAP+Y-27632 group than those in the SAP group(P<0.05).Conclusion RhoF/ROCK signaling pathway-mediated polarization of Th17 cells is involved in the pathogenesis of SAP.
8.Wang Xingkuan's Experience in Treating Perimenopausal Women with Burning Mouth Syndrome Based on the Theory of"Yin Injury and Internal Dryness"

This article introduced Professor Wang Xingkuan's academic thoughts and clinical experience in diagnosing and treating burning mouth syndrome(BMS)in perimenopausal women based on the theory of"yin injury and internal dryness".Professor Wang advocates the principle of"treating the root cause of the disease and harmonizing multiple organs".Starting from the unique physiological characteristics of perimenopausal women,he believes that the BMS in perimenopausal women should be attributed to the kidneys,liver,heart,and small intestine.The fundamental cause of the disease is the deficiency of kidney essence and the gradual decline of liver blood,and the key to the onset is the dryness of the kidney,liver,heart,and small intestine.In treatment,the overall strategy is to take"yin injury and internal dryness"as the main guideline,focusing on the liver and kidney,with nourishing and replenishing the liver and kidney as the main approach and clearing heat and moistening as auxiliary methods.The basic formula for treating BMS is a combination of Erdong Decoction and Baihe Dihuang Decoction,which has shown significant clinical efficacy.
9.Expression of NME3 in gastric cancer and its clinical significance

Objective:To investigate the expression level of NME3 in gastric cancer and its correlation with clinicopathological characteristics and prognosis.Methods:A retrospective case series study was conducted. The clinicopathological data of 156 patients with gastric cancer who received radical gastrectomy in Zhejiang Cancer Hospital between January 2013 and December 2017 were collected. The samples of cancer tissues and paracancerous tissues were taken and partial paracancerous tissues were not meet the standard. Finally, immunohistochemical staining was conducted on both cancer tissues (156 cases) and paracancerous tissues (139 cases) to detect the expression of NME3 protein; H scoring system was used to score the expression of NME3 protein and the patients were divided into NME3 high expression group (H score ≥ 6 points) and NME3 low expression group (H score < 6 points). The clinicopathological characteristics of the 2 groups were analyzed. Kaplan-Meier method was used for overall survival (OS) analysis of the 2 groups, and log-rank test was used for comparison. Univariate and multivariate Cox proportional risk models were used to determine the poor independent factors affecting the poor OS in patients with gastric cancer.Results:The median age of the 156 patients was 61 years (53 years, 68 years), including 110 males (70.5%) and 46 females (29.5%). The proportion of patients with NME3 high expression in cancer tissues was lower than that in paracancerous tissues [51.9% (81/156) vs. 75.5% (105/139)], and the difference was statistically significant ( χ2 = 17.60, P < 0.001). The proportion of patients with NME3 high expression in moderate-low differentiation and moderate differentiation group was lower than that of those in low-differentiation group [63.3% (50/79) vs. 39.4% (28/71)], the proportion of patients with NME3 high expression in pTNM staging group Ⅲ-Ⅳ was higher than that of those in pTNM staging group Ⅰ-Ⅱ [55.5% (76/137) vs. 26.3% (5/19)], and the difference was statistically significant (both P < 0.05). The proportion of patients with NME3 high expression was 62.2% (46/74), 52.0% (13/25), 39.3% (22/56), respectively in patients with Lauran intestinal type, mixed type and diffused type, and the differences were statistically significant ( χ2 = 6.69, P = 0.035). In addition, OS of patients with the NME3 high expression group was better than that of those with the NME3 low expression group, and the difference was statistically significant ( P < 0.001). The further analysis of gender subgroup showed that OS of male patients with the NME3 high expression group was better than that of those with the NME3 low expression group, and OS of female patients with the NME3 high expression group was better than that of those with the NME3 low expression group, and the differences was statistically significant (all P < 0.05). Univariate and multivariate Cox regression analysis showed that the expression of NME3 in cancer tissues (high expression vs. low expression: HR = 0.342, 95% CI: 0.207-0.564, P < 0.001), family history (yes vs. no: HR = 2.240, 95% CI: 1.285-3.907, P = 0.004), pN staging (N 2-3vs. N 0-1: HR = 2.133, 95% CI: 1.114-4.083, P = 0.022), pM staging (M 1vs. M 0: HR = 2.761, 95% CI: 1.386-5.500, P = 0.004), carcinoma embryonic antigen (CEA) level (CEA > 5 ng/ml vs. CEA ≤ 5 ng/ml: HR = 1.688, 95% CI: 1.018-2.798, P = 0.042), carbohydrate antigen 125 (CA125) level (CA125 > 35 U/ml vs. CA125 ≤ 35 U/ml: HR = 2.913, 95% CI: 1.403-6.047, P = 0.004) were independent factors influencing OS in patients with gastric cancer. Conclusions:NME3 is lowly expressed in gastric cancer tissues, and it is highly expressed in higher-differentially, late staged and intestinal type gastric cancer. NME3 low expression is an independent risk factor for the poor prognosis of gastric cancer. It is speculated that NME3 may play a inhibitory role in gastric cancer.
10.Analysis of clinical characteristics of food allergy in children

Objective To investigate the food allergy situation of children in Changping District of Beijing,and to explore the influence of allergic family history,gender and mode of delivery on food allergy in children,the distribution of food allergy in different age groups,the types of food allergy that are easy to cause in this area,and the comorbidities of food allergy.Methods A total of 515 children aged 0 to 14 years who were admitted to the general pediatric outpatient and emergency department and inpatient of Beijing Changping Hospital from April to November 2023.Using immunoblotting to detect specific immunoglobulin E in the serum of pediatric patients,and using SPSS 26.0 statistical software to perform binomial tests on gender and delivery mode non parameters;Using custom Excel functions to statistically analyze the family history of allergies,the number of people in different age groups,and the frequency of allergic foods in each group;Use a self-made mini program to statistically analyze the combination of comorbid allergic diseases.Results ① The number of male children with food allergies(306 cases)was higher than that of female children(209 cases),and the difference was statistically significant(P<0.05).(2)There were 109 cases of pediatric patients with parents who had no history of allergic diseases,accounting for 21.17%of the total cases;There were a total of 406 cases where at least one parent had a family history of allergies,accounting for 78.83%of the total cases.Among them,228 cases(44.27%)had one parent with a history of allergies,and 178 cases(34.56%)had both parents with a history of allergies.③ Among 515 children with positive food allergens,there were 10 cases(1.94%)in infancy,37 cases(7.19%)in early childhood,235 cases(45.63%)in preschool,192 cases(37.28%)in school age,and 41 cases(7.96%)in adolescence.The highest positivity rate for food allergens is in milk(444 times),followed by egg white(70 times),cashew nuts(57 times),crab(37 times),beef(26 times),mango(24 times),shrimp(21 times),pineapple(6 times),and shellfish(1 time).Milk protein is the most common allergen in all age groups.(4)Among 515 children with food allergies,399 cases were single food allergies,accounting for 77.47%;116 cases of multiple food allergies(2 or more types of food allergies),accounting for 22.53%.⑤ The most common comorbidity of food allergies is food allergy related gastrointestinal diseases combined with allergic rhinitis,with a total of 267 cases;Secondly,there were 192 cases of allergic rhinitis combined with chronic cough,and 124 cases of food allergy related gastrointestinal diseases combined with chronic cough.Conclusion Milk is the main allergen of food allergy in people of age 14 and under,and gastrointestinal symptoms are the most common in children with food allergy.
