1.Value of Revolution CT dual-low technique coronary angiography in coronary heart disease

Objective To investigate the application value of Revolution CT dual-low technique coronary angiography in coronary heart disease.Methods One hundred patients with suspected coronary heart disease were retrospectively selected and randomly divided into control group and observation group,who all were examined by Revolution CT.In the control group(50 cases),the tube voltage was 120 kV,the contrast agent iohexol was 350 mg I/mL,and the dose of contrast agent was 1 mL/kg.In the observation group(50 cases),the tube voltage was 100 kV,the contrast agent iohexol was 350 mg I/mL,and the dose of contrast agent was 0.8 mL/kg.All patients underwent coronary digital subtraction angiography(DSA)examination at the same time.The image quality(subjective and objective evaluation indexes)and radiation dose of the two groups were compared,the diagnostic value of the two methods for coronary artery stenosis was compared,and the diagnostic consistency with coronary DSA between the two methods were compared using the Kappa test,as well as the typical coronary images of the two groups were analyzed.Results There were no significant differences in the sub-jective image quality grade and excellence rate between the observation group and the control group(P>0.05).The CT value and noise of the observation group were higher than those of the control group(P<0.05).There were no statistically significant differ-ences in the signal-to-noise ratio(SNR)and contrast-to-noise ratio(CNR)of the right coronary proximal and the left coronary main trunk between the two groups(P>0.05).Compared with the control group,the volume CT dose index(CTDIvol),dose length product(DLP),and effective dose(ED)were lower in the observation group(P<0.05).Compared with the results of coronary DSA,the positive predictive value,negative predictive value,sensitivity,specificity and accuracy of the two methods were>80%,while there was no significant difference between the two groups(P>0.05).The Kappa value of the observation group versus coronary DSA diagnosis was 0.871,the Kappa value of the control group ver-sus coronary DSA diagnosis was 0.889,and the consistency of both methods versus coronary DSA diagnosis was good.Conclusion Revolution CT dual-low technigue coronary angiography has good diagnostic value for coronary heart disease and can significantly reduce the radiation dose while ensuring image quality.

Patients with severe trauma require an extremely timely treatment and transfusion plays an irreplaceable role in the emergency treatment of such patients. An increasing number of evidence-based medicinal evidences and clinical practices suggest that patients with severe traumatic bleeding benefit from early transfusion of low-titer group O whole blood or hemostatic resuscitation with red blood cells, plasma and platelet of a balanced ratio. However, the current domestic mode of blood supply cannot fully meet the requirements of timely and effective blood transfusion for emergency treatment of patients with severe trauma in clinical practice. In order to solve the key problems in blood supply and blood transfusion strategies for emergency treatment of severe trauma, Branch of Clinical Transfusion Medicine of Chinese Medical Association, Group for Trauma Emergency Care and Multiple Injuries of Trauma Branch of Chinese Medical Association, Young Scholar Group of Disaster Medicine Branch of Chinese Medical Association organized domestic experts of blood transfusion medicine and trauma treatment to jointly formulate Chinese expert consensus on blood support mode and blood transfusion strategies for emergency treatment of severe trauma patients ( version 2024). Based on the evidence-based medical evidence and Delphi method of expert consultation and voting, 10 recommendations were put forward from two aspects of blood support mode and transfusion strategies, aiming to provide a reference for transfusion resuscitation in the emergency treatment of severe trauma and further improve the success rate of treatment of patients with severe trauma.
3.Recent advance in diagnosis, treatment and pathogenesis of Parkinson's disease comorbid depression and anxiety

Parkinson's disease (PD) is a kind of neurodegenerative disease based on damage of striatal nigrostriatal dopaminergic neurons, with motor dysfunction as typical clinical feature. However, PD is often accompanied by a series of non-motor symptoms (NMS) represented by depression and anxiety, which not only interferes with clinical management of PD patients, but also seriously affects quality of life of patients. Pathophysiological mechanism of PD comorbid depression and anxiety is still unclear, but is closely related to the disease process, severity of motor dysfunction, and metabolic disorders of neurotransmitter cytokines. This article reviews the epidemiological characteristics, diagnostic criteria, pathophysiological mechanism and treatment strategies of PD comorbid depression and anxiety, in order to improve the NMS clinical management and quality of life of PD patients.
4.Discovery of novel covalent selective estrogen receptor degraders against endocrine-resistant breast cancer.

