1.Chinesization of the HEMO-FISS-QoL questionnaire and its reliability and validity

Objective:To evaluate the reliability and validity of the Chinese version of HEMO-FISS-QoL(HF-QoL) questionnaire (HF-QoL-C) in the Chinese population with hemorrhoids.Methods:From November 2021 to November 2022, a self-constructed general information questionnaire, HF-QoL-C, and the 36-item short form health survey (SF-36), Goligher classification, and Giordano severity of hemorrhoid symptom questionnaire (GSQ) were used to conduct a questionnaire survey on 760 hemorrhoid patients in the anorectal department of six hospitals. The data was analyzed for reliability and validity using SPSS 21.0 and AMOS 26.0 software.Results:The Cronbach's α coefficient of HF-QoL-C and its dimension ranged from 0.831 to 0.960, and the split coefficient was 0.832-0.915. Four common factors were extracted through principal component exploratory factor analysis. Confirmatory factor analysis indicated acceptable structural validity( χ2/ df=8.152, RSMEA=0.097, CFI=0.881, IFI=0.881, NFI=0.867). HF-QoL-C was correlated with SF36 and GSQ( r=-0.694, 0.501, both P<0.01). There were differences in the total score and dimensional scores of HF-QoL-C between surgical and drug treated patients, different grades of Goligher classification for hemorrhoidal disease, and different ranges of hemorrhoid prolapse (all P<0.001). No ceiling effect was found in the total score and the scores of each dimension(0.3%-2.0%). There was a floor effect in both psychological function and sexual activity dimensions (16.7%, 35.1%). Conclusion:HF-QoL-C has good reliability and validity, which can be used to measure the quality of life of Chinese hemorrhoid patients.
2.Recent progress in the construction and application of patient-derived pancreatic cancer organoid models

Pancreatic cancer is a worldwide medical and health problem.Due to its high heterogeneity and malignant phenotype,the 5-year survival rate of pancreatic cancer is only 7.2%,which is a serious threat to human health.The biological mechanism and drug development research of pancreatic cancer often rely on the traditional two-dimensional culture models(cell lines)and patient-derived xenograft models.However,cell lines lack the three-dimensional environment and heterogeneity of the tumor,while xenograft models have the disadvantages of long culture time,low success rate and difficulty in carrying out high-throughput drug screening.There is an urgent need to develop three-dimensional culture models that can highly reflect the characteristics and molecular variation of pancreatic cancer.As a three-dimensional culture model developed in recent years,human-derived pancreatic cancer organoids are multicellular units extracted from tissue samples and embedded in extracellular matrix gels after mechanical and enzymatic digestion,which can reproduce the histological characteristics and organ characteristics of the original patient,and even have the functions of the original organ.With the continuous development and improvement of pancreatic cancer organoids culture system,its simple,economical and stable culture technology has been gradually established,which promotes the application of human pancreatic cancer organoids to high-throughput drug screening,individualized precision treatment and deeper pathogenesis and targeted drug development research.At the same time,human-derived pancreatic cancer organoids are also a novel semi-in vivo model for the study of clinical molecular typing of pancreatic cancer,especially for the study of etiology,molecular characteristics,histomorphology and somatic mutation burden of patients with high mutation rate and high heterogeneity.The construction of human pancreatic cancer organoid biobank with large sample size is a new platform for biology,basic medicine and clinical science oncology research,which provides a reliable resource for the in-depth study of the pathogenesis of pancreatic cancer and the development of treatment strategies.In summary,the research progress of human pancreatic cancer organoids promotes the application of laboratory models that recapitulate the characteristics of clinical patients,improves the connection between basic laboratory research and clinical patients,and provides clinical sources for drug screening,pathogenesis research,individualized treatment strategies and the construction of biobanks.Therefore,this article reviewed the latest research progress of human pancreatic cancer organoids,in order to provide reference for researchers engaged in the research of human pancreatic cancer organoids.
3.Clinical efficacy of transcatheter tricuspid valve replacement in cardiac implantable electronic lead-related tricuspid regurgitation: A multi-center retrospective cohort study

