2.Analysis of vaccination recommendations and follow-up for children with special health status

Objective:To analyze the vaccination recommendations and follow-up for children with special health status.Methods:In this retrospective cohort study, 509 children who attended the Consultation Clinic of Vaccination for Special Health Children in Beijing Children′s Hospital from August 2020 to February 2023 were selected, the children were given vaccination planning advice after the assessment. The clinical data were collected, including the general situation, special health conditions, vaccination recommendations and implementation status, occurrence and outcomes of suspected adverse events following immunization (AEFI) after vaccination. The vaccination situation and safety in these children were evaluated.Results:Among the 509 children, the most common special health conditions were cardiovascular system diseases (103 cases), followed by neurological diseases (88 cases) and neonatal problems (82 cases). After comprehensive evaluation and multidisciplinary collaboration, 399 children (78.4%) were recommended to receive vaccination/catch-up vaccination according to the immunization program, 63 children (12.4%) were recommended to receive some vaccines but temporarily suspend others, and 47 children (9.2%) were recommended to temporarily suspend vaccination. A total of 449 children (88.2%) were actually vaccinated, AEFI occurred in 49 children and 45 cases were considered as general reactions.Conclusions:The majority of children with special health status can be vaccinated, and the overall compliance and safety are high. The individualized immunization evaluation model of multidisciplinary collaboration is conducive to the completion of the immunization program of children with special health status.
3.Impact of overweight or obesity on outcome in patients with hypertensive intracerebral hemorrhage

Objective:To investigate the correlation between overweight or obesity defined by body mass index (BMI) and functional outcome in patients with hypertensive intracerebral hemorrhage (HICH).Methods:Patients with HICH admitted to the Department of Neurosurgery, the First People's Hospital of Lianyungang from October 2020 to February 2022 were included retrospectively. According to BMI, they were divided into underweight or normal group (<24 kg/m 2) and overweight or obese group (≥24 kg/m 2). At one year after onset, the functional outcome were evaluated using the modified Rankin Scale. 0-2 was defined as good outcome, while >2 were defined as poor outcome. Cox proportional hazards regression model was used to analyze the correlation between BMI and the outcome of patients. Results:A total of 394 patients with HICH were enrolled, including 263 males (66.8%), aged 60.2±12.0 years. At one year after onset, 145 patients (36.8%) had poor outcome and 54 (13.7%) died. Compared with the underweight or normal group, the overweight or obese group had a higher proportion of previous type 2 diabetes history, as well as higher baseline diastolic blood pressure, low-density lipoprotein cholesterol and liver enzyme levels, better early outcome after discharge, and lower mortality at 1 year (all P<0.05). Multivariate analysis showed that the overweight or obese group had significantly better functional outcome compared to the underweight or normal group (hazard ratio 0.598, 95% confidence interval 0.419-0.854; P=0.005), but there was no statistically significant difference in all-cause mortality risk (hazard ratio 1.201, 95% confidence interval, 0.462-3.126; P=0.707). Conclusion:The overweight or obese patients with HICH have significantly better functional outcome at one year after onset, indicating that the obesity paradox also exists in patients with HICH.
4.Application effect of"three-station"teaching rounds in standardized training of general surgery resi-dents

Objective To explore the application effect of"three-station"teaching rounds in standardized training teach-ing rounds of general surgery residents.Methods 50 trained doctors from the general surgical base from July 2022 to July 2023 were randomly selected and divided into two groups,with 25 patients in each.The observation group used a"three-station"teaching ward round,while the control group used a traditional teaching ward round mode.The exam scores,ward round effects,satisfaction,and experience were compared.Results The total scores of the observation group were significantly higher than those of the control group(P<0.05),and the scores of the medical history collection and physical examination in the case anal-ysis test and skill test were significantly higher than those of the control group(P<0.05).There was no significant difference in the theoretical test scores between the two groups(P>0.05).The proportion of"Yes"in the questionnaire of the observation group was higher than that of the control group in the aspects of improving the diagnosis and treatment of diseases,clinical think-ing,doctor-patient communication ability and increasing learning interest(P<0.05).The satisfaction of questionnaire teaching round in the observation group was 84.0%,significantly higher than that in the control group(60.0%)(P<0.05).In the observation group,the percentages of the attending physicians and the teaching rounds were 64.0%and 68.0%,respectively,which were significantly higher than those of the control group(28.0%and 32.0%,P<0.05).Conclusion The"three-sta-tion"teaching ward round can improve the exam scores,ward round effectiveness and satisfaction,improve the experience of attending physicians and teaching ward round subjects,and is worth promoting in various resident training bases.
5.Correlation between overactive bladder and allergies in children