Endocrine-resistance remains a major challenge in estrogen receptor α positive (ERα+) breast cancer (BC) treatment and constitutively active somatic mutations in ERα are a common mechanism. There is an urgent need to develop novel drugs with new mode of mechanism to fight endocrine-resistance. Given aberrant ERα activity, we herein report the identification of novel covalent selective estrogen receptor degraders (cSERDs) possessing the advantages of both covalent and degradation strategies. A highly potent cSERD 29c was identified with superior anti-proliferative activity than fulvestrant against a panel of ERα+ breast cancer cell lines including mutant ERα. Crystal structure of ERα‒ 29c complex alongside intact mass spectrometry revealed that 29c disrupted ERα protein homeostasis through covalent targeting C530 and strong hydrophobic interaction collied on H11, thus enforcing a unique antagonist conformation and driving the ERα degradation. These significant effects of the cSERD on ERα homeostasis, unlike typical ERα degraders that occur directly via long side chains perturbing the morphology of H12, demonstrating a distinct mechanism of action (MoA). In vivo, 29c showed potent antitumor activity in MCF-7 tumor xenograft models and low toxicity. This proof-of-principle study verifies that novel cSERDs offering new opportunities for the development of innovative therapies for endocrine-resistant BC.
5.Advances in NLRP3-mediated neuroinflammation in Parkinson disease

Parkinson disease (PD) is a neurodegenerative disorder characterized by striatal dopamine deficiency due to degenerative death of midbrain nigrostriatal dopaminergic neurons and the presence of intracellular α-synuclein aggregates.The inflammation caused by NLRP3 inflammasome mediated by microglia overactivation in the midbrain substantia nigra promotes the degeneration of dopamine neurons and plays a key role in the development of PD.α-synuclein aggregation, mitochondrial dysfunction, autophagy and dopamine receptor defects are key regulators of the degeneration of substantia nigra neurons mediated by microglia NLRP3 inflammasome activation.In this paper, we summarize the role of molecular mechanisms of microglia NLRP3 inflammasome activation and regulation in PD, and discuss the main potential PD therapeutic targets or approaches targeting NLRP3 inflammasome mechanisms, with the aim of providing the theoretical basis and insight for PD treatment.

The article introduces the content and organization of the training program for newly recruited nurses in the three hospitals of Massachusetts General Hospital, Mayo Clinic, and University of Illinois Chicago Medical Center in the United States. This article also summarizes the characteristics and highlights of the program implementation, and discusses and analyzes the training program by combining the current situation of newly recruited nurses training in my country, and makes suggestions for further improvement in the future.
7. Correlation between androgen receptor expression and surrogate molecular subtypes in invasive breast carcinoma

Objective:
To investigate androgen receptor(AR)expression in invasive breast carcinoma and the correlation with surrogate molecular breast carcinoma subtypes.
Methods:
Immunohistochemical staining of AR and other biomarkers was performed in a cohort of 870 cases of primary invasive breast carcinomas collected from August to December, 2016. The association of AR expression with different histological and surrogate molecular subtypes was analyzed.
Results:
The positive expression rate of AR in the immunohistochemistry-based surrogate subtypes was 96.3%(207/215) for Luminal A, 89.8%(378/421) for Luminal B, 82.4%(75/91) for HER2 overexpression and 37.1%(53/143) for triple negative breast carcinoma, with significant differences among the four groups (
8. Clinicopathologic study of infiltrating epitheliosis of the breast