Objective To analyze the clinical efficacy of transcatheter tricuspid valve replacement (TTVR) in cardiac implantable electronic lead-related tricuspid regurgitation (TR). Methods The patients with severe TR who underwent LuX-Valve TTVR in 9 Chinese medical centers from June 2020 to August 2021 were retrospectively enrolled. They were divided into a cardiac implantable electronic device (CIED) group and a non-CIED group based on whether they had pre-existing CIED implantation. Success of the procedure was defined as safe implantation of the LuX-Valve and complete withdrawal of the delivery system. Prognostic improvement was defined as a decrease of TR grade to≤2+ and an improvement of cardiac function by≥2 grades. Surgical success and postoperative prognosis were compared between the two groups. Results A total of 190 patients were collected, including 50 males and 140 females with a mean age of 66.2±7.8 years. There were 29 patients in the CIED group, and 161 patients in the non-CIED group. In the CIED group, 28 patients were implanted with a permanent pacemaker and 1 patient with a cardioverter-defibrillator. Preoperative New York Heart Association (NYHA) cardiac function class, TR degree, left ventricular ejection fraction, tricuspid annular plane systolic excusion, and cardiac risk scores were comparable between the two groups (P>0.05). Postoperative TR was reduced to≤2+ in all patients, and there was no statistical difference in the incidence of perivalvular leakage between the two groups (P=0.270). Postoperative CT of CIED patients showed the valve was in place, and the lead was not extruded, twisted, or deflected. The in-hospital mortality of the two groups were 10.3% and 1.9%, respectively, and the difference was statistically significant (P=0.047). In addition, there was no statistical difference between the two groups in terms of postoperative improvement of cardiac function and mortality in the 1- and 2-year follow-up. Conclusion TTVR is feasible, safe, and effective in patients with CIED implantation, and the pre-existing lead has no significant effect on the clinical efficacy.
4.The value of myeloid-derived suppressor cell and T lymphocyte subsets in the diagnosis and treatment of active pulmonary tuberculosis

Objective:To investigate the levels and correlation between myeloid-derived suppressor cell (MDSC) and T lymphocyte subsets in peripheral blood of patients with active pulmonary tuberculosis.Methods:A total of 38 patients with active pulmonary tuberculosis in Nanjing Second Hospital from February 2019 to June 2020 were selected as the tuberculosis group, and 23 healthy outpatient physical examination patients were selected as the healthy control group during the same period. The levels of MDSC, clinically related indicators, inflammatory cytokines and lymphocyte subsets were compared between each group, and the correlation between MDSC and lymphocyte subsets was analyzed. Meanwhile, the levels of MDSC and lymphocyte subsets before and after treatment were compared.Results:The MDSC and CRP in tuberculosis group were higher than those in healthy group: (16.41 ± 2.13)% vs. (1.82 ± 0.54)%, (25.42 ± 10.56) mg/L vs. (5.82 ± 1.39) mg/L ( P<0.05). Serum inflammatory cytokines tumor necrosis factor (TNF)-α, interleukin (IL)-6, IL-1β, IL-10 and interferon (IFN)-γ in tuberculosis group were significantly higher than those in healthy control group ( P<0.05). T lymphocyte subsets CD 3+ T cell, CD 4+ T cell, CD 8+ T cell and CD 16/56+ nature killer (NK) cell in tuberculosis group were significantly lower than those in healthy control group ( P<0.05), while the number of CD 19+ B cell was not statistically significant ( P>0.05). Correlation analysis showed that MDSC was negatively correlated with T lymphocyte subsets CD 3+ T cell ( r = -0.73, P<0.001), CD 4+ T cell ( r = -0.68, P<0.001) and CD 8+ T cell ( r = -0.53, P = 0.001), but had no significant correlation with CD 16/56+ NK cell ( r = -0.10, P = 0.561). CD 3+ T cell, CD 4+ T cell, CD 8+ T cell and CD 16/56+ NK cell were significantly different in peripheral blood MDSC before and after treatment ( P<0.05). Conclusions:MDSC, CD 3+ T cell, CD 4+ T cell, CD 8+ T cell and CD 16/56+ NK cell have a guiding role in the diagnosis and evaluation of the curative effect of active pulmonary tuberculosis, with high value in clinical application.
5.Transcatheter tricuspid valve replacement via vena jugularis interna: A case report

We reported a 54-year-old female patient with severe tricuspid regurgitation, who received mechanic valve in the mitral position 15 years ago. The patient’s Society of Thoracic Surgeons score was 8.27%, and was intolerant to open heart surgery, so she was recommended for transcatheter tricuspid valve replacement via right vena jugularis interna. The procedures were guided by echocardiography and X ray fluoroscopy on November 13, 2021, the prosthesis was implanted successfully, and the patient was recoved without any adverse events. After 1 month follow-up, her general condition was apparently improved.

