Objective To investigate the expressions of interleukin-1β(IL-1β), interleukin-6(IL-6) and tumor necrosis factor alpha(TNF-α) in cartilage of children with Kashin-Beck disease(KBD) in order to provide a possible mechanism of the disease. Methods Articular cartilage tissues of 5 KBD children(KBD group) were selected from KBD children autopsy samples keeping in Institute of Endemic Diseases, Medical School of Xi’an Jiaotong University; articular cartilage tissues of 5 normal children ( control group ) were selected from non-KBD areas of Shaanxi Province, three cases were from accident death children, two cases were the samples of congential malformation of six finger. Expressions of IL-1β, IL-6 and TNF-α in the cartilage were detected using immunohistochemistry; the cells of articular cartilage were divided into three areas (superficial zone, middle zone and deep zone) to analyze the expressions of IL-1β, IL-6 and TNF-α. Results The expressions of IL-1β in superficial zone , middle zone and deep zone of articular cartilage of KBD group (63.50 ± 7.19, 54.75 ± 5.50, 66.20 ± 9.91) were significantly higher than those of control group(5.75 ± 1.26, 0.00 ± 0.00, 0.00 ± 0.00, all P<0.05). The expression of IL-6 in superficial zone of articular cartilage in KBD group(55.25 ± 6.24) was significantly higher than that of control group(0.00 ± 0.00, P<0.05). The expressions of TNF-αin all zone of articular cartilage of KBD group(33.25 ± 6.50, 3.75 ± 0.96, 29.80 ± 1.92) were significantly higher than those of control group (3.74 ± 0.82, 0.00 ± 0.00, 0.00 ± 0.00, all P < 0.05). Conclusion The levels of IL-1β, IL-6 and TNF-α are up-regulated in articular cartilage of KBD children, suggesting that cytokines may play an important role in matrix degradation in KBD children cartilage.

ObjectiveTo discuss the effect of preoperative psychological communication on psychological state of elderly patients before destined operation.Methods136 cases of elderly patients underwent elective operation were divided into the observation group and the control group with 68 cases in each group,the control group was given routine nursing of general surgery,the observation group received targeted psychological communication based upon routine nursing for consecutive 3 days before surgery.The psychological state such as anxiety,depression and fear was compared between two groups.ResultsThe anxiety,depression and fear in the observation group was significantly improved compared with the control group.ConclusionsPreoperative appropriate and targeted psychological communication can alleviate the anxiety,depression and fear of the elderly patients with elective operation,and contribute to the smooth operation.

Ninety elderly patients with coronary heart disease were randomly divided into 2 groups:in study group (n = 45) the cognitive-behavioral intervention as well as conventional medical treatment were performed and in control group ( n = 45 ) only conventional treatment was given. Short Form-36 ( SF-36),symptom checklist 90(SCL-90), Hamilton anxiety scale( HAMA), Hamilton depression scale( HAMD)were evaluated before and after intervention, the serum total cholesterol (TC), triglyceride (TG)and body mass index (BMI)were measured, the smoking and drinking behaviors were recorded. After 6-month of intervention the smoking [(0. 6 ± 0. 2)cigarettes/d] and drinking [(10. 1 ± 2. 3)g/d] behaviors, the scores of SF-36 (85 ± 16), SCL-90 (104 ±4), HAMA (10 ±4) and HAMD (12 ±3), the systolic blood pressure [( 123 ± 11 ) mm Hg ( 1 mm Hg = 0. 133 kPa)], diastolic blood pressure [(77 ± 5 ) mm Hg], the fasting blood glucose [(4. 2 ± 2. 6 ) mmol/L] and BMI ( 22.5 ± 0. 5 ) in study group were significantly improved compared to control group ( P ＜ 0. 01 ). The results indicate that cognitive-behavioral intervention can improve the mental state and quality of life in elderly patients with coronary heart disease.

Objective To explore the effects of behavioral intervention on mental heath and quality of life of inpatients with coronary heart disease. Methods Eighty cases with coronary heart disease were randomly divid-ed into intervention group(n=40) and the drug treatment group(n=40). SF-36,SCL-90,EPQ,HAMA,HAMD were applied to assess the mental health state and quality of life. Results After 6-months, the score of SF-36,SCL-90,EPQ,HAMA, HAMD,and N score of EPQ in intervention group were more significantly proved than the drug treatment group SF-36:(84.7±16.2)vs(68.3±10.6); P <0.01; SCL-90:(83.5±4.2)vs(148.6±3.2)], P<0.05;EPQ-N:(43.8±4.2)vs(52.8±3.1), P<0.01;HAMA:(10.4±3.4)vs(15.1±5.1), P <0.01; HAMD: (11.4±3.8) vs (17.9±4.6), P< 0.01). Conclustion Behavioral intervention effectively im-proves mental state and quality of life of patients with coronary heart disease.