Objective To assess the clinical correlation between overactive bladder(OAB)and allergies in children.Methods The clinical characteristics of 663 children diagnosed with OAB in outpatient clinics between January 2020 and January 2022 were retro-spectively analyzed,and logistic regression was used to analyze the risk factors for OAB and evaluate the effect of antihistamine drugs on OAB.Blood and urine samples of some children with OAB were collected to analyze the expression levels of bradykinin and substance P.Results A history of eczema,urticaria,pruritus,mosquito bites,allergic rhinitis,allergic cough with asthma,food allergies,constipation,and total blood IgE levels were risk factors for OAB in children(P<0.05).The efficacy rate of antihistamine treatment in children with OAB was 95.5%,and no adverse reactions were observed,indicating that the efficacy and safety of OAB in children receiving antihista-mine treatment were good,and the proportion of children with OAB and urticaria in the antihistamine treatment group was higher(P<0.05).The total blood IgE levels in children with OAB in the effective group of antihistamine treatment were higher than those in the ineffective group(P<0.05).In addition,the expression level of bradykinin in the urine of children with OAB was significantly higher than in healthy children(P<0.05),while the level of substance P was not significantly increased(P>0.05).Conclusion OAB in chil-dren are related to their allergic status,children's previous respiratory,digestive tract,and skin allergies,and significantly elevated blood IgE levels are risk factors for OAB in children.Antihistamine treatment of OAB in children is safe and effective.OAB in children may be related to urine bradykinin.
6.Characteristics of neuropsychological development in infants with different types of morphological cranial malformation

ObjectiveTo compare the neuropsychological development of infants with different types of morphologic cranial deformities. MethodsA total of 954 children aged 0 to 18 months who came to Beijing Children's Hospital from January, 2020 to August, 2021 for cranial measurement and neuropsychological development measurement were selected. They were divided into brachycephaly group, plagiocephaly group, asymmetric brachycephaly group, scaphocephaly group and normal group according to the cranial measurement. The development quotient (DQ) was calculated from Children Neuropsychological Development Scale (0-6). ResultsThere were 449 cases in the normal group, 94 cases in the brachycephaly group, 201 cases in the plagiocephaly group, 82 cases in the asymmetric brachycephaly group and 128 cases in the scaphocephaly group. The detection rate of Developmental Edge and Delay (DQ < 85) for gross motor area was the most in brachycephaly group (60.6%), and it was the most for fine motor (64.6%), language (45.1%), adaption (51.2%) and social behavior areas (48.8%) in the asymmetrical brachycephaly group. The DQ was different among the five groups for all the areas except the language area (F > 14.835, P < 0.001); compared with the normal group, DQ decreased for all the four areas in all the groups except the scaphocephaly group; DQ of the areas of gross motor, fine motor and adaption was more in the plagiocephaly group than in the asymmetric brachycephaly group (P < 0.05), while DQ of the areas of gross motor and fine motor was more in the plagiocephaly group than in the brachycephaly group (P < 0.05). Linear regression analysis showed that, DQ negative linear correlated with the cephalic ratio and cranial vault asymmetry index (|B| > 0.967, P < 0.05). ConclusionAmong four kinds of cranial malformation in infants, the neuropsychological development of the scaphocephaly group is almost normal, and somehow delays for brachycephaly, plagiocephaly and asymmetric brachycephaly, especially in the aspects of gross motor, fine motor, adaption and social behavior. The more serious the cranial deformity, the greater the risk of developmental delay in each functional area.
7.Relationship between exposure to mobile phones and other electronic products and the ability development in children

Objective:To identify the relationship between exposure to mobile phones and other electronic products and the ability development in children.Methods:Retrospective study.A total of 218 children aged 0.5-6.0 years presented to the Department of Health Care, Beijing Children′s Hospital, Capital Medical University form September 2019 to June 2020 for developmental examination were included.Those with nervous system, mental system diseases, endocrine system diseases and premature children were excluded.Their development was evaluated and the information about exposure to mobiles phones and other electronic products were collected. Chi- square test and Fisher′ s exact probability method were used to analyze the relationship between the exposure to mobile phones and other electronic products and the ability development in children.The influencing factors of children′s development in various fields were analyzed by the binary Logistic regression. Results:Abnormal fine movements of children aged 0.5-0.9 years were related to the latest time every night of exposure ( P<0.05). Abnormalities of adaptability, language and social self-care in children aged 1.0-2.9 years and abnormality of language in children aged 3.0-5.9 years were correlated with the number of hours of daily exposure (all P<0.05). Abnormalities of language and social self-care in children aged 1.0-2.9 years and abnormalities of adaptability, language and social self-care in children aged 3.0-5.9 years were correlated with the purposes of exposure (all P<0.05). Binary Logistic regression analysis indicated that the children′s gross motor was affected by the number of hours of daily exposure ( OR=1.868, P<0.05). The children′s fine motor movements were affected by mother′s educational level and the latest time every night of exposure ( OR=1.722, 2.355, all P<0.05). The children′s adaptability was affected by mother′s educational level, child caregivers, the number of hours of daily exposure and the latest time every night of exposure ( OR=1.711, 2.866, 1.895, 1.650, all P<0.05). The children′s speech was affected by the number of hours of daily exposure, the latest time every night of exposure, and the purposes of exposure (telephone or video phone, early education or study)( OR=2.348, 1.806, 0.328/0.350, all P<0.05). The children′s social interaction and self-care delay were affected by mother′s educational level, the number of hours of daily exposure and the purposes of exposure (telephone or video phone, early education or study)( OR=1.647, 2.678, 0.307/0.363, all P<0.05). Conclusions:The adverse effects of exposure to mobile phones and other electronic products on children should be well concerned.The exposure time of developing young children should be strictly controlled to prevent the adverse effects on the nervous system and development in children.For children who were already affected, relevant habits and behaviors should be timely corrected to avoid irreversible damages.
8. Influencing factors of health literacy among college students in a university

