1.Liver Injury Caused by Psoraleae Fructus: A Review

Psoraleae Fructus (PF) is a non-toxic Chinese herbal medicine, while the liver injury caused by PF has aroused wide concern in recent years. At present, animal experiments and in vitro studies have been carried out to explore the mechanism, targets, and toxic components of PF in inducing liver injury, which, however, have differences compared with the actual conditions in clinical practice, and there are still some potential hepatotoxic components and targets of PF that have not been discovered. With the continuous progress in systems biology, establishing the drug-induced liver injury model and the liver injury prediction model based on network toxicology can reduce the cost of animal experiments, improve the toxicity prediction efficiency, and provide new tools for predicting toxic components and targets. To systematically explain the characteristics of liver injury in the application of PF and explore the potential hepatotoxic components and targets of PF, we reviewed the related articles published by China National Knowledge Infrastructure (CNKI), Wanfang Data, VIP, and PubMed from 1962 to 2021 and analyzed the characteristics and influencing factors of liver injury caused by PF in the patients. Furthermore, we summarized the chemical components of PF and the components entering blood. By reviewing the mechanism, targets, and components of PF in inducing liver injury that were discovered by in vivo and in vitro experiments, we summarized the known compounds in PF that may cause liver injury. Finally, the current methods for building the prediction model of PF-induced liver injury were summarized, and the predicted toxic components and targets were introduced. The possible factors of PF in causing liver injury were explained from three aspects: clinical characteristics, preclinical studies, and computer-assisted network prediction, which provide a reference for predicting the risk of PF-induced liver injury.
2.Analysis of the suspected cases of measles and rubella in Pudong New Area of Shanghai, 2013‒2022

ObjectiveTo analyze the epidemic characteristics of measles and rubella in Pudong New Area of Shanghai from 2013 to 2022, and to provide data support for the elimination of measles and rubella. MethodsEnzyme linked immunosorbent assay was used to detect IgM antibodies in serum samples. The sequence of 630 nucleotides at the C-terminal of N gene of measles virus was amplified by reverse transcription-polymerase chain reaction and the phylogenic tree was constructed. ResultsA total of 1 529 suspected cases of measles were detected from 2013 to 2022, among which the positive rate of measles IgM antibody was 33.55% (513/1 529). The highest positive rate (20.73%) was from March to May , and the positive rate of rubella IgM antibody was 6.80% (104/1 529). The positive rate of both IgM was higher in males than that in females (P<0.05). The IgM against measles was mainly detected in 0‒ years old (63.16%, 96/152) and 20‒ years old (45.61%, 161/353). The IgM against rubella was mainly detected in 10‒20 years old (27.27%, 18/66). The IgM antibody could be detected more easily from 4 to 28 days after eruption, and the IgM antibody positive rate of measles/rubella from 2020 to 2022 was significantly lower than previous years (2013‒2019). There were 2 D8 genotype strains, and the rest were H1a gene subtypes. ConclusionThe positive rate of IgM antibodies against measles/rubella in Pudong New Area of Shanghai decreased significantly. People aged 0‒ years and 20‒ years old are more susceptible to measles, and rubella is concentrated in 10‒ years old. It is necessary to strengthen the vaccination of school-age children, in order to achieve the goal of eliminating measles. The age group with high risk of exposure should be checked for vaccination status to ensure the enhanced immunization, and the surveillance of imported measles cases should be strengthened.