Objective To evaluate the clinical application of female orthotopic ileal neobladder.Methods Modified radical cysteetomy plus orthotopic ileal neobladder was performed on 19 female pa-tients with bladder cancer from June 1999 to January 2008.The mean age of the patients was 52(45-66) years,mean course of disease was 4.4 months (16 days-1.9 years).Of all the patients,there were 10 cases with grade 1,7 cases with grade 2 and 2 cases grade 3.According to the UICC stage system,5 patients were T1 stage,12 T2 and 2 T3a.All the patients received modified radical cystecto-my without resection of uterus and anterior vagina,meanwhile the nerves around urethra were protec-ted.0.8-1.2 cm proximal end of the urethra was excised and 30 cm distal ileum was used for the re-construction of the neobladder.Results Sixteen cases were followed up for 6-102 months,mean 71 months.Fifteen patients survived without disease recurrence,1 patient died of myocardial infarc-tion 17 months postoperation.The daytime and night continent rate was 100$,93% at 9 months postoperative.The average voiding volume of the 15 patients was 519.0 ml.The average residual vol-ume was 29.2 ml,and Qmax was 18.6 ml/s.The average filling and voiding pressure was 16.7 cm H2O and 53.0 cm H2O.Intravenous urography showed slight hydronephrosis in 1 case.Conclusion Female orthotopic ileal neobladder could be a good choice because of the continence,fewer complica-tions,lower pressure and enough bladder capacity.

Congenital absence of major salivary glands is a sort of infrequent salivary glands abnormality.This disease can be diagnosed by nuclide raster display and its anatomy image can be identified by CT or MRI.We reviewed the etiological factor,clinical situation,diagnosis and therapy of this disease,and pointed out that gene and development checks are necessary to classify it as a hereditary disease.

OBJECTIVETo prepare purified and concentrated coltivirus high titer antigen in order to further detect antibodies against coltivirus in serum sample of patients.

METHODSThe coltivirus in C6/36 cells was cultured and harvested at different time, and the titer was titrated. The virus was purified and concentrated by polyethylene glycol (PEG), and stored at -20 degrees and 4 degrees, with and without glycerol, respectively, then the titer of coltivirus antigen was tested by indirect ELISA. By using the antigen, coltivirus antibodies in serum samples from both suspected Japanese encephalitis (JE) and viral encephalitis (VE) patients were detected.

RESULTSThe highest titer of coltivirus was found at 3-4 weeks of culturing. The antigen titer could be maintained at least for 6 months, especially antigen with glycerol either at 4 degrees or at -30 degrees even for two years. Totally 1141 serum samples from patients diagnosed clinically as JE and VE were tested. The results showed that 130 samples were coltivirus IgM antibody positive and the average positive rate was 11.4% (130/1141). Among 41 samples of paired-serum from patients in Guangzhou Children's Hospital, 9 samples were positive, the positive rate was 22.0% (9/41) in which 5 samples were diagnosed clinically as VE.

CONCLUSIONSStable and purified coltivirus antigen was obtained in order to test coltivirus antibodies as well as development of kits. Coltivirus probably can cause summer-autumn encephalitis in China.

Objectives:The effects of pingyangmycin(PYM) in the treatment of cavernous hemangioma, strawberry hemangioma and mixed hemangioma in the oromaxillofacial and intraoral regions is evaluated.　Methods:450 cases of hemangioma in the oromaxillofacial regions from January 1993 to January 1999 were reviewed. PYM was injected into the cavity or in the circumference of the hemangioma, and the injection may be repeated every 7～10 days for 3～5 times.　Results:450 patients were followed-up for 6 months～6 years. 86.89% were cured and nearlly cured,and 12% were improved. The total efficiency rate was 98.86%. The cure and elementary cure rates of cavernous hemangioma, strawberry hemangioma and mixed hemangioma were 91.04%, 86.08% and 78.01%, respectively, but the elementary cure rate of the wine color stainscapillary hemangioma was 14.29% only.　Conclusions:This method may be a safe, simple and effective therapy for cavernous hemangioma, strawberry hemangioma and mixed hemangioma in the oromaxillofacial regions.

Objectives:To evaluate the efficiency of packing jaw defects with a composite of coral and bone marrow.　Methods:Twelve patients (9 with jaw cyst and 3 with ameloblastoma) underwent enucleation of jaw lesion and packing with a composite of coral and bone marrow.Repair of the bone defects was evaluated at 1 week,1,6 and 12 months postoperatively by clinical examination and X-ray films.　Results:Wound healing after the operations on 10 patients was uneventful,and definite ossification around the implanted material could be detected at 1 month postoperatively.A lot of bone formation and partial resorption of coral were observed at 6 months postoperatively.Complete resorption of coral and complete bone repair were obtained at 12 months postoperatively.Wound breakdown was observed on two other patients,and the composite had to be removed completely.　Conclusions:A composite of coral and bone marrow may enhances bone healing in jaw defects after cyst or ameloblastoma removal.

Objectives: To study the histopathological grading and the clinical-staging system on the surgical prognosis in patients with mucoepidermoid carcinoma(MEC) of the salivary glands. 　Methods: Analysis were made to the primary sites, clinical findings and the correlation between the histopathological-grading/clinical-staging system and the surgical prognosis in 43 patients with pathological evidence of MEC. 　Results: Recurrence or regional lymph node metastasis were diagnosed in 6 of the 36 MEC patients, among which 4 were well differentiated and 2 were poorly differentiated, 1 was in stage Ⅰ to Ⅱ, and 5 were in the stage of Ⅲ or Ⅳ, there were 23 survivors. Thirteen of the 36 patients died, 4 of whom from MEC and 9 from reasons not related to MEC. All the died were in stage Ⅲ or Ⅳ. 　Conclusions: Recurrence or metastasis of MEC correlates with clinical-staging, but not related to the histopathological grading, this suggested that ablative surgery of the primary tumor and dissection of the regional lymphatic system are required in cases of Ⅲ and Ⅳstage MEC. Postoperative irradiation is proposed in the event of inadequate resection.