Objective:
To evaluate the morphological and immunohistochemical features of infiltrating epitheliosis and its differential diagnosis.
Methods:
Nine consultation and routine cases of infiltrating epitheliosis diagnosed from January 2015 to December 2016 in Fudan University Shanghai Cancer Center were collected. All tissues were formalin-fixed paraffin-embedded and routinely HE stained. The HE slides were reviewed. Immunohistochemical staining of CKpan, CK7, CK19, CK5/6, CK14, p63, SMMHC, Calponin, ER, PR, HER2, Ki-67 and S-100 protein was performed using Ventana BenchMark automated immunostainer.
Results:
The morphological features of infiltrating epitheliosis included: (1) Florid proliferation of epithelial cells forming solid nests or papillary, glandular and cord-like pattern. The proliferative cells possessed nuclei of varying size and shape without atypia. (2) The stroma was altered, showing varying degrees of fibrosis or sclerosis. (3) The proliferative epithelial nests might flow into the spaces within small ducts and lobules at the periphery of the lesion, resulting in pseudo-infiltration. Immunohistochemically, infiltrating epitheliosis was non-uniformly positive for ER/PR, and was positive for high molecular weight CK5/6 and CK14. Myoepithelial markers p63, SMMHC and Calponin demonstrated intact, partial or entire loss of myoepithelial cells around the epithelial nests. The loss of myoepithelial markers staining was more frequent at the periphery of the lesion. The most important differential diagnoses included invasive ductal carcinoma, ductal carcinoma in situ (DCIS), and low grade adenosquamous carcinoma, etc.
Conclusions
Infiltrating epitheliosis is an important pseudo-infiltrating lesion. The lack of atypia, non-uniform ER/PR expression, positivity for high molecular weight cytokeratins, and the intact to partial to entire loss of myoepithelial markers around the proliferating cell nests are the key points to differentiate it from invasive carcinomas and DCIS.
9.Methodology of the epidemiology studies of personality disorder in senior high school students in Beijing

Objectives: To describe prevalence of personality disorders (PD) and personality dysfunction in senior high school students in Beijing using cross-sectional study, to explore genetic and environmental risk factors of PD in adolescence using a case-control study, to establishand test the hypothesis of genetic and environmental risk factors of PD using a cohort study and describe the natural history of personality dysfunction, and to develop and test an intervention model of mental health education to community adolescents in Beijing using a community intervention trail. Methods: By means of cross-sectional study, case-control study, cohort study, and community intervention trial, 10003 senior high school students and their parents were investigated by Personality Disorder Questionnaire-fourth edition, Parental Rearing Behavior Questionnaire (Egma Minnen av Bardndosnauppfor-stran, EMBU), General Information Questionnaire, and International Personality Disorder Examination (IPDE) to diagnose PD according to the criteria of ICD-10 and DSM-IV. The subjects were followed-up for three years to observe the dynamic change of personal dysfunction and to evaluate the effect of intervention measure of mental health education. The univariate and multivariate analysis methods were applied for data analyses.
10.A cross-sectional study of personality disorder in senior high school students in Beijing

Objectives: To describe prevalence of personality dysfuction and personality disorders (PD) and their distribution in senior high school students in Beijing. Methods: By means of cross-sectional study, 10003 senior high school students were investigated by Personality Disorder Questionnaire-fourth edition (PDQ-4), Parental Rearing Questionnaire (Egma Minnen av Bardndosnauppfor-stran, EMBU), General Information Questionnaire, and International Personality Disorder Examination (IPDE) to diagnose PD according to the criteria of ICD-10 and DSM-IV. Results: In the students of first grade, the mean score of PDQ-4 was (25. 38 ± 9. 64). Male students had higher PDQ-4 scroes than those of female. Students in rural high schools had higher PDQ-4 scroes than those of students in urban high schools. Students in general high schools had the highest PDQ-4 scroes, followed by students in disctrict-level key high schools, and the students from city-level key high schools had the lowest PDQ-4 scroes. The prevalence rate of personality dysfunction was 8. 3% with similar distributions of PDQ-4 scroes by gender, residential area, and rank of school. In the students of third grade, the mean score of PDQ-4 was (23. 10 ± 10. 20). Male students had higher PDQ-4 scroes than those of female. The prevalence rates of personality dysfunction and PD were 5. 8% and 1. 83%, respectively. The correlation coefficients of EMBU had statistical significances and four common factors were identified by factor analysis. The heritability of overall PD of father and mother were 0. 71 and 0. 72, respectively. The heritability of each PD cluster was higher than 0. 6. Conclusion: The prevalence rates of personality dysfunction and personality disorder in Beijing, China is lower than those in other countries. The genetic factor plays an important role in the development of PD.