N6-methyladenosine modification is regulated by methylase and demethylase,resulting in a dynamic and reversible process.Changes in m6A levels are involved in a wide range of cellular processes,including nuclear RNA output,mRNA metabolism,protein translation,and RNA splicing,and have strong associations with various diseases.The purpose of this paper is to generalize and summarize the role and mechanism of the change of m6A expression level in mRNA in three common diseases,as well as the research trend based on the change of m6A level in mRNA as a drug intervention target.
4.Qualitative Analysis of Metabolites of Aristolochiae Fructus Aqueous Extract in Rats

ObjectiveBased on ultra performance liquid chromatography-quadrupole-time-of-flight mass spectrometry(UPLC-Q-TOF-MSE) technique, we identified qualitatively the metabolites of aristolochic acid(AAs) in rat in order to analyze the metabolic differences between water extract of Aristolochiae fructus(AFE) and Aristolochic acid Ⅰ(AAⅠ). MethodSD rats were selected and administered AFE(110 g·kg-1·d-1) or AAⅠ(5 mg·kg-1·d-1) by oral for 5 days, respectively. Serum, urine and feces were collected after administration. Through sample pretreatment, ACQUITY UPLC BEH C18 column(2.1 mm×100 mm, 1.7 μm) was used with the mobile phase of 0.01% formic acid methanol(A)-0.01% formic acid water(B, containing 5 mmol·L-1 ammonium acetate) for gradient elution(0-1 min, 10%B; 1-7 min, 10%-75%B; 7-7.2 min, 75%-95%B; 7.2-10.2 min, 95%B; 10.2-10.3 min, 95%-10%B; 10.3-12 min, 10%B) at a flow rate of 0.3 mL·min-1. Positive ion mode of electrospray ionization(ESI+) was performed in the scanning range of m/z 100-1 200. In combination with UNIFI 1.9.4.053 system, the Pathway-MSE was used to qualitatively analyze and identify the AAs prototype and related metabolites in biological samples(serum, urine and feces), and to compare the similarities and differences of metabolites in rats in the subacute toxicity test between AFE group and AAⅠ group. ResultCompared with AAⅠ group, 6, 10, 13 common metabolites and 14, 20, 30 unique metabolites were identified in biological samples(serum, urine and feces) of AFE group, respectively. Moreover, the main AAs components always followed the metabolic processes of demethylation, nitrate reduction and conjugation. Compared with common metabolites in AAⅠ group, prototype components of AAⅠ in serum and most metabolic derivatives of AAⅠ[AAⅠa, aristolochic lactam Ⅰ(ALⅠ)a, 7-OHALⅠ and its conjugated derivatives] in biological samples were significantly increased in AFE group(P<0.05, P<0.01), except that the metabolic amount of ALⅠ in feces of AFE group was remarkably lowed than that of AAⅠ group(P<0.01). In addition, a variety of special ALⅠ efflux derivatives were also identified in the urine and feces of the AFE group. ConclusionAlthough major AAs components in AFE all show similar metabolic rules as AAⅠ components in vivo, the coexistence of multiple AAs components in Aristolochiae Fructus may affect the metabolism of AAⅠ, and achieve the attenuating effect by increasing the metabolic effection of AAⅠ and ALⅠ.
5.Effect of escitalopram combined with repetitive transcranial magnetic stimulation on efficacy and attention function of patients with first-episode unipolar depression

