1.Comprehensive characterization of genomic features and clinical outcomes following targeted therapy and secondary cytoreductive surgery in OCCC: a single center experience

Objective:
Ovarian clear cell carcinoma (OCCC) is associated with chemoresistance. Limited data exists regarding the efficacy of targeted therapies such as immune checkpoint inhibitors (ICI) and bevacizumab, and the role of secondary cytoreductive surgery (SCS).
Methods:
We retrospectively analyzed genomic features and treatment outcomes of 172 OCCC patients treated at our institution from January 2000 to May 2022. Next-generation sequencing (NGS) was performed where sufficient archival tissue was available.
Results:
64.0% of patients were diagnosed at an early stage, and 36.0% at an advanced stage.Patients with advanced/relapsed OCCC who received platinum-based chemotherapy plus bevacizumab followed by maintenance bevacizumab had a median first-line progressionfree survival (PFS) of 12.2 months, compared with 9.3 months for chemotherapy alone (hazard ratio=0.69; 95% confidence interval [CI]=0.33, 1.45). In 27 patients who received an ICI, the overall response rate was 18.5% and median duration of response was 7.4 months (95% CI=6.5, 8.3). In 17 carefully selected patients with fewer than 3 sites of relapse, median PFS was 35 months (95% CI=0, 73.5) and median overall survival was 96.8 months (95% CI=44.6, 149.0) after SCS. NGS on 58 tumors revealed common mutations in ARID1A (48.3%), PIK3CA (46.6%), and KRAS (20.7%). Pathogenic alterations in PIK3CA, FGFR2, and NBN were associated with worse survival outcomes. Median tumor mutational burden was 3.78 (range, 0–16). All 26 patients with available loss of heterozygosity (LOH) scores had LOH <16%.
Conclusion
Our study demonstrates encouraging outcomes with bevacizumab and ICI, and SCS in select relapsed OCCC patients. Prospective trials are warranted.

4.Budget Impact Analysis of Ferric Derisomaltose for the Treatment of Iron-Deficiency in Malaysia

Introduction: Untreated iron deficiency (ID) can lead to severe anaemia, requiring blood transfusion, or increased
mortality risk. Globally intravenous (IV) iron is increasingly recognised as a recommended option for patients. This
study aims to evaluate the budget impact associated with introducing a new intravenous (IV) iron, ferric derisomaltose (Monofer® [IIM]) as one of the treatment options for the management of ID in the Ministry of Health Malaysia
(MOHM) setting. Methods: A 5-year budget impact model was developed from 2020 to 2024 for patients with ID
that require a high iron dose (≥500 mg), using the perspective of MOHM. The model was built with four external
medical specialists, each with experience and deep knowledge of ID management, to support estimations on the
future development of iron use in Malaysia. Results: Compared to the current market mix with the existing IV iron
products (i.e., iron sucrose and iron dextran), a cost-saving of MYR 53,910 could be achieved with the introduction
of IIM in 2020. The uptake of IIM into MOHM over five years is estimated to lead to an overall budget saving of MYR
11,837,524 over a 5-year time horizon. Conclusion: The use of IIM in place of the current IV iron products in MOHM
resulted in a significant cost saving by reducing the number of visits required to achieve the targeted iron dose and
the shorter IV infusion time with IIM.
5.Evaluation of Dental Students’ Medical History Records on Hypertension and Diabetes Mellitus at The National University of Malaysia

Introduction: A thorough medical history ensures safe dental practice. A good medical history guides clinicians in
risk stratification to avoid medical emergencies and improve preparedness to prevent patient morbidity and mortality. This clinical audit aims to analyse the medical history taken by the dental students in patients with hypertension
and/or diabetes mellitus (DM) and subsequently, recommend improvements in history-taking components in the
dental practice. Methods: Hundred and two patients’ folders from the Faculty of Dentistry were examined by two
independent auditors using a validated history-taking evaluation form. Six components of the medical history were
classified as good or bad practices. Sociodemographic factors and distribution of the American Society of Anesthesiologists (ASA status) were described. The level of completeness of medical history records with years of study was
assessed using the Chi-square test. Results: None of the students met 100% of the components required in medical
history taking. Year three undergraduates performed poorly in the completeness of diagnosis and control of the
medical condition whereby none of them had a good level of practice. The completeness of records did not differ
between years of study except for diagnosis (p=0.026), control (p<0.001) and updating medical history (p=0.009)
whereby the postgraduates had the best practice. Conclusion: This study highlighted marked deficiencies in taking a
thorough medical history. Adaptation of the European Medical Risk Related History (EMRRH) form is recommended
to be implemented in dental schools.

