Non-small cell lung cancer (NSCLC) is the most common pathological type of lung cancer, most NSCLC patients are at advanced stage at the time of diagnosis. For patients without sensitive driven-oncogene mutations, chemotherapy is still the main treatment at present, the overall prognosis is poor. Improving outcomes and obtaining long-term survival are the most urgent needs of patients with advanced NSCLC. In recent years, immunotherapy has developed rapidly. Immune checkpoint inhibitors (ICIs), especially targeting programmed death-1 (PD-1)/programmed death-ligand 1 (PD-L1), have made a breakthrough in the treatment of NSCLC, beneficial to patients' survival and changed the treatment pattern for NSCLC. It shows more and more important role in the treatment of NSCLC. Led by NSCLC expert committee of Chinese society of clinical oncology (CSCO), relevant experts in this field were organized. On the basis of referring to domestic and foreign literature, systematically evaluating the results of Chinese and foreign clinical trials, and combining the experiences of the experts, the experts group reached an agreement to develop this consensus. It will guide domestic counterparts for better application of ICIs to treat NSCLC.
2.Application of ultrasound elastography in evaluating portal hypertension

Portal hypertension (PH) is a common clinical syndrome, and the main risk factor for death is esophageal variceal bleeding. Therefore, the early and accurate diagnosis of PH can reduce the incidence rate of serious complications and mortality rate. Hepatic venous pressure gradient, as a main method for the diagnosis of PH, has certain limitations in clinical application, and thus it is necessary to explore new noninvasive diagnostic techniques. Up to now, ultrasound elastography has become one of the important methods for the noninvasive assessment of PH. This article elaborates on its current application and prospects in the assessment of PH.
3.Relationship between urinary 8-oxo-Gsn and physical function in the elderly

Objective:To investigate the correlation between urinary 8-oxo-7, 8-dihydroguanosine(8-oxo-Gsn)and physical function in community-dwelling elderly people.Methods:A total of 210 community-dwelling elderly people aged 75 years and over were enrolled in this cross-sectional analysis.According to the scores of short physical performance battery(SPPB), subjects were divided into three groups: the high performance group(summary score 10-12), the intermediate performance group(summary score 7-9)and the low performance group(summary score 0-6). All participants received a comprehensive geriatric assessment.Urinary 8-oxo-dGsn, serum high-sensitivity C-reactive protein(hs-CRP)and white blood cell count were measured.The correlation between urinary 8-oxo-Gsn and physical function was analyzed by using Pearson correlation analysis and the ordered Logistic regression model.Results:The scores of activities of daily living(ADL), instrumental activities of daily living(IADL), grip strength and gait speed were lower in the low performance group than in the other two groups( H=47.002, 110.902, F=11.962, 235.952, all P<0.001). Levels of urinary 8-oxo-Gsn and serum hs-CRP were higher in the low performance group than in the other two groups( F=23.780 and 13.259, both P<0.001). There was no significant difference in levels of urinary 8-oxo-dGsn or white blood cell count between the three groups(both P>0.05). Urinary 8-oxo-Gsn was negatively correlated with gait speed, grip strength and SPPB score( r=-0.559, -0.302 and-0.450, all P<0.001)and was positively correlated with the time of five-times-sit-to-stand test( r=0.290, P<0.001). Adjusting for age, gender and Charlson comorbidity index, the ordered Logistic regression analysis showed that elevated levels of urinary 8-oxo-Gsn( OR=1.38, 95% CI: 1.06-1.77, P<0.05)and hs-CRP( OR=1.23, 95% CI: 1.04-1.48, P<0.05)decreased physical function. Conclusions:Urinary 8-oxo-Gsn is independently associated with the decline of physical function in community-dwelling elderly adults, suggesting that the oxidative stress level is increased in the elderly with poor physical function.
4.Preparation and antimicrobial assay of ceramic brackets coated with TiO2 thin films.

OBJECTIVE: Different methods have been utilized to prevent enamel demineralization and other complications during orthodontic treatment. However, none of these methods can offer long-lasting and effective prevention of orthodontic complications or interventions after complications occur. Considering the photocatalytic effect of TiO2 on organic compounds, we hoped to synthesize a novel bracket with a TiO2 thin film to develop a photocatalytic antimicrobial effect. METHODS: The sol-gel dip coating method was used to prepare TiO2 thin films on ceramic bracket surfaces. Twenty groups of samples were composed according to the experimental parameters. Crystalline structure and surface morphology were characterized by X-ray diffraction and scanning electron microscopy, respectively; film thickness was examined with a surface ellipsometer. The photocatalytic properties under ultraviolet (UV) light irradiation were analyzed by evaluating the degradation ratio of methylene blue (MB) at a certain time. Antibacterial activities of selected thin films were also tested against Lactobacillus acidophilus and Candida albicans. RESULTS: Films with 5 coating layers annealed at 700℃ showed the greatest photocatalytic activity in terms of MB decomposition under UV light irradiation. TiO2 thin films with 5 coating layers annealed at 700℃ exhibited the greatest antimicrobial activity under UV-A light irradiation. CONCLUSIONS: These results provide promising guidance in prevention of demineralization by increasing antimicrobial activities of film coated brackets.

