1.Development status analysis and suggestions of TCM pharmacists in Chinese public TCM hospitals

OBJECTIVE To understand the development status and existing problems of traditional Chinese medicine(TCM) pharmacists in public TCM hospitals in China, aiming to provide suggestions for the competent departments to formulate management policies for TCM pharmacists and promote the healthy development of TCM. METHODS The data on the number and professional titles of TCM pharmacists in public TCM hospitals in China from 2019 to 2023 were collected. Descriptive analysis was employed to analyze the number, distribution and professional titles of TCM pharmacists in public TCM hospitals across the country, and to measure the quantity shortfalls of the number of TCM pharmacists in these hospitals. RESULTS From 2019 to 2023, the number of TCM pharmacists in public TCM hospitals in China grew slowly, with an average annual growth rate of 2.56%. However, the proportion of TCM pharmacists to the total number of pharmacists in public TCM hospitals gradually decreased, with an average annual growth rate of -0.65%. In terms of hospital grades, the number of TCM pharmacists in tertiary public TCM hospitals showed positive growth, while those in secondary and primary public TCM hospitals showed negative growth. In terms of hospital types, the average annual growth rate of TCM pharmacists in TCM hospitals was 2.22%, in integrated Chinese and Western medicine hospitals it was 7.97%, and in ethnic minority medicine hospitals it was 2.74%. The development of TCM pharmacists in different provinces was uneven. The annual growth rate of TCM pharmacists in Guizhou exceeded 10%, while the growth rate in Hunan and Heilongjiang was negative. In 2023, the number of TCM pharmacists per thousand population in public TCM hospitals was 0.03, indicating a relatively low staffing level. The professional titles of TCM pharmacists in public TCM hospitals were mainly primary and E-mail:601907549@qq.com intermediate, with a total of 67.33%. According to the calculation that the proportion of TCM pharmacists to pharmacists was not less than 60%, public TCM hospitals and hospitals of integrated TCM and Western medicine should be reconfigured with TCM pharmacists 6 212 and 1 288 people, respectively. CONCLUSIONS The number of TCM pharmacists in public TCM hospitals is growing slowly, with insufficient staffing levels, relatively low professional titles, and uneven distribution and development across provinces. It is suggested that relevant competent departments strengthen policy guidance, increase the attention given by the state level to TCM pharmacists, strengthen the construction of the talent team for TCM pharmacists, improve the quality and optimize the allocation of TCM pharmacist talents in order to promote the high-quality development of TCM services.
2.Amplitude of low-frequency fluctuations of behavioral characteristics in different phenotypes of attention deficit hyperactivity disorder

Objective:
To investigate the difference in behavioral characteristics among different phenotypes of attention deficit hyperactivity disorder (ADHD) using amplitude of low-frequency fluctuation (ALFF), so as to provide insights into clinical differentiation of behavioral characteristics among different phenotypes of ADHD.
Methods:
The children with ADHD admitted to The Affiliated Hospital of Hangzhou Normal University were enrolled and classified into the inattentive type (ADHD-I), hyperactive/impulsive type (ADHD-HI) and combined type (ADHD-C). The reaction time (RT) was measured using integrated visual and auditory continuous performance test, and the mean (RT-mean) and standard deviation of RT (RT-SD) were estimated. In addition, the ALFF was calculated at 0.010 to 0.027, 0.027 to 0.073, 0.073 to 0.167 Hz, and the difference of ALFF was compared among children with different types of ADHD.
Results:
A total of 107 children with ADHD were enrolled, including 95 boys and 12 girls, with a mean age of (8.89±1.93) years. There were 69 children with ADHD-I, 8 children with ADHD-HI and 30 children with ADHD-C. The RT-SD was significantly higher among children with ADHD-C than among children with ADHD-I [(126.003±51.619) ms vs. (97.720±45.302) ms; P=0.007]; however, there was no significant difference in RT-mean among children with various ADHD phenotypes (F=1.386, P=0.255). There was an interaction between frequency and ADHD phenotypes (F=2.754, P=0.032), and the ALFF was significantly higher among children with ADHD-C than among children with ADHD-I at 0.010 to 0.027 [(5 590.567±231.595) ms vs. (4 694.001±154.397) ms; P=0.002] and 0.073 to 0.167 Hz [(4 312.609±174.709) ms vs. (3 690.805±116.473) ms; P=0.005].
Conclusions
The ALFF varies in ADHD phenotypes, and there is a frequency-specific difference.
3.A preliminary study on the dynamic imaging of chest high resolution CT in patients with mild COVID-19