OBJECTIVE: To analyze the status and influencing factors of the health literacy(HL) of college students in a comprehensive university. METHODS: A total of 3 360 students from in a comprehensive university of Xinjiang Production and Construction Corps was selected using multi-stage stratified cluster random sampling method. The HL level of college students was investigated and evaluated with self-edited Xinjiang Construction Corps College Students Health Literacy Questionnaire. RESULTS: The HL level of college students was 17.1%. The HL level of medical students was higher than that of non-medical students(35.4% vs 10.0%, P<0.01). Logistic regression analysis results showed that among the medical students in grade three or four, those with medium and excellent academic achievement, and Han nationality had a positive effect on their HL level(P<0.01). Among the non-medical students, female and medicine related optional courses had a positive effect on their HL level(P<0.05). Students in the sophomore year had a negative effect on their HL level(P<0.05). CONCLUSION: There is a big difference in the level of HL between medical students and non-medical students. Medical college students and non-medical college students have different factors affecting HL, medical education is related to improving HL.
9.Investigation on the health status of children in an organization in the urban area of Beijing

Objective:To understand the health status of children in an organization in the urban area of Beijing.Methods:From October to November 2019, 558 children aged from 0 to 13 of employees in an organization in Beijing received physical examination, including height, weight, ophthalmology and otorhinolaryngology, heart and lung examination, etc.Statistical analysis was conducted on the items related to physical status and with higher detection rate. Body mass index (BMI)≥P 85(P is the percentile compared to the standardized growth curve) was defined as overweight and BMI≥P 95 as obese. The rate and composition ratio were used to describe the relevant data of each item in children′s physical examination. The differences between different age groups and gender groups were compared with the chi-square test. Results:The items with high detection rate were overweight, obesity, rhinitis, caries, phimosis, etc. The ratio of children′s weight and height ≤P 10 were 3.4% and 2.7%, ≥P 97 were 12.0% and 7.0%respectively; the rates of overweight (BMI≥P 85) and obesity (BMI≥P 95) in children were 13.8% and 13.3% respectively, the ratio of weight ≥P 97, height ≥P 97 and BMI≥P 95all increased with age in all children and in boys (all P<0.05), the ratio of BMI≤P 5 increased with age ( P<0.05), and the ratio of body weight ≥P 75 decreased with age in boys ( P<0.05); the ratio of weight ≥P 75 in boys was significantly higher than that in girls ( P<0.05). The prevalence of rhinitis in children was 39.8%, among which the prevalence of allergic rhinitis was 14.9%.The prevalence of allergic rhinitis in the 3.0-5.9 age group of all children was significantly higher than that in 0.0-0.9 age group and 6.0-9.9 age group (both P<0.05), the prevalence of total rhinitis in the 3.0-5.9 age group of girls was significantly higher than that in 0.0-0.9 age group, 1.0-2.9 age group and 10.0-13.9 age group (all P<0.05). The prevalence of dental caries in children (excluding the group of 0.0-0.9 years old) was 26.0%, the prevalence of dental caries in the 3.0-5.9 age group of all children was significantly higher than that in 1.0-2.9 age group and 10.0-13.9 age group (both P<0.05), the prevalence of dental caries in 3.0-5.9 age group and 6.0-9.9 age group of boys and girls were both significantly higher than that in 1.0-2.9 age group (all P<0.05). The prevalence of phimosis in boyswas 31.5%, the phimosis rates were significantly higher in the 1.0-2.9 age group and 3.0-5.9 age group than that in 6.0-9.9 age group and 10.0-13.9 age group (all P<0.05). No statistical differences were found in the prevalence of allergic rhinitis and dental caries between obese and non-obese children ( P>0.05). Conclusion:The severity of overweight and obesity in children is beyond our expectation, the allergenicity problem is becoming more and more prominent, the prevention and treatment of dental caries needs to be improved urgently, and the phimosis problem in boys needs to be paid attention to.