5.Antimicrobial properties of TiO2-xNx antibacterial bracket

BACKGROUND:TiO2-xNx film has good antibacterial effects on Streptococcus mutans, Actinomyces viscosus and Candida albicans. OBJECTIVE:To investigate the antimicrobial property of TiO2-xNx bracket. METHODS: Ten orthodontic patients were selected, and their teeth were divided into two parts by oral midline and respectively treated with ordinary metal brackets and TiO2-xNx bracket. Orthodontic plaque index, gingival index, gingival sulcus bleeding index and enamel demineralization index were detected before, and at 1, 2, 3, 4 weeks after orthodontic treatment. RESULTS AND CONCLUSION:For the ordinary metal brackets, the orthodontic plaque index, gingival index, gingival sulcus bleeding index and enamel demineralization index were al increased after orthodontic treatment (P < 0.05); for the TiO2-xNx bracket, the gingival index was increased at 3 weeks after treatment (P < 0.05), the gingival sulcus bleeding index was increased at 1, 2, 3 weeks after treatment (P < 0.05), and the enamel demineralization index was increased at 2, 3, 4 weeks after treatment (P < 0.005). Compared with the ordinary metal brackets, the orthodontic plaque index and gingival sulcus bleeding index were lower in the TiO2-xNx bracket group at 1-4 weeks after treatment (P < 0.05), and the gingival index and enamel demineralization index were lower in the TiO2-xNx bracket group at 2-4 weeks after treatment (P < 0.05). These findings indicate that the TiO2-xNx film bracket has excelent antibacterial properties, which is better to control and removal dental plaques.
6.A comparison of efficacy of sedation with dexmedetomidine versus midazolam in mechanically ventilated patients

Objective To compare the efficacy of sedation with dexmedetomidine versus midazolam in mechanically ventilated patients.Methods Sixty patients aged 20-64 yr,with body mass index 21-25 kg/m2,APECHE Ⅱ score 10-25,requiring 24 h of nechanical ventilation in intensive care unit (ICU),were randomly divided into 2 groups (n=30 each): midazolam group (group M) and dexmedetomidine group (group D).A loading dose of midazolam 0.05 mg/kg was injected intravenously,followed by infusion at 0.03-0.20 mg· kg-1 · h-1 in group M.A loading dose of dexmedetomidine 1 μg/kg was injected intravenously,followed by infusion at 0.2-0.7 μg· kg-1 · h-1 in group D.Ramsay sedation score was maintained at 2-4.The satisfaction of the doctors in ICU with the sedative efficacy,hypotension and bradycardia were recorded.Delirium was recorded starting from the begging of sedation to 2 h after the end of sedation.The emergence time and occurrence of falling asleep again within 2 h after waking were also recorded.Results Compared with M group,the satisfactory level of the doctors in ICU with the sedative efficacy was significantly increased,the emergence time was significantly shortened and the incidences of falling asleep again within 2 h after waking and delirium were significantly decreased (P < 0.05 or 0.01),and no significant change was found in the incidence of hypotension and bradycardia in group D (P > 0.05).Conclusion The efficacy of sedation with dexmedetomidine is better than that of midazolam in mechanically ventilated patients.

8.The postburn change in hypothalamic paraventricular beta-endorphin and the roles of delta-receptor in scalded rats.

OBJECTIVETo investigate the postburn change in hypothalamic paraventricular beta-endorphin and the roles of delta-receptor in scalded rats.
METHODSMale Sprague-Dawley (SD) rats were randomly divided into 3 groups, i.e. ICI174864, DPDPE and control groups. The rats were inflicted with 20% TBSA of III degree scalding on the back by boiling (100 degrees ) water. The postburn change in the tissue content of the hypothalamic paraventricular beta-endorphin was determined by radioimmuno assay (RIA). The effects of delta-receptor in scalded shock rats were investigated by observing the change of the rats'survival time and cardiac indices after the micro-injection of delta-receptor agonist DPDPE or antagonist ICI174864 into the hypothalamic paraventricle.
RESULTS(1) The tissue content of the hypothalamic paraventricular beta-endorphin increased significantly (P < 0.01) at 1, 2 and 4 postburn hours (PBHs) in the scalded rats. (2) When compared with that of control group, the ratio of the cardiovascular parameters [mean arterial pressure (MAP), dp/dt(max) and HR] were obviously increased at different time points in rats with pre-injection of ICI174864 whereas the ratio was decreased when DPDPE was used. Nevertheless, the change in the heart rate ratio was not obvious whether ICI174864 or DPDPE was used. (3) The average animal survival time in ICI174864 group was much longer than that in DPDPE group.
CONCLUSIONAn excessive increase in hypothalamic paraventricular beta-endorphin was one of the factors leading to the aggravation of burn shock and earlier death. delta-receptor located in the tissue might have played important roles in the mediation of the action of hypothalamic paraventricular beta-endorphin. It is beneficial to antagonize the action of delta-receptor for the correction of burn shock and for the prolongation of the of life of animals.

9.COMPARISON OF EFFECTS OF DIFFERENT OPIOID ANTAGONISTS ON SCALD SHOCK IN RATS

Indexes of cardiovascular function and survival time were obser-vedin rats given the following antagonists respectively after scald injury: anti-p-endorphin serum at 10?l, Naloxone at 2mg, ICI174864 at 0.2mg, or TRH at 2 mg, and half of the doses were administered at 1, 2, 3h after scald. The results showed that the cardiac indexes (dP/dtmax, -dP/dtmax and LVSP)were improved, the decrease of mean arterial pressure ( MAP ) and heart rate ( HR ) were delayed after the injections of anti-p-endorphin serum, naloxone or ICIi748e4, and survival time was significantly prolonged in anti ? - endorphin serum group. TRH had little effect on cardiac indexes, MAP and HR were maintained at high level at earlier period, but sharply sloped down in about 210 min after burn. The result suggests that intraventri-cular administration of anti-?-endorphin, naloxone or ICI174864 had much benefit on scald shock, but TRH was uncertain at least in the treatment of scald shock.