Objective:To explore the evolution of imaging findings on dynamic chest high resolution CT(HRCT) in patients with mild COVID-19.Methods:The data of epidemiology, clinical data and continuous dynamic chest high-resolution CT images of the patients with mild COVID-19 were analyzed retrospectively. Twenty-five patients (including 24 common type and 1 mild) were enrolled in the group, including 14 males and 11 females, with age of (42±12) years and hospital stay of (19±5) days. The basic images and dynamic images of HRCT were analyzed and compared by the radiologists.Results:The clinical manifestations were fever (22 cases), cough (18 cases), expectoration (8 cases), pharyngeal pain (6 cases). Most laboratory tests lacked specificity. There were no significant abnormalities on chest CT of one mild patient. HRCT findings of the common type were as follows: (1) the distribution of the lesions: most of the multiple lesions involved both lungs (19 cases), with average of 3 (3±1) lobes, located in the peripheral pulmonary zone near the pleura (22 cases); (2) the morphology and density of the lesions: most of the lesions were ground glass density foci (22 cases), which were patchy and massive (18 cases), nodular (10 cases) and arc broadband (7 cases); with the development of the disease, the density of some lesions increased with localized pulmonary consolidation (10 cases), accompanied by air bronchus sign (5 cases) and halo sign (5 cases). Dynamic changes of HRCT images in the chest: the positive manifestations were found on the 5th (5, 6) day after the onset of the disease, the progressive time of CT lesions was 5 (5, 7) days, the peak time of CT lesions was 11 (10, 13) days, and the turning time of CT lesions was 9 (8, 11) days.Conclusions:Dynamic chest HRCT can monitor the basic evolution process of the disease in patients with mild COVID-19, and provide a more intuitive basis for clinical early diagnosis and treatment.
4.Real-time measurement of cell contractile force during activation of human hepatic stellate cell line LX-2.

The contractile force of hepatic stellate cells plays a very important role in liver damage, hepatitis and fibrosis. In this paper, a method based on polydimethylsiloxane (PDMS) thin micropillar arrays is proposed to measure the contractile force of human hepatic stellate cell line LX-2, which enables dynamic measurement of the subcellular distribution of force magnitude and direction. First, thin micropillar arrays on glass bottom dish were fabricated using two-step casting process in order to meet the working distance requirement of 100× objective lens. After hydrophilic treatment and protein imprint, cells were seeded on the micropillar arrays. LX-2 cells, which were quiesced by growth in serum-free medium, were activated by adding fetal bovine serum (FBS). The deflections of the micropillars were achieved by image processing technique, and then the contractile force of cells exerted on the micropillars was calculated according to mechanical simulation results, and was analyzed under both quiescent and activated conditions. The experimental results show that the average traction force of quiescent cells is about 20 nN, while the contractile force of activated cells increased to 110 nN upon adding FBS. This method can quantify the contractile force of LX-2 cell on subcellular scale in both quiescent and activated states, which may benefit pathology study and drug screen for chronic liver diseases resulted from liver fibrosis.