A new definition of metabolic dysfunction-associated fatty liver disease (MAFLD) has recently been proposed. We aim to examine the associations of MAFLD, particularly its discordance from non-alcoholic fatty liver disease (NAFLD), with the progression of elevated brachial-ankle pulse wave velocity (baPWV) and albuminuria in a community-based study sample in Shanghai, China. After 4.3 years of follow-up, 778 participants developed elevated baPWV and 499 developed albuminuria. In comparison with the non-MAFLD group, the multivariable adjusted odds ratio (OR) of MAFLD group for new-onset elevated baPWV was 1.25 (95% confidence interval (CI) 1.01-1.55) and 1.35 (95% CI 1.07-1.70) for albuminuria. Participants without NAFLD but diagnosed according to MAFLD definition were associated with higher risk of incident albuminuria (OR 1.77; 95% CI 1.07-2.94). Patients with MAFLD with high value of hepamet fibrosis score or poor-controlled diabetes had higher risk of elevated baPWV or albuminuria. In conclusion, MAFLD was associated with new-onset elevated baPWV and albuminuria independently of body mass index, waist circumference, and hip circumference. Individuals without NAFLD but diagnosed as MAFLD had high risk of albuminuria, supporting that MAFLD criteria would be practical for the evaluation of long-term risk of subclinical atherosclerosis among fatty liver patients.

8.Comparative study on the shade matching performance of dental students under D55 and D65 light sources conditions.

OBJECTIVE:
This study aimed to compare the effect of D55 and D65 light sources on the visual colorimetry performance of dental students by using a homemade light-source shelf.
METHODS:
Two Vitapan 3D-Master shade guides were randomly selected. One set was used as shade guides. Ten commonly used shade tabs of 2L2.5, 2M2, 2R2.5, 3M2, 3R2.5, 3L1.5, 3R1.5, 3L2.5, 4R1.5, and 4L1.5 were selected from the other set with covered value marks and numbered from 1 to 10. After the colorimetric training, 49 undergraduate dental students were randomly divided into two groups. Each student randomly selected two of the 10 shade tabs, and the colors were subsequently matched under D65 and D55 light sources from a distance of approximately 40 cm. The average color difference (ΔE) between the color selected by each participant and the actual color of shade tabs was calculated. Paired t test was used for statistical analysis.
RESULTS:
The ΔE values between the color selected by each participant and the actual color of the shade tabs under the D55 light source varied from 0 to 6.540. The average value was 2.501. The ΔE values between the color selected by each participant and the actual color of the shade tabs under the D65 light source varied from 0 to 6.610. The average value was 2.530. No statistically significant difference was observed between the results under the two light sources (P=0.921).
CONCLUSIONS
Both D55 and D65 daylight lamps can be used for daily dental colorimetry. These two different color temperatures showed no significant difference.

9.Research progress of three-dimensional finite element analysis in unilateral maxillary defect restorations

Unilateral maxillary defects are common clinical maxillofacial deformities. Because of their large area and the complexity of the maxillary structure, the distribution of pressure from dental prostheses and on the sustentacular tissue is usually uneven, which often results in pain or ulceration of the soft tissue and agomphiasis during the therapeutic process. Recently, the finite element method has been used to guide prosthesis design and implantation. This method is conducive to the restoration and stability of the dental prosthesis and the protection of the remaining tissue, which improves restoration quality and patient satisfaction. This paper summarizes the establishment of a three-dimensional finite element model of unilateral maxillary defects and its application in repairing unilateral maxillary defects with traditional prostheses, implant-supported prostheses and surgical flap transplantation combined with prostheses.
10.Study on the Resistance Mechanisms of Carbapenem-Resistant Klebsiella Oxytoca

Objective To investigate the resistance mechanism of Carbapenem-Resistant Klebsiella oxytoca.Methods Car-bapenem-Resistant Klebsiellaoxytoca were collected from Fujian Medical University Union Hospital.The modified hodge test (MHT)was used for carbapenemase phenotype screening.The minimum inhibit concentration(MIC)was detected using agar dilution method for 1 7 drugs.PCR and DNA sequencing were used to detect commonβ-Lactamase genes and carbapene-mases genes.Conj ugation experiments demonstrated the transferability of the carbapenem-resistant determinants.Results 5 Carbapenem-Resistant Klebsiella oxytoca of 4 isolates were positive detected by MHT.Minimum inhibit concentration was detected by using agar dilution method for 17 drugs.More than 80% isolates were resistance to nine drugs.2 isolates conju-gated successfully of 5 Carbapenem-Resistant Klebsiella oxytoca Isolates.There were 2 isolates included carbapenemases gene (1 isolates were only IMP producers,1 isolate contained the IMP and KPC),3 isolates produce ESBLs gene.Conclution The due to CRE strains isolated from Fujian Medical University Union Hospital may be metallo-enzyme carbapenemase and KPC gene.And the isolate that produce two Carbapenem-Resistant gene had been found in this hospital.
