1.Correlation between resilience,post-traumatic stress disorder and quality of life of front-line medical staff in public health emergencies

Objective:To explore the correlation between post-traumatic stress disorder(PTSD),resilience and quality of life of front-line medical staff in public health emergencies.Methods:From Nov to Dec 2020,the medical staff of 4 COVID-19 designated hospitals in Wuhan were investigated with the general demographic questionnaire,10-item Connor-Davidson Resilience Scale,PTSD Checklist for DSM-5 and Simplify Qualify of Life Scale.Spearman correlation analysis and hierarchical regression analysis were used to investigate the correlation between PTSD,resilience and quality of life.Results:A total of 545 questionnaires were collected in this survey and the valid effective rate was 97.8% (533/545).The score of psychological resilience,PTSD and quality of life of medical staff were 26(20,30),17(8,25),and 20(18,23),respectively.And 13.1% (70/533)of medical staff had obvious PTSD symptoms.There were significant differences in the score of quality of life among medical staff with different genders,occupations and PTSD levels.Spearman correlation analysis results showed that the score of PTSD was negatively correlated with quality of life and psychological resilience(r=-0.488 and-0.464,P<0.01).The score of psychological resilience was positively correlated with the score of quality of life(r =0.578,P<0.01).Psychological resilience and PTSD were important predictors of quality of life,with an explanatory capacity of 37.0% .Conclusions:PTSD is a risk factor for quality of life,and psychological resilience is a protective factor for quality of life.In public health emergencies,improving psychological resilience,preventing and treating PTSD can improve the quality of life of medical staff.
2.Application of Global Leadership Initiative on Malnutrition criteria for the diagnosis of malnutrition in patients with ulcerative colitis

Objective:To investigate the prevalence of malnutrition in hospitalized patients with ulcerative colitis (UC) using the Global Leadership Initiative on Malnutrition (GLIM) criteria, compare the body composition, clinical indicators, and occurrence of poor UC outcomes between the malnutrition group and the non-malnutrition group, and analyze the risk factors of malnutrition.Methods:A total of 104 UC patients hospitalized in the Affiliated Hospital of Qingdao University during 2019 to 2021 and with relevant data collected through UC-sarcopenia Prospective Study were included. Data including nutritional risk screening 2002 (NRS 2002) scores, diagnosis of malnutrition per GLIM criteria, body composition data, clinical data and occurrence of poor outcomes were extracted. All patients were screened for nutritional risk using NRS 2002 at admission, and those at nutritional risk were further evaluated for malnutrition diagnosis per GLIM criteria. The prevalence of malnutrition in UC inpatients, the differences in indicators between malnutrition and non-malnutrition groups, and the risk factors of malnutrition were analyzed.Results:Among the enrolled UC patients , 35 (33.7%) were at nutritional risk, and 30 (28.8%) were diagnosed as malnutrition by GLIM criteria. Compared with the non-malnutrition group, the body composition indexes, including body fat mass, protein, skeletal muscle mass, segmental muscle mass, body water, waist circumference, arm circumference, visceral fat area, basal metabolic rate and body cell mass, all decreased to some extent in the malnutrition group ( P < 0.05). Prealbumin and albumin levels were lower ( P < 0.01). The modified Mayo endoscopic score (MMES), C-reactive protein level and erythrocyte sedimentation rate were higher ( P < 0.01). The rates of re-admission and surgery due to disease activity at 90 days were higher ( P < 0.05). High MMES ( OR =1.534, P = 0.044) and low albumin level ( OR =0.781, P = 0.013) were risk factors for malnutrition in UC patients. Conclusions:GLIM criteria is suitable for the diagnosis of malnutrition in hospitalized UC patients, and malnutrition patients are more likely to experience poor outcomes such as short-term re-admission and surgery. UC patients with higher MMES and lower albumin level are more likely to suffer from malnutrition.
3.A Meta-Analysis of Efficacy and Adverse Effects of Lobaplatin and Cisplatin in the Treatment of Malignant Pleural Effusion.