Objective:To observe the effect of escitalopram combined with repetitive transcranial magnetic stimulation (rTMS) on efficacy and attention function in patients with first-episode unipolar depression.Methods:Fifty-two first-episode initial-naive unipolar depression patients were enrolled in Department of Neurology of Guangzhou First People's Hospital from March 2022 to April 2023 were chosen. They were randomly allocated to active stimulation group ( n=27) and sham stimulation group ( n=25); both were treated with escitalopram, and active treatment or sham treatment in the left dorsolateral prefrontal cortex (DLPFC) were given for 4 weeks (5 d per week, 20 d totally). Before treatment and 2 and 4 weeks after treatment, Hamilton Depression Rating Scale (HAMD)-24 was used to evaluate depressive symptoms, and Birmingham Cognitive Screening Scale-Chinese (BCoS-C) was used to evaluate the attention function. Results:(1) In terms of depressive symptoms: HAMD-24 scores of the active stimulation group 2 and 4 weeks after treatment (20.63±2.73, 15.85±2.43) were significantly lower than those before treatment (25.74±2.68, P<0.05); HAMD-24 scores of sham stimulation group 4 weeks after treatment were also significantly lower than those before treatment ([20.48±2.33] vs. [25.80±2.57], P<0.05); HAMD-24 scores of the active stimulation group 2 and 4 weeks after treatment were significantly lower than those of sham stimulation group ( P<0.05). (2) In terms of auditory attention indicators: total correct number (selective attention) in active stimulation group 4 weeks after treatment was significantly larger than that before treatment (51.74±1.38 vs. 47.48±1.60), and the sustained index (sustained attention) was significantly lower than that before treatment (0.74±0.71 vs. 4.37±1.15, P<0.05); total correct number in active stimulation group 4 weeks after treatment was significantly larger than that in sham stimulation group (48.00±1.66), and the sustained index was significantly lower than that in sham stimulation group (3.72±1.28, P<0.05). Conclusion:Combined with escitalopram, rTMS can more effectively mitigate the depressive symptoms in first-episode unipolar depression patients, and depressive symptoms improve more quickly than attentional function.
6.An experimental study of magnetic anchor technique-assisted endoscopic submucosal dissection for early esophageal cancer

Objective:To investigate the feasibility of magnetic anchor technique for endoscopic submucosal dissection (ESD) in the treatment of early esophageal cancer.Methods:A self-designed magnetic anchoring device (including an anchor magnet and a target magnet) was used to perform ESD on the hypothesized esophageal lesion mucosa of six isolated esophagus of Beagle dogs. The feasibility and convenience of the operation was evaluated.Results:ESD of 6 isolated esophagus of dogs was successfully completed. Through adjusting the position of anchor magnet, the pulling direction and force of the target magnet on the mucosa could be flexibly controlled, the mucosal peeling surface was fully exposed, and tissue tension was provided to ensure the smooth removal of the diseased mucosa. The entire operation was smooth, and the target magnet was conveniently retained. No target magnet slippage or mucosal laceration occurred during the operation.Conclusion:The magnetic anchor technique is safe and feasible for the ESD, effectively pulling the diseased mucosa in treatment of early esophageal cancer, which can greatly improve the endoscopic operation experience.

8.Functional connectivity of resting-state attention network in first-episode treatment-naive depressed patients

Objective To observe the functional connectivity of attentional network in first-episode treatment-naive depressed patients with resting-state functional MR imaging (rfMRI),and to investigate the neuropathologic mechanism of attention dysfunction.Methods Eighteen first-episode treatment-naive depressed patients were collected in our hospital from August 2015 to March 2017,and 16 healthy controls matched with age,gender and education were chosen at the same time period.All subjects accepted rfMRI.Pearson correlation analysis was performed on all the time series values of the two regions of interest (ROIs),and their correlation coefficient was obtained.Fisher z transformation was performed to reflect the functional connection strength of the two ROIs through z scores.Results As compared with healthy controls,depressed patients showed abnormal enhancement on functional connectivity of the attentional network in the left anterior cingulate and supplementary motor area (z scores:0.21 ±0.10 vs.0.13±0.09,P=0.029),right basal ganglia region and dorsolateral prefrontal cortex (z scores:0.18±0.12 vs.0.10±0.08,P=0.048),right frontal eye field and intraparietal suleus (z scores:0.19± 0.12 vs.0.10±0.05,P=0.006),right frontal eye field and nucleus ceruleus (z scores:0.18±0.13 vs.0.09± 0.07,P=0.018).Conclusion Abnormal functional connectivity of attentional network exists extensively in first-episode treatment-naive depressed patients,which may be due to attention impairment in depressed patients.
9.Correlation between emotional intelligence, psychological resilience and sense of career successfulness in community nurses