Grey literature is a valuable source of information for evidence synthesis in public health, particularly when swift action is needed to address issues. In 2020, the COVID-19 pandemic was an example where rapid knowledge sharing was quintessential as the world grappled with the management of a novel coronavirus that was spreading at an alarming rate. To document and contextualise the health systems strategies used to address the COVID-19 pandemic in Malaysia from January 2020 to April 2020, we conducted a rapid review of publicly available documents from WHO Global Research on Coronavirus Disease (COVID-19) (WHO database), official government websites and local newspapers. This paper aims to describe the methods and discuss the lessons learnt from the review. In the early stage of the pandemic, published articles in the WHO database focused on clinical knowledge, hence we relied on grey literature as a primary source of information, mainly official government websites, which provided real-time information relevant to our study. Grey literature can be a good source of information for a rapid review of nascent and urgent topics particularly in the area of public health, however, a trade-off between comprehensiveness and efficiency has to be considered.

The complete picture regarding transmission modes of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) is unknown. This review summarises the available evidence on its transmission modes, our preliminary research findings and implications for infection control policy, and outlines future research directions. Environmental contamination has been reported in hospital settings occupied by infected patients, and is higher in the first week of illness. Transmission via environmental surfaces or fomites is likely, but decontamination protocols are effective in minimising this risk. The extent of airborne transmission is also unclear. While several studies have detected SARS-CoV-2 ribonucleic acid in air samples, none has isolated viable virus in culture. Transmission likely lies on a spectrum between droplet and airborne transmission, depending on the patient, disease and environmental factors. Singapore's current personal protective equipment and isolation protocols are sufficient to manage this risk.

9.Impact of Knowledge and Attitudes on Lifestyle Practices in Preventing Type 2 Diabetes Mellitus.

INTRODUCTION:
Diabetes mellitus is a major public health issue in Singapore. To shape healthcare policies for the primary prevention of diabetes, it is crucial to understand Singaporeans' knowledge, attitudes and practices related to diabetes and its prevention. This study aimed to assess the knowledge, attitudes and lifestyles of individuals without diabetes.
MATERIALS AND METHODS:
A cross-sectional household survey was performed between 31 January to 3 February 2019 to examine knowledge, attitudes and practices related to diabetes. Inclusion criteria of the participants included: 1) Singaporeans/permanent residents, 2) between 30 to 64 years old, and 3) who did not have a diagnosis of diabetes. Logistic and linear regression models were used to analyse the association of knowledge and attitudes with physical activity and diet habits, respectively.
RESULTS:
Among 806 participants, 72.2% did not meet the Health Promotion Board's physical activity recommendation. Physical activity was associated with better diabetes knowledge (odds ratio [OR] 5.38, 95% confidence interval [CI] = 1.65-17.53, = 0.049), stronger beliefs in diabetes prevention (OR 3.36, 95% CI = 1.02-11.12, = 0.047) and lower levels of worry about diabetes (OR 0.41, 95% CI 0.17-1.00, = 0.049). Neither knowledge nor beliefs or worries about diabetes was associated with diet.
CONCLUSION
There is a need to reinforce the importance of physical activity and healthy diet in preventing diabetes. Although improving the knowledge level of diabetes may increase physical activity of the population, it is unlikely to improve dietary choices without effective behavior change interventions.
10.Pre-independence medical journals in British Malaya: A content analysis

Introduction:
This study aims to describe the content of several pre-independence medical journals published in
British Malaya.
Methods:
The content of five journals were retrieved from National Library of Singapore e-resources and print collection of the Reference Library, Institute for Medical Research, Kuala Lumpur, Malaysia. The content of these journals was classified and descriptive analysis performed.
Results:
The five journals identified were Journal of the Straits Medical Association, Journal of the Malaya Branch of the British Medical Association, Malaya Medical Journal, Malayan Medical Journal and Journal of the Malaya Branch of the British Medical Association (new series). A total of 81 issues with 873 articles were retrieved. Almost one-third of articles were concerned with these topics: malaria, mosquito control, beri-beri, tuberculosis, sexually transmitted diseases, helminthiasis, leprosy and cholera. One-fifth of the articles were case reports and 95% of the articles were written by single authors.
Discussion
The content of these journals is a good resource for those who are interested in the history of medicine – it provided substantial details on the state of public health and chronicled the medical writings of medical workers in British Malaya. It illustrates well the introduction of Western medicine to tackle health problems that arose from the interplay of immigration, poor environmental sanitation and economic exploitation.