BACKGROUND:
The aim of this study is to systematically evaluate the efficacy and adverse effects of Lobaplatin and Cisplatin in the treatment of malignant pleural effusion.
METHODS:
The databases of Medline (PubMed), Embase, Web of Science, Cochrane, Wanfang, CNKI and VIP were retrieved so as to search the studies about the randomized controlled clinical trials (RCT) that compared the Lobaplatin and Cisplatin for malignant pleural effusion. The main outcome indicators include objective response rate, complete response, partial response, nephrotoxicity, chest pain, gastrointestinal reaction, myelosuppression, fever response and hepatotoxicity. Relative risk was used as the effect size, which was expressed as 95% confidence interval. The meta-analysis was performed using Stata 14.0 statistical software.
RESULTS:
A total of 12 RCTs and 720 MPE patients were included. The results showed that the ORR (RR=1.27, 95%CI: 1.15-1.40, P<0.001), CR (RR=1.39, 95%CI: 1.09-1.78, P=0.007), PR (RR=1.21, 95%CI: 1.02-1.42, P=0.026) in LBP thoracic perfusion chemotherapy were significantly higher than those in DDP thoracic perfusion chemotherapy. The incidence of nephrotoxicity (RR=0.31, 95%CI: 0.13-0.71, P=0.005) and gastrointestinal reactions (RR=0.44, 95%CI: 0.31-0.62, P<0.001) in the LBP group were significantly lower than those in DDP group.
CONCLUSIONS
Compared with DDP pleural perfusion chemotherapy, the ORR, CR and PR of LBP pleural perfusion chemotherapy for MPE are significantly better than DDP, and its nephrotoxicity and gastrointestinal reactions are remarkably lower than DDP.

4.Study on correlation between serum uric acid level and brachial ankle pulse wave velocity in patients with systemic lupus erythematosus

Objective To investigate the correlation between cumulative serum uric acid (cumUA) and brachial-ankle pulse wave velocity (baPWV).Methods Among the workers who participated in the four health check-up of Kailuan Group from 2010 to 2017,subjects who completed one PWV test were selected.The subjects who met the selection criteria were 20 688,subjects who lacked the first three uric acid tests and sex data were excluded.The subjects who had ischemic stroke (excluding lacunar infarction),transient ischemic attack and myocardial infarction were excluded.Decreased subjects were excluded and the extreme value were also excluded,20 295 subjects eventually meet the inclusion criteria and were included for statistical analysis.Stepwise linear regression,multivariate logistic regression and natural spline function were used to analyze the relationship between cumUA and baPWV and the influence of cumUA on baPWV.Results Among 20 295 subjects,the incidence of baPWV ≥ 14 m/s (criteria for judging atherosclerosis) increased with the increase of cumUA.There was significant difference in the incidence of baPWV ≥ 14 m/s (53.07%,54.35%,56.42%,58.41%,61.91%) among different cumUA partition groups (β=0.11,P<0.01).In stepwise linear regression analysis,after adjusting for other confounding factors,it was found that cumUA was positively correlated with baPWV.In multivariate logistic regression analysis,after adjusting for other confounding factors,the results showed that baPWV ≥aPWVm were all risk factors for the third,fourth and fifth subgroups of cumUA compared with the first subgroup,and the OR05%CI) was 1.35(1.13,1.62) (P=0.01),1.60(1.29,1.97) (P<0.01) and 2.14(1.64,2.80) (P<0.01),respectively.Natural spline analysis exhibited a similar J curve relationship between cumUA and increased baPWV.Conclusion CumUA is a risk factor for increased baPWV.
6.Nursing care of total hip replacement

Objective To explore the effective nursing methods and effect of total hip replacement.Methods A total of 523 patients with artificial hip replacement in our department from January 2010 to December 2013 were selected as the study subjects, and 280 patients in the observation group were treated with stage nursing, and 243 cases in the control group were treated with routine nursing.The rehabilitation of the enthusiasm, complications and rehabilitation effect were compared.Results The degree of demand, the compliance degree and the coordination degree of rehabilitation in the observation group were higher than that in the control group, and the difference was statistically significant (P<0.05).The complications in the observation group were less than those in the control group, and the hip function score and the excellent rate were higher than that in the control group, the differences were statistically significant (P<0.05).Conclusion Stage nursing can improve the rehabilitation of patients with hip joint replacement, reduce postoperative complications and promote the rehabilitation of hip joint function.