5.Bladder autophagy after spinal cord injury

Objective To observe the expression of autophagy-related genes and proteins in the smooth bladder muscle of rats after spinal cord injury (SPI).Methods Twenty-four male Wistar rats were randomly and evenly divided into a model group and a control group.The model group had SPI induced using the modified Allen's method,while the control group was only given laminectomy.Six hours after the operation,the Basso Beattle Bresnahan (BBB) locomotor rating scale was used to evaluate the rats' hindlimb locomotor function.Nissl staining was used to observe the morphological changes in their spinal cords,while Western blotting and immunofluorescence staining were employed to assess the expression of microtubule-associated protein 1 light chain 3 (LC3) and protein 62 (P62).The expression of autography gene Beclin1 mRNA was determined using a reverse transcription polymerase chain reaction (RT-PCR).Results The average BBB score in the model group was significantly lower than in the control group.After Nissl's staining,a decreased number of neurons and Nissl bodies was observed.Western blotting showed that the expression of LC3-Ⅱ had increased significantly and that of P62 had decreased significantly in the model group compared with the control group.The immunofluorescence staining showed LC3 and P62 dots in the bladders' smooth muscle cells.RT-PCR detected significantly higher LC3 and Beclinl mRNA levels in the model group than in the control group;in contrast the average P62 mRNA level was significantly lower.Conclusions Autophagy was activated in rats' bladder muscles after SPI.That may be related to the pathogenesis of a neurogenic bladder after spinal cord injury.
6.Curative effect of ALA-PDT combined with Quxietang douching in the treatment of cervical HPV infection

Objective To investigate the efficacy and safety of ALA-PDT combined with Quxietang douc-hing in the treatment of cervical HPV infection.Methods 208 female cervical PCR-HPV positive patients were randomly divided into treatment group and control group,each group had 104 cases.The control group received 10%(ALA)in cervical for 3 hours,then red light for 30 minutes once a week,a total of 3 times.Treatment group was given 10% (ALA)in cervical for 3 hours,then red light for 30 minutes once a week,a total of 3 times,and douching vagina with Quxietang,one day at a time,stay 3 weeks.After treatment for 3 months,every month,a cervical PCR-HPV check was conducted,summarized virus removal efficiency.Results The virus removal efficiency of treatment group was 89.42%,that of the control group was 73.08%,the difference was statistically significant (χ2 =7.471,P<0.05 ).The patients in the treatment group had light local mild erythema,swelling,burning pain,without treatment subside.Conclusion ALA -PDT combined with Quxietang douching method is efficient,with mild adverse reactions,the treatment group was better than the control group in removing cervical HPV infection and effectively eliminating the hidden trouble of cervical HPV.
7.Protective effects of pretreatment with ifenprodil on learning, memory and cognitive dysfunction induced by sevoflurane in neonatal rats

Objective To evaluate the role and mechanism of ifenprodil, which is the selective antagonist of N-methyl-D-aspartic acid subtype receptor NR2B, in soflurane-induced cognitive dysfunction in neonatal rats.Methods Twenty-eight 7-day-old Sprague Dawley rats, weighing 15-18 g, were randomly divided into 4 groups (n=7 each): control group (group C), ifenprodil group (group I), sevoflurane group (group S) and ifenprodil+sevoflurane group (group IS).Normal saline 0.2 ml was injected intraperitoneally in group C.Specific NR2B receptor antagonist ifenprodil 5 mg/kg was injected intraperitoneally at the corresponding time points in group I.Normal saline 0.2 ml was injected intraperitoneally and 2.0% sevoflurane was inhaled for 4 h in group S.Ifenprodil 5 mg/kg was injected intraperitoneally 2 h before sevoflurance inhalation, and 2.0% sevoflurance was inhaled for 4 h in group IS.The rats were then sacrificed 3 weeks after administration, their brains were immediately removed and hippocampal slices were prepared for electrophyisological experiments.The value of population spike amplitude (PSA) and long-term potentiation (LTP) were measured every 10 minutes.Induced LTP was recorded.Results Compared with group C, the values of PSA and rates of induced LTP were significantly decreased in group S (P<0.01).The values of PSA and rates of induced LTP were significantly increased in group IS than those in group S (P<0.01).Conclusion NR2B receptor is involved in sevoflurance-induced cognitive dysfunction in the neonatal rats.Pretreatment with ifenprodil 5.0 mg/kg can improve the neurotoxicity and protect the brain.
8.Application of micro course in health education for prevention shoulder-hand syndrome in patients with stoke