Objective? To explore the status quo of sense of career successfulness, emotional intelligence and psychological resilience and the effects of emotional intelligence and psychological resilience on sense of career successfulness, and to provide a reference for stabilizing and managing the community nursing team. Methods? Totally 380 in-service nurses were selected from a community health service center in Beijing using convenient sampling between April and May 2019. A cross-sectional questionnaire survey was conducted with the self-designed questionnaire, Sense of Career Successfulness Scale (SCSS), Schutte′s Emotional Intelligence Scale-Chinese (SSEIS-C) and Connor Davidson Resilience Scale (CD-RISC). Results? A total of 380 questionnaires were distributed and 352 valid questionnaires were returned. The SCSS score of the 352 community nurses was (73.09±13.61), their total SSEIS-C score was (78.98±12.29), their total CD-RISC score was (88.01±14.79). Univariate analysis results showed that different marital status, with or without children, post satisfaction, job satisfaction, monthly income satisfaction, nurse-patient relationship satisfaction, colleague relationship satisfaction, cooperation relationship satisfaction, interest in management, research or specialty affairs, occupational development, manifestation of self-worth and decision to or not to quit were the influencing factor of the professional success of community nurses(P< 0.05). Psychological resilience and emotional intelligence were significantly and positively correlated with sense of career successfulness. A hierarchical regression analysis revealed that general demographic data could account for 32.4% of the total variable of sense of career successfulness in community nurses; emotional intelligence could account for 14.4%;and psychological resilience could account for 11.7%. Conclusions? The sense of career successfulness in community nurses stands at a medium level, which is correlated with multiple factors such as their research work, nurse-patient relationship satisfaction, special interest, emotional intelligence and psychological resilience. Community health services should pay attentional to these factors and improve their sense of career successfulness by enhancing their emotional intelligence and psychological resilience.
10. Relationship between hyperuricemia and cardiovascular risk factors among outdoor male traffic policemen

OBJECTIVE: To comprehend the prevalence status of elevated serum uric acid(SUA) and investigate the relationship between elevated SUA and cardiovascular risk factors and the clustering of the cardiovascular risk factors among outdoor male traffic policemen. METHODS: Selected by convenience sampling,1 039 outdoor traffic policemen in Guangzhou were asked to complete a questionnaire survey,physical and laboratory examination. According to the level of SUA > 420. 00 μmol/L or not,they were divided into elevated SUA group and control group. RESULTS: The median SUA level of outdoor male traffic policemen was 431. 00 μmol/L,and the elevated SUA prevalence was 56. 3%. The length of working years,systolic blood pressure,diastolic blood pressure,and levels of serum triglyceride(TG),total cholesterol(TC),low density lipoprotein cholesterol(LDL-C),and serum creatinine(Scr) in the elevated SUA group were statistically higher than the control group(P < 0. 01). The high density lipoprotein cholesterol level was lower in the control group than that in the elevated SUA group(P < 0. 01). After adjusting for age and alcohol consumption,the results of multivariate logistic regression analyses showed that outdoor traffic policemen who suffer from overweight or obesity,elevated TG and elevated TC have more risk in suffering from elevated SUA(P < 0. 05). The odds ratios(ORs) and 95%confidence intervals(CIs) were 2. 347(1. 772-3. 109),2. 040(1. 517-2. 743) and 1. 431(1. 080-1. 896) respectively.The risk factors of suffering from elevated SUA increased along with the increase of outdoor working years or Scr level(P <0. 05). The ORs and 95% CIs were 1. 028(1. 004-1. 054) and 1. 048(1. 033-1. 062) respectively. The proportion of people with elevated SUA among outdoor traffic policemen increased with the increase of cardiovascular risk factors(P <0. 01). The risk of elevated SUA among outdoor male traffic policemen who have 1,2,3,4 and ≥5 cardiovascular risk factors were 1. 583,2. 351,4. 657,2. 865 and 13. 576 times higher than those without cardiovascular risk factor respectively(P < 0. 05). CONCLUSION: Among outdoor male traffic policemen,elevated SUA are closely associated with the cardiovascular risk factors. The risk factors of suffering from elevated SUA increased with clustering of cardiovascular risk factors.