8.Evaluation of right heart function in patients with acute pulmonary embolism by 256-slice spiral pulmo-nary artery angiography

Objective:To explore value of 256-slice spiral CT pulmonary artery angiography (CTPA)evaluating right heart function in patients with acute pulmonary embolism (APE).Methods:According to death risk assessment of APE patients, a total of 67 APE patients were divided into high risk APE group (n=41)and non-high risk APE group (n=35).CTPA was used to analyze and compare CTPA obstructive index and right heart function indexes between two groups.Correlations among CTPA obstructive index and right heart function indexes were also analyzed.Results:Compared with non-high risk APE group,there were significant rise in CTPA constructive index [8.58% vs.24.69%],percentages of straight or left protruded ventricular septum (31.46% vs.73.17%)and bronchial artery dilation (5.71% vs.24.69%);Except the supe- rior vena cava diameter,the rest right heart function parameters had difference significance in two groups,P<0.05 or<0.01. Spearman correlation analysis indicated that CTPA obstructive index was significant positively correlated with right ventricular maximal short axis (RVMSA),RV:LV,diameters of main pulmonary artery,superior vena cava and azygos (r=0.684~0.954),and significant inversely correlated with LVMSA (r=-0.786),P<0.01 all.ROC curve analysis indicated that AUCs of CTPA RV:LV (0.949)was the biggest diagnosing APE severity.Conclusion:Right heart func- tion indexes measured by multi-detector CT are accurate and practical indexes evaluating APE severity,and it possesses im- portant significance.
9.Analysis of the current status of the prevalence and its relative factors of non-alcoholic fatty liver disease in urban community in Ningxia province

Objective To investigate the current status of prevalence and its relative factors of non-alcoholic fatty liver disease (NAFLD) in the elderly in urban community in Ningxia province and to compare the differences in prevalence of NAFLD between Han and Hui ethnicities.Methods 1046 residents aged 55 years and over from five communities were selected.Questionnaire survey and clinical examination were conducted in all subjects.Fasting vein blood samples were collected.The serum total cholesterol,triglyceride,high density lipoprotein cholesterol,low density lipoprotein cholesterol were detected.Results There were 1043 subjects with average age of (66.41 6.65) years finishing the whole examination,including 696 (66.7%) Han and 347 (33.3%) Hui people.The total prevalence of NAFLD was 27.0% (286/1043),and there was a significant difference in the prevalence [23.4% (94 cases) vs.29.9% (192 cases),x2 =5.18,P=0.023]between male and female,but no difference between Han and Hui ethnicities [28.2% (98 cases) vs.27.0% (188 cases) x2 =0.17,P=0.675].Logistic regression showed that age(OR=0.96,95% CI:0.94-0.99),BMI(OR=1.31,95% CI:1.24-1.38),TG(OR=1.71,95% CI:1.47-1.98),central obesity (CO)(OR=5.20,95%CI:2.21-12.23) were the factors correlating with NAFLD.Conclusions The prevalence of NAFLD in Hui elderly people was is similar to that in Han elderly people.The elderly people with overweight,central obesity and high serum level of triglyceride and high level TG have higher risk for NAFLD.
10.Application values of multislice spiral CT in children malformation of large blood vessels outside the heart

Objective To analyze the application value of multislice spiral CT in diagnosis of children malformation of large blood vessels outside the heart.Methods The diagnosis results were compared between echocardiography and multislice spiral CT in 110 children with congenital heart disease,and the children had been confirmed by operation.Congenital heart disease was divided three kinds according to the region:heart,connections between heart and large blood vessels and large blood vessels outside the heart.Results One hundred and ten children with congenital heart disease were selected,and they had 366 malformations.The accuracy rates of echocardiography and multislice spiral CT were 90.7% (332/366) and 95.6% (350/366) respectively,and there was no statistical difference (x2 =3.284,P =0.070).In 366 malformations,160 malformations were in heart region.The accuracy rates of echocardiography and multislice spiral CT were 98.1% (157/160) and 97.5% (156/160) respectively,and there was no statistical difference (x2 =0.776,P =0.378).There were 55 malformations in connections between heart and large blood vessels.The accuracy rates of echocardiography and multislice spiral CT were 94.5% (52/55) and 100.0% (55/55) respectively,and there was no statistical difference (x2 =3.083,P =0.243).There were 151 malformations in large blood vessels outside the heart.The accuracy rates of echocardiography and multislice spiral CT were 81.5%(123/151) and 92.1% (139/151) respectively,and there was statistical difference (x2 =7.377,P =0.01).Conclusion Multisltce spiral CT has great application value in diagnosis of children congenital heart disease,especially in malformation of large blood vessels outside the heart.