Objective To explore the clinic effect of micro course in health education for prevention shoulder-hand syndrome in patients with stoke.Method The 42 stoke patients treated in our sector from October 2013 to September 2014 were observed (observation group) under health education by micro course,taking the 45 stoke patients treated in our sector from October 2012 to September 2013 as control group under traditional health education.Heahh education evaluation,occurrence rate of shoulder-hand syndrome,shoulder joint passive range of motion (ROM) and simple test for evaluating hand function (STEF) of the two groups of patients were compared after one and three months,respectively.Results Compared to the control group,the observation group made higher evaluation of health education method,showed lower occurrence rate of shoulder-hand syndrome,better shoulder joint passive ROM and upper limb movement function.In the time point of 1 and 3 months after the intervention,the incidence rate of shoulder-hand syndrome in the observation group was 2.5%(1/40) and 7.5%(3/40) respectively,which was significant lower than that of in the control group (19.05%,8/42 and 47.63%,20/40 respectively),thex2 value was 4.173 and 16.340 respectively,P<0.05.In the time point of 1 and 3 months after the intervention,the passive ROM in the observation group was (172.80 ± 15.18)° and (174.76 ± 7.45)°,which was significant higher than that of in the control group,(143.81 ± 22.76)° and (132.12 ± 16.67)° respectively,the t value was 6.816 and 15.074respectively,P<0.01.Conclusion Micro course used in health education of stroke patients could stimulate their study interests and activity,which played an important role in shoulder-hand syndrome occurrence prevention,shoulder injury reduction and upper limb function restoration acceleration.
9.The effects of citicoline combined with rehabilitative training on neural plasticity after focal cerebral ischemia

Objective To investigate the effects of citicoline combined with rehabilitative training on motor function after focal cerebral ischemia. Methods One hundred and twenty male adult Sprague-Dawley rats ( 3 months old) were subjected to left middle cerebral artery occlusion (MCAO) by suturing. Ninety-six of them were randomly divided into four groups of 24: a control group, a drug group, a rehabilitative training group, and a drug combined with rehabilitative training group. The rats in the control group did not receive any treatment. Three days after reperfusion, those in the drug group received 500 mg/kg of citicoline daily; those in the rehabilitative training group received motor training including balancing, grasping, rotating and walking exercises; those in the combined group received both citicoline and the motor training program. Behavioral tests were administered at the 7th, 14th and 21st days after MCAO. At the same time points, immunohistochemistry was used to detect calcium-binding protein S100 and β-tubulin expression in the peri-ischemia region of the cortex. Results ①As compared with the control group, the drug group at the 7th, 14th and 21st day after MCAO and the other two groups at the 7th day after MCAO showed no significant difference in average behavior scores. But at the 14th and 21st day the rehabilitative training group as well as the drug combined with rehabilitative training group had average behavior scores significantly superior to those of the control group, especially in the drug combined with rehabilitative training group. ②As compared with the control group, the expression of S100 and β-tubulin in the drug and rehabilitative training groups at the 7th day after MCAO showed no significant difference. In the drug combined with rehabilitative training group the average score was significantly higher than in the other groups at the 7th day after MCAO. All the other groups had average scores significantly higher than that of the controls at the 14th and 21st day after MCAO, especially the drug combined with rehabilitative training group. Conclusions Citicoline combined with rehabilitative training can significantly improve motor function recovery in rats. The effect of the combined modalities was more obvious, and the functional enhancement might be partially attributable to the up-regulation of S100 and β-tubulin in the cortex.
10.Ultrasonic diagnosis of nontramatic hiatal gastric hernia of esophagus in middle-aged and elderly people

Objective To discuss the clinical values of transabdominal ultrasonic diagnosis of nontraumatic hiatal gastric hernia of esophagus in middle-aged and elderly people.Methods Thirty-two cases were examined with fasting and filing methods in half-sitting,standing,supine and supine right anterior oblique positions.The findings were contrasted with X-ray barium meal examination,endoscopy and operation.Results Twenty-nine cases were of sliding hiatal gastric hernia of esophagus,3 of impacted hernia.The ultrasonic diagnosis coincidence rate was 100%.The signs and symtoms of acoustic images were widening of esophageal hiatus diaphragmatic muscle(≥1.37 cm),echoes of gastric mucosa were found in superior phrenic hernail sac and inside hernial sac.The display rates were 100%,100%,15.6%respectively.Conclusions The transabdominal ultrasonic diagnosis of nontraumatic hiatal gastric hernia of esophagus in middle-aged and elderly people is brief and accurate.
